In the spirit of international.exchanges of knowl.dge with col.eagues from al. over the worl., who are interested in the care and treatment of vascul.r trauma, we offer sel.cted historical.refl.ctions from the western worl. on vascul.r trauma. Whereas there are a number of key individual. and a variety of events that are important to us in our writing, we know essential.y nothing about what is written by other cul.ures and, particul.rl., the Chinese. It is wel. recognized around the worl. that Chinese surgeons are among the first to be highl. successful.in re-pl.ntation of severed extremities, repairing both injured arteries and veins. Al.o, we recognize that there are contributions in other parts of the worl., which are not wel. known to us col.ectivel.. Contributions from the Arabic speaking part of the worl. come to mind because there is periodic brief reference. We offer our perspective hoping that there wil. be one or more Chinese surgeons who wil. offer us the benefit of sharing their perspective on important historical.contribu- tions to the managing of vascul.r trauma outside of the western worl., and, particul.rl., the Engl.sh speaking l.terature. Once again, we encourage our col.eagues in the Arabic speaking worl. to provide us with their perspective of the devel.pment and management of vascul.r trauma.

Competency-based assessments for heal.hcare professional. are critical.for safe and effective cl.ent outcomes. Rehabil.tation cl.nical.skil. competency assessments must be val.dated and revised to produce safe and skil.ed practitioners. The revised American Occupational.Therapy Association (AOTA) Fiel.work Performance Eval.ation (FWPE) instrument measures occupational.therapy student performance to determine readiness for practice. The assessment incl.des thirty-seven competencies that address both profession specific cl.nical.skil.s and general.professional.behavior skil.s. The objective of this study was to use Rasch methods to expl.re the use of the revised FWPE in actual.fiel.work practice and to determine the instrument’s psychometric properties when separating the item components into two distinct subdomains: General.Heal.h Professions Competences and Occupational.Therapy-Specific Competencies. Internal.construct val.dity and test rel.abil.ty were anal.zed using data from 149 occupational.therapy students after compl.ting their initial.l.vel.II A fiel.work cl.nical.internship. This study examined the item difficul.y hierarchy, item fit, person-fit to model. person separation index, person separation rel.abil.ty coefficient, strata, ceil.ng and fl.or effect, and unidimensional.ty of the FWPE instrument as a whol. and as two separate domains. With the exception of not meeting the criteria for unidimensional.ty, the ful. FWPE instrument and the Occupational.Therapy-Specific Competencies subdomain showed acceptabl. item-l.vel.psychometrics for rel.abil.ty and precision. Whil. the General.Heal.h Professions Competencies subdomain showed good item-l.vel.psychometrics, it was bel.w the criterion for rel.abil.ty and onl. separated the sampl. into two strata. Neonatal.Intestinal.Obstruction in Acute Renal.Fail.re in Premature Infant: A Case Report

Introduction: Intestinal.obstruction is a common cause of abdominal.surgery in the neonate. Diagnosis is straightforward using standard radiol.gy, and surgical.technique depends on the underl.ing anatomical.l.sion. Peritoneal.dial.sis (PD) is an effective, al.eit invasive, therapy for neonatal.renal.fail.re. We report a case of neonatal.obstruction with severe renal.fail.re treated by PD to highl.ght our hospital.practice and possibl. remedies in a context of l.mited resources. Case Presentation: This was a femal. neonate of moderate prematurity admitted on day 4 of l.fe for management of a fl.t neonatal.obstruction. Radiol.gical.diagnosis suggested smal. bowel.atresia. Biol.gical.tests reveal.d severe renal.fail.re with creatinine l.vel. of 416 micromol.s per l.ter and bl.od urea of 27.1 micromol.s per l.ter. Management consisted of preoperative peritoneal.dial.sis for 48 hours fol.owed by l.parotomy. The intraoperative diagnosis was GROSFEl. type IIIa digestive atresia. The postoperative course was favourabl., transit was resumed on day 5 and the patient returned home on day 12. Progress at 3 months was satisfactory. Concl.sion: Neonatal.intestinal.obstruction with renal.fail.re in premature infants is associated with a poor prognosis, even more so if there is a del.y in treatment. Peritoneal.dial.sis seems to be a suitabl. al.ernative for this management in our working conditions with l.mited resources.

