We comment on the article by Jia et al,in the World Journal of Gastrointestinal *** focus mainly on the factors that impair gastric motility and cause gastric retention in the pre-operative setting of endoscopic retrograde cholan-giopancreatography(ERCP).ERCP is a complex endoscopic therapeutic proce-dure,which demands great skill from the endoscopist but also has recognized *** retention impairs the endoscopist’s visibility but also increases the risk of complications,such as aspiration ***,identifying the factors that predispose to gastric retention alerts the endoscopists of the possible risks and enables them to take evasive *** authors in the current study by Jia et al developed and validated a predictive model,which in-corporates five different factors,i.e.,gender,primary disease,jaundice,opioid use,and gastrointestinal obstruction,which were found to influence gastric *** model was shown to have a high predictive value to accurately identify pa-tients at risk for gastric retention before a therapeutic ERCP.

Brain integrity and cognitive aptitude are often impaired in patients with diabetes mellitus, presumably a result of the metabolic complications inherent to the disease. However, an increasing body of evidence has demonstrated the central role of insulin-like growth factor 1(IGF1) and its relation to sex hormones in many neuroprotective processes. Both male and female patients with diabetes display abnormal IGF1 and sexhormone levels but the comparison of these fluctuations is seldom a topic of interest. It is interesting to note that both IGF1 and sex hormones have the ability to regulate phosphoinositide 3-kinase-Akt and mitogen-activated protein kinases-extracellular signal-related kinasesignaling cascades in animal and cell culture models of neuroprotection. Additionally, there is considerable evidence demonstrating the neuroprotective coupling of IGF1 and estrogen. Androgens have also been implicated in many neuroprotective processes that operate on similar signaling cascades as the estrogen-IGF1 relation. Yet, androgens have not been directly linked to the brain IGF1 system and neuroprotection. Despite the sex-specific variations in brain integrity and hormone levels observed in diabetic patients, the IGF1-sex hormone relation in neuroprotection has yet to be fully substantiated in experimental models of diabetes. taken together, there is a clear need for the comprehensive analysis of sex differences on brain integrity of diabetic patients and the relationship between IGF1 and sex hormones that may influence brain-health outcomes. As such, this review will briefly outline the basic relation of diabetes and IGF1 and its role in neuroprotection. We will also consider the findings on sex hormones and diabetes as a basis for separately analyzing males and females to identify possible hormone-induced brain abnormalities. this paper presents the results from unidirectional shaking table tests of two reduced scale steel models of a building frame, with one and two floors, respectively. these frames incorporate friction dissipators at every floor. the inputs are sine-dwells and artificial and registered earthquakes. this study is part of a larger research project aiming to assess the seismic efficiency of friction dissipators by means of an integrated numerical and experimental approach. Inside this framework, the main objectives of these experiments are to：（i） collect a wide range of results to calibrate a numerical model derived within the project, （ii） clarify some of the most controversial issues about friction dissipators （including behavior for inputs containing pulses, capacity to reduce resonance peaks, introduction of high frequencies in the response, and self- generated eccentricities）, （iii） better understand their dynamic behavior, （iv） provide insight on the feasibility and reliability of using simple friction dissipators for seismic protection of building structures and （v） characterize the hysteretic behavior of these devices. Most of these objectives are satisfactorily reached and relevant conclusions are stated.

Background:Despite efficacy in HCV eradication,direct-acting antiviral(DAA)therapy has raised controversies around their impact on hepatocellular carcinoma(HCC)*** we reported the first Australian data on HCC incidence in DAA-treated HCV patients with advanced fibrosis/***:We conducted a retrospective single center study of DAA-treated HCV patients with advanced fibrosis/cirrhosis from April 2015 to December *** with prior HCC were included if they had complete response to HCC ***:Among 138 patients who completed DAA therapy,133(96.4%)achieved sustained virologic response(median follow-up 23.8 months).ten had prior HCC and 5/10(50.0%)developed recurrence,while de novo HCC developed in 7/128(5.5%).Median time from DAA to HCC diagnosis was 34 weeks in recurrent HCC *** novo 52 weeks(P=0.159).In patients with prior HCC,those with recurrence(***)had shorter median time between last HCC treatment and DAA(12 vs.164 weeks,Pte analysis,failed sustained virologic response at 12 weeks(SVR12)(P=0.011),platelets(P=0.005),model for end-stage liver disease(MELD)score(P=0.029),alpha fetoprotein(AFP)(P=0.013),and prior HCC(Pted with HCC *** multivariate analysis,significant factors were prior HCC(OR=4.80;95%CI:1.47–48.50;P=0.010),failed SVR12(OR=2.83;95%CI:1.71–16.30;P=0.016)and platelets(OR=0.97;95%CI:0.95–0.99;P=0.009).Conclusions:Our study demonstrates a high incidence of recurrent HCC in HCV patients with advanced fibrosis/cirrhosis treated with *** associated with HCC development post-DAA were more advanced liver disease,failed SVR12 and prior HCC,with higher rates of recurrence in those who started DAA earlier.
