Percutaneous kidney stone surgery and radiation exposure: A review

During the past 3 decades,radiation exposure(RE)has increased drastically among patients undergoing percutaneous nephrolithotomy(PCNL),thus potentially causing new cases of cancer each year.The effective dose received by patients comes from pre-and postoperative computed tomography(CT)and intraoperative fluoroscopy(FL).We reviewed literature to find novel techniques and approaches that help to decrease RE of patients and personnel.We performed PubMed search using keywords percutaneous nephrolithotomy,intraoperative fluoroscopy,radiation exposure,imaging,percutaneous access,ultrasound,computed tomography,endoscopy,reconstruction,innovations,and augmented reality.Forty-four relevant articles were included in this review.As much as 20%of patients with first diagnosed urolithiasis exceed background RE level almost 17-fold.For diagnosing purposes using low-dose and ultra-low-dose CT,as well as low-dose dual energy scan protocols can be efficient ways to decrease RE while maintaining decent accuracy.Patients with urinary stones can be effectively monitored with digital tomosynthesis,ultrasound alone or ultrasound combined with plain film of the abdomen.Percutaneous access(PCA)into the kidney can be performed with reduced or even no RE,using novel PCA methods.REs from conventional imaging techniques during diagnosis and treatment increase probability of non-stochastic radiation effects.Urologists should be aware of protocols that decrease RE from CT and FL in diagnosis and management of urinary stones.Consideration of recently developed imaging modalities and PCA techniques will also aid in adherence to the“as low as reasonably achievable”principle.

The genetic framework for development of nephrolithiasis

Over 1%-15% of the population worldwide is affected by nephrolithiasis,which remains the most common and costly disease that urologists manage today.Identification of atrisk individuals remains a theoretical and technological challenge.The search for monogenic causes of stone disease has been largely unfruitful and a technological challenge;however,several candidate genes have been implicated in the development of nephrolithiasis.In this review,we will review current data on the genetic inheritance of stone disease,as well as investigate the evolving role of genetic analysis and counseling in the management of nephrolithiasis.

Combined Effect of a Catalytic Reduction Device with Waste Frying Oil-Based Biodiesel on NOx Emissions of Diesel Engines

Internal combustion engines with application in automobiles and other relevant industries constitute significant environmental pollution via the release of toxic exhaust gasses like carbon monoxide (CO), hydrocarbons (HC), particulate matter (PM), and nitrogen oxide (NOx). Engine researchers and manufacturers are challenged to develop external and internal measures to ensure environmentally friendly solutions to accommodate and conform to the growing list of emission standards. Therefore, this work presents an experimental investigation of the NOx emission profile of a diesel engine that is fuelled and fitted with waste frying oil-based biodiesel and catalytic converter. Using a single-cylinder, four-stroke air-cooled CI engine at a constant speed of 1900 rpm and different loadings of 25%, 50%, 75%, and 100%;fitted with a catalytic converter at the exhaust outlet of the engine and linked to a dynamometer and a gas analyser, an experiment was conducted at biodiesel/diesel volume blends of B0 (0/10), B5 (5/95), B20 (20/80), B30 (30/70), B70 (70/30), B100 (100/0);and 30% concentration (v/v), 0.5 litre/hr flow rate of aqueous urea from the catalytic converter. The results show an increasing NOx emission as the biodiesel component increased in the blend. The catalytic converter showed a downward NOx reduction with a significant 68% reduction in efficiency at high exhaust gas temperatures. It is concluded that the combined utilisation of waste frying oil-based biodiesel and the catalytic converter yields substantial NOx emission reduction.

Network Reliability Analysis as a Tool to Guide Investment Decisions in Distributed Generation

Distributed Generation (DG) in any quantity is relevant to supplement the available energy capacity based on various locations, that is, whether a site specific or non-site specific energy technology. The evacuation infrastructure that delivers power to the distribution grid is designed with appropriate capacity in terms of size and length. The evacuation lines and distribution network however behave differently as they possess inherent characteristics and are exposed to varying external conditions. It is thus feasible to expect that these networks behave stochastically due to fault conditions and variable loads that destabilize the system. This in essence impacts on the availability of the evacuation infrastructure and consequently on the amount of energy delivered to the grid from the DG stations. Reliability of the evacuation point of a DG is however not a common or prioritized criteria in the decision process that guides investment in DG. This paper reviews a planned solar based DG plant in Uganda. Over the last couple of years, Uganda has seen a significant increase in the penetration levels of DG. With a network that is predominantly radial and experiences low reliability levels, one would thus expect reliability analysis to feature significantly in the assessment of the proposed DG plants. This is however not the case. This paper, uses reliability analysis to assess the impact of different evacuation options of the proposed DG plant on its dispatch levels. The evacuation options were selected based on infrastructure options in other locations with similar solar irradiances as the planned DG location. Outage data were collected and analyzed using the chi square method. It was found to be variable and fitting to different Probability Distribution Functions (PDF). Using stochastic methods, a model that incorporates the PDFs was developed to compute the reliability indices. These were assessed using chi square and found to be variable and fitting different PDFs as well. The viability of the project is reviewed

Leptotrichia hongkongensis sp.nov.,a novel Leptotrichia species with the oral cavity as its natural reservoir

A straight, non-sporulating, Gram-variable bacillus (HKU24T) was recovered from the blood culture of a patient with metastatic breast carcinoma. After repeated subculturing in BACTEC Plus Anaerobic/F blood culture broth, HKU24T grew on brucella agar as non-hemolytic, pinpoint colonies after 96 h of incubation at 37 °C in an anaerobic environment and aerobic environment with 5% CO2. Growth was enhanced with a streak of Staphylococcus aureus. HKU24T was non-motile and catalase-negative, but positive for alkaline phosphatase, β-glucosidase, and α-glucosidase. It hydrolyzed phenylphosphonate and reduced resazurin. 16S rRNA, groEL, gyrB, recA, and rpoB sequencing showed that HKU24T occupies a distinct phylogenetic position among the Leptotrichia species, being most closely related to Leptotrichia trevisanii. Using HKU24T groEL, gyrB, recA, and rpoB gene-specific primers, fragments of these genes were amplified from one of 20 oral specimens. Based on phenotypic and genotypic characteristics, we propose a new species, Leptotrichia hongkongensis sp. nov., to describe this bacterium.
