Cryptococcal Meningitis in Patient with Chronic Myeloid Leukemia

Objective: This study aimed to report the case of a female patient with chronic myeloid leukemia affected by cryptococcal meningitis. Case report: ML, white, 48 years old, female sex, previously diagnosed with chronic myeloid leukemia that has been refractive to the use of imatinib and who has recently begun using nilotinib, was admitted complaining of sudden and disabling migraine in the last 1 month associated with asthenia, adinamia, anorexia, disinterest for daily activities, dizziness, nausea, and vomiting. She evolved with ataxia, and started to stroll with help and showed decrease of muscular strength in her upper limbs. She also presented episodes of decrease of consciousness, with look fixation, no respond to sound stimulation, and short-term hearing loss. The cerebrospinal fluid showed presence of Cryptococcus sp. and, therefore, we began treatment with intravenous liposomal amphotericin B in the dose of 3 mg/kg/day, for 6 weeks. A new cerebrospinal fluid analysis, at the end of treatment, also showed rare structures that are compatible with Cryptococcus sp. As sequelae, she continued with hearing loss in her right ear and enhancement in her right auditory canal, seen in the magnetic resonance imaging. After stabilization and clinical improvement, she was discharged. After 3 weeks, she was hospitalized again with degeneration of the condition, and died due to intracranial hypertension secondary to cryptococcal infection. Final Considerations: This report reinforces the need of reflecting on fungi pathologies, especially in immunosuppressant patients, as well as the importance of early diagnosing and making a fast intervention, with the aims of providing quality of life and comfort to the patient and of minimizing neurological sequelae to the patient.

Intimate partner violence(IPV)is simultaneously assumed as a serious crime and a major public health issue,having recurrences as one of its main characteristics and,consequently,re-entries of some alleged offenders in the criminal justice system(CJS).The main goal of this study is to assess if in cases of female victims of IPV,violence decreases after the first entry of the alleged offender in the CJS.A retrospective study was performed based on the analysis of police reports of alleged cases of IPV during a 4-year *** final sample(n=1488)was divided into two groups according to the number of entries in the CJS(single or multiple)followed by a comparative *** suggest that violence decreases after the first entry of alleged offenders in the ***-entries were found in only 15.5%of the cases but they were accountable for 3.3 times more crimes on ***,victims of recidivism presented more injuries and required more medical ***,a small group of alleged offenders seems to be more violent and accountable for most of the IPV crimes registered in the CJS suggesting that regardless of legal sanctions aiming to deter violence,these measures may not be enough for a certain group of *** study sustains the need for a predictive model to quantify the risk of repeated IPV cases within the Portuguese population.

以多壁碳纳米管(MWCNTs)为原料,经肽键形成氨基功能化后合成出新型多色荧光碳量子点(CQDs)。所制CQDs由准球形石墨纳米晶体(平均尺寸为10 nm)包裹不同烃类,如丙胺、辛胺、十二胺和八乙胺得到的氨基。在CQDs表面引入氮原子和烃类对其荧光性能、量子产率和溶解性起重要影响。这种新型有机-溶解CQDs的荧光发射波长取决于激发波长。荧光量子产率根据CQDs表面上烃类取代物的链长而变化。

分别以Fe40Al、Ni3Al和TiAl(NbCr)金属间化合物为基体,在一定的压力和温度下,使其和低孔隙率TiC粉体烧结体有效结合。采用扫描电镜及能谱仪等对其界面的组织结构进行分析。结果表明,所有金属间化合物均与TiC烧结体形成了冶金结合的界面。TiC烧结体在高温保压过程中有微量分解,扩散进入了Fe40Al和Ni3Al基体表层,降低了其熔点,从而使其成为可流动状态,被挤压进入TiC烧结体的孔隙。但TiAl(NbCr)合金未能进入TiC烧结体孔隙,而是在与TiC的界面处形成了一层Ti含量高于基体约10at%的反应层。

This work deals with the analysis of soil-structure interaction modeling of pipeline problems in static behavior using the coupling between FEM （finite element method） and BEM （boundary element method）. The representation of the pipe is made by MEF using one fmite element in the cylindrical panel formulated from the theory of equivalent discrete layers （Layerwise theory）, proposed by J. N. Reddy. The soil is represented by elastic continum infimite or semi-infinite and modeled using boundary elements with special curved surface, associated with cylindrical panel used to represent the soil-structure interaction within the soil, especially at the contact surface with the pipe.

