Endoscopic management of esophageal stenosis in children:New and traditional treatments

Post-esophageal atresia anastomotic strictures and postcorrosive esophagitis are the most frequent types of cicatricial esophageal stricture. Congenital esophageal stenosis has been reported to be a rare but typical disease in children; other pediatric conditions are peptic, eosinophilic esophagitis and dystrophic recessive epidermolysis bullosa strictures. The conservative treatment of esophageal stenosis and strictures(ES) rather than surgery is a well-known strategy for children. Before planning esophageal dilation, the esophageal morphology should be assessed in detail for its length, aspect, number and level, and different conservative strategies should be chosen accordingly. Endoscopic dilators and techniques that involve different adjuvant treatment strategies have been reported and depend on the stricture's etiology, the availability of different tools and the operator's experience and preferences. Balloon and semirigid dilators are the most frequently used tools. No high-quality studies have reported on the differences in the efficacies and rates of complications associated with these two types of dilators. There is no consensus in the literature regarding the frequency of dilations or the diameter that should be achieved. The use of adjuvant treatments has been reported in cases of recalcitrant stenosis or strictures with evidence of dysphagic symptoms. Corticosteroids(either systemically or locally injected), the local application of mitomycin C, diathermy and laser ES sectioning have been reported. Some authors have suggested that stenting can reduce both the number of dilations and the treatment length. In many cases, this strategy is effective when either metallic or plastic stents are utilized. Treatment complications, such esophageal perforations, can be conservatively managed, considering surgery only in cases with severe pleural cavity involvement. Rituximab has provided a revolutionary contribution to the treatment of B-cell non-Hodgkin’s lymphomas (NHL). A high prevalence of hepatitis C virus (HCV) infection has been described in B-cell NHL patients. Cases of liver dysfunction in HCV-positive patients have been reported with Rituximab-containing regimens. In this paper we review the recent data regarding the effects of Rituximab in NHL patients with HCV infection. We also added a section devoted to improving communication between oncohaematologists and hepatologists. Furthermore, we propose a common methodological ground to study hepatic toxicity emerging during chemotherapy.

The“oil stain disease”has become a phytosanitary status that involves economic losses for farmers who produce gulupa(Passiflora edulis Sims).For this reason,this research aimed to evaluate the biocontrol activity of Bacillus subtilis ATCC 55033 in a culture medium based on fermented millet as a biocontrol product of the ***,metabolite production of seven strains of *** and a reference strain B.s(ATCC 55033)was evaluated;in addition,the in vitro antagonistic ability was evaluated against Xanthomonas *** experiments were performed under in vitro conditions:(1)study of the presence of halos of hemolysis due to metabolite production;(2)analysis of the antagonistic ability by using double *** was observed that concerning the average,a number five times bigger of halos of hemolysis and 10 times bigger halos of inhibition in vitro on the growth of *** in the strains B.s(ATCC 55033)and B.s TB2 over other ***,the evaluation was performed under greenhouse conditions,based on a randomized block design,in which two treatments were applied over the soil:(1)application of the fermented medium based on millet with B.s ATCC 55033,24 h before X.a inoculation;(2)application of 5 d *** was observed that the treatment,previous infective process,obtained an average percentage of severity equal to 42.044%.In addition,after the evaluation month,a foliage restoration was *** conclusion,it is suggested that these products could act as inducers in the plant systemic resistance to trigger defense responses to infection of X.a.

Liver biopsy:analysis of results of two specialist teams

AIM:To analyze the safety and the adequacy of a sample of liver biopsies(LB)obtained by gastroenterologist(G)and interventional radiologist(IR)***:Medical records of consecutive patients evaluated at our GI unit from 01/01/2004 to31/12/2010 for whom LB was considered necessary to diagnose and/or stage liver disease,both in the setting of day hospital and regular admission(RA) care,were retrieved and the data entered in a *** were divided into two groups:one undergoing an ultrasonography(US)-assisted procedure by the G team and one undergoing US-guided biopsy by the IR *** the first group,an intercostal approach(US-assisted) and a Menghini modified type needle 16 G(length 90 mm) were *** IR team used a subcostal approach(US-guided) and a semiautomatic modified Menghini type needle 18 G(length 150 mm).All the biopsies were evaluated for appropriateness according to the current *** number of portal tracts present in each biopsy was assessed by a revision performed by a single pathologist unaware of the previous pathology ***,laboratory and demographic patient characteristics,the adverse events rate and the diagnostic adequacy of LB were ***:During the study period,226 patients,126 males(56%) and 100 females(44%),underwent LB:167(74%) were carried out by the G team,whereas 59(26%) by the IR *** was mostly performed in a day hospital setting by the G team,while IR completed more procedures on inpatients(P < 0.0001).The groups did not differ in median age,body mass index(BMI),presence of comorbidities and coagulation *** occurred in 26 patients(16 G team vs 10 IR team,P = 0.15).Most gross samples obtained were considered suitable for basal histological evaluation,with no difference among the two teams(96.4% G team vs 91.5% IR,P = 0.16).However,the samples obtained by the G team had a higher mean number of portal tracts(G team 9.5 ± 4.8; range 1-29 vs IR team 7.8 ± 4.1; range 1-2
