Endoscopic balloon dilation of crohn's disease stricturessafety,efficacy and clinical impact

AIM To evaluate the incidence of anastomotic strictures after intestinal resection in Crohn's disease(CD), demonstrate long-term efficacy and safety of endoscopic balloon dilation(EBD) in CD strictures and its impact on the diagnosis of subclinical postoperative endoscopic recurrence. METHODS Retrospective single tertiary center study based on prospectively collected data between 2010 and 2015including anastomotic and non-anastomotic strictures. RESULTS29% of 162 CD patients included developed an anastomotic stricture. 43 patients with anastomotic strictures and 37 with non-anastomotic strictures underwent EBD; technical success was 97.7% and 100%, respectively, however, 63% and 41% needed repeat dilation during the 4.4-year follow-up. longer periods between surgery and index colonoscopy and higher lactoferrin levels were associated with the presence of stricture after surgery. Calprotectin levels > 83.35 μg/g and current or past history of smoking were associated with a shorter time until need for dilation(HR = 3.877, 95%CI: 1.480-10.152 and HR = 3.041, 95%CI: 1.213-7.627). Anastomotic strictures had a greater need for repeat dilation(63% vs 41%, P = 0.047). No differences were found between asymptomatic and symptomatic cohorts. Disease recurrence diagnosis was only possible after EBD in a third of patients. CONCLUSION EBD is an effective and safe alternative to surgery, with a good short and long-term outcome, postponing or even avoiding further surgery. EBD may allow to diagnose disease recurrence in patients with no clinical signs/biomarkers of disease activity.

Branch retinal vein thrombosis and visual loss probably associated with pegylated interferon therapy of chronic hepatitis C

Ophthalmological complications with interferon therapy are usually mild and reversible, not requiring the withdrawal of the treatment. We report a case of a patient who had visual loss probably associated with interferon therapy. Chronic hepatitis C virus infection （genotype la） was diagnosed in a 33-year old asymptomatic man. His past medical history was unremarkable and previous routine ophthalmologic check-up was normal. Pegylated interferon alpha and ribavirin were started. Three weeks later he reported painless reduction of vision. Ophthalmologic examination showed extensive intraretinal hemorrhages and cottonwool spots, associated with inferior branch retinal vein thrombosis. Antiviral therapy was immediately discontinued, but one year later he persists with severely decreased visual acuity. This case illustrates the possibility of unpredictable and severe complications during pegylated interferon therapy.

Antifungal activity of plant extracts with potential to control plant pathogens in pineapple

Objective:To evaluate the in vitro antifungal activity of extracts,resins,oils and mother tinctures from plants against the filamentous fungi Fusarium guttiforme(***)and Chalara paradoxa,and to evaluate the control of the pineapple fusariosis in situ using mother ***:The screening of the antifungal potential of 131 extract forms from 63 plant species was performed in vitro by using plate-hole *** control pineapple fusariosis in situ,preventive and post-infection treatments were performed on detached pineapple leaves of ***(susceptible).Results:The quantitative study indicated that among the 49 mother tincture samples analyzed,46% were effective against *** and 29% for the Chalara *** natural plant extracts,mother tincture of Glycyrrhiza glabra(MTGG1),mother tincture of Myroxylon balsamum(MTBT2),mother tincture of Aloe vera(MTAV3),mother tincture of Allium sativum(MTAS4),resin of Protium heptaphyllum(RESAM5) and crude extracts of Rhizophora mangle(CEMV6),exhibited an antifungal activity against *** the preventive treatment against pineapple fusariosis,MTAV3,MTAS4 and MTGG1 were statistically similar to the treatment with tebuconazol *** curative treatments with MTAV3,MTAS4,MTGG1 and MTBT2 presented similar activity to fungicide(P lon balsamum showed antifungal activity and was used here for the first time for inhibition of phytopathogenic *** study paves the way for the development of bioactive natural products with phytosanitary applications,with the added benefits of an environmentally safe and economically viable product.

In this study,the sperm ultrastructure of three species of Idarnes genus was investigated using light and transmission electron *** morphology of the three species was similar to that of most Chalcidoidea,with helicoidally twisted nucleus and *** head region consists of an acrosome and a nucleus;the nucleus-flagellum transition region characterized by the presence of mitochondrial derivatives and the centriolar adjunct;a flagellum region,which includes the axoneme with microtubular arrangement 9+9+2 and two mitochondrial ***,the sperm of these three species exhibit features that discriminate one species from each other:(1)only one species,Idarnes sp.2(carme group)exhibited an extracellular sheath surrounding the anterior portion of the nucleus,which extends to the anterior region of the flagellum,but it did not present filaments;(2)the acrosome in the three species was quite different,Idarnes sp.1 and Idarnes sp.2(carme group)has two compartments(acrosomal and subacrosomal vesicles)while Idarnes sp.3(flavicollis group)has a third compartment(perforatorium);(3)the centriolar adjunct elongated and its location among the mitochondrial derivatives is similar for the three species analyzed;(4)mitochondrial derivatives differ between the species,with triangular(Idarnes sp.1 and sp.3)and elongated or flat shaped(Idarnes sp.2)*** data shows that sperm structure may differ within the same genus and confirms the potential of these cells in phylogenetic and taxonomic analyses in the Chalcidoidea superfamily,as well as in Hymenoptera in general.

