A family nurse-led intervention for reducing health services’utilization in individuals with chronic diseases:The ADVICE pilot study

Objectives:Intensive health services'utilization is common in older individuals affected by chronic *** study assessed whether a structured family nurse-led educational intervention would be effective in reducing health services'use(readmissions and/or emergency service access)among older people affected by chronic ***:This is a non-randomized before-after pilot study.A sample of 78 patients was recruited from two general practices in Italy and 70 among them were followed for 8 *** home care was provided during the first four months'period(months 1-4),followed by the educational intervention until the end of the study(months 5-8).The intervention,based on the teach-back method,consisted of by-weekly 60-min home sessions targeting aspects of the disease and its treatment,potential complications,medication adherence,and health *** of health services'use were collected immediately before(T0),and after the interventions(T1).Differences in utilization rates were examined by the Mcnemar's *** factors associated with the risk of health services'use were explored with a Cox proportional hazard regression ***:The sample(n=78)was predominantly female(n=50,64.1%),and had a mean age of 76.2(SD=4.8)*** mellitus was the most frequent disease(n=27,34.6%).Mcnemar's test indicated a significant reduction in health services'use at T1(Mcnemar χ^(2)==28.03,Pns indicated that time and patient education,as well as their interaction,were the only variables positively associated with the probability of health services'***:A teach-back intervention led by a family nurse practitioner has the potential to reduce health services'use in older patients with chronic diseases.

Estimated Binding Energies of Drug-Like and nondrug-Like Molecules in the Active Site of HIV-1 Integrase, ***, and Two Mutant Models: Y143R and n155H

Lipinski’s “Rule of Five” was introduced for predicting oral bioavailability to describe drug-like molecules. For the purpose of this research the rules were used to separate potential inhibitors of HIV-1 integrase (***) into two groups: drug-like and nondrug-like. If one of Lipinski’s “Rule of Five” was not followed the potential inhibitor was classified as nondrug-like. Thirty molecules were identified from the literature, twenty-four drug-like and six nondrug-like, that were docked into the active site of *** (considered the non-mutated protein) and two mutant models, Y143R and n155H. These are two of the mutations that have led to increased resistance to HIV-1 integrase drugs such as raltegravir and elvitegravir. The computational software, ICM-Pro (Molsoft L.L.C.), was used to determine the estimated binding energy (EBE) of the drug/protein complex. It was found that the nondrug-like molecules generally had a more negative EBE, that is, tighter binding with 1BIS. pdb, though there were several exceptions in the drug-like group. With the protein mutant model Y143R, the majority of drug-like (58%) and nondrug-like molecules (67%) had tighter binding. However, for the mutant model n155H, there was the same percent (46%) of drug-like molecules with tighter binding with the mutant model as with ***. The drug-like molecules were used when there was a ≥1 kcal/mole difference between *** and either of the two mutant models to suggest a pharmacophore with structural characteristics for an HIV-1 integrase inhibitor.

The aim of the present study is to determine the effects of fish meal substitution by plant protein(soybean meal,wheat gluten and corn gluten) in turbot Psetta maxima juveniles’ diet in terms of growth,nitrogen budget and gut *** isoproteic and isolipidic diets(protein: 53%,lipid:22%) were manufactured using extrusion technology and common feed ingredients. Diets were formulated containing 55(FM55),45(FM45),35(FM35) and 25%(FM25) fishmeal (FM).480 juveniles(initial body weight:23-24g) were equally distributed into twelve 450 liters tanks connected with a recirculation system(temperature 18℃±1,photoperiod 12hrs of light, dissolved oxygen above 6.5 ppm).Animals have been hand-fed twice a day to apparent satiation during the week and once on *** were tested in triplicate for 77 *** the beginning and at the end of the trial,fish were weighed to determine performances and pools of animals were taken for carcass proximate composition analysis,nutrient retention calculation and histopathological gut *** the end of the growth trial,ammonia excretion has been also measured during a 24 ***-way AnOVA with newmann - Keuls’ post test were used to analyze data(P≤0.05).no differences in terms of palatability were observed and feed intake was similar among groups(from 1.14 to 1.19%day).At the end of the experiment,final weight ranged from 126.6(FM55) to 99.5 g(FM45).A significant decreasing of specific growth rate(SGR) was observed from fish fed diet FM55(2.14%day) to those fed diets FM35 and FM25(2.01 and 1.82%day,respectively) together with an increasing of feed conversion rate (FCR)(0.65 vs.0.70-074).Fish fed diet FM45 showed intermediate values for both indices(SGR: 2.09%day;FCR:0.67).no statistical differences were found in terms of PER or GPE between fish fed diet FM55 and *** fed diet FM55 presented a lower ammonia excretion in comparison with those fed diet FM 25(2.61 vs.3.41 g n-nH 100g protein intake).Experimental diets

