Epidemiology of pediatric burns and future prevention strategies—a study of 475 patients from a high-volume burn center in North India

Background: Pediatric burns have a long-term social impact. This is more apparent in a developing country such as India, where their incidence and morbidity are high. The aim of this study was to provide recent prospective epidemiological data on pediatric burns in India and to suggest future preventive strategies. Methods: Children up to 18 years old admitted to the Department of Burns, Plastic&Maxillofacial Surgery, VMMC&Safdarjung Hospital, New Delhi, between January and December 2014 were included in the study. Data regarding age, sex, etiology, total body surface area (TBSA), circumstances of injury, and clinical assessment were collected. The Mann-Whitney test or Kruskal-Wallis test or ANOVA was used to compare involved TBSA among various cohort groups accordingly. Univariate and multivariate linear regression analyses were used to determine the predictors of TBSA. Results: There were a total of 475 patients involved in the study, including seven suicidal burns, all of whom were females with a mean age greater than the cohort average. Age, type of burns, mode of injury, presence or absence of inhalation injury, gender, and time of year (quarter) for admission were found to independently affect the TBSA involved. Electrical burns also formed an important number of presenting burn patients, mainly involving teenagers. Several societal issues have come forth, e.g., child marriage, child labor, and likely psychological problems among female children as suggested by a high incidence of suicidal burns. Conclusions: This study also highlights several issues such as overcrowding, lack of awareness, dangerous cooking practices, and improper use of kerosene oil. There is an emergent need to recognize the problems, formulate strategies, spread awareness, and ban or replace hazardous substances responsible for most burn accidents.

Circum-Indian Holocene sea-level database:A repository of distinct relative sea-level plots across the Indian Peninsula

The Indian Peninsula is one of the most well-studied regions for Holocene sea-level fluctuations in the world, however, standardized relative sea-level datasets are missing. This study provides an archive of sealevel indicators(n = 162, 20 locations) along the western and the eastern sides of the peninsula, that have been used to develop Relative Sea Level(RSL) plots. Each dated sea-level indicator is recalibrated for its elevation based on tidal and tectonic correction, as well as age with reservoir correction, and have been separated into six zones based on coastal geomorphology and number of datasets. The database spans throughout the Holocene and covers sea-level depth/elevations from-45 m to +5 m from mean sea-level(MSL). Approximately 90 % of the dataset range from 8 ka to the present day. The first transgression is highly variable and identified between 8.5-8 ka BP in Gujarat(Zone 1), ~ 5.5 ka BP in Maharashtra(Zone 2), between 8 and 7 ka BP in Tamil Nadu(Zone 4) and between 8 and 7.5 ka BP in the Bengal coasts(Zone 6). No transgression above present sea-level is observed along Andhra Pradesh(Zone 5)(no data for Kerala-Zone 3).Further, Zones 1, 2, 4 and 6 show a strong uplift component(tectonic), whereas Zone 5 exhibits subsidence during the Holocene(Zone 3-insufficient data). Based on these findings, and given the region's coastal topography and tidal components, Zones 6 and 1 will likely undergo the largest coastal inundation, followed by Zones 5, 4, 2, and 3. These insights are critical in planning future coastal inundation measures across the Indian Peninsula.

The main aim of this opinion review is to comment on the recent article published by Garg et al in the World Journal of Gastroenterology 2023;29:4593–4603.The authors in the published article developed a new scoring system,Garg incon-tinence scores(GIS),for fecal incontinence(FI).FI is a chronic debilitating disease that has a severe negative impact on the quality of life of the patients.Rome IV criteria define FI as multiple episodes of solid or liquid stool passed into the clothes at least twice a month.The associated social stigmatization often leads to significant under-reporting of the condition,which further impairs management.An important point is that the complexity and vagueness of the disease make it difficult for the patients to properly define and report the magnitude of the problem to their physicians.Due to this,the management becomes even more difficult.This issue is resolved up to a considerable extent by a scoring ques-tionnaire.There were several scoring systems in use for the last three decades.The prominent of them were the Cleveland Clinic scoring system or the Wexner scoring system,St.Marks Hospital or Vaizey’s scores,and the FI severity index.However,there were several shortcomings in these scoring systems.In the opinion review,we tried to analyze the strength of GIS and compare it to the existing scoring systems.The main pitfalls in the existing scoring systems were that most of them gave equal weightage to different types of FI(solid,liquid,flatus,etc.),were not comprehensive,and took only the surgeon’s perception of FI into view.In GIS,almost all shortcomings of previous scoring systems had been addressed:different weights were assigned to different types of FI by a robust statistical methodology;the scoring system was made comprehensive by including all types of FI that were previously omitted(urge,stress and mucus FI)and gave priority to patients’rather than the physicians’perceptions while developing the scoring system.Due to this,GIS indeed looked like a paradigm shift in the evaluat