Screening of Environmental Bacterial Strains for Wild Aedes Mosquito Larval Biocontrol

The effectiveness of current control measures against Aedes mosquitoes remains low, resulting in persistent epidemics in urban areas. The emergence of resistant mosquito populations to chemical insecticides highlights the need for novel, environmentally friendly, cost-effective control strategies. This study explored the potential of environmental bacterial isolates to biocontrol wild Aedes larvae. Initially, we collected bacterial samples from infectious masses of Aedes fluviatilis larvae. The isolated bacteria were identified using biochemical, enzymatic, and molecular methods, including 16S rRNA sequencing and MALDI-TOF. Previously, Aeromonas hydrophila and Bacillus thuringiensis isolated from these infectious masses showed limited Aedes larval inhibition. Consequently, we screened additional environmental isolates from the bacteriotheque. Six isolates previously identified were tested: Chromobacterium violaceum, Enterobacter cloacae, Bacillus cereus, Bacillus sphaericus, and two strains of Bacillus thuringiensis israelensis. Among these strains, B. thuringiensis and C. violaceum exhibited significant inhibitory activities against wild Aedes larvae. Bacillus thuringiensis cultures grown under daylight conditions showed a slight ability to inhibit Aedes larvae. The potential of B. thuringiensis and C. violaceum strains studied, along with optimized culture growth conditions, will be further investigated to develop bioinsecticide products to provide safer and more sustainable alternatives for controlling larvae of Aedes mosquitoes.

The detection of dysplasia in patients with inflammatory bowel disease(IBD)continues to be important given the increased risk of colorectal cancer in this ***,in 2017,we performed a review and update of the recommendations for the management and follow-up of patients with IBD based on the clinical practice guidelines of various scientific *** present manuscript focuses on new aspects of the detection,follow-up,and management of dysplasia according to the latest studies and *** chromoendoscopy with targeted biopsy continues to be the technique of choice for the screening and detection of dysplasia in IBD,the associated difficulties mean that it is now being compared with other techniques(virtual chromoendoscopy),which yield similar results with less technical ***,the emergence of new endoscopy techniques that are still being researched but seem promising(e.g.,confocal laser endomicroscopy and full-spectrum endoscopy),together with the development of devices that improve endoscopic visualization(e.g.,Endocuff Vision),lead us to believe that these approaches can revolutionize the screening and follow-up of dysplasia in patients with ***,further studies are warranted to define the optimal follow-up strategy in this patient population.

Prevalence and Factors Associated with Diabetes among City Hall Workers, Bohicon, Benin

Introduction: Living and working conditions are increasingly conducive to a sedentary lifestyle. Many administrative workers are exposed daily to long hours of work in a prolonged sitting position, which can favour the onset of diabetes. The aim of the study was to determine the prevalence and factors associated with diabetes among workers at a town hall in Benin in 2019. Methods: This was a descriptive cross-sectional study conducted from December 2018 to January 2019. Exhaustive recruitment was carried out. Data were collected during a face-to-face interview, using a standardized questionnaire including socio-professional factors, behavioral factors, anthropometric measurements, and blood glucose. Frequencies were calculated. Frequencies were compared using the chi-square and Fisher tests. The significance threshold was 5%. Results: A total of 85 workers were included. Their mean age was 41.55 ± 9.82 years, with a sex ratio of 6.1. = The prevalence of diabetes was estimated at 8.2% [95% CI: 3.38 - 16.23%] and 3 of the workers knew they had diabetes. A family history of diabetes was associated with diabetes (p = 0.032). Occupationally, 10.77% of permanent employees had diabetes, compared with 0.0% of non-permanent employees;12.50% of employees with at least 10 years’ seniority had diabetes, compared with 7.25% of those with less seniority;19.05% of employees who spent at least 6 hours a day sitting at work had diabetes, compared with 4.69% of those who spent less time sitting. Conclusion: Preventing chronic disease in the workplace is part of promoting workers’ health, and must take into account occupational factors such as prolonged sitting.
