A Retrospective Study on Hepatitis C Virus Infected Individuals Lost to Follow-Up in Ticino: The Hub and Spoke Approach

Background: Approximately 71.1 million individuals are chronically infected with hepatitis C virus (HCV). The global incidence of HCV was 23.7 cases per 100,000 population in 2015, with an estimated 1.75 million new HCV infections diagnosed in 2015. In Switzerland, it is estimated that 37,000 patients have chronic HCV infection. In the Southern part of Switzerland liver disease patients are mainly followed at the six outpatient clinics of Epatocentro Ticino (EPT) connected with its hub in Lugano. The aim of the study was to identify lost to follow-up patients and reconnect them to specialist care. Methods: We used the database from EPT to identify HCV positive patients, who were lost to follow-up in the years 2007-2017. We consider lost to follow-up patients who had their last visit more than 2 years before closing date of analysis. We contacted the patients on the phone or by mail. Results: Overall 1271 patients have been at EPT between 2007 and 2017;of those 74 were lost to follow-up at the time of our screening. We received an answer from 60 of them. The results we collected were that 12 (6%) have died, 31 (42%) were currently followed by the General Practitioner (GP) or a different specialist, 9 (12%) were cured from HCV infection and did not need any further visit, and 4 (5%) refused to come for a check-up visit. In total, we rescued 4 (5%) patients and follow them at the EPT. Conclusion: Our look-back study revealed that our “Hub and Spoke-approach” had a high retention rate such that only 6% of our HCV patients were lost to follow-up. The look-back effort was still useful: the number of patients to recall identifying one patient needing treatment was 19.

Human endogenous retroviruses(HERVs) are retroviruses that infected human genome millions of years ago and have persisted throughout human evolution. About 8% of our genome is composed of HERVs, most of which are nonfunctional because of epigenetic control or deactivating mutations. However, a correlation between HERVs and human cancer has been described and many tumors, such as melanoma, breast cancer, germ cell tumors, renal cancer or ovarian cancer, express HERV proteins, mainly HERV-K(HML6) and HERV-K(HML2). Although the causative role of HERVs in cancer is controversial, data from animal models demonstrated that endogenous retroviruses are potentially oncogenic. HERV protein expression in human cells generates an immune response by activating innate and adaptive immunities. Some HERV-derived peptides have antigenic properties. For example, HERV-K(HML-6) encodes the HER-K MEL peptide recognized by CD8+ lymphocytes. In addition, HERVs are twoedged immunomodulators. HERVs show immunosuppressive activity. The presence of genomic retroviral elements in host-cell cytosol may activate an interferon type I response. Therefore, targeting HERVs through cellular vaccines or immunomodulatory drugs combined with checkpoint inhibitors is attracting interest because they could be active in human tumors.

After short introducing the crucial role of e‐fuels to meet net‐zero emissions targets,this perspective paper discusses the differences between reactive catalysis(electro‐,photo‐and plasma‐catalysis,with focus on the first for conciseness)and thermal catalysis used at *** main point is to evidence that to progress in producing e‐fuels,the gap is not in terms of scaling‐up and pilot testing,but rather in the fundamental needs to turn the current approach and methodologies to develop reactive catalysis,including from a mechanistic perspective,to go beyond the current methods largely derived from thermal *** thus new fundamental bases to understand reactive catalysis is the challenge to accelerate the progress in this area to enable the potential role towards a sustainable net‐zero emissions *** novel aspects are highlighted,but the general aim is rather to stimulate discussion in rethinking catalysis from an alternative perspective.

Topics discussed regard （i） the new sustainable energy scenario, （ii） the role of energy storage （from smart grids to chemical storage of energy）, （iii） the outlooks and role of solar （bio）refineries and solar fuels, （iv） how to integrate hio- and solar-refineries to move to new economy, （v） the role of methanol at the crossover of new energy-chemistry nexus, （vi） the role of chemistry in this new scenario, （vii） the role of nanomaterials for a sustainable energy, （viii） the use of nanocarbons to design advanced energy conversion and storage devices, and （ix） possibilities and routes to exploit solar energy and methane （shale gas）. The contribution provides a glimpse of the emerging directions and routes with some elements about their possible role in the future scenario, but does not orovide a detailed analysis of the state of the art in these directions

Etherification of HMF to biodiesel additives: The role of NH4+ confinement in Beta zeolites

The role of NH4^+ ion confinement in the catalytic etherification of HMF(5-hydroxymethylfurfural) with ethanol to biodiesel additives was evidenced by studying the catalytic behavior of NH4^+-Beta zeolites with SiO2/Al2O3 ratios of 25 and *** order to affect the strength and distribution of the acidic sites, as well as the mobility of NH4^+ ions in the zeolites cages, a secondary level of porosity was introduced in the NH4^+-Beta, presenting a different stability versus alkaline treatment, by using a thermal or an ultrasound assisted *** analyzing the catalytic behavior in these two series of samples with respect to the changes in porosity by nonlocal density functional theory, structure by XRD, amount of acid sites by FT-IR and mobility of NH4^+ cations by measurements of reversible NH4^+ exchange capacity, was evidenced a decrease in catalytic performances both in terms of rate of HMF depletion and productivity to the main products, when confinement of the ammonium ions is lost due to the introduction of *** high capability of ammonium ions release, associated to the mono-dentate configuration,and the minor confinement effect inside the zeolite pore system, due to the more opened pores structure of mesoporous zeolites, hinders both the direct etherification of HMF to EMF [5-(ethoxymethyl)furan-2-carbaldehyde] and the parallel reaction pathway via acetalization, favoring the rapid desorption of the HMFDEA [5-(hydroxymethyl)furfural diethyl acetal] product out of the crystal and the consequent inhibition of the consecutive reactions to EMFDEA [5-(ethoxymethyl)furfural diethyl acetal] and EMF.

Objective To investigate the effects of extremely low frequency magnetic and electric fields （ELFEMFs） emitted from 380 kV transmission lines on some leukocyte differentiation antigens in dairy cows. Methods The study was carded out in 5 cows exposed to 1.98-3.28 μT of ELFEMFs and in 5 control cows exposed to 0.2-0.7 μT of ELFEMFs. Following haematological and immunologic parameters were measured in both groups： WBC, CD45R, CD6, CD4, CD8, CD21, and CD11B leukocyte antigen expression. Results Some of the haematological and immunologic parameters under investigation were similar in both groups. However, CD8 （T lymphocyte surface antigen） was higher in the exposed group （1.35 ±0.120 vs 0.50 ±0.14×10^3/mL）. Furthermore, the CD4/CD8 ratio （0.84 ±0.05 and 2.19±0.16 for exposed and not exposed cows respectively） and circadian rhythm were different between the two groups. Conclusion Exposure to ELFEMFs is responsible of the abnormal temporal variations and distribution of some haematological and immunological parameters in dairy cows.
