A Competing Risk Hazard Model for Complications of Diabetes Mellitus

Diabetes mellitus is a major public health problem globally. It is the cause of serious and sometimes life-threatening complications leading to death. It is one of the most common non communicable diseases and leading cause of death in most developed countries including Bangladesh. It is associated with the increased risk of microvascular (nephropathy and retinopathy), macrovascular (cardiovascular diseases, stroke) and other mixed (cataract, pulmonary tuberculosis, foot ulcer etc.) complications. The aim of this study was to determine the risk factors related to diabetes complications and to propose competing risk models for analyzing complications of diabetes mellitus. We study 2887 diabetic patients from the BIRDEM registry record books for the period from 1984 to 1997 who have at least two follow-up visits and who are free from complications at the first visit. The data on variables such as 2-hBG, age, sex, family history of diabetes, area of residence, educational level, BMI, SBP, DBP and treatment status are obtained. Also, the time from the diagnosis of type 2 diabetes mellitus (T2DM) to the occurrence of complications was recorded. We use the competing risk hazard model and examined the potential risk factors for determining the development of such complications. This study reveals that increase in blood pressure is a potential risk factor for CHD and nephropathy in T2DM. We also found that male and illiterate patients are more affected by nephropathy. In our study, we also revealed that female and illiterate patients are also more influenced by cataract.

Backgrounds and Aims: Several epidemiological investigations revealed that the prevalence of Type 2 Diabetes Mellitus (T2DM) has been increasing in the adult population of Bangladesh. But the prevalence of diabetes among the children and adolescents in Bangladesh has not been reported. This study addressed the prevalence of diabetes among the younger people in Bangladesh. Subjects and Methods: We investigated school children of age group 10 - 18 years in rural, suburban and urban communities. Investigations included socio-demographic information, height, weight, mid-upper-arm circumference (MUAC), blood pressure (SBP, DBP) and fasting plasma glucose (FPG). We used WHO criteria (1999) for impaired fasting glucose (IFG) and diabetes mellitus (T2DM). Results: A total of 2152 students (boys/girls: 1064/1088) volunteered the study. Their mean (SD) age was 13.3 (2.0) y, BMI was 18.5 (3.1) and MUAC was 21.2 (3.4) cm. The mean (SD) of FPG was 4.6 (0.87) mmol/l. The prevalence of IFG (95% CI) was 3.4% (2.63 - 4.17) and T2DM was 1.8% (1.23 - 2.37). BMI showed no association with FPG in either sex. In assessing risk for hyperglycemia (FPG > = 5.6mmol/l), logistic regression showed [odds ratio (OR) with 95% CI] that compared with lower age (16 y) higher age had excess risk (OR 5.2, 2.92 - 9.23). Compared with the rural the urban children had higher risk (OR 14.7, 6.41 - 33.78). Higher family income was also found to have higher risk (BDT 8000: OR 2.03, 1.30 - 3.18);whereas, higher BMI and MUAC were proved to be not significant. Conclusions: The prevalence of IFG and T2DM in Bangladeshi children and adolescents appears to be high. The urban children from the higher family income are the most vulnerable for developing diabetes, obesity and hypertension. A prospective cohort including other variables like physical activities, waist-girth, insulin or macro-/micronutrients may explain which risk factors are significant for developing diabetes among children and adolescents in Bangla

Diabetes and Hypertension in a Santhal Tribe in Bangladesh: A Population Based Study

