骨痛、发热、胸壁肿块、胸水

患者，女，55岁。因全身多处骨痛、双下肢麻木伴发热、右侧胸背部多发肿块7月余于2 001年2月21日收住院。患者于2000年6月起出现臀部骨痛、双下肢麻木伴抬腿活难，之后逐渐出现背部疼痛并进行性加重，外院骨科拟诊"骨质疏松"给予对症治疗无效。2000 年8月初出现午后低热盗汗，体温最高为37．9℃。8月19日起右侧胸背部出现多处肿块伴疼痛，体积从小核桃大至6 Cm×Cm，质硬，与胸壁固定，与皮肤不粘连，同时双下肢麻木加重伴大小便失禁。当地医院行右胸部肿块穿刺活检见浆细胞型恶性肿瘤细胞。全身骨扫描示全身多发性浓集灶，双肾下垂。X线胸片检查示心包、胸腔积液。未行骨髓细胞学检查，拟诊多发性骨髓瘤（MM），给予环磷酰胺（CTX）十泼尼松（Pred)化疗。肿块均有不同程度的缩小或消失，全身骨痛减轻，大小便失禁消失。10月给予MP（马法兰、泼尼松）方案化疗，肿块反逐渐增大，疼痛加剧。遂转院，再次行肿块穿刺活检见散在幼稚浆细胞。骨髓涂片送上海中山医院会诊为骨髓增生活跃，浆细胞占0．005，疑MM。l1月给予CHOP（环磷酰胺、阿素、长春新碱、泼尼松）方案化疗后肿块又有缩小，疼痛减轻，但麻木加重，考虑与长春新碱（VCR）有关，故之后又行3个疗程的CAP（不含VCR）方案化疗，效果渐差，肿块增大。200年1月底口服美法仑6 mgx7d。因治疗效果差，为进一步明确诊治而来我院。幼年时曾患"伤寒、结核"，1992年患"甲状腺腺瘤"。家族史无殊。体格检查：体温36．5℃，脉率110次／min，呼吸23次／min，血压正常。神志清楚，无明显贫血貌全身皮肤、豁膜无黄染、痪点及疲斑。全身浅表淋巴结无肿大，甲状腺右叶II度肿大，无血管杂音。胸骨无压痛，右侧胸壁及右乳房外下象限见两个肿块，分别为10 cm×12 cm和3cm ×4 Cm，质硬，与胸壁固定，局部皮肤发红。右肺呼吸音低，右侧语颤减弱，叩诊呈浊音。肝、脾肋缘下未触及。双下肢感觉异常，四肢肌力正常，生理反射存在，病理反射未引出。实验室检查：血常规（200l年2月22日）：Hb 102g/WBC 3．42×10~9／L，BPC236 ×10~9/L。尿、粪功能：胆红素（一），A／G＝36／18，AU，12 U／L，BUN 3 ．7mmocr26μmol/L，LDH343 U／L［正常值（1（一），血清AKP 87 U／L（正常值15～l12U／L）。肝炎免疫全套（一）。T_3、T_4、FT_3、FT_4、TSH（一），AFP、CEA（一），CA15-3（-），CA125 73 ．5 u/ml（正常值0～mg/L（正常值0～1．21mg/L），cRP 2．4mg/L）。血清IgG6．80g/L，IgA0．85g/L，IgM蛋白鉴定（一）。2001年2月26日胸水常规检查：红混浊，李凡特试验（＋＋），WBC 5 800 x10~6/L，RBC 49 l10~6/L,见瘤细胞。胸水生化检查：蛋白55g/L，LDH 2 03ADA 19 ．lu，IgH重排（一）。胸水细胞涂片检查：片中有胞密布，巨大异常细胞占0．96，直径l5一100μm，呈类圆部分有伪足，胞浆丰富，呈灰蓝色或深蓝色，部分有核旁淡染区及少量空泡，细胞核大，部分核偏位，有瘤状突起、母子核、分叶等核畸形，双核、三核及丝状分裂可见，核染色质粗，核仁大，1～3个。细胞组织化学染色结果：光镜下POX （一）pas（十），呈弥漫性；ACP呈强阳性。胸水细胞流式细胞仪（FCM）分析（2001年3月2日）：Cq_2、CD_3、CD_7CD_33、CD_68、MPO、CD性率100％，浆细胞抗原阳性率100％，P53阳性率100％阳性率100％，Bcl- 2阳性率87％，异常大细胞EMA阳性。胸水FCM检测（200X）1年3月16日）：细胞分两群，集中区域细占团％，C从阳性率99．26％，C几阳性率97．以％，C几阳性率97．88％，CD7阳性率26．59％，cD19阳性率3．27％，c巩阳性率20．35％，c几阳性率1 ．55％，c巩阳性率0．37％，c几：阳性率55．71％，c巩。阳性率26．19％，'阳性0．34％，入阳性0．46％，519阳性1．37％；分散细胞占40％，表型难以判断，有C玛：，C巩的部分表达，519（一）。骨髓象（姗l年2月26日）：骨髓有核细胞增生活跃，粒：红为2．05：l，淋巴细胞比例及形态正常，全片未见异常细胞；骨髓病理活检（2（"l年2月26日）：骨髓造血成分占20％，结构脂肪存在，见到三系细胞，浆细胞可见。肿块穿刺活检（2001年3月1日）：细胞异型，深染，染色质细，细胞质淡嗜酸性，瘤细胞呈小片状排列。免疫酶标：EMA（＋＋），CRL（一），CEA、LCA、AB一pas（一），提癌可能。x线胸片＋全身扁骨平片（2001年2月2