Background: Many studies have underlined as caregiving for people with Alzheimer’s disease (AD) is highly stressful and has significant negative consequences. Objectives: The study of the structure of personality, can help to understand the association between depression, intrapsychic and interpersonal processes of caregivers of Alzheimer’s disease patients and what kind of intervention can be planned to favor the stress burden management. Methods: Case group: Caregivers (CG) (n = 75);control group, Subjects not Caregivers (nCG) (n. 104). Tests: SASB questionnaire (Structural Analysis of Interpersonal Behavior) describing intrapsychic and interpersonal processes of the structure of personality validated on the basis of DSMIV;CDQ questionnaires—depression. Results: Intrapsychic level: From the results it emerged that CGs had lower autonomy in their choices, and lower acceptance of their own feelings, and exercised greater self-control exhausting themselves toward predetermined goals, and more depression compared to the control group. They may be not able to achieve psychic equilibrium in the presence of stress: they may likely become disoriented and engage in behaviors that may be self-defeating. The knowledge of these modalities could allow to plan a psychotherapeutic and multidisciplinary intervention aimed at facing and overcoming the psychological distress of the caregiver.

The north Coastal Region of the State of S?o Paulo, which comprises the Municipalities of Caraguatatuba, S?o Sebasti?o, Ilhabela and Ubatuba, is one of the most prone to flooding and debris flow deposition Brazilian areas, owing to hydrological extreme rainfall events usually coupled with extreme tidal levels. This risk is also high due to human lives and material assets, with increasing population rates and the establishment of large companies such as the Oil industry, with reduced defense/prevention measures and *** catastrophic scenario of the city of Caraguatatuba, in March 1967, resulting from one of the most serious natural disasters in Brazil, fosters discussions about probabilities of heavy rainfall-caused events and rise in the sea level in coastal areas. Hence, this research is a consequence of this reality. The research is founded on an innovative methodology based on the analysis of past data of rainfall and tidal stations, complemented with debris flow registers in the region of the north coastal zone of the State of S?o Paulo (Brazil). The anaysis developed involved the meteorological, hydraulic, geotechnical and statistical knowledge *** results are intended to be used for urban planning, designs of macro-drainage, fluvial, maritime projects and debris flow retention structures. These practical applications will then associate the probability of occurrence of certain types of heavy rainfall-caused events such as flooding or debris flow coupled with a corresponding increase in tidal levels.

Multiple Detector-Row CT in Gastric Cancer Staging: Prospective Study

The aim of this study was to evaluate the accuracy of multiple detector computed tomography (MDCT) in the preoperative staging of gastric cancer, prospectively comparing CT findings with pathological findings at surgery, in a single-center study. A total of 19 consecutive patients with primary cancer recruited between March and July 2014 were submitted to preoperative MDCT staging according to a standard protocol. All diagnostic procedures were performed by dedicated radiologists who were unaware of the final pathological results. Subsequently, 16 patients underwent surgical treatment and 15 were finally included in the study. The primary tumor was detected at CT in all 15 cases. CT results for T staging were in agreement with pathological findings in 12 of 15 cases, with overall accuracy of 80%. Stage-specific sensibility was high for advanced stages (sensibility for T1, T3, and T4 resulted 60%, 85.7%, and 100%, respectively), while earlier stages showed higher specificity (specificity for T1, T3, and T4 resulted 100%, 75%, and 91.7%, respectively). Overall n staging accuracy was 86.7%, with 13 of 15 patients correctly staged. Stage-specific sensibility was 75% for n0 and 100% for n3, while specificity was 100% for n0 and lower for advanced stages. Accuracy for peritoneal involvement was 100%. Our findings show a good performance of the diagnostic protocol performed with MDCT tested in this study.