Atherosclerotic plaque classification in carotid ultrasound images using machine learning and explainable deep learning

Objective The incidence of cardiovascular diseases(CVD)is rising rapidly worldwide.Some forms of CVD,such as stroke and heart attack,are more common among patients with certain conditions.Atherosclerosis development is a major factor underlying cardiovascular events,such as heart attack and stroke,and its early detection may prevent such events.Ultrasound imaging of carotid arteries is a useful method for diagnosis of atherosclerotic plaques;however,an automated method to classify atherosclerotic plaques for evaluation of early-stage CVD is needed.Here,we propose an automated method for classification of high-risk atherosclerotic plaque ultrasound images.Methods Five deep learning(DL)models(VGG16,ResNet-50,GoogLeNet,XceptionNet,and SqueezeNet)were used for automated classification and the results compared with those of a machine learning(ML)-based technique,involving extraction of 23 texture features from ultrasound images and classification using a Support Vector Machine classifier.To enhance model interpretability,output gradient-weighted convolutional activation maps(GradCAMs)were generated and overlayed on original images.Results A series of indices,including accuracy,sensitivity,specificity,F1-score,Cohen-kappa index,and area under the curve values,were calculated to evaluate model performance.GradCAM output images allowed visualization of the most significant ultrasound image regions.The GoogLeNet model yielded the highest accuracy(98.20%).Conclusion ML models may be also suitable for applications requiring low computational resource.Further,DL models could be more completely automated than ML models.

Appraisal of pollution and health risks associated with coal mine contaminated soil using multimodal statistical and Fuzzy-TOPSIS approaches

The present study assesses the concentration,probabilistic risk,source classification,and dietary risk arising from heavy metal(HMs)pollution in agricultural soils affected by coal mining in eastern part of India.Analyses of soil and rice plant indicated significantly elevated levels of HMs beyond the permissible limit in the contaminated zones(zone 1:PbSoil:108.24±72.97,CuSoil:57.26±23.91,CdSoil:8.44±2.76,CrSoil:180.05±46.90,NiSoil:70.79±25.06 mg/kg;PbGrain:0.96±0.8,CuGrain:8.6±5.1,CdGrain:0.65±0.42,CrGrain:4.78±1.89,NiGrain:11.74±4.38 mg/kg.zone 2:PbSoil:139.56±69.46,CuSoil:69.89±19.86,CdSoil:8.95±2.57,CrSoil:245.46±70.66,NiSoil:95.46±22.89 mg/kg;PbGrain:1.27±0.84,CuGrain:7.9±4.57,CdGrain:0.76±0.43,CrGrain:8.6±1.58,NiGrain:11.50±2.46 mg/kg)compared to the uncontaminated zone(zone 3).Carcinogenic and non-carcinogenic health risks were computed based on the HMs concentration in the soil and rice grain,with Pb,Cr,and Ni identified as posing a high risk to human health.Monte Carlo simulation,the solubility-free ion activity model(FIAM),and severity adjusted margin of exposure(SAMOE)were employed to predict health risk.FIAM hazard quotient(HQ)values for Ni,Cr,Cd,and Pb were>1,indicating a significant non-carcinogenic risk.SAMOE(risk thermometer)results for contaminated zones ranged from low to moderate risk(CrSAMOE:0.05,and NiSAMOE:0.03).Fuzzy-TOPSIS and variable importance plots(from random forest)showed that Ni and Cr were mostly responsible for the toxicity in the rice plant,respectively.A self-organizing map for source classification revealed common origin for the studied HMs with zone 2 exhibiting the highest contamination.The positive matrix factorization model for the source apportionment identified coal mining and transportation as the predominant sources of HMs.Spatial distribution analysis indicated higher contamination near mining sites as compared to distant sampling sites.Consequently,this study will aid environmental scientists and policymakers controlling HM pollution in agricultural soils near coal m

The extensive research and development in perovskite solar cells (PSCs) have rekindled the hopes of converting solar energy into electricity.An elusive understanding of underlying mechanisms is required for the development of efficient PSCs.Over the years,Impedance Spectroscopy (IS) characterization,along with complementary techniques,has proven to be an effective way to understand and analyze the charge transport and recombination at interface and bulk of PSCs.The IS of PSCs have been analyzed,interpreted,and improvised continuously,revealing intricate details about the work.However,there is a lack of centralized source of these details,which make it tougher to account for the generalized approach to understand the device properties.The present work is focused on compiling the research done on various PSC device architectures via IS to construct a comprehensive foundation of information on impedance plots,equivalent circuits,and associated processes.