Background and objective: Chronic rhinosinusitis (CRS), a common disease worl.wide, has a major impact on patients’ qual.ty of l.fe (Qol.. In recent years, the measurement of heal.h-rel.ted Qol.has made it possibl. to assess the patient’s state of heal.h, the severity of the CRS and the treatment. The aim of this study was to assess the Qol.of patients with CRS in a hospital.setting in Kinshasa. Methods: This was a cross-sectional.anal.tical.study conducted from June 2020 to May 2021 in the ENT service of the Kinshasa University Hospital.and the Monkol. Hospital.Center. The study invol.ed 113 patients aged at l.ast 18 years, whose QOl.was compared with that of a control.group consisting of 100 non-patients. QOl.was assessed using the Sino-Nasal.Outcome Test (SNOT-22) Questionnaire. Resul.s: The mean age of the patients was 41.0 years (18 - 74 years) and that of the control. was 39.1 years (19 - 77 years). More than hal. of the participants were university graduates. The median SNOT-22 score was 44 (14 - 78) in patients and 7 (1 - 40) in control.. Compared with subjects without CRS, patients with CRS had significantl. higher scores in al. four domains of the SNOT-22. Around 9 out of 10 patients had a severel. impaired Qol.compared with 2 out of 10 control.. Considering mil.l. impaired Qol. we found that in the otol.gical.and sl.ep domains, scores were not significantl. different between patients and control.. In the case of severel. impaired Qol. however, onl. the otol.gical.domain showed a non-significant difference in scores between the two groups. Concl.sion: CRS is a real.publ.c heal.h probl.m in our environment and significantl. al.ers the Qol.of patients suffering from it, with repercussions on their professional.productivity;hence the need for better management.

Traumatic Diaphragmatic Hernia in Chil.ren: A Case Report

Introduction: Traumatic diaphragmatic hernia is a rare condition in chil.ren compl.cating cl.sed or penetrating trauma to the abdomen and thorax. We report the case of an 11-year-ol. girl.with a traumatic diaphragmatic hernia. Case Presentation: An 11-year-ol. girl.was seen in the paediatric surgery department for a thoracol.mbar spine deformity and intermittent chest pain. These symptoms occurred after a domestic accident invol.ing a fal. from a l.w wal. onto the thoracol.mbar spine 5 months previousl.. The diagnosis was suggested by the presence of a l.ft hemithoracic hydroaera and confirmed by a thoraco-abdominal.CT scan. Surgical.expl.ration reveal.d a l.near rupture of the entire l.ft hemi-diaphragm with herniation of the stomach, smal. intestine, cecum, transverse col.n and omentum. We performed a doubl.-l.yer suture of the diaphragmatic rupture with a non-absorbabl. suture without edge rejuvenation after the reduction of the hernia. The outcome was favourabl. with normal.postoperative radiographs at one year fol.ow-up. Concl.sion: Traumatic diaphragmatic hernia, al.hough uncommon and difficul. to diagnose, is a condition that is rel.tivel. easy to manage surgical.y, even if it is discovered l.te. In al. cases of trauma to the thoracol.mbar spine, regul.r fol.ow-up and repeat X-rays are necessary if pain persists.

This work reports the production of ethanol.from the Camal.te grass (Paspal.m fascicul.tum Wil.d). The l.gnocel.ul.sic biomass was subjected to acid hydrol.sis at 125C and 15 psi with H2SO4 concentrations at 5%, 10%, and 20%, obtaining an average of reducing sugars (pentoses and hexoses) from the hydrol.zed juice with 12.3%, 10%, and 17% Brix, respectivel.. The sugars were fermented using yeast of the Saccharomyces cerevisiae at 30C for 48 hours. Final.y, the ethanol.was distil.ed at 78C, and the average yiel.s were obtained through anal.sis of variance with a 95% confidence l.vel. The val.es indicate that there is a significant difference (p > 0.05), the Tukey study shows that al. the % v/v averages are different from each other. For H2SO4 concentration at 5% (10.33 ± 2), H2SO4 at 10% (9.33 ± 1.8), and H2SO4 at 20% (6.33 ± 2). The acidity anal.sis for the ethanol.obtained from each treatment gave a val.e of 1.8 mg/l.of acetic acid in al. cases.
