Effectiveness of an Secondary Prevention Program in Chronic Kidney Disease

Background: There are many programs which focus on late-stage chronic kidney disease (CKD), and it is considered that further evidence needs to be generated regarding the effectiveness of the programs used before renal replacement therapy. Study Design: A cohort study. Settings & Participants: Patients over 15 years of age who had been diagnosed with CKD according to the KDOQI (Kidney Disease Outcomes Quality Initiative) guidelines and who had undergone conventional treatment (CT) or a renal protection program (RPP). These were patients of two Colombian health insurance companies. Predictors: Age, sex, marital status, comorbidities, CKD stage, and clinical indicators. Outcomes: First CKD progression, and need for renal replacement therapy (RRT). Measures: Clinical marker. Results: The RPP is structurally and functionally different from the CT. It offers the interdisciplinary management of patients, a greater number of medical appointments, and patients start to receive treatment at younger ages and at earlier stages of their condition. The clinical markers of the patients following the RPP are within adequate ranges, and their renal function is less impaired, despite the differences in basal conditions. Upon finishing the study, we found that patients who received CT had a higher risk of receiving nephrotoxic drugs and not receiving nephroprotective drugs. The explanatory variables for the first progression were age, stage, history of dyslipidemia, and hemoglobin, potassium, and albumin levels. These variables, together with glycemia levels were also valid for RRT, except for history of dyslipidemia, as it was not significant. Upon adjusting for the explanatory variables, it was found that belonging to the RPP and attending more appointments had a protective effect in the process of controlling renal damage. Background: This document includes recommendations and guidelines issued by a group of Mexican researchers and specialists gathered in the First National Colloquium for the Diagnosis and Management of Chronic Myeloid Leukaemia (CML) by initiative of Instituto Nacional de Cancerología and with the support of the Leukaemia Department of the MD Anderson Cancer Center. Mexico lacks of updated information taken from its own reality on the diagnosis and treatment of CML and other haematological disorders;besides, there are no national guidelines. Aim: To publish a consensus document with guidelines for the management of CML adjusted to the national environment and overall characteristics. Method: The participants answered a DELPHI questionnaire about the overall aspects of the disease, aiming to target controversial topics, discuss them in the colloquium, and to agree on the best ones. After those meetings, a final document was drawn up. Results: The group presents recommendations for definition, diagnosis, prognosis, monitoring, and treatment of CML in Mexico. Conclusions: Having consensus guidelines for the clinical management of CML in our country will enable the consensual practice of Mexican specialists regarding the clinical approach to CML, as well as optimize the resources which allow the rational planning of the medical care strategies.

Tribological behavior of ammonium-based protic ionic liquid as lubricant additive

In this study, the tribological behavior of an ammonium-based protic ionic liquid(PIL) as an additive in a base mineral oil(MO) is investigated on a steel–steel contact at room temperature and 100℃. Tri-[bis(2-hydroxyethylammonium)] citrate(DCi) was synthesized in a simple and low-cost way, and the ionic structure of DCi was confirmed by proton nuclear magnetic resonance(^(1)H NMR). The stability measurement of 1 wt% DCi to a MO was investigated, and the lubricating ability and anti-wear properties of DCi as an additive in MO were also examined using a custom-designed reciprocating ball-on-flat tribometer. Optical microscope and profilometry were used to obtain the worn morphology of the steel disks. Scanning electron microscopy(SEM) and energy dispersive X-ray spectroscopy(EDS) were carried out to investigate the wear mechanism and to analyze the surface interactions between the rubbing components. When 1 wt% DCi is added into the base MO, frictional performance is improved at both temperatures studied with a friction reduction of 29.0% and 35.5%, respectively. Moreover, the addition of 1 wt% DCi to MO reduced the wear volume 59.4% compared to the use of MO. An oxygen-richened tribolayer is confirmed by EDS on the disk surface when DCi was used as additive under 100℃.

Synchronous waving in fiddler crabs: a review

Many animals that use acoustic communication synchronize their mate attract!on signals: individuals precisely time their calls to overlap those of their neighbors. In contrast, synchrony in the mate attraction displays of species with visual/motion-based signals is rare. It has only been documented in five species of fiddler crabs. In all of them, small groups of males wave their single large claw in close synchrony. Here, I review what we know about synchrony in fiddler crabs, comparing the five species with each other to determine whether similar mechanisms and functions are comm on to all. I also propose future research questions that, if answered, would shed light on synchronous behavior in both visual and acoustic signallers.

Orbit orientation in didelphid marsupials （Didelphimorphia： Didelphidae）

Usually considered a morphologically conservative group, didelphid marsupials present consider- able variation in ecology and body size, some of which were shown to relate to morphological structures. Thus, changes on orbit morphology are likely and could be related to that variation. We calculated orbit orientation in 873 specimens of 16 Didelphidae genera yielding estimates of orbits convergence （their position relative to midsagittal line） and verticality （their position relative to frontal plane）. We then compared similarities in these variables across taxa to ecological, morpho- logical and phylogenetic data to evaluate the influencing factors on orbit orientation in didelphids. We found an inverse relation between convergence and verticality. Didelphids orbits have low ver- ticality but are highly convergent, yet orbit orientation differs significantly between taxa, and that variation is related to morphological aspects of the cranium. Rostral variables are the only morpho- logical features correlated with orbit orientation： increasing snout length yields more convergent orbits, whereas increase on snout breadth imply in more vertical orbits. Size and encephalization quotients are uncorrelated with orbit orientation. Among ecological data, diet showed significant correlation whereas locomotion is the factor that less affects the position of orbits. Phylogeny is uncorrelated to any orbital parameters measured. Ecological factors seemingly play a more import- ant role on orbit orientation than previously expected, and differentiation on orbit orientation seems to be more functional than inherited. Thus, despite the apparent homogeneity on didelphid morphology, there is subtle morphological variability that may be directly related to feeding behavior.

Dependence of Upper Atmosphere Photochemistry on the Shape of the Diurnal Cycle of the Photolysis Rates

A photochemical model of the atmosphere constitutes a non-linear, non-autonomous dynamical system, enforced by the Earth＇s rotation. Some studies have shown that the region of the mesopause tends towards non-linear responses such as period-doubling cascades and chaos. In these studies, simple approximations for the diurnal variations of the photolysis rates are assumed. The goal of this article is to investigate what happens if the more realistic, calculated photolysis rates are introduced. It is found that, if the usual approximations-sinusoidal and step fiunctions—are assumed, the responses of the system axe similar： it converges to a 2-day periodic solution. If the more realistic, calculated diurnal cycle is introduced, a new 4-day subharmonic appear.
