Genotype-Phenotype Correlation in a MODY 2 Family: An Under-Diagnosed Disease

Objective: We report a case of a MODY 2 family: a disease frequently under-diagnosed. Patients and Methods: We analyzed the case of three brothers that we suspected as affected by Type 1 diabetes because of their low BMI without clinical or biochemical parameters for this diagnosis. Their father was diagnosed as affected from Type 2 diabetes at the age 31 years old. Results: Genetic analysis revealed the presence in all analyzed family members of non-sense Ser 383x GCK mutation mapping in exon 9 of the gene. Conclusions: We described a case of a patient misdiagnosed as T2DM. Only after the observation of a mild hyperglicemia also in his three sons, we supposed the diagnosis of MODY 2 and we confirmed it through the genetic test. These observations enforce the validity of the designed clinical algorithm for the identification of patients to be selected for the genetic diagnosis of MODY 2.

During the carrying out of the monitoring plan as part of the Regional Law n. 3/2005 emerged, in 2008, the well known “buffalo milk crisis” that led to the enactment of other monitoring plans targeted to the research of dioxins in food. From the last monitoring plan on PCDD/Fs and DL-PCBs in Campania Region, named as “Surveillance plan 2011-2014”, all dairy products from N. 50 farms intended to the production of bovine, buffalo and ovine milk, resulted compliant to the maximum levels fixed by EU in Regulation 1259/2011, suggesting the effectiveness of the restrictive measures on farming practices applied after the sanitary emergency of 2008. On the other hand, the action limits reported in EU Recommendations 516/2011 and 711/2013 were exceeded in two sheep farms and two buffalo farms, to represent the critical issues that nowadays exist.

Treatment for isolated loco-regional recurrence of gastric adenocarcinoma: Does surgery play a role?

AIM: To evaluate the role of surgical treatment for isolated loco-regional recurrences of operated gastric adenocarcinoma. METHODS: Among the 837 patients operated for gastric adenocarcinoma between December 1979 and April 2004, 713 (85%) underwent resection with curative intent. A retrospective review of a prospectively collected gastric cancer database was carried out. Overall recurrence rate was 44% (315 cases), with 75% occurring within the first 2 years from the operation. Isolated L-R recurrences were observed in 38 (12%) patients. Symptomatic lesions were observed in 27 (71%). RESULTS: Six (16%) patients were macroscopically resected with curative intent. The recurrence was located in the gastric stump after a STG in three patients, in the esophagojejunal anastomosis after a TG in two patients and in the gastric bed after a TG in one patient. Surgical procedures consisted of three secondary TG, two esophagojejunal resection and one excision of an extraluminal recurrence. Postoperative complications occurred in two patients (33%), including one anastomotic leakage and one hemorrhage. The latter patient died of sepsis 35 d after the surgery (mortality rate 17%). All patients died of recurrent gastric cancer: 2 within 1 year from surgery (8 and 11 mo, respectively), 2 after 16 and 17 mo respectively and 1 after 28 mo from the second operation. CONCLUSION: Surgery plays a very limited role in the treatment for isolated loco-regional recurrence of gastric adenocarcinoma.

The present study examined disclosure and use of privacy settings in online social networking profiles as a function of the media context (i.e., online versus hard copy (paper and pencil) FacebookTM profiles). Gender was also examined. Overall, participants disclosed more information when constructing a profile for another person when using a hard copy paper and pencil format than an online context. Gender differences were not uniform across media contexts, however, in contrast to traditional disclosure theory, females censored their disclosures more so than males but only for some topics. Only 20% of the sample increased their use of privacy settings. Consistent with patterns of disclosure, descriptive comparison suggests that more settings were employed in the paper and pencil than online context and more privacy settings were employed by females.

Helicobacter pylori(H. pylori) is a widespread pathogen infecting about 40% of people living in urban areas and over 90% of people living in the developing regions of the world. H. pylori is well-documented as the main factor in the pathogenesis of peptic ulcer disease, chronic gastritis, and gastric malignancies such as cancer and mucosa-associated lymphoid tissuelymphoma; hence, its eradication is strongly recommended. the Maastricht Ⅳ consensus, which focused on the management of H. pylori infection, set important new strategies in terms of treatment approaches, particularly with regards to first- and second-line treatment protocols and led to improved knowledge and understanding of H. pylori resistance to antibiotics. In recent years, various fluoroquinolone-based protocols, mainly including levofloxacin, have been proposed and effectively tested at all therapeutic lines for H. pylori eradication. the aim of the present paper is to review the scientific literature focused on the use of fluoroquinolones in eradicating H. pylori.

Illegally paid blood donation was a risk factor for HIV acquisition exclusively in Henan and Hubei Provinces of China,and not in *** sequences in the gag and env genes of HIV-1 were compared between isolates from Henan and Shanghai regions of China to test whether an expected higher degree of a common source of infections from this unique blood donation transmission risk would be evident as decreased variation among Henan isolates in an exploratory cross-sectional *** 38 isolates studied,23 of 23(100%)from Henan and 8 of 15(54%)from Shanghai were subtype *** addition,fewer sequence differences were found in gp41 of subtype B isolates from Henan than from Shanghai *** studies with additional controls are therefore warranted to confirm the role of the degree of a common source of infections in differences in HIV variation across populations.

BACKGROUND Hepatobiliary manifestations occur in ulcerative colitis(UC)*** effect of laparoscopic restorative proctocolectomy(LRP)with ileal pouch anal anastomosis(IPAA)on hepatobiliary manifestations is *** To evaluate hepatobiliary changes after two-stages elective laparoscopic restorative proctocolectomy for patients with *** Between June 2013 and June 2018,167 patients with hepatobiliary symptoms underwent two-stage elective LRP for UC in a prospective observational *** with UC and having at least one hepatobiliary manifestation who underwent LRP with IPAA were included in the *** patients were followed up for four years to assess the outcomes of hepatobiliary *** The patients'mean age was 36±8 years,and males predominated(67.1%).The most common hepatobiliary diagnostic method was liver biopsy(85.6%),followed by Magnetic resonance cholangiopancreatography(63.5%),Antineutrophil cytoplasmic antibodies(62.5%),abdominal ultrasonography(35.9%),and Endoscopic retrograde cholangiopancreatography(6%).The most common hepatobiliary symptom was Primary sclerosing cholangitis(PSC)(62.3%),followed by fatty liver(16.8%)and gallbladder stone(10.2%).66.4%of patients showed a stable course after *** or regressive courses occurred in 16.8%of *** was 6%,and recurrence or progression of symptoms required surgery for 15%.Most PSC patients(87.5%)had a stable course,and only 12.5%became ***-thirds(64.3%)of fatty liver patients showed a regressive course,while one-third(35.7%)showed a stable *** rates were 98.8%,97%,95.8%,and 94%at 12 mo,24 mo,36 mo,and at the end of the *** In patients with UC who had LRP,there is a positive impact on hepatobiliary *** caused an improvement in PSC and fatty liver *** most prevalent unchanged course was PSC,while the most common improvement was fatty liver disease.
