Bioequivalence Study of Two Hydrochlorothiazide Formulations after Oral Administration in Healthy Chilean Volunteers

The study was carried out in healthy Chilean volunteers in order to compare the pharmacokinetics （rate and extent absorption） of two commercial oral formulations of 50 mg hydrochlorothiazide. Thirty nine subjects were administered hydrochlorothiazide tablets of test （T） and reference （R） formulation in a single blind, randomized, fasting, 2 × 2 crossover study, seven washout days. Blood samples were taken during a 48 h period after drug administration. Plasma concentrations were quantified by HPLC-MS/MS. The primary parameters log-transformed Cmax （maximum plasma concentration）, A UC0-t and A UC0-∞ （area under the plasma concentration-time curve from zero to the last time and zero to infinity） were tested for bioequivalence considering the ratios of geometric means （test/reference）; whereas tmax （the time of maximum plasma concentration） was analyzed nonparametrically. The 90% confidence intervals for the geometric mean values of test/reference ratios for Cmax, A UCo.t and A UC0-∞ were 95.37%, 93.59% and 96.34%, respectively, and were located within the bioequivalence acceptance range of 80-125%, as were tmax and elimination constants. Together, we conclude that the test formulation of the hydrochlorothiazide 50 mg tablet is bioequivalent to the reference product and suitable for generic prescription.

Prime Enzymatic Exocellular Background of Lysinibacillus louembei

Many high-reading journals reject several studies based on the basic aspects of microbiology by forgetting that this could open doors and windows of great scientific discoveries. A new spore-forming bacteria species called Lysinibacillus louembei has been previously discovered in our research unit. This new study aims to assess a real enzymatic machine produced with such bacteria. Using agarose mixed with half skimmed milk, LB medium supplemented with tween 20, Olive oil, egg yolk, cellulose, casein, pectin and starch, we showed that L. louembei has lipolytic, proteolytic (2.8 cm ± 0.1), cellulolytic, amylolytic pectinolytic activities, with percentage ranging from 30% to 80%. This species is able to secrete lipase, thermostable protease up to 60°C. Cellulase, pectinase and amylase are secreted with more stability between 30°C and 50°C with an optimum at 45°C. The effect of pH was determined after 24 h of incubation at 37°C and 50°C. The proteolytic activity is stable at pH 8 at 50°C. In addition to this first enzymatic landscape, L. louembei is able to degrade and/or tolerate gasoline and/or diesel fuel hydrocarbons in 36 hours. In this work, we also showed that L. louembei can produce biosurfactant in the presence of gasoline and SAE140 with the E24 up to 50%.

Endoscopic management of esophageal stenosis in children:New and traditional treatments

Post-esophageal atresia anastomotic strictures and postcorrosive esophagitis are the most frequent types of cicatricial esophageal stricture. Congenital esophageal stenosis has been reported to be a rare but typical disease in children; other pediatric conditions are peptic, eosinophilic esophagitis and dystrophic recessive epidermolysis bullosa strictures. The conservative treatment of esophageal stenosis and strictures(ES) rather than surgery is a well-known strategy for children. Before planning esophageal dilation, the esophageal morphology should be assessed in detail for its length, aspect, number and level, and different conservative strategies should be chosen accordingly. Endoscopic dilators and techniques that involve different adjuvant treatment strategies have been reported and depend on the stricture's etiology, the availability of different tools and the operator's experience and preferences. Balloon and semirigid dilators are the most frequently used tools. No high-quality studies have reported on the differences in the efficacies and rates of complications associated with these two types of dilators. There is no consensus in the literature regarding the frequency of dilations or the diameter that should be achieved. The use of adjuvant treatments has been reported in cases of recalcitrant stenosis or strictures with evidence of dysphagic symptoms. Corticosteroids(either systemically or locally injected), the local application of mitomycin C, diathermy and laser ES sectioning have been reported. Some authors have suggested that stenting can reduce both the number of dilations and the treatment length. In many cases, this strategy is effective when either metallic or plastic stents are utilized. Treatment complications, such esophageal perforations, can be conservatively managed, considering surgery only in cases with severe pleural cavity involvement. Quantitative Evaluation of Inflammatory Response in Rats Submitted to the Abdominal Wall Surgery Using Peritoneum of Paca Preserved in 300% Supersaturated Solution of Sucrose or 98% Glycerin

