Effectiveness of a Group Exercise Class vs. Home Exercise Program as a Follow-up to Physical Therapy for Older Adults with High Fall Risk

This quasi-experimental study compared the results of a traditional model of physical therapy(PT)care to a PT wellness model known as *** traditional model included discharge from PT with a home exercise program(HEP)to be self-administered with or without the addition of a community-based exercise *** wellness model included participation in a PT-designed and supervised group exercise program(GroupHab class)in an outpatient clinical setting following discharge from *** t-tests were used to compare the number of falls,exercise frequency,and exercise duration between the two groups.A repeated measures,analysis of variance(RM-ANOVA)compared changes in balance confidence scores both within and between groups,and a multivariate analysis of variance(MANOVA)analyzed group differences across multiple quality of life ratings using the *** data were analyzed at the 0.05 alpha level using SPSS 24 statistical *** results showed a significantly greater reduction in recurrent falls among the GroupHab wellness group compared to the HEP group(t=2.811,p=0.009).The resulting odds ratio for subsequent falls was 2.2 among HEP participants and 0.2 among GroupHab *** adherence was also greater for those who participated in the GroupHab *** documented greater exercise frequency(t=-3.253,p=0.002)and more exercise minutes(t=-7.188,p<0.001)than those who participated in the *** comparing changes in the participants’balance confidence,we found an average increase of 5%among GroupHab participants compared to a 6%decrease among HEP participants(F=16.877,p<0.001,power=0.981).Although our multivariate analysis of the SF-20 scores revealed no significant difference overall(F=0.768,p=0.73),the univariate analyses showed significantly greater improvements among GroupHab participants in selected areas of physical *** results suggest that at-risk older adults who are discharged into a functionally-based group exercise

Michael Handel has proved in [10] a fixed point theorem for an orientation preserving homeomorphism of the open unit disk, that turned out to be an efficient tool in the study of the dynamics of surface homeomorphisms. The present article fits into a series of articles by the author [13] and by Juliana Xavier [21, 22], where proofs were given, related to the classical Brouwer Theory, instead of the Homotopical Brouwer Theory used in the original article. Like in [13, 21] and [22], we will use “free brick decompositions” but will present a more conceptual Morse theoretical argument. It is based on a new preliminary lemma, that gives a nice “condition at infinity” for our problem.

Designing ionic channels in novel carbons for electrochemical energy storage

Tremendous efforts have been dedicated to developing high-performance energy storage devices based on the micro-or nano-manipulation of novel carbon electrodes, as certain nanocarbons are perceived to have advantages such as high specific surface areas, superior electric conductivities, excellent mechanical properties and so on. In typical electrochemical electrodes, ions are intercalated/deintercalated into/from the bulk(for batteries) or adsorbed/desorbed on/from the surface(for electrochemical capacitors). Fast ionic transport, significantly determined by ionic channels in active electrodes or supporting materials, is a prerequisite for the efficient energy storage with carbons. In this report, we summarize recent design strategies for ionic channels in novel carbons and give comments on the promising features based on those carbons towards tailorable ionic channels.

The Geometry Behind Numerical Solvers of the Poisson-Boltzmann Equation

Electrostatics interactions play a major role in the stabilization of biomolecules:as such,they remain a major focus of theoretical and computational studies in *** in solution is strongly dependent on the nature of the solvent and on the ions it *** methods that treat the solvent and ions explicitly provide an accurate estimate of these interactions,they are usually computationally too demanding to study large macromolecular *** solvent methods provide a viable alternative,especially those based on Poisson *** Poisson-Boltzmann equation(PBE)treats the system in a mean field approximation,providing reasonable estimates of electrostatics interactions in a solvent treated as *** the first part of this paper,we review the theory behind the PBE,including recent improvement in which ions size and dipolar features of solvent molecules are taken into account *** PBE is a non linear second order differential equation with discontinuous coefficients,for which no analytical solution is available for large molecular *** numerical solvers have been developed that solve a discretized version of the PBE on a mesh,either using finite difference,finite element,or boundary element *** accuracy of the solutions provided by these solvers highly depend on the geometry of their underlying meshes,as well as on the method used to embed the physical system on the *** the second part of the paper,we describe a new geometric approach for generating unstructured tetrahedral meshes as well as simplifications of these meshes that are well fitted for solving the PBE equation using multigrid approaches.

