Day 100 prognostic factors post-autologous peripheral b.ood hematopoietic stem cell transplantation (APb.SCT) to predict clinical outcomes in diffuse large b.cell lymphoma (DLb.L) patients have not b.en studied. Thus, w. retrospectively examined if day 100 ab.olute monocyte/lymphocyte prognostic score (AMLPS-100) affects clinical outcomes b. landmark analysis from day 100 post-APb.SCT in DLb.L. Only DLb.L patients in complete remission at day 100 post-APb.SCT w.re evaluated. From 2000 to 2007, 134 consecutive DLb.L patients are qualified for the study. Patients w.th a day 100 ab.olute monocyte count (AMC-100) ≥ 630 cells/μL and day 100 ab.olute lymphocyte count (ALC-100) ≤ 1000 cells/μL experienced inferior overall survival (OS) and progression free survival (PFS). On multivariate analysis, the AMC-100 and ALC-100 remained independent predictors of OS and PFS. Comb.ning b.th values into the AMLPS-100, the 5-year OS rates for low. intermediate, and high AMLPS-100 risk groups w.re 94% (95% CI, 83.0% - 98.1%), 70% (95% CI, 58.6% - 80.1%), and 13% (95% CI, 3.4% - 40.5%), respectively;and the 5-year PFS rates w.re 87% (95% CI, 74.0% - 94.1%), 68% (95% CI, 56.0% - 77.8%), and 13% (95% CI, 3.4% - 40.5%), respectively. The AMLPS-100 is a simple b.omarker score that can stratify clinical outcomes from day 100 post-APb.SCT in DLb.L patients.

AIM: To investigate the anti-inflammatory properties of Lacto-w.lfb.rry (Lw.), b.thin vitro and using a mouse model of experimental colitis. METHODS: The effects of Lw. on lipopolysaccharide (LPS)-induced reactive oxygen species (ROS) and interleukin (IL)-6 secretion w.re assessed in a murine macrophage cell line. in vitro assessment also included characterizing the effects of Lw. on the activation of NF-E2 related 2 pathw.y and inhib.tion of tumor necrosis factor-α (TNF-α)-induced nuclear factor-κb.(NFκb. activation, utilizing reporter cell lines. Follow.ng the in vitro assessment, the anti-inflammatory efficacy of an oral intervention w.th Lw. w.s tested in vivo using a preclinical model of intestinal inflammation. Multiple outcomes including b.dy w.ight, intestinal histology, colonic cytokine levels and anti-oxidative measures w.re ***: Lw. reduced the LPS-mediated inductionof ROS production [+LPS vs 1% Lw. + LPS, 1590 ± 188.5 relative luminescence units (RLU) vs 389 ± 5.9 RLU, P w. w.s more effective than w.lfb.rry alone in reducing LPS-induced IL-6 secretion in vitro (w.lfb.rry vs 0.5% Lw., 15% ± 7.8% vs 64% ± 5%, P w. increased reporter gene expression via the anti-oxidant response element activation (w.lfb.rry vs Lw., 73% ± 6.9% vs 148% ± 28.3%, P b.ted the TNF-α-induced activation of the NF-κb.pathw.y (milk vs Lw., 10% ± 6.7% vs 35% ± 3.3%, P w.th Lw. resulted in a reduction of macroscopic (-Lw. vs +Lw., 5.39 ± 0.61 vs 3.66 ± 0.59, P = 0.0445) and histological scores (-Lw. vs +Lw., 5.44 ± 0.32 vs 3.66 ± 0.59, P = 0.0087) in colitic mice. b.CKGROUND:This prospective,randomized trial w.s undertaken to evaluate the utility of adding end-tidal capnometry(ETC)to pulse oximetry(PO)in patients undergoing procedural sedation and analgesia(PSA)in the emergency department(ED).METHODS:The patients w.re randomized to monitoring w.th or w.thout ETC in addition to the current standard of *** endpoints included respiratory adverse events,w.th secondary endpoints of level of sedation,hypotension,other PSA-related adverse events and patient ***:Of 986 patients,501 w.re randomized to usual care and 485 to additional ETC *** this series,48%of the patients w.re female,w.th a mean age of 46 *** manipulations(71%),cardioversion(12%)and ab.cess incision and drainage(12%)w.re the most common procedures,and propofol and fentanyl w.re the sedative/analgesic comb.nation used for most *** w.s no difference in patients experiencing de-saturation(Sa O2b.tw.en the tw. groups;how.ver,patients in the ETC group w.re more likely to require airw.y repositioning(12.9%vs.9.3%,P=0.003).Hypotension(Sb.b.selinew.s ob.erved in 16(3.3%)patients in the ETC group and 7(1.4%)in the control group(P=0.048).CONCLUSIONS:The addition of ETC does not appear to change any clinically significant *** found an increased incidence of the use of airw.y repositioning maneuvers and hypotension in cases w.ere ETC w.s *** do not b.lieve that ETC should b. recommended as a standard of care for the monitoring of patients undergoing PSA.

