Low Temperature Growth of Hydrogenated Silicon Prepared by PECVD from Argon Diluted Silane Plasma

In order to contribute to the understanding of the optoelectronics properties of hydrogenated nanocrystalline silicon thin films, a detailed study has been conducted. The samples were deposited by 13.56 MHz PECVD (Plasma-Enhanced Chemical Vapor Deposition) of silane argon mixture. The argon dilution of silane for all samples studied was 96% by volume. The substrate temperature was fixed at 200oC. The influence of depositions parameters on optical proprieties of samples was studied by UV-Vis-NIR spectroscopy. The structural evolution was studied by Raman spectroscopy and X-ray diffraction (XRD). Intrinsic-layer samples depositions were made in this experiment in order to obtain the transition from the amorphous to crystalline phase materials. The deposition pressure varied from 400 mTorr to 1400 mTorr and the rf power from 50 to 250 W. The structural evolution studies show that beyond 200 W, we observed an amorphous-nanocrystalline transition, with an increase in crystalline fraction by increasing rf power and working pressure. Films near the amorphous to nanocrystalline transition region are grown at reasonably high deposition rates (~10 /s), which are highly desirable for the fabrication of cost effective devices. The deposition rate increases with increasing rf power and process pressure. Different crystalline fractions (21% to 95%) and crystallite size (6 - 16 nm) can be achieved by controlling the process pressure and rf power. These structural changes are well correlated to the variation of optical proprieties of the thin films.

Acute anemia after dental extraction:A case report

A patient treated with a vitamin K antagonist (VKA) since the implantation of two mechanical heart valves developed acute anemia after the extraction of a tooth. This case report and data in the literature indicate a need for specific measures before, during, and after oral surgery in patients taking anticoagulant therapy: 1) the bleeding risk should be evaluated before the procedure. The INR should be measured routinely, 2) the procedure should be scheduled early in the week to allow an evaluation at the fibrinolysis peak, i.e., 48 to 72 hours after surgery, which is the time of greatest risk of delayed bleeding, 3) the surgical procedure should be appropriate for the elevated bleeding risk, 4) postoperative monitoring is of the most importance, as bleeding is usually delayed in patients on VKA therapy, 5) when poor treatment adherence is expected, day-hospital admission is useful to ensure that the postoperative protocol is implemented properly and to detect early bleeding. The treatment of post-extraction acute anemia includes local hemostasis protocol with the revision of the socket followed by red-blood-cell pack transfusion. A daily fluindione dosage control, and a normal hemoglobin level will allow the patient to leave the hospital.

There has been a growing interest over the past few years in the impact of male nutrition on fertility. Infertility has been linked to male overweight or obesity, and conventional semen parameter values seem to be altered in case of high body mass index （BMI）. A few studies assessing the impact of BMI on sperm DNA integrity have been published, but they did not lead to a strong consensus. Our objective was to explore further the relationship between sperm DNA integrity and BMI, through a 3-year multicentre study. Three hundred and thirty male partners in subfertile couples were included. Using the terminal uridine nick-end labelling （TUNEL） assay, we observed an increased rate of sDerm DNA damage in obese men （odds ratio （95% confidence interval）： 2.5 （1.2-5.1））.

Community Based Management of Severe Acute Malnutrition: The MSF Experience from an Urban Slum Setting in Bangladesh

Background: Until recently, the experience on implementing community based management of acute malnutrition (CMAM) among children has been largely based in African settings. While the government in Bangladesh is yet to scale up CMAM approach, there is still paucity of knowledge on the experience of CMAM within the complex milieu of an urban slum context. In Kamrangirchar slum, Dhaka, Bangladesh, this paper describes a CMAM programme performance and outcomes run by Médecins Sans Frontières (MSF)/Doctors without Borders, in light to performance indicators set by MSF and the Sphere minimum standards. Methods: This was a descriptive retrospective study using routinely collected programme data of children admitted with severe acute malnutrition between May 2010 and November 2011. Kamrangirchar is an urban slum of a large migrant population in Dhaka, Bangladesh. Results: There was a total of 640 new admissions, of whom 333 (52%) were males. The median age was 18 months (Inter-quartile range (IQR) 12 - 41). 599 children had a reported nutritional outcome at discharge from ambulatory therapeutic feeding centre (ATFC), this included: cure rate of 69% with an average length of stay of 68.8 (SD ± 46.0) days and average weight gain of 3.8 g/kg/day (SD ± 2.7). The lost-to-follow-up rate was 18% and 5% reported to the programme that they will leave the slum and go back to their villages. These performance indicators did not meet the threshold level indicators set by MSF and Sphere standards. Role of blood AFP mRNA and tumor grade in the preoperative prognostic evaluation of patients with hepatoceiluiar carcinoma

AIM： To explore the potential prognostic role of preoperative tumor grade and blood AFP mRNA in a cohort of patients with hepatocellular carcinoma （HCC） eligible for radical therapies according to a well-defined treatment algorithm not including nodule size and number as absolute selection criteria. METHODS： Fifty patients with a diagnosis of HCC were prospectively enrolled in the study. Inclusion criteria were：（1） histological assessment of tumor grade by means of percutaneous biopsies; （2） determination of AFP mRNA status in the blood; （3） patient＇s eligibility for radical therapies. RESULTS： At preoperative evaluation, 54% of the study group had a well-differentiated HCC, 42% had AFP mRNA in the blood, 40% had a tumor larger than 5 cm and 56% had more than one nodule. Surgery （resection or liver transplantation） was performed in 29 patients, while 21 had percutaneous ablation procedures. After a median follow-up of 28 too, 12-, 24-, and 36-mo survival rates were 78%, 58%, and 51%, respectively. Surgical therapy, performance status and three tumor-related variables （AFP mRNA, HCC grade and gross vascular invasion） resulted as significant survival predictors at univariate analysis. Nodule size and number did not perform as significant prognosticators. Multivariate study selected only surgical therapy and a biologically early HCC profile （AFP mRNA negative and well-differentiated tumor without gross vascular invasion） as independent survival variables. CONCLUSION： The preoperative determination of tumor grade and blood AFP mRNA status may potentially refine the prognostic evaluation of HCC patients and improve the selection process for radical therapies.

