Laparoscopic Surgery of Incisional Hernia: Technique and Short-Term Results in Three Surgical Units in Cameroon

Introduction: Incisional hernia is a common medical situation and its treatment has always been a challenge for general surgeons. If laparoscopic repair has become a gold standard elsewhere, it is increasingly done in our milieu, but no data has been found. The aim of this study was to review this surgery done in our country so far, to describe the operative technique and to evaluate the postoperative outcomes. Patients and methods: We conducted an observational descriptive study with retrospective collection of data from the 1st of July 2018 to 30th of June 2022, in three hospitals in Cameroon. Data on socio-demographic and clinical characteristics, surgical technique and postoperative outcomes of patients who had a laparoscopic repair of their incisional hernia were collected. Results: We reviewed 20 files. There were 14 women with an average age of 54.6 years. The average BmI was 32.8 kg/m2. Thirteen patients had a moderate size hernia and the hernia was localized at the midline in 18 cases. Only composite biface meshes were used with dimensions chosen to obtain at least 5 cm overlap. Spiral tacks were the only fixation means used. We registered 3 complications. One case of generalized peritonitis secondary to missed digestive perforation, one case of seroma and one case of postoperative ileus. We registered no death. Conclusion: Laparoscopic repair of incisional hernias is feasible in a resource-limited setting like ours. Compliance with the operative technique and recommendations is important to obtain short-term results closed to that put forward by literature.

The contribution of chronic peripheral inflammation to the pathogenesis of neurodegenerative diseases is an outstanding question. Sustained activation of the peripheral innate and adaptive immune systems occurs in the context of a broad array of disorders ranging from chronic infectious diseases to autoimmune and metabolic diseases. In addition, progressive systemic inflammation is increasingly recognized during aging. Peripheral immune cells could potentially modulate the cellular brain environment via the secretion of soluble molecules. There is an ongoing debate whether peripheral immune cells have the potential to migrate into the brain under certain permissive circumstances. In this perspective, we discuss the possible contribution of chronic peripheral inflammation to the pathogenesis of age-related neurodegenerative diseases with a focus on microglia, the resident immune cells of the brain parenchyma.

Cathodic activated stainless steel mesh as a highly active electrocatalyst for the oxygen evolution reaction with self-healing possibility

The oxygen evolution reaction(OER)represents one of the major bottlenecks for broad-based applications of many clean energy storage/conversion *** key to solving this problem lies in developing high-performing,cost effective and stable catalysts for the ***,we demonstrate that ubiquitous stainless steel mesh(SSm)materials activated by a facile cathodization treatment can be employed as a high performing OER catalyst,as showcased by the impressively low overpotentials of 275 and 319 mV to reach the benchmark current densities of 10 and 100 mA cm^−2(1.0 m KOH),*** SSm also exhibits excellent performance in a two-electrode water electrolyzer,which requires a low cell voltage of 1.58 at 10 mA cm^−2 and outperforms many of water electrolyzers using earth-abundant OER ***,cathodized SSm with minor performance degradation after the stability test can also be readily healed by subjecting it to an additional cathodization *** is disclosed that the superior performance of cathodized SSms stems from the surface enrichment of OER active Ni(oxy)hydroxide,facile gas-bubble removal and transportation over the unique mesh-structured surfaces,while the abundant reservoir of nickel in the bulk allows healing of the catalyst by a facile cathodization.

The emerging role and targetability of the TCA cycle in cancer metabolism

The tricarboxylic acid （TCA） cycle is a central route for oxidative phosphorylation in cells, and fulfills their bioenergetic, biosynthetic, and redox balance require- ments. Despite early dogma that cancer cells bypass the TCA cycle and primarily utilize aerobic glycolysis, emerging evidence demonstrates that certain cancer cells, especially those with deregulated oncogene and tumor suppressor expression, rely heavily on the TCA cycle for energy production and macromolecule synthesis. As the field progresses, the importance of aberrant TCA cycle function in tumorigenesis and the potentials of applying small molecule inhibitors to perturb the enhanced cycle function for cancer treatment start to evolve. In this review, we summarize current knowledge about the fuels feeding the cycle, effects of oncogenes and tumor suppressors on fuel and cycle usage, common genetic alterations and deregulation of cycle enzymes, and potential therapeutic opportunities for targeting the TCA cycle in cancer cells. With the application of advanced technology and in vivo model organism studies, it is our hope that studies of this previously overlooked biochemical hub will provide fresh insights into cancer metabolism and tumorigenesis, subsequently revealing vulnerabilities for thera- peutic interventions in various cancer types.