BACKGROUND: Santhal (Santals) tribe is one of the oldest and largest aboriginal pre Aryan populations in India and Bangladesh. There was no published report on the prevalence of diabetes and hypertension. OBJECTIVE: To determine the prevalence of type 2 diabetes mellitus (T2DM) and hypertension in a Santhal tribe of Bangladesh. RESEARCH DESIGN AND METHODS: Eight villages inhabited largely by Santhal tribe were purposively selected. All Santhals aged 20 years or more were considered eligible and enlisted for the study. Investigations included socio-demographic information (age, sex, education, income), clinical history (general illness), anthropometry (height, weight, waist-girth, hip-girth) and blood pressure. Body mass index (BMI) and waist-to-hip ration (WHR) were calculated. Blood samples were collected for fasting plasma glucose (FPG), total cholesterol (Chol), triglycerides (TG), urea and creatinine. RESULTS: Thirteen hundred eligible Santhals were enlisted. Of them, 1049 (80.7%) participated in the study. The male and female participants were 40% and 60%, respectively. The prevalence of T2DM was 0.6% and hyperglycemia (FPG > 5.5 mmol/l) was 10.0%. The prevalence of systolic hypertension (sHTN) was 24.4% and diastolic hypertension (dHTN) was 24.6%. Compared with the males the females had significantly higher prevalence of sHTN (OR, 2.20 with 95% CI, 1.62 - 3.02) and dHTN (OR, 1.81 with CI, 1.34 - 2.0);whereas, the prevalence of T2DM and IFG did not differ. Regarding obesity 45% of the participants had BMI 23.1. Logistic regression estimated that the increasing age, female sex, higher FPG (>5.5 mmol/l) and higher Chol (>160 mg/dl) had independent risk for sHTN and dHTN. CONCLUSIONS: The Santhals had less risk for diabetes but increased risk for hypertension. The Santhal females had excess risk of hypertension. Advancing age, female sex, hyperglycemia and hypercholesterolemia were found to have significant risk for hypertension. Obesity had no effect on diabetes

Comparison of Individual and Synergistic Antimicrobial Activity of Common Spices against Certain Infectious Pathogen in Bangladesh

Aims: The aim of this undertaken investigation was designed to determine the comparative antimicrobial potential of ethanol extract of six commonly consumed spices such as Garlic (Allium satilyvum), Ginger (Zingiber officinale), Turmeric (Curcuma longa), Cinnamon (Cinnamomum zeylanicum), Cumin (Cuminum cyminum) and Black cumin (Nigella sativa). Method: This study includes, the efficacy of individual and synergistic effect of these extracts that was tested against bacteria by agar well-diffusion method employing 100 μL spices-extract solution per well and was conducted in (Centre of Excellence Laboratory) Department of Microbiology, Primeasia University during November 2018 to April 2019. Minimum inhibitory concentration (MIC) was determined by the micro-broth dilution method and compared with commercial antibiotic discs such as Amoxicillin, Vancomycin, Erythromycin, Ceftriaxone, Chloramphenicol, and Ciprofloxacin. Result: According to the findings of the antibacterial assay, the ethanol extracts of the spices showed inhibitory activity against common infectious bacterial pathogens. Spice extracts have the most significant activity against B. cereus and E. coli was the least sensitive among the tested organisms. The ethanol extract had individual antibacterial activity with mean zone of inhibition 22 ± 0.5 and 20.08 ± 0.58 mm and the synergistic effect of ethanol extract had a mean zone of inhibition 30 ± 0.75 and 28.25 ± 0.9 mm against B. cereus and V. cholera, respectively, which is highly comparable to the commercial antibiotic, Ciprofloxacin (25 mm). Conclusion: The ethanol extract of indigenous spices was shown to be highly potential to be applied as an alternative of commercial drugs.

Deep Neural Network Based Spam Email Classification Using Attention Mechanisms

Spam emails pose a threat to individuals. The proliferation of spam emails daily has rendered traditional machine learning and deep learning methods for screening them ineffective and inefficient. In our research, we employ deep neural networks like RNN, LSTM, and GRU, incorporating attention mechanisms such as Bahdanua, scaled dot product (SDP), and Luong scaled dot product self-attention for spam email filtering. We evaluate our approach on various datasets, including Trec spam, Enron spam emails, SMS spam collections, and the Ling spam dataset, which constitutes a substantial custom dataset. All these datasets are publicly available. For the Enron dataset, we attain an accuracy of 99.97% using LSTM with SDP self-attention. Our custom dataset exhibits the highest accuracy of 99.01% when employing GRU with SDP self-attention. The SMS spam collection dataset yields a peak accuracy of 99.61% with LSTM and SDP attention. Using the GRU (Gated Recurrent Unit) alongside Luong and SDP (Structured Self-Attention) attention mechanisms, the peak accuracy of 99.89% in the Ling spam dataset. For the Trec spam dataset, the most accurate results are achieved using Luong attention LSTM, with an accuracy rate of 99.01%. Our performance analyses consistently indicate that employing the scaled dot product attention mechanism in conjunction with gated recurrent neural networks (GRU) delivers the most effective results. In summary, our research underscores the efficacy of employing advanced deep learning techniques and attention mechanisms for spam email filtering, with remarkable accuracy across multiple datasets. This approach presents a promising solution to the ever-growing problem of spam emails.