The influence of solvent and the rate of addition of water on the characteristics of alumina-zirconia powders obtained by sol-gel method were investigated. The al2O3-ZrO2 powders (1:1 molar ratio) were prepared using aluminum tri-sec-butoxide and zirconium n-propoxide as precursors. Ethanol (EtOH), isopropanol (iPrOH) and isobutanol (iBuOH) were used as solvents. The al2O3-ZrO2 powders were characterized by nitrogen physisorption (SBET), Fourier transformed infrared spectroscopy (FT-IR), thermogravimetric analysis (TGA), differential thermal analysis (DTA), X-ray diffraction (XRD) and scanning electron microscopy (SEM). Prepared oxides calcined at 700℃ showed high specific surface area (200 - 240 m2/g). Obtained results suggest that the homogeneity of the mixed oxides is favored by using a water addition rate of 0.06 and 0.10 mL/min with ethanol as solvent.

A laboratory study was conducted in order to explain successive extractions of reserved potassium by using concentrated HCI under reflux for 13 surface and sub-surface soil samples representing three chosen pedons of soil series （472 ECC Bashika, 463 EKC Rabeaa and 461 ECC Talafar） locations of Nineveh Province in Northern of Iraq. The aim is to investigate role of clay minerals on release of reserved potassium from soils. Results showed that dominated clay minerals were （smectite 〉 illite 〉 kaolinite 〉 chlorite） for both 472 ECC and 461 ECC soil series and were （illite 〉 smectite 〉 kaolinite 〉 chlorite） for 463 EKC soil series. also results appeared that exchangeable K＋ released values were 2,483-4,575 mg/kg at 461 ECC and 463 EKC soil series, respectively; non-exchangeable phase ranged from 752-1,390 mg/kg at 461 ECC and 472 ECC soil series, respectively. Soil fertility was evaluated according to its K＋ mica release referring for high K＋ release content with range between 3,324-5,516 mg/kg at 461 ECC and 463 EKC soil series, but rate ofK~ released was very law according to parabolic diffusion me,del with range from 195-359 mg/kg at latter soil series respectively. Results reflect effect of clay minerals on potassium release as amount and rates in arid and semi-arid regions. It can be concluded the study soils have a high amount of potassium content with low rate release of it, in turn these soil need particular management.

The cormsion in air of a two-phase Cu-75Cr alloy was studied at 700-900℃. Thealloy cormded nearly parabolically at 700-800℃, but at 900℃ its instantaneousparabolic mte constant decreased with time. The scales were complex and consistedof an outermost layer of copper oxide generally followed bg a layer of the double ox-ide Cu2 Cr2 O4 sometimes containing particles of unoxidized chromium surmunded bya chromia layer. An innermost layer of chromia was also observed in some *** metallic copper was al8o frequently mixed with chromia particles. No Cr de-pletion was observed in the alloy close to the inteffoce with the scale. In any case,this alloy was not able to form an exclusive continuous protective chromia layer. Thespecial cormsion behavior Of this alloy is typical of two-phase binary systems with alange solubility gaP in which the outwaof dthesion fiux of the most-reactive componentin the alloy is strongly reduced and may be inswncient to fOrm a protective externallayer of the cormsponding oxide. In paTticular, the presence of particles of Cr withina double oxide layer is very unusual and is allowed only for the kinetic reason.

In this study the characterization of an aluminosilicate synthesized from commercial al2(SO4)3 and colloidal SiO2 is presented, as well as its capacity for the removal of copper from aqueous solution. Characterization of the synthesized material was performed using X-ray diffraction, BET nitrogen adsorption-desorption, mass titration and the Boehm method. In order to obtain stable agglomeration and enhance its surface area (165 - 243 m2/g) and solid adsorbing capabilities, the molar ratio SiO2:al2O3 (1:3, 1:1 and 3:1) was studied, the solubility of the preparation material, synthesis-procedure time and solution pH function were also examined. The maximum capacity to remove copper ions from an aqueous solution by synthesized aluminosilicate was 16 mg/g at pH 4 and 25℃. The Langmuir model fitted better to the copper adsorption experimental data.