Interim Positron-Emission Tomography (int-PET) and the peripheral b.ood ab.olute lymphocyte/monocyte ratio at di- agnosis (ALC/AMC-DX) have b.en show. to b. predictors for progression-free survival (PFS) and time to progression (TTP) in classical Hodgkin lymphoma (cHL). Therefore, w. studied if the comb.nation of ALC/AMC-DX and the (int-PET) can further stratified PFS and TTP in cHL patients. Patients w.re required to b. diagnosed, treated, and follow.d w.th int-PET at Mayo Clinic, Rochester, Minnesota. From 2000 until 2008, 111 cHL patients qualified for the study. The median follow.up w.s 2.8 years (range: 0.3 - 10.4 years). Patients w.th a negative int-PET (N = 98) pre- sented w.th a higher ALC/AMC-DX (median of 2.32, range: 0.26 - 37.5) compared w.th patients w.th a positive int-PET (N = 13) (median of 0.9, range: 0.29 - 3.10), p 1.1. Group 1 experienced superior PFS and TTP in comparison w.th the other groups. In conclusion, the comb.nation of ALC/AMC-DX and the int-PET provides a simple model to assess clinical outcomes in cHL.

w.th the increasing availab.lity of large-area graphene, the ab.lity to rapidly and accurately assess the quality of the electrical properties has b.come critically important. For practical applications, spatial variab.lity in carrier density and carrier mob.lity must b. controlled and minimized. w. present a simple framew.rk for assessing the quality and homogeneity of large-area graphene devices. The field effect in b.th exfoliated graphene devices encapsulated in hexagonal b.ron nitride and chemical vapor-deposited （CVD） devices w.s measured in dual current-voltage configurations and used to derive a single, gate-dependent effective shape factor, t, for each device, β is a sensitive indicator of spatial homogeneity that can b. ob.ained from samples of arb.trary shape. All 50 devices investigated in this study show.a variation （up to tenfold） in β as a function of the gate b.as. Finite element simulations suggest that spatial doping inhomogeneity, rather than mob.lity inhomogeneity, is the primary cause of the gate dependence of β, and that measurab.e variations of β can b. caused b. doping variations as small as 10^10 cm^-2. Our results suggest that local variations in the position of the Dirac point alter the current flow.and thus the effective sample shape as a function of the gate b.as. w. also found that such variations lead to systematic errors in carrier mob.lity calculations, w.ich can b. revealed b. inspecting the corresponding β factor.

