In some disease systems,the process of waning immunity can be subtle,involving a complex relationship between the duration of immunitydacquired either through natural infection or vaccinationdand subsequent boosting of immunity through asymptomatic *** present and analyse a model of infectious disease transmission where primary and secondary infections are distinguished to examine the interplay between infection and *** we allow the duration of infection-acquired immunity to differ from that of vaccine-acquired immunity to explore the impact on long-term disease patterns and prevalence of infection in the presence of immune *** model demonstrates that vaccination may induce cyclic behaviour,and the ability of vaccinations to reduce primary infections may not lead to decreased *** the boosting of vaccine-acquired immunity delays a primary infection,the driver of transmission largely remains primary *** contrast,if the immune boosting bypasses a primary infection,secondary infections become the main driver of transmission under a sufficiently long duration of *** results show that the epidemiological patterns of an infectious disease may change considerably when the duration of vaccine-acquired immunity differs from that of infection-acquired *** study highlights that for any particular disease and associated vaccine,a detailed understanding of the waning and boosting of immunity and how the duration of protection is influenced by infection prevalence are important as we seek to optimise vaccination strategies.

Role of ion channels in gastrointestinal cancer

In their seminal papers Hanahan and Weinberg described oncogenic processes a normal cell undergoes to be transformed into a cancer *** functions of ion channels in the gastrointestinal(GI)tract influence a variety of cellular processes,many of which overlap with these hallmarks of *** this review we focus on the roles of the calcium(Ca^2+),sodium(Na^+),potassium(K^+),chloride(Cl^-)and zinc(Zn^2+)transporters in GI cancer,with a special emphasis on the roles of the KCNQ1 K+channel and CFtR Cl-channel in colorectal cancer(CRC).Ca^2+is a ubiquitous second messenger,serving as a signaling molecule for a variety of cellular processes such as control of the cell cycle,apoptosis,and *** members of the tRP superfamily,including tRPM8,tRPM7,tRPM6 and tRPM2,have been implicated in GI cancers,especially through overexpression in pancreatic adenocarcinomas and down-regulation in colon ***-gated sodium channels(VGSCs)are classically associated with the initiation and conduction of action potentials in electrically excitable cells such as neurons and muscle *** VGSC NaV1.5 is abundantly expressed in human colorectal CRC cell lines as well as being highly expressed in primary CRC *** have demonstrated that conductance through NaV1.5 contributes significantly to CRC cell invasiveness and cancer ***2+transporters of the ZIP/SLC39A and Znt/SLC30A families are dysregulated in all major GI organ cancers,in particular,ZIP4 up-regulation in pancreatic cancer(PC).More than 70 K+channel genes,clustered in four families,are found expressed in the GI tract,where they regulate a range of cellular processes,including gastrin secretion in the stomach and anion secretion and fluid balance in the intestinal *** distinct types of K+channels are found dysregulated in the GI *** are hERG1 upregulation in PC,gastric cancer(GC)and CRC,leading to enhanced cancer angiogenesis and invasion,and KCNQ1 down-regulation in CR

“En bloc ” caudate lobe and inferior vena cava resection following cytoreductive surgery and hyperthermic intraperitoneal chemotherapy for peritoneal and liver metastasis of colorectal cancer

there are diverse protocols to manage patients with recurrent disease after primary cytoreductive surgery(CRS) with hyperthermic intraperitoneal chemotherapy(HIPEC) for peritoneal carcinomatosis. We describe a case of metachronous liver metastasis after CRS and HIPEC for colorectal cancer, successfully treated with a selective metastectomy and partial graft of the inferior vena cava. A 35-year-old female presented with a large tumour in the cecum and consequent colonic stenosis. After an emergency right colectomy, the patient received adjuvant chemotherapy. One year later she was diagnosed with peritoneal carcinomatosis, and it was decided to carry out a CRS/HIPEC. After 2 years of total remission, an isolated metachronous liver metastasis was detected by magnetic resonance imaging surveillance. the patient underwent a third procedure including a caudate lobe and partial inferior vena cava resection with a prosthetic graft interposition, achieving an R0 situation. the postoperative course was uneventful and the patient was discharged on postoperative day 17 after the liver resection. At 18-mo follow-up after the liver resection the patient remained free of recurrence. In selected patients, the option of re-operation due to recurrent disease should be discussed. Even liver resection of a metachronous metastasis and an extended vascular resection are acceptable after CRS/HIPEC and can be considered as a potential treatment option to remove all macroscopic lesions.

Clostridium paraputrificum septicemia and liver abscess

We report the first case of a healthy 23-year-old female who underwent an interventional radiology-guided embolization of a hepatic adenoma, which resulted in a gas forming hepatic liver abscess and septicemia by Clostridium paraputrificum. A retrospective review of Clostridial liver abscesses was performed using a Pub Med literature search, and we found 57 clostridial hepatic abscess cases. the two most commonly reported clostridial species are C. perfringens and C. septicum(64.9% and 17.5% respectively). C. perfringens cases carried a mortality of 67.6% with median survival of 11 h, and 70.2% of the C. perfringens cases experienced hemolysis. All C. septicum cases were found to have underlying liver malignancy at the time of the presentation with a mortality of only 30%. the remaining cases were caused by various Clostridium species, and this cohort's clinical course was significantly milder when compared to the above C. perfringens and C. septicum cohorts.

Learning to combine multiple string similarity metrics for effective toponym matching

Several tasks related to geographical information retrieval and to the geographical information sciences involve toponym matching,that is,the problem of matching place names that share a common *** this article,we present the results of a wide-ranging evaluation on the performance of different string similarity metrics over the toponym matching *** also report on experiments involving the usage of supervised machine learning for combining multiple similarity metrics,which has the natural advantage of avoiding the manual tuning of similarity *** with a very large dataset show that the performance differences for the individual similarity metrics are relatively small,and that carefully tuning the similarity threshold is important for achieving good *** methods based on supervised machine learning,particularly when considering ensembles of decision trees,can achieve good results on this task,significantly outperforming the individual similarity metrics.

One parabolic p-Laplacian-like differential equation with mixed boundaries is au/at in the corresponding studies is replaced by a（au/at）, studied in this paper, where the item au/at which makes it more general. the sufficient condition of the existence and uniqueness of non-trivial solution in L2（O, t; L2 （Ω）） is presented by employing the techniques of splitting the boundary problems into operator equation. Compared to the corresponding work, the restrictions imposed on the equation are weaken and the proof technique is simplified. It can be regarded as the extension and complement of the previous work.

Characterization of the Genetic Diversity of trachemys dorbigni and Phrynops hilarii

the utilization of RAPD and ISSR molecular markers is proposed to initiate studies of genetic variability in Phrynops hilarii（Chelidae） and trachemys dorbigni（Emydidae）, two species of fresh water turtles distributed in South America. three primers of RAPD and four of ISSR were selected and the amplified products of these markers were evaluated by electrophoretic runs in agarose and polyacrylamide gels. the levels of heterozygosity, Shannon index and different allele numbers were slightly higher in P. hilarii for both types of markers. Levels of polymorphism were also higher in P. hilarii than t. dorbigni and both were elevated compared to those recorded for other species. the fact that similar results were obtained with both types of markers for all estimates of diversity highlights the usefulness and validity of the RAPD technique. the molecular markers used were found potentially useful for analysing future temporal and spatial distribution of genetic diversity in both species, expanding scales work.
