BACKGROUND Coronavirus disease 2019(COVID-19)is a highly contagious infection caused by the severe acute respiratory syndrome coronavirus 2 virus and has a unique underlying ***(HD)patients experience high risk of contamination with COVID-19 and are considered to have higher mortality rates than the general population by most but not all clinical *** aim to highlight the peculiarities in the immune state of HD patients,who seem to have both immune-activation and immune-depression affecting their outcome in COVID-19 *** SUMMARY We report the opposite clinical outcomes(nearly asymptomatic course vs death)of two diabetic elderly patients infected simultaneously by COVID-19,one being on chronic HD and the other with normal renal *** were both admitted in our hospital with COVID-19 symptoms and received the same treatment by *** non-HD sibling deteriorated rapidly and was intubated and transferred to the Intensive Care Unit,where he died despite all supportive *** HD sibling,although considered more“high-risk”for adverse outcome,followed a benign course and left the hospital alive and *** These cases may shed light on aspects of the immune responses to COVID-19 between HD and non-HD patients and stimulate further research in pathophysiology and treatment of this dreadful disease.

Fat Embolism Syndrome in a Patient with Bilateral Tibial Fractures: Report of the Case and Review of the Literature

Background: Although the original clinical description of fat embolism syndrome (FES) dates from 1873, the condition remains a diagnostic challenge for modern clinicians. The syndrome is described as a serious consequence of fat emboli producing a distinct pattern of clinical symptoms and signs. It is mainly associated with fractures of the long-bones and the pelvis. The present paper describes the case of a trauma patient with bilateral tibial fractures that present the syndrome and highlights the importance of early identification and aggressive treatment of FES which is obligatory for the patient’s survival. Case Description: A 32 year-old man reached the hospital as a trauma call after a road traffic accident. Clinical examination was unremarkable with the only finding that the patient was unable to straight leg raise and weight bear. Both shins were painful and swollen at their middle third, while the left one was in varus and posterior displacement. Neurovascular status of both lower extremities was normal. Plain radiographic control (X-Rays) revealed an oblique right tibial fracture of the mid-shaft with a distal third fibular fracture and a comminuted fracture of the left tibia with a fracture of the fibula at the same level. The legs were immobilised on a splint and the decision was to treat the patient surgically. Within a few post admission hours the haemoglobin dropped by 2.9 mg/dL, however a source of active bleeding could not be allocated. The abdomen was soft and not tender in palpation and the central nervous system did not present any abnormalities. The following day the haemoglobin dropped but the new ultrasonography control did not reveal any free abdominal fluid. Both lower limbs were not compromised. The second post-injury day the patient went to theatre and the right tibia was stabilised with an antegrade nail (T2-Stryker) while the left one with an external fixation. An object-based approach for flood area delineation in a transboundary area using ENVISAT ASAR and LANDSAT TM data

The aim of this study was to develop a straightforward approach for flood area mapping in a transboundary riverbed using Geographic Object-Based Image *** bilateral/multilateral cooperation among neighboring countries hampers effective disaster management and mitigation activities over transboundary areas and strengthens the demand for reliable remote-sensing-derived *** object-based classification approaches using ENVISAT/ASAR and multi-temporal LANDSAT TM data were developed and validated for flood area *** accuracy assessment of the classification results was based on oblique air photo interpretation and an area-based comparison with the official flood *** bi-level object-based model using the Normalized Difference Water Index and the original post-flood TM bands attained 92.67%overall accuracy in inundated-areas detection,while the ENVISAT/ASAR classification was the least accurate(85.33%),probably due to the lower spatial resolution of the Synthetic Aperture Radar image.A strong agreement(92.14%)was found between the LANDSAT flood extent and the official flood map,suggesting that the proposed method has the potential to be employed in the future as a standard part of a flood crisis management process.

3D extrusion of multi-biomaterial lattices using an environmentally informed workflow

The conventional building material palette has been proven limited in terms of adaptability to our current environmental challenges. Innovations in computational design and digital manufacturing have supported the broadening of biomaterial applications as an alternative. While biomaterials are characteristically responsive to stimuli such as temperature and humidity, their unpredictable behaviour is a hurdle to standardization and architectural utilisation. To examine the nexus between material formulation, computation and manufacturing, multi-biomaterial lattice structures were produced through an environmentally informed workflow. Customized biomaterial development resulted in three candidate biopolymer blends with varying levels of hydro-responsiveness and transparency. The computational strategy included a machine learning clustering algorithm to customise results and dictate material distribution outputs. To test the workflow, environmental data of solar radiation exposure and solar heat gain from a specific location was used to inform the material deposition via pneumatic extrusion for the design and digital fabrication of a deformation-controlled prototype of 350 mm × 350 mm. This led to a series of multi-biomaterial wall panel components that can be applied at architectural scale. In future, these techniques can support the incorporation of living elements to be embedded within the built environment for truly animate architecture.

Laparoscopic transvesical vesicovaginal fistula repair with the least invasive way:Only three trocars and a limited posterior cystotomy

Exploiting the Cellular Infrastructure for Data Transmission in Smart Metering Systems

To implement the access and backhaul networks for Smart Metering (SM) systems various technologies are combined with the existing communications infrastructure. This paper deals with data transmission in SM systems, focusing on how the existing cellular networks infrastructure is employed to implement SM access communication networks. The analysis aims at analyzing the role of the cellular communications infrastructure taking into account the spatial distribution and installation points of the smart meters, the urban and topological characteristics of the SM deployment areas and the common practice so far followed by the utilities. It is demonstrated that cellular communications, either exclusively or combined with power line communications, enable immediate and scalable deployment of SM access communication networks at low installation cost, thus constituting the basic option for the implementation of smart metering.
