The purpose of this study was to investigate whether obese rats with a strong anorectic response to growth hormone also showed signs of increased hepatic ketogenesis as reflected in circulating β-hydroxybutyrate levels. Rats with diet-induced obesity were allocated to one of two groups, receiving either vehicle (n = 7) or 4 mg/kg/d of growth hormone (n = 13) for 4 days. This latter group was later split into a group of responders (n = 8) showing a cumulated reduction of food intake of more than 4 g from base line during the last two days of administrations and a group of non-responders (n = 5). The cumulated reduction of food intake from baseline among the responders was 10.8 ±1.5 g. The corresponding marginal reductions in the non-responder and vehicle groups were 0.5 ±3.4 gand 0.5 ±3.7 g, respectively. Growth hormone administration generally increased serum levels of β-hydroxybutyrate and free fatty acids, compared with vehicle, whereas triglycerides were decreased. Among the responders this effect was statistically significant in all instances whereas the same trend was weaker among non-responders. The main finding of the present study was that the serum β-hydroxybutyrate levels of 0.76 ± 0.11 mmol/l among responders was three times higher than non-responders

In the present study, the selection of an etchant for a particular crystal was purely made on empirical basis. The gel grown barium oxalate crystals were etched by HCl, HNO3, BaCl2, NH4Cl, and NH4Cl-HCl solutions. Micro-topographical studies have been made and it was found that elongated triangular etch pits and pits within pits were formed. Rectangular growth layers in the form of staircase and leaf like dendrite pattern were seen. Kinetics of etching was studied. Quantitative estimation of dissolved crystals in etchants was used for the determination of activation energy of reaction and pre-exponential factor with the help of Arrhenius equation.

Background: Diaphragmatic agenesis or complete absence of a hemidiaphragm in adulthood is rare. The significance of presence of a lung cyst occupying the common thoracoabdominal space lies in the absence of associated diaphragmatic hernia which has not been documented previously. Case Presentation: We report a case of previously undiagnosed complete absence of a hemidiaphragm in a 62-year-old male who presented with recurrent respiratory infection, episodes of nausea and postprandial fullness. Computed Tomography scan showed a large cyst in relation to the lower lobe of left lung. During surgery, he was found to have complete absence of a left hemidiaphragm and a large cyst arising from the lower lobe of left lung occupied the common thoracoabdominal space. Abdominal viscera were found displaced downwards and fixed in a thick fibrous membrane. There was no herniation of abdominal organs into the thoracic cavity after excision of the cyst. The left lung could be fully expanded and chest was closed after putting a chest drain. Diaphragmatic agenesis did not require any additional procedure. The cyst was excised with closure of bronchocystic opening. The patient had an uneventful recovery. Conclusions: The association of lung cyst with diaphragmatic agenesis (presented in the adulthood) is rare. It has precluded a diaphragmatic hernia and survived the patient acting as a protective barrier. The significance of lung cyst in presence of diaphragmatic agenesis lies in the absence of associated diaphragmatic hernia which has not been documented previously.

A Rare Case of Aortico-Right Ventricular Tunnel Along with Ventricular Septal Defect Presented as Ruptured Sinus of Valsalva in an Adult Female

Background: Aortico-right ventricular tunnel is an extremely rare congenital defect rarely described in an infant. This diagnosis is likely to be missed due to its rare entity and similar clinical presentations with other aortico-right ventricular communications like ruptured sinus of valsalva. Methods: We report a case of previously undiagnosed aortico-right ventricular tunnel along with a perimembraneous ventricular septal defect in an 18-year-old female. She had history of exertional dyspnoea, palpitation, history of recurrent lower respiratory tract infection. She was diagnosed as a case of ruptured sinus of valsalva (RSOV) elsewhere. She had “to and fro” murmur, features of congestive cardiac failure. Her echo diagnosis was RSOV. On surgical exploration, after opening the aorta, we found a tunnel like opening in the aorta leading to the roof of RV cavity in between right and non coronary sinuses at the commissural level. Cusps were splayed wide part. Ventricular septal defect was conspicuous from right atrial approach. Post operative CT angio was done. Results: Venricular septal defect was closed from the right atrial approach and aortico-right ventricular tunnel was repaired through aortic and right venricular approach. Postoperative CT angio also confirmed the location and closure of the defects. Postoperative recovery was uneventful. Conclusions: Aortico-right ventricular tunnel in an adult female has not been reported in the literature previously. This rare entity should be considered in the differential diagnosis of a critically ill patient with a “to and fro” murmur, and signs of right heart failure.

Association of procedure length on outcomes and adverse events of endoscopic retrograde cholangiopancreatography

Objective:The aims of this study were to determine the effects of length of procedure on endoscopic retrograde cholangiopancreatography(ERCP)outcomes and adverse ***:All ERCP procedures,performed by experienced advanced endoscopists,in patients without prior papillary intervention from 2006 to 2008 were *** were arbitrarily divided into two groups:shorter procedures(SP),with a duration shorter than the overall mean procedure length,and longer procedures(LP),with a duration longer than overall mean procedure *** of procedure was defined as the time from endoscope insertion to endoscope ***:Two hundred and ninety-five procedures were included in the *** procedure length was 45.630.1 *** hundred and seventy-seven procedures(60%)were SP and 118(40%)were *** were no differences between the groups with regard to patients’ages,genders,race,or trainee *** cases were more likely to be biliary vs pancreatic or bi-ductal evaluations(P=0.03).LP had significantly higher complexity scores(34%with>3 vs 13%;P=0.046)and were more likely to require pre-cut papillotomy(39%vs 15%;P<0.001).There was no significant difference between the groups in overall completion rates(91.5%LP vs 96%SP;P=0.10)or adverse events(10.2%LP vs 6.2%SP;P=0.21).However,LP cases were associated with higher rates of post-ERCP bleeding(4.2%vs 0.6%;P=0.029).Conclusion:There was no significant difference in outcomes or overall adverse events between shorter and longer ERCP ***,longer procedures were associated with higher procedure complexity,higher utilization of pre-cut technique,and increased risk of bleeding.

Pyro and Kinetic Studies of Barium Oxalate Crystals Grown in Agar Gel

Single crystals of barium oxalate have been grown using gel method at ambient temperature. Thermal characteristics and kinetic parameters of barium oxalate crystals were determined by thermo-gravimetric (TG) analysis under non-isothermal heating conditions. The pyrolysis experiments were performed with increasing temperature up-to 600℃ at heating rate of 5℃, 7℃ and 10℃ in nitrogen gas atmosphere. The pyrolysis curve showed that loss of mass took place mainly in the range of 220℃ - 400℃. At higher temperature there was a significant mass loss due to decomposition of oxalates. Ozawa and Coats & Redfern methods were used to determine the apparent activation energies of material degradation. The apparent activation energies for barium oxalate crystals were obtained 187.42 KJ·mol-1 and 185.4 KJ·mol-1 for the respective methods.
