Progressive supranuclear palsy,corticobasal degeneration,multiple system atrophy and dementia with Lewy bodies are the most common causes of atypical Parkinsonism and enter the differential diagnosis of Parkinson's *** system atrophy,dementia with Lewy bodies and Parkinson's disease are synucleinopathies,whereas progressive supranuclear palsy and corticobasal degeneration are *** cerebrospinal fluid markers have been applied on cohorts of patients with Parkinsonism,with the aim to develop biomarkers for these *** tau(τΤ),phosphorylated tau at threonine 181(τP-181)and amyloid-beta with 42 amino acids(Aβ42)are considered classical biomarkers for Alzheimer's *** aim of the present study is to review the literature regarding these classical cerebrospinal fluid biomarkers in cohorts with Parkinsonism,as well as present data on novel approaches regarding analysis of these proteins.

Paleostress magnitude in a fold-thrust belt （External Hellenides, greece）： evidence from twinning in calcareous rocks

Based on twinned calcite in carbonate samples collected from the External Hellenides, we calculated paleostress magnitude and its across-strike variation into the Pindos Zone （PZ） and the gavrovo Zone （gZ）. These two zones are located west of the suture zone between the External and the Internal Hellenides. Paleostress magnitude is in the order of 110 MPa close to the suture, dropping toward the foreland and reaching a value of 50 MPa 80 km from the suture. The obtained distribution of paleostress levels in the External Hellenides resembles that recorded in other orogens, such as the Appalachian and Northern Pyrenean. The paleodepths of sampled carbonates during the initial stage of thrusting, which were estimated using the differential stress values, are structurally sensible. Comparing our results to available information on the evolution of paleostress magnitude with depth in compressional settings at the upper crust, we suggest that the mechanical behaviour of the cover thrust sheets of the External Hellenides is governed by the law of friction under nearly hydrostatic conditions. Based on this finding, as well as on published rheological data from rocks exposed at the deeper structural levels of the External Hellenides, a rheological profile was proposed. On this profile, brittle-ductile transition occurs at a depth of around 9-10 kin, slightly deeper than the basal decollement of the External Hellenides.

Offering routine carotid endarterectomy(CEA)or carotid artery stenting(CAS)to patients with asymptomatic carotid artery stenosis(ACS)is no longer considered as the optimal management of these *** suboptimal,however,is the policy of offering only best medical treatment(BMT)to all patients with ACS and not considering any of them for prophylactic *** the last few years,there have been many studies aiming to identify reliable predictors of future cerebrovascular events that would allow the identification of patients with high-risk ACS and offer a prophylactic carotid intervention only to these patients to prevent them from becoming *** patients with ACS should receive *** present article will summarise the evidence suggesting ways to identify these high-risk asymptomatic individuals,namely:(1)microemboli detection on transcranial Doppler,(2)plaque echolucency on Duplex ultrasound,(3)progression in the severity of ACS,(4)silent embolic infarcts on brain CT/MRI,(5)reduced cerebrovascular reserve,(6)increased size of juxtaluminal hypoechoic area,(7)identification of intraplaque haemorrhage using MRI and(8)carotid *** evidence suggests that approximately 10%-15%of patents with asymptomatic stenosis might benefit from intervention;this will become more clear after publication of ongoing studies comparing stenting or endarterectomy with best medical *** the meantime,no patient should be offered intervention unless there is evidence of high risk of ipsilateral stroke,from modalities such as those discussed here.

Colonoscopy is the gold standard for colorectal cancer prevention; however, it is still an imperfect modality. Precancerous lesions can be lost during screening examinations, thus increasing the risk of interval cancer. A variety of factors either patient-, or endoscopist dependent or even the procedure itself may contribute to loss of lesions. Sophisticated modalities including advanced technology endoscopes and add-on devices have been developed in an effort to eliminate colonoscopy's drawbacks and maximize its ability to detect potentially culprit polyps. Novel colonoscopes aim to widen the field of view. They incorporate more than one cameras enabling simultaneous image transmission. In that way the field of view can expand up to 330°. On the other hand a plethora of add-on devices attachable on the standard colonoscope promise to detect lesions in the proximal aspect of colonic folds either by offering a retrograde view of the lumen or by straightening the haustral folds during withdrawal. In this minireview we discuss how these recent advances affect colonoscopy performance by improving its quality indicators(cecal intubation rate, adenoma detection rate) and other metrics(polyp detection rate, adenomas per colonoscopy, polyp/adenoma miss rate) associated with examination's outcomes.

Villous adenomas of the bile ducts are extremely uncommon. We describe a 58-year-old man presenting with clinical signs and laboratory findings of acute pancreatitis and obstructive jaundice. Preoperative investigation demonstrated a dilated papillary orifice with mucus exiting （fish-mouth sign） and a filling defect in the distal common bile duct. He underwent a modified Whipple operation and histological examination of the surgical specimen showed villous adenoma with rich secretion of mucus.

Despite the effectiveness of endoscopic retrograde cholangiopancreatography（ERCP） for the treatment of choledocholithiasis, various complications have been described. We herein report the first case of prolonged postERCP jaundice due to toxicity of the contrast agent Iobitridol（~174;XENETIX, guerbet, Roissy Cd g Cedex, France） in a patient who underwent ERCP with sphincterectomy and common bile duct stone removal. While clinical improvement and normalization of aminotransferases and cholestatic enzymes after the procedure, an unexplained increase of direct bilirubin was noticed. A second ERCP was performed one week later, excluding possible remaining choledocholithiasis. Nevertheless, serum direct bilirubin increased further up to 15 mg/d L. Other potential causes of direct hyperbilirubinemia were ruled out and patient＇s liver biopsy was compatible with drug-induced liver toxicity. Additionally, the cause-result time connection between the use of Iobitridol and bilirubin increase indicated the possibility of a toxic effect related to the repeated use of the particular contrast agent. Iobitridol, a contrast agent, can induce prolonged direct hyperbilirubinemia.

Although colonoscopy has been proven effective in reducing the incidence of colorectal cancer through the detection and removal of precancerous lesions, it remains an imperfect examination, as it can fail in detecting up to almost one fourth of existing adenomas. Among reasons accounting for such failures, is the inability to meticulously visualize the colonic mucosa located either proximal to haustral folds or anatomic curves, including the hepatic and splenic flexures. In order to overcome these limitations, various colonoscope attachments aiming to improve mucosal visualization have been developed. All of them-transparent cap, Endocuff, Endocuff Vision and Endorings-are simply mounted onto the distal tip of the scope. In this review article, we introduce the rationale of their development, present their mode of action and discuss in detail the effect of their implementation in the detection of lesions during colonoscopy.

In the present paper the problem of disturbance rejection of single input-single output neutral time delay systems with multiple measurable disturbances is solved via dynamic controllers. In particular, the general form of the controller matrices is presented, while the necessary and sufficient conditions for the controller to be realizable are offered. The proposed technique is applied to a test case neutral time delay central heating system. In particular, the nonlinear model of the plant and its linearized approximation are presented. Based on the linearized model, a two-stage controller is designed in order to regulate the room temperature and the boiler effluent temperature. The performance of the closed loop system is investigated through computational experiments.
