Hemophilia A is an X-linked recessive disorder characterized by a deficiency of coagulation factor Ⅷ(FⅧ) and therefore by a greater risk of bleeding during percutaneous interventional procedures and during the dual antiplatelet therapy(DAPT) in patients with ischemic heart disease. Information regarding safe percutaneous procedures in hemophiliacs is limited. Since the introduction of FVⅧ concentrates, the life expectancy of hemophiliac patients has improved and consequently, the rate of ischemic heart disease in this population is increased. Frequently the replacement therapy can trigger the onset of an acute coronary syndrome. We report a case of a patient with mild Hemophilia A, who presents with unstable angina, treated successfully with coronary angioplasty and drug eluting stent implantation without replacement of FVⅧ, treated with long term DAPT without major bleeding after six months of follow up.

BACKGROUND Disorders of gut-brain interaction(DGBI)are common,but knowledge about their physiopathology is still poor,nor valid tools have been used to evaluate them in *** To develop a psycho-gastroenterological questionnaire(PGQ)to assess the psycho-gastroenterological profile and social characteristics of a pediatric population with and without *** One hundred and nineteen Italian children(age 11-18)were included:28 outpatient patients with DGBI(Rome IV criteria)and 91 healthy *** filled the PGQ,faces pain scale revised(FPS-R),Bristol stool chart,ga-strointestinal symptoms rating scale,state-trait anxiety inventory,Toronto alexithymia scale 20,perceived self-efficacy in the management of negative emotions and expression of positive emotions(APEN-G,APEP-G),irritable bowel syndrome-quality of life questionnaire,school performances,tobacco use,early life events,degree of *** Compared to controls,patients had more medical examinations(35%of them went to the doctor more than five times),a higher school performance(23%vs 13%,P<0.05),didn’t use tobacco(never vs 16%,P<0.05),had early life events(28%vs 1%P<0.05)and a higher percentage of pain classified as 4 in the FPS-R during the examination(14%vs 7%,P<0.05).CONCLUSION Pediatric outpatients with DGBI had a higher prevalence of early life events,a lower quality of life,more medical examinations rising health care costs,lower anxiety levels.

The present study focuses on the flow of a yield-stress (Bingham) nanofluid, consisting of suspended Fe 3 O 4 nanoparticles, subjected to a magnetic field in a backward-facing step duct (BFS) configuration. The duct is equipped with a cylindrical obstacle, where the lower wall is kept at a constant temperature. The yield-stress nanofluid enters this duct at a cold temperature with fully developed velocity. The aim of the present investigation is to explore the influence of flow velocity (Re = 10 to 200), nanoparticle concentration ( φ = 0 to 0.1), magnetic field intensity (Ha = 0 to 100), and its inclination angle (γ = 0 to 90) and nanofluid yield stress (Bn = 0 to 20) on the thermal and hydrodynamic efficiency inside the backward-facing step. The numerical results have been obtained by resolving the momentum and energy balance equations using the Galerkin finite element method. The obtained results have indicated that an increase in Reynolds number and nanoparticle volume fraction enhances heat transfer. In contrast, a significant reduction is observed with an increase in Hartmann and Bingham numbers, resulting in quasi-immobilization of the fluid under the magnetic influence and radical solidification of this type of fluid, accompanied by the suppression of the vortex zone downstream of the cylindrical obstacle. This study sheds light on the complexity of this magnetically influenced fluid, with potential implications in various engineering and materials science fields.

This paper presents an approach for the optimal design of a new retrofit technique called weakening and damping that is valid for civil engineering inelastic structures. An alternative design methodology is developed with respect to the existing ones that is able to determine the locations and the magnitude of weakening and/or softening of structural elements and adding damping while insuring structural stability. An optimal polynomial controller that is a summation of polynomials in nonlinear states is used in Phase I of the method to reduce the peak response quantities of seismically excited nonlinear or hysteretic systems. The main advantage of the optimal polynomial controller is that it is able to automatically stabilize the structural system. The optimal design of a shear-type structure is used as an example to illustrate the feasibility of the proposed approach, which leads to a reduction of both peak inter-story drifts and peak total accelerations.