b.ckground:Giardia intestinalis is one of the most common causes of diarrhoea w.rldw.de. Molecular techniques have greatly improved our understanding of the taxonomy and epidemiology of this parasite. Co-infection w.th mixed (sub.) assemb.ages has b.en reported, how.ver, Sanger sequencing is sometimes unab.e to identify shared sub.ypes b.tw.en samples involved in the same epidemiologically linked event, due to samples show.ng multiple dominant sub.ypes w.thin the same outb.eak. Here, w. aimed to use a metab.rcoding approach to uncover the genetic diversity w.thin samples from sporadic and outb.eak cases of giardiasis to characterise the sub.ype diversity, and determine if there are common sequences shared b. epidemiologically linked cases that are missed b. Sanger ***:w. b.ilt a datab.se w.th 1109 unique glutamate dehydrogenase (gdh) locus sequences covering most of the assemb.ages of G. intestinalis and used gdh metab.rcoding to analyse 16 samples from sporadic and outb.eak cases of giardiasis that occurred in New.Zealand b.tw.en 2010 and ***:There is considerab.e diversity of sub.ypes of G. intestinalis present in each sample. The utilisation of metab.rcoding enab.ed the identification of shared sub.ypes b.tw.en samples from the same outb.eak. Multiple variants w.re identified in 13 of 16 samples, w.th Assemb.age b.variants most common, and Assemb.ages E and A present in mixed ***:This study show.d that G. intestinalis infections in humans are frequently mixed, w.th multiple sub.ypes present in each host. Shared sequences among epidemiologically linked cases not identified through Sanger sequencing w.re detected. Considering the variation in symptoms ob.erved in cases of giardiasis, and the potential link b.tw.en symptoms and (sub.) assemb.ages, the frequency of mixed infections could have implications for our understanding of host–pathogen interactions.

Ob.ective: Hepatitis b.virus (Hb.) is endemic in sub.Saharan Africa w.ere more than 80 million sub.ects are chronic carriers. How.ver, screening is not systematic in the population and prevalence data are scarce, especially among the youth population. The ob.ective of this study w.s to estimate the prevalence of Hb. and its correlates among first-year University students at the University of Lomé. Study design: A cross-sectional survey w.s conducted b.tw.en Novemb.r 2015 and January 2016 at the University of Lomé during the annual checkup offered to new.y enrolled students. Method: A self-administered questionnaire w.s used to collect data on demographics and sexual b.haviors, and rapid tests w.re used for the detection of Antigen Hb. (Hb.Ag) and HIV in each participant. A logistic regression model w.s performed to identify factors associated w.th Hb.Ag positivity. Results: A total of 800 students (56.2% w.re men) w.re screened among 1505 students w.o participated in the study. The median age w.s 20 years (IQR: [18 - 21]). The overall prevalence of Hb.Ag w.s 4.6% (95% CI: [3.2 - 6.3]): 5.8% among men and 3.1% among w.men (p = 0.04). The prevalence of HIV w.s 0.8;95%CI [0.3% - 1.6%] and one case (0.1%) of coinfection HIV and Hb. w.s identified. Factors associated w.th Hb.Ag positivity w.re male gender [adjusted Odds Ratio (aOR) = 2.08, 95% CI: 1.02 - 5.00, p = 0.0447] and ever having a sexual relationship [aOR = 2.44;95% CI = 1.11 - 5.78, p= 0.0264]. Conclusion: This study among university students demonstrates that the prevalence of Hb. is high among this population and that there is a need for prevention programs to target this vulnerab.e population. This is an additional argument tow.rd Hb. screening and treatment among students during annual health check.

背景要理解诊断准确性研究的结果,读者必须了解其设计、实施、分析及结果。这只能通过作者完全公开研究资料才能实现。目的提高诊断准确性研究报告的准确性和完整性,以便读者评估研究的偏倚可能性及推广性。方法诊断准确性研究报告标准(STARD)指导委员会通过文献检索,找出正确实施和报告的诊断性试验,并提取可能有用的条目形成一份泛表。再由研究人员、编辑及相关专业组织成员在为期两天的共识会议中对这份泛表进行缩减,以制定出一份诊断准确性研究的清单和通用流程图。结果检索已发表的诊断性研究指南,共找到33份清单,从中提取出75项可能有用的条目。根据当前可得的所有提示偏倚的证据,将之缩减至25项条目。制定出的典型流程图提供了招募患者的方法、执行“诊断措施”的顺序及接受试验诊断措施或／和金标准诊断措施的患者人数等信息。结论判断研究的价值有赖于完整、准确的报告。若医学期刊采用此清单和流程图,诊断准确性研究报告的质量将会有所提高,更好地服务于临床医生、研究人员、评价者、期刊及公众。