Thiopurine metabolites variations during co-treatment with aminosalicylates for inflammatory bowel disease:Effect of n-acetyl transferase polymorphisms

AIM: To evaluate variation of the concentration of thiopurine metabolites after 5-aminosalicylate(5-ASA) interruption and the role of genetic polymorphisms of n-acetyl transferase(nAT) 1 and 2. METHODS: Concentrations of thioguanine nucleotides(TGn) and methymercaptopurine nucleotides(MMPn), metabolites of thiopurines, were measured by high performance liquid chromatography in 12 young patients(3 females and 9 males, median age 16 years) with inflammatory bowel disease(6 Crohn's disease and 6 ulcerative colitis) treated with thiopurines(7 mercaptopurine and 5 azathioprine) and 5-ASA. Blood samples were collected one month before and one month after the interruption of 5-ASA. DnA was extracted and genotyping of nAT1, nAT2, inosine triphosphate pyrophosphatase(ITPA) and thiopurine methyl transferase(TPMT) genes was performed using PCR assays. RESULTS: Median TGn concentration before 5-ASA interruption was 270 pmol/8 x 108 erythrocytes(range: 145-750); after the interruption of the aminosalicylate, a 35% reduction in TGn mean concentrations(absolutemean reduction 109 pmol/8 × 108 erythrocytes) was observed(median 221 pmol/8 × 108 erythrocytes, range: 96-427, P value linear mixed effects model 0.0011). Demographic and clinical covariates were not related to thiopurine metabolites concentrations. All patients were wild-type for the most relevant ITPA and TPMT variants. For nAT1 genotyping, 7 subjects presented an allele combination corresponding to fast enzymatic activity and 5 to slow activity. nAT1 genotypes corresponding to fast enzymatic activity were associated with reduced TGn concentration(P value linear mixed effects model 0.033), putatively because of increased 5-ASA inactivation and consequent reduced inhibition of thiopurine metabolism. The effect of nAT1 status on TGn seems to be persistent even after one month since the interruption of the aminosalicylate. no effect of nAT1 genotypes was shown on MMPn concentrations. nAT2 genotyping revealed that 6 patients

目的　比较18F 脱氧葡萄糖 (FDG)和18F 酪氨酸 (FET)PET显像对垂体腺瘤的诊断价值。方法　正常对照者 7例同期行18F FDGPET脑显像和13 n nH3 PET脑血流显像。 2 0例垂体腺瘤患者行18F FDGPET脑显像 ,其中 10例同期行18F FETPET脑显像 ,并参照近期MRI结果进行分析。 2 0例患者均行经口鼻蝶窦垂体腺瘤切除术 ,术后行病理检查及免疫组织化学分型。结果　①正常对照组垂体18F FDG摄取明显低于周围组织。②对于分泌激素和无分泌功能的垂体腺瘤 ,18F FDGPET和18F FETPET均可显示病灶。③垂体腺瘤在18F FDG显像中多表现为均匀的放射性摄取增高 ,而18F FET显像则多表现为散在的摄取增高 ,与MRI所示等T1长T2 或稍长T1长T2 信号的肿瘤组织分布相似。④ 7例微腺瘤 18F FDGPET检出 5例 ,MRI检出 3例。⑤不同激素分泌型和无分泌功能垂体腺瘤的瘤体大小、激素水平与18F FDG或18F FET显像标准摄取值 (SUV)间无明显相关。结论　18F FDG和18F FETPET脑显像均可显示垂体腺瘤。

Role of endoscopic ultrasonography in the loco-regional staging of patients with rectal cancer

The prognosis of rectal cancer(RC) is strictly related to both T and n stage of the disease at the time of diagnosis. RC staging is crucial for choosing the best multimodal therapy: patients with high risk locally advanced RC(LARC) undergo surgery after neoadjuvant chemotherapy and radiotherapy(nAT); those with low risk LARC are operated on after a preoperative short-course radiation therapy; finally, surgery alone is recommended only for early RC. Several imaging methods are used for staging patients with RC: computerized tomography, magnetic resonance imaging, positron emission tomography, and endoscopic ultrasound(EUS). EUS is highly accurate for the loco-regional staging of RC, since it is capable to evaluate precisely the mural infiltration of the tumor(T), especially in early RC. On the other hand, EUS is less accurate in restaging RC after nAT and before surgery. Finally, EUS is indicated for follow-up of patients operated on for RC, where there is a need for the surveillance of the anastomosis. The aim of this review is to highlight the impact of EUS on the management of patients with RC, evaluating its role in both preoperative staging and follow-up of patients after surgery.