Differential Impact of an Executive-Function and a Social Cognition Training on Preschoolers with Externalizing Behavior Problems

In preschoolers, externalizing behavior problems are a primary cause of consultation. It is known that externalizing behavior (EB) could result from individual and environmental risk factors. Individual risk factors included notably social cognition (SC) and executive functions (EF). A high level of EB has usually been related to dysfunction in SC and to poor skills in EF. The aim of the present experimental study is to compare the impact of two very targeted child-oriented trainings in the increasing of social competence and decreasing of EB in preschoolers. One training targeted SC abilities while the second one targeted EF capacities. These two trainings were compared on 48 preschoolers presenting clinically relevant levels of EB. The comparison of those results highlighted how each training could help preschoolers with EB in their behavior, emotion regulation and social adjustment. In comparison to a waiting-list control-group, the two trainings were effective in decreasing EB and differentiated impacts of the two trainings were obtained on different dimensions of profiles of social competence and emotion regulation. Results are discussed for their research and clinical implications.

Multi-phase post-mortem CT-angiography： a pathologic correlation study on cardiovascular sudden death

Multi-phase post-mortem CT-angiography （MPMCTA） has the great potential to increase the quality of the postmortem investigation, especially in the area of sudden death; however, its role as routine complement to the pathology toolbox is still questioned as it needs to be further standardized. The aim of this study is to investigate the contribution of MPMCTA in cases of sudden unexplained death in adults and in particular in sudden cardiovascular death. Sixty-eight sudden unexpected deaths of adults were investigated at our institution between 2012 and 2013. Ten cases underwent MPMCTA and autopsy and were included in the study. Before the angiographic step by complete filling of the vascular system, prior to any manipulation of the body, a non-contrast CT-scan was carried out. Image reconstructions were performed on a CT workstation （Vitrea） and two radiologists experienced with post mortem imaging interpreted the MPMCTA findings. In all 10 cases, we could state a good correlation between combination of post-mortem CT and MPMCTA and autopsy procedures, confirming a high diagnostic sensitivity. With this case series we want to illustrate the advantages offered by performing MPMCTA when facing a sudden death, regardless of specific suspicion for acute coronary syndrome or other vascular or ischemic disease.

Antioxidant and Neuroprotective Activities of the Mesocarp of <i>Raphia hookeri</i>Fruit against Aluminum Chloride-Induced Neurotoxicity in Rats

This work aims to evaluate the antioxidant and protective effect of Raphia hookeri (Rh) fruit extracts and powders against aluminum induced neurotoxicity in rats. Extracts and powder were prepared using fruit mesocarp. Phytochemical contents (phenolic content and flavonoïd) and antioxidant in vitro activity were evaluated through the DPPH radical scavenging capacity, the ability to reduce ferric ion and Hydroxyl radical scavenging ability. The neurocognitive dysfunction, the activity of acetylcholinesterase (AChE) and activities of antioxidant in vivo indices were evaluated in the plasma, liver and brain of aluminum chloride (AlCl3) induced neurotoxicity in rats. The highest total phenolic (76.34 mg Eq AG/g of extract), flavonoïds’ contents (13.32 mg Eq CAT/g of extract), the best DPPH scavenging activity and the ability to reduce ferric ion (Fe3+) were obtained with aqueous extract. The administration of aqueous extract (200 and 400 g/kg bw) and powder (5% and 10 %) during 28 days resulted in a significant decrease (P < 0.05) of the time use to find platform and the increase of the acquisition speed of food in rats. Also the evaluation of some biochemical parameters shows the decrease of malondialdehyde, nitric oxide and acetylcholine esterase activity and the increase of total protein level, catalase activities (CAT), superoxide dismutase (SOD) and glutathione level in plasma, liver and brain compared to the positive control group (PC). The best activities were obtained with groups ARh400 and Rh10%. Treatment with aqueous extract and powder of Rh mesocarp ameliorated neurobehavioral changes by enhancing antioxidant activities, cognitive functions. Therefore Rh would protect oxidative damage and preserve neurone functions.

欧洲脑瘫监测组织(SurveillanceofCerebralPalsyinEurope,SCPE)协作网络的主要目的是建立欧洲脑瘫病例的中央数据库。对脑瘫患病率变化趋势的监控有利于对脑瘫危险因素的研究以及提高脑瘫患儿的生活质量。在脑瘫多中心协作登记工作中,为了准确一致地鉴定脑瘫病例,研究人员采用明确、统一的脑瘫定义。SCPE协作组发现每1000个存活儿中脑瘫的患病例为1.5～3例。在1980至1996年间的出生组群中(n=9128),53.9%的脑瘫患儿为双侧痉挛性脑瘫,31.0%的为单侧痉挛性脑瘫,6.6%为运动障碍,4.1%为共济失调。在脑瘫患儿中,20.4%患儿出生体重不足1500g,而25.5%的孕龄不足32周。29.5%的脑瘫患儿出现了智力障碍(IQ<50)。不能步行(甚至在协助下)的脑瘫患儿比例为30.3%。12.5%的脑瘫患儿出现了严重的视力缺损。做好登记工作有助于了解脑瘫流行病学的特征,是进行脑瘫流行病学研究的一种最佳方式。