The so-calledPhiladelphia(Ph) chromosome is present in more than 90% of chronic myeloid leukemia (CML) cases. It results in juxtaposition of the 5' part of the BCR gene on chromosome 22 and the 3' part of the ABL1 gene on chromosome 9. An additional acquired monosomy 7 or deletion of 7q is associated with poor prognosis in a variety of myeloid disorders. Here we report a novel Ph chromosome positive CML case with a ring chromosome 7 [r(7)]. Immunophenotyping was compatible with CML, although 4.5% of total leucocytes appeared like acute myelogeneous leukemia (AML) subtype M2. The r(7) was characterized in detail by array-proven multicolor banding (aMCB), the latter being of enormous significance to characterize breakpoint regions in detail. Underlying mechanisms and prognostic are discussed, as ring chromosomes are rare cytogenetic abnormalities in hematopoietic malignancies.

Objective: To investigate the age at menarche, pubarche and telarche in a representative sample of residents of an urban zone of Leon, Guanajuato, Central Mexico and to evaluate the secular change of the age at menarche between 1985 and 2000. Methods: Using status quo and retrospective methods, menarcheal, pubarcheal and telarcheal status of girls 8 - 17 years of age (n = 1093) were determined. Multiple regression analysis and logistic regression were used to determine factors that were related to or predictive of age at menarche. Results: Median ages for menarche, pubarche and telarche in our studied sample were: 11.9 ± 0.04;11.13 ± 1.25 and 10.84 ± 1.34 years, respectively. A direct correlation was found between the age of menarche of adolescents and that of their mother (p p p = 0.001), but an indirect correlation was found with the girls’ hip circumference (p = 0.004). We demonstrated a clear secular tendency towards an earlier onset of menarche by comparing our results (11.9 ± 0.04) with data from 1985 (12.8 ± 1.3) for adolescents from the same city. Conclusion: We demonstrated a clear reduction in age at menarche of approximately 0.75 year/decade, higher than Tanner’s finding in 1962, but similar than that of Malina et al. in 2004.

隔震技术通过限制传递到上部结构的地震能量来保护结构免受强震破坏。针对高层建筑中有LRB隔振装置的结构抗震性能已有部分研究者开展了少量研究。然而,对基础隔震的有效性进行探讨,以此保障并有效地提升高层建筑的抗震性能仍需进一步探索。根据IBC（2012）和ACI 318（2014）的规定,选择了一栋12层钢筋混凝土框架住宅,并考虑其位于软弱土（D类场地）的高烈度区,采用振型分解反应谱法设计,框架结构满足设计地震（DBE）强度水平下的设计限值。采用铅芯橡胶支座（LRB）隔震系统,分析了该隔震系统的力学性能、选取了支座尺寸参数。利用计算机软件SAP2000对基础隔震和基础固定RC框架在四次地震事件中、不同强度下的地震响应,进行了非线性时程分析,每一个事件都有三个记录,分别是近断层脉冲（NFP）、近断层无脉冲（NFNP）和远断层记录（FF）。记录按比例缩放以匹配设计地震水平（del）。上部结构的非弹性行为通过在框架构件端部分配塑性铰来模拟,而铅橡胶支座的行为则模拟为双线性滞回单元,并在SAP2000中定义为连杆单元。建立了相应的数值模型进行了线性时程分析,研究了土-结构相互作用对基础隔震框架和固定框架性能的影响。考虑LRB隔振器被建模为非弹性单元,而土壤和上部结构被假定为弹性元件。将软土作为平面应变单元,采用二维平面有限元进行了考虑和模拟。将边界条件定义为代表远场的人工粘弹性边界,采用弹性半空间直接分析法进行了分析。从屋顶加速度峰值、层间位移峰值、基底剪力、最大基底位移和产生的塑性铰数量等方面,详细比较了隔震结构与固定基础结构的抗震性能。非线性时程分析结果表明,与固定基础相比,无脉冲近场和远场时框架结构响应明显减小,且相比有脉冲近断层时框架结构响应明显小很多。同时,近断层脉冲地震动作用下的隔震框架的最大位移比远断层地震动作用下的最大位移更大,而考虑SSI效应的线性时程分析结果表明,隔震框架和固定框架的结构响应显著增加,且对地震反应的影响较小,即SSI降低了LRB的隔离效率。