This study aimed to quantitative evaluate the implantation of paca’s peritoneum, a new biomaterial option, preserved in 300% supersaturated sugar solution or 98% glycerin on the abdominal wall of Wistar rats. 60 males, weighing between 150 and 200 g, are organized into the following experimental groups: control group (GI);group peritoneum preserved in 300% supersaturated sugar solution (GII);and group peritoneum preserved in 98% glycerin (GIII). Each group had 20 animals. Five rats from each group underwent euthanasia at four different moments: 7th, 15th, 30th, and 60th post-operatory day for histological and stereological evaluations of the graft/native tissue interface. On histological examination, one found out an extensive presence of inflammatory infiltrate at the early periods (7th and 15th day) and an extensive presence of connective tissue at the late moments (30th and 60th day). Through stereological analysis, one found out that the number of mononuclear cells decreased throughout the evaluation moments. One concluded that the paca’s peritoneum as a biological membrane preserved in 98% glycerin, when implanted on the abdominal wall of rats, microscopically allowed one to obtain, through stereological analyses, a lower inflammatory response than the paca’s peritoneum preserved in 300% supersaturated sugar solution.

Rituximab has provided a revolutionary contribution to the treatment of B-cell non-Hodgkin’s lymphomas (NHL). A high prevalence of hepatitis C virus (HCV) infection has been described in B-cell NHL patients. Cases of liver dysfunction in HCV-positive patients have been reported with Rituximab-containing regimens. In this paper we review the recent data regarding the effects of Rituximab in NHL patients with HCV infection. We also added a section devoted to improving communication between oncohaematologists and hepatologists. Furthermore, we propose a common methodological ground to study hepatic toxicity emerging during chemotherapy.

Case Report: Caseous Calcification of the Mitral Annulus in an 88-Year-Old Woman

The caseous calcification of the mitral annulus (CCMA) is a rare variant of the mitral annulus calcification (MAC), with prevalence of 0.067% in general population. It is usually an asymptomatic condition but rarely it can cause severe mitral valve dysfunction, embolization, and conduction abnormalities. Transthoracic echocardiography (TTE) is a useful diagnostic tool. However, it can be necessary a multi-modality imaging approach for a differential diagnosis among other intra-cardiac masses. We are presenting a case of CCMA in an 88-year-old woman admitted to our department for acute decompensation of chronic heart failure. TTE has shown pathognomonic findings of CCMA. Tridimensional trans-esophageal echocardiography (3D TEE) gave a better characterization of the CCMA, avoiding further diagnostic procedures.

Clinical Facial Study of the Parents of Children with OAV (Oculo-Auriculo-Vertebral) Syndrome. Cases and Controls

Introduction: The oculo-auriculo-vertebral syndrome is a complex disease of the head and neck. Aim: The purpose of this study was to look for facial asymmetry, measuring both sides of the mandible and external ear in the parents of these patients in a clinical way, to see if it could be considered as a minor manifestation of the disease. Patients and Methods: A clinical, observational study was done with 50 trios and their parents’ controls, at the Instituto Nacional de Rehabilitación LGII in 2019, from January to May. Mandible and ear measurements were done. Results: All of our patients had microtia and facial asymmetry, and in 17 cases, other anomalies. We compared both sides of the mandible in the patients’ parents and in the parents’ controls. The results showed differences only in the fathers, and in the comparison with the controls, we found statistical differences only in women, family and non-family cases. The ears did not show any differences. Discussion: It is very interesting that we found differences in the measurements of the mandible of the patients’ mothers and their controls;in the mothers the mandible was bigger than in the controls, this probably means that the development of the cranial structures is modified by some genetic variants. No differences were found in the measurements of the ears. Conclusion: It is very difficult to know exactly if the differences we found are important or not. The clinical measurements could be a good option to establish if the parents have minor manifestations that could explain some heritability.