Image Noise Analysis of a Large Ring PET Scanner

Image noise analysis of a large ring PET scanner “macro PET” performed using two different phantoms, namely a Jaszczak SPECT phantom and a uniform cylindrical phantom. In the present work, simple 2D filtered back projection was used to reconstruct all the images, and in almost all the cases a Hamming filter of cutoff frequency 0.4 and a 256 by 256 matrix with zoom factors from 1 to 4 were used in order to investigate the imaging capabilities of the new scanner and the influence of filter and cut-off frequency on the filtered back projected images. Results indicate that 11.1 mm cold rod in the Jaszczak phantom images can consistently be seen. The Coefficient of variation (CV) results for Hann and Hamming filters are very similar and increase approximately in linear fashion with higher cutoff frequency. The value of CV for the Parsen filter is lower than the value for Hann and Hamming filters. It concludes that all filters with low cut off-frequency (0.6) would suppress image noise but decrease contrast.

Hypoglycemic Effect Study of a Combination of Some Stipulated Spices in Alloxan Induced Diabetic Wistar Albino Rats along with Nutritional Value Evaluation

The main purpose of the study was to reveal the hypoglycemic effect of a combination of some selected spices along with the evaluation of nutritional values. A sum of fourteen different types of spices at different percentages were taken into consideration to propel the research activities forward, viz;ginger, garlic, onion, red chili, turmeric, cumin, cinnamon, clove, coriander, fenugreek, black pepper, nigella, carom seed and cardamom. The healthy experimental wistar albino rats were made diabetic by injecting alloxan monohydrate intraperitoneally a dose of 150 mg/kg body weight. The rats of Group-D and Group-E were treated with the sample, mixed spices at a dose of 100 mg/kg and 200 mg/kg body weight respectively with the lab diet while in case of Group-C, glibenclamide at a dose of 0.5 mg/kg body weight was introduced with the lab diet for a total period of 21 days where as for the same days span, Group-A and Group-B were continued feeding with only lab diet. Data suggest that the mixed spices of both doses applied in Group-D and Group-E had exerted effects significantly (p 0.05) in lowering the blood glucose level. On 22nd day, in Group-D and Group-E, the FBG level was found to be reduced by around 40.66% and 41.18% respectively compared to their respective initial days where in Group-C, the reduction was around 51.90% in comparison to its initial day. Again, in case of 2hPG level measurement, on 22nd day, both the groups, Group-D and Group-E were found with the decreased level of around 19.38% and 39.11% respectively compared to their respective initial days while the Group-C was shown with the reduced of 44.26% in comparison adopting same manner. Furthermore, various analyses confirm the presence of several micro and macro nutrients and high amount of dietary fiber in the spices mix. Thus the result affirms that this combined spice possesses strong hypoglycemic properties.

Study of Image Contrast Recovery Coefficient (CRC) of a Large Ring PET Scanner

Image contrast recovery coefficient (CRC) of a large ring PET scanner “macroPET” was studied with septa and without septa configuration by ac-quiring data from a laboratory made 35.7 cm square phantom filled with 18-F. Images were reconstructed with simple 2D filtered back projection using Hann, Hamming and Parsen filters with different cut-off frequencies aiming to investigate the influence of filter and cut-off frequency on image contrast with septa and without septa mode. Results indicate that the CRC, for both hot and cold lesions, is excellent for diameters ≥ 3 cm using cut-off frequencies > 0.4. For a 2 cm hot lesion CRC is around 0.8 to 0.9. CRC for 1 cm hot and cold lesions is ~0.3, as expected. There is surprisingly little difference between results with and without septa. For hot lesions, septa appear to improve CRC slightly, but for cold lesions CRC is slightly poorer using septa.