Isolated versus Drug Combined Vestibular Rehabilitation for the Treatment of Metabolic Dizziness

Introduction: Systemic processes and inappropriate use of medications may affect vestibular adaptation. Objectives: To assess the efficacy of isolated and combined drug therapy for treating metabolic dizziness and to determine the effect of multidrug treatment. Methodology: We analyzed 100 questionnaires of patients diagnosed with dizziness of metabolic origin. Results: The group treated with vestibular rehabilitation alone showed improvement with 9 months of therapy (p = 0.01). The group treated with vestibular rehabilitation and medication showed improvement with 45 days of therapy (p = 0.01). The group treated with a single type of drug did not show significant improvement. The group treated with several drugs showed improvement with 45 days of therapy (p = 0.03). Conclusion: Combined vestibular rehabilitation resulted in immediate improvement of symptoms without contributing to long-term compensation. Vestibular rehabilitation alone resulted in improve- ment of symptoms over time. Multidrug treatment was beneficial for immediate relief of dizziness.

Detection of carotid artery plaques in the dental setting

It has been suggested that atherosclerotic plaques can be visualized in a panoramic radiograph but this tool is not reliable for systematic screening of patients due to its low sensitivity. This study investigated the potential role for dental practitioners in using an ultra- sound machine to detect carotid artery plaques. 100 subjects completed a survey concerning theirathero-sclerosis risk factors. Bilateral images and digital video clips of the carotid arteries of each person were obtained with a portable high-resolution ultrasound device equipped with an L38/10-5 MHz linear-array transducer. Images were subsequently interpreted. We found images suggestive of atherosclerotic plaque build-up in two Caucasian females: one 59 year-old with risk factors including: past smoker, high choles- terol, and family history of atherosclerosis and one 54 year-old with risk factors including: past smoker, overweight, and reported no physical activity. Both individuals were referred to their primary care providers for further assessments. 2% of the patient adult population may have carotid blockages detectable by ultrasound. The frequency can be as high as 5.5% in individuals older than 50. In the dental setting, ultrasound imaging can be used to capture bilateral pictures of the carotid arteries. With these images, at-risk patients may be screened for atherosclerotic plaques that could ultimately form a stroke-causing embolus. The dentist is particularly well suited for this role because he/she sees patients every six months while a physician can go years without seeing his/her patients.

Cardiac Myeloperoxidase Activity Is Elevated in Hypertensive Pregnant Rats

Myeloperoxidase（MPO） is released from activated neutrophils. The inflammation in preeclampsia was found to be associated with endothelial dysfunction. We hypothesized that cardiac and circulating MPO levels are elevated in hypertensive pregnancy. Systolic and diastolic blood pressure and heart rate were measured on pregnancy days 14, 16, 18 and 20 in normal pregnant and hypertensive pregnant rats. Left and right ventricle weights, the number of viable fetuses, litter size, fetal and placenta weights were recorded on gestational day 21. Circulating and cardiac MPO activities, soluble fms-like tyrosine kinase-1（sFlt-1） and vascular endothelial growth factor（VEGF） and nitric oxide（NO） were detected. The results showed increases in cardiac（left, but not right ventricle） and circulating MPO activities, and concomitantly lower number of viable fetuses, litter size, and fetal and placenta weights, and decreases in NO in hypertensive pregnant rats. Also, the increases in circulating sFlt-1 and VEGF were found in hypertensive pregnant group. In conclusion, maternal and fetal detrimental changes along with increases in circulating sFlt-1 and VEGF in hypertensive pregnancy may be associated with increases in cardiac and circulating MPO activities, confirming the causative role of inflammatory response in preeclampsia.
