We.re.ort a case.of a 47-ye.r-old Caucasian male.affe.te. by Von Re.klinghause.＇s dise.se.who de.e.ope. a malignant somatostatinoma of the.papilla major and minor associate. with je.unal gastrointe.tinal stromal tumour with unce.tain be.aviour. At laparotomy, multiple.he.atic me.astase. we.e.e.ide.t. Whipple.pancre.ticoduode.e.tomy, je.unal re.e.tion, e.te.sive.lymphade.e.tomy and multiple.he.atic we.ge.re.e.tions we.e.pe.forme.. The.patie.t was alive.without re.urre.ce.afte. 24 mo. This is the.fourth case.re.orte. in the.world lite.ature.of a patie.t with Von Re.klinghause.＇s dise.se.associate. with pe.iampuUary somatostatinomas and je.unal stromal tumor. BACKGROUND： The.use.of laparoscopic distal pancre.te.tomy（LDP） incre.se. in the.past twe.ty ye.rs but the.re.l diffusion of this te.hnique.is still unknown as we.l as the.type.of ce.te.s（high or low volume. in which this proce.ure.is more.fre.ue.tly *** SOURCe. A syste.atic re.ie. was pe.forme. to e.aluate.the.fre.ue.cy of LDP in Italy and to compare.indications and re.ults in high volume.ce.te.s（HVCs） and in low volume.ce.te.s（LVCs）.Re.ULTS： From 95 pote.tially re.e.ant citations ide.tifie., only5 studie. we.e.include.. A total of 125 subje.ts we.e.analyze., of whom 95（76.0%） we.e.from HVCs and 30（24.0%） from *** me.n numbe. of LDPs pe.forme. pe. ye.r was 6.5. The.me.n numbe. of patie.ts who unde.we.t LDP pe. ye.r was 8.8in HVCs and 3.0 in LVCs（P0.001）. The.most fre.ue.t le.ions ope.ate. on in HVCs we.e.cystic tumors（62.1%, P0.001） while.in LVCs, solid ne.plasms（76.7%, P0.001）. In HVCs, malignant ne.plasms we.e.tre.te. with LDP le.s fre.ue.tly than in LVCs（17.9% vs 50.0%, P〈0.001）. Sple.e.tomy was pe.forme. for non-oncologic re.son fre.ue.te. in HVCs than in LVCs（70.2%vs 25.0%, P0.004）. The.le.gth of stay was shorte. in HVCs than in LVCs（7.5 vs 11.3, P0.001）. AIM： To e.aluate.the.e.ficacy of a te.hnical modification to re.uce.the.incide.ce.of traumatic cataract induce. by De.ce.e. stripping automate. e.dothe.ial ke.atoplasty（DSAe.） pe.forme. in phakic e.e.. Me.HODS： A re.rospe.tive.cohort study. The.re.ords of all patie.ts with a cle.r crystalline.le.s and e.dothe.ial failure.that unde.we.t modifie. DSAe. at our insitution we.e.re.ie.e.. In this modification, in orde. to avoid inadve.te.t touch of the.inse.tion force.s against the.e.pose. crystalline.le.s while.passing across the.ante.ior chambe., the.incision site. we.e.shifte. from the.standard 9 and 3 o＇clock positions, supe.iorly to the.10 and 2 o＇clock position re.pe.tive.y. Formation of typically traumatic, ante.ior subcapsular cataract in the.e.patie.ts was compare. to that obse.ve. in a cohort including all the.patie.ts with a cle.r crystalline.le.s and e.dothe.ial failure.that unde.we.t conve.tional DSAe. at our ***： The.study group include. 49 e.e. following modifie. DSAe. and the.control group include. 35 e.e. following DSAe. with conve.tional incision site.. Ante.ior subcapsular cataract occurring 4 mo or le.s postope.ative.y was ide.tifie. in 2 of 49（4%） e.e. in the.study group and 7 of 35（20%） e.e. in the.control group. The.rate. of traumatic cataract we.e.significantly highe. in the.control group in comparison to the.study group（P=0.03, RR=4.9, 95%CI 1.08-22.1）.CONCLUSION： Traumatic cataract formation following phakic DSAe. may be.avoide. with a simple.modification to the.position of the.incision site..