Betalains are natural coloring pigments with betalamic acid as the core structure of all subclasses.Besides their coloring properties,betalains exhibit various biological activities,including antioxidant and anti-inflammatory properties,which are highly imperative.Further in-vivo studies reported that betalains protect various body organs,leading to health enhancement.Body organs,including the heart,liver,kidney,lung,etc.,are important for a healthy life.However,these organs can be affected or damaged by various stress factors,toxicants,and harmful substances.Recent studies have claimed that betalains could protect all vital organs of the body through antioxidant and anti-inflammatory mechanisms.This review article described the in-vivo antioxidant and anti-inflammatory activities of betalains in various cell-line or animal models.A comprehensive discussion has been provided on the mechanism of action of betalains in protecting various body organs,including cardio-protective effect,hepato-protective ability,renal protection capacity,repro-protective ability,neuro-protective effect,lung protection,and gut protection ability.Finally,future research directions and conclusions have been outlined.

Penoscrotal defect:a functional,esthetic,and psychological challenge

Aim:Penoscrotal defects may be caused by a variety of events.Reconstruction of the penoscrotal region is required not only for aesthetic appearance but also for functional and psychological reasons.Numerous techniques have been described for penoscrotal reconstruction reflecting the challenge and complexity of the region involved.This suggests that no single method is satisfactory for all types and degrees of tissue defects.This prospective study was conducted in a tertiary care hospital in India,over a period of 5 years.Methods:Eighteen patients with penoscrotal defects of varying etiology were included in the study and underwent different surgical techniques.Age of the patients ranged from 20 to 60 years.The etiology of penoscrotal defect was Fournier’s gangrene in 12 cases,trauma in 4 cases,and burn in 2 cases.The patients with Fournier’s gangrene were initially treated by debridement,drainage,and antibiotics.The penoscrotal defects were treated with local flap advancement with skin grafting(n=7),pedicled anterolateral thigh flap(n=4),gracilis muscle flap with split skin grafting(n=4),and medial thigh flap(n=3).Results:There was complete healing in 16 patients with minor complications in the form of partial skin graft loss(n=1)and wound dehiscence(n=1).Results were highly satisfactory in 6 patients,satisfactory in 8 patients,and not satisfactory in 4 patients.Scarring at the donor site was limited and acceptable.Conclusion:The vast arsenal of options for penoscrotal defect coverage ranges from skin grafting to flaps,and every case needs a customized approach with regard to its feasibility,outcome,and complication rate.Flaps should be the preferred choice over the skin grafts because of the superior functional and aesthetic results and better compliance.

Characteristics, phenotypes, mechanisms and management of severe asthma

Severe asthma is "asthma which requires treatment with high dose inhaled corticosteroids (ICS) plus a second controller (and/or systemic corticosteroids) to prevent it from becoming ’’uncontrolled’ or which remains ’’uncontrolled’ despite this therapy." The state of control was defined by symptoms, exacerbations and the degree of airflow obstruction. Therefore, for the diagnosis of severe asthma, it is important to have evidence for a diagnosis of asthma with an assessment of its severity, followed by a review of comorbidities, risk factors, triggers and an assessment of whether treatment is commensurate with severity, whether the prescribed treatments have been adhered to and whether inhaled therapy has been properly administered. Phenotyping of severe asthma has been introduced with the definition of a severe eosinophilic asthma phenotype characterized by recurrent exacerbations despite being on high dose ICS and sometimes oral corticosteroids, with a high blood eosinophil count and a raised level of nitric oxide in exhaled breath. This phenotype has been associated with a Type-2 (T2) inflammatory profile with expression of interleukin (IL)-4, IL-5, and IL-13. Molecular phenotyping has also revealed non-T2 inflammatory phenotypes such as Type-1 or Type-17 driven phenotypes. Antibody treatments targeted at the T2 targets such as anti-IL5, anti-IL5Rα, and anti-IL4Rα antibodies are now available for treating severe eosinophilic asthma, in addition to anti-immunoglobulin E antibody for severe allergic asthma. No targeted treatments are currently available for non-T2 inflammatory phenotypes. Long-term azithromycin and bronchial thermoplasty may be considered. The future lies with molecular phenotyping of the airway inflammatory process to refine asthma endotypes for precision medicine.