Better together?Comparing physical activity of parents walking outdoors with and without their child

This study compares the physical activity levels and enjoyment of parents(n=50;age=41.8±4.0 years)during outdoor,self-paced walking sessions while walking alone and when walking with their 6-to-12-year-old *** counts,moderate-to-vigorous physical activity(mVPA),vigorous physical activity,and enjoyment were compared using paired *** modeling was used to examine the impact of children's age and adults'and children's body mass index on the disparity in step counts experienced by *** attained high percentages of time spent in mVPA while walking alone(99.8%)and when walking with their child(96.6%).However,more of this time was categorized as vigorous physical activity when walking alone(13.6%vs.3.0%,p=0.006).Adults'step counts decreased by an average of 6.4%while walking with their child compared to walking alone(pmal-weight adults(p=0.042).Although adults obtained higher step counts and vigorous physical activity while walking alone,they enjoyed walking more with their child(pmaking recommendations for parent-child co-participation in physical activity,and parents may wish to supplement their physical activity obtained while walking with their child(ren)with additional activities that result in vigorous physical activity.

Background: Evidence regarding the effectiveness of prenatal nutritional supplements has mainly considered anthropometric pregnancy outcomes. The effect on markers of health and disease， such as offspring telomere length (TL) and mitochondrial DNA content (mtDNAc) is unknown. Objectives: We assessed the efficacy of maternal multiple micronutrient (mmN)-fortified balanced-energy protein (BEP) and iron-folic acid (IFA) supplementation on newborn TL as a secondary outcome and mtDNAc as a non-declared outcome. Design: We conducted a randomized controlled trial in rural Burkina Faso， among pregnant females (15-40 years old) enrolled at mothers received either mmN-fortified BEP and IFA (intervention) or IFA only (control) throughout pregnancy. Whole arterial blood samples were collected from the umbilical cord of 104 control and 90 intervention group infants， respectively. Average relative TL and mtDNAc were measured using quantitative polymerase chain reaction. Linear regression models were fitted to assess TL and mtDNAc differences across trial arms. Results: We found that a combined daily mmN-fortified BEP supplement and IFA tablet did not affect newborn TL [β = -0.010 (95% CI: -0.057， 0.036); P = 0.662] or mtDNAc [β = 0.065 (95% CI: -0.203， 0.073); P = 0.354]， as compared to an IFA tablet alone. These findings were confirmed (P > 0.05) by adjusting the regression models for potential prognostic factors of study outcomes at enrollment. Exploratory analyses indicated higher， but non-significantly different mtDNAc among children born either small-for-gestational age， low birthweight， or preterm. Conclusions: Newborns from mothers who received daily nutritional supplements across gestation did not have different relative TL or mtDNAc.

mechanistic considerations for the use of monoclonal antibodies for cancer therapy

Since the approval of rituximab in 1997, monoclonal antibodies(mAbs) have become an increasingly important component of therapeutic regimens in oncology. The success of mAbs as a therapeutic class is a result of great strides that have been made in molecular biology and in biotechnology over the past several decades. Currently, there are 14 approved mAb products for oncology indications, and there are ten additional mAbs in late stages of clinical trials. Compared to traditional chemotherapeutic agents, mAbs have several advantages, including a long circulating half-life and high target specificity. Antibodies can serve as cytotoxic agents when administered alone, exerting a pharmacologic effect through several mechanisms involving the antigen binding(Fab) and/or Fc domains of the molecule, and mAbs may also be utilized as drug carriers, targeting a toxic payload to cancer cells. The extremely high affinity of mAbs for their targets, which is desirable with respect to pharmacodynamics(i.e., contributing to the high therapeutic selectivity of mAb), often leads to complex, non-linear, target-mediated pharmacokinetics. In this report, we summarize the pharmacokinetic and pharmacodynamics of mAbs that have been approved and of mAbs that are nearing approval for oncology indications, with particular focus on the molecular and cellular mechanisms responsible for their disposition and efficacy.

Recent scientific consensus suggests that climate-related disasters are becoming more frequent and ***,increasing importance is given to disaster risk reduction(DRR)and climate change adaptation(CCA)in global *** projected global warming at 1.5℃and the climate variability that the Philippine archipelago experiences make DRR-CCA the key priorities of both the national and local government *** this study,we assessed and measured the degree of mainstreaming of DRR-CCA in the comprehensive development plans(CDPs)of the five component cities in the province of Nueva Ecija in the *** are among the areas in Central Luzon that are susceptible to hydrometeorological and geologic *** distributed survey questionnaires to 25 employees of the local government units in the five component cities in Nueva Ecija who are directly involved in planning *** triangulated their responses using archival data(review of written policies and plans).The main findings of the study indicate that in spite the presence of laws and policies on DRR-CCA,their implementation is only in the preliminary *** results also provide insights to policymakers and future researchers on the challenges and opportunities influencing the systemic mainstreaming of DRR-CCA in the province.

Effects of human disturbance on liana community diversity and structure in a tropical rainforest, malaysia: implication for conservation