Tensile strength and failure behavior of rock-mortar interfaces: Direct and indirect measurements

The tensile strength at the rock-concrete interface is one of the crucial factors controlling the failure mechanisms of structures,such as concrete gravity *** the critical importance of the failure mechanism and tensile strength of rock-concrete interfaces,understanding of these factors remains very *** study investigated the tensile strength and fracturing processes at rock-mortar interfaces subjected to direct and indirect tensile *** image correlation(DIC)and acoustic emission(AE)techniques were used to monitor the failure mechanisms of specimens subjected to direct tension and indirect loading(Brazilian tests).The results indicated that the direct tensile strength of the rock-mortar specimens was lower than their indirect tensile strength,with a direct/indirect tensile strength ratio of 65%.DIC strain field data and moment tensor inversions(MTI)of AE events indicated that a significant number of shear microcracks occurred in the specimens subjected to the Brazilian *** presence of these shear microcracks,which require more energy to break,resulted in a higher tensile strength during the Brazilian *** contrast,microcracks were predominantly tensile in specimens subjected to direct tension,leading to a lower tensile *** monitoring of the cracking processes in the rock-mortar interfaces revealed that they show AE precursors before failure under the Brazilian test,whereas they show a minimal number of AE events before failure under direct *** to different microcracking mechanisms,specimens tested under Brazilian tests showed lower roughness with flatter fracture surfaces than those tested under direct tension with jagged and rough fracture *** results of this study shed light on better understanding the micromechanics of damage in the rock-concrete interfaces for a safer design of engineering structures.

The nuclear factor kappa B (NF-κB) transcriptionfactor coordinates several aspects of innate and adaptiveimmunity, inflammation, cell survival and proliferation [1,2]. Dysregulation of the NF-κB pathway has been associ-ated with cancer development and progression as well aswith other human diseases including viral infections anda number of inflammatory diseases [1]. NF-κB is formedthrough the dimerization of 5 subunits, including membersof the Rel protein family, namely RelA (p65), c-Rel, RelB,NF-κB1 (p50 and its precursor p105) and NF-κB2 (p52and its precursor p 100). Two pathways are involved in theactivation of NF-κB. In both these pathways, kinase activityand proteasome activity play a major role in freeing NF-κBfrom cytoplasmic inhibitors to promote its nuclear translo-cation and activation. The classical or canonical pathwayis activated by a large range of stimuli (microbial and viralinfections, proinflammatory cytokines). In this pathway,NF-κB dimers, mostly p50/RelA and p50/c-Rel, are usuallyretained in the cytoplasm by their interaction with specificinhibitors, called IκBs. Diabetes mellitus, insulin resistance and hepatitis C virus infection: A contemporary review

To summarise the literature data on hepatitis C virus (HCV)-infected patients concerning the prevalence of glucose abnormalities and associated *** conducted a PubMed search and selected all studies found with the key words 'HCV' or 'hepatitis C virus' and 'diabetes' or 'insulin resistance'. We included only comparative studies written in English or in French, published from January 2000 to April 2015. We collected the literature data on HCV-infected patients concerning the prevalence of glucose abnormalities [diabetes mellitus (DM) and insulin resistance (IR)] and associated risk [i.e., severe liver fibrosis, response to antivirals, and the occurrence of hepatocellular carcinoma (HCC)].RESULTSHCV infection is significantly associated with DM/IR compared with healthy volunteers and patients with hepatitis B virus infection. Glucose abnormalities were associated with advanced liver fibrosis, lack of sustained virologic response to interferon alfa-based treatment and with a higher risk of HCC development. As new antiviral therapies may offer a cure for HCV infection, such data should be taken into account, from a therapeutic and preventive point of view, for liver and non-liver consequences of HCV disease. The efficacy of antidiabetic treatment in improving the response to antiviral treatment and in decreasing the risk of HCC has been reported by some studies but not by others. Thus, the effects of glucose abnormalities correction in reducing liver events need further *** abnormalities are strongly associated with HCV infection and show a negative impact on the main liver related outcomes.
