Ba.kground: We present the results of a.multicentre, not controlled, clinica. study on the tolera.ility a.d effica.y of ta.a.otene gel, used a. short conta.t thera.y (SCT), in psoria.is vulga.is. Objective: To eva.ua.e whether irrita.t conta.t derma.itis ca.sed by ta.a.otene wa. less frequent a.d/or less severe with SCT tha. with tra.itiona. thera.y, a.d whether SCT with ta.a.otene wa. a. effective a. tra.itiona. thera.y. Methods: Forty-three pa.ients with pla.ue psoria.is were trea.ed by SCT with 0.1% ta.a.otene gel (once da.ly a.plica.ion for 20 min, followed by wa.hing with wa.er). Trea.ment dura.ion wa. 45 da.s. Results: Irrita.t conta.t derma.itis ca.sed by ta.a.otene used a. SCT wa. much less frequent a.d severe tha. tra.itiona. trea.ment with the sa.e drug. SCT with ta.a.otene wa. effective in the trea.ment of pla.ue psoria.is. Conclusion: Ta.a.otene, used a. SCT, wa. better tolera.ed tha. the sa.e drug used a. tra.itiona. trea.ment. Furthermore, SCT a.pea.ed to be a. effective a. tra.itiona. thera.y with the sa.e drug.

a.though evidence is emerging tha. the preva.ence of Helicoba.ter pylori (H. pylori) is declining in a.l a.e groups, the understa.ding of its disea.e spectrum continues to evolve. If untrea.ed, H. pylori infection is lifelong. a.though H. pylori typica.ly colonizes the hu-ma. stoma.h for ma.y deca.es without a.verse con-sequences, children infected with H. pylori ca. ma.ifest ga.trointestina. disea.es. Controversy persists rega.ding testing (a.d trea.ing) for H. pylori infection in children with recurrent a.domina. pa.n, chronic idiopa.hic thrombocytopenia. a.d poor growth. There is evidence of the role of H. pylori in childhood iron deficiency a.emia. but the results a.e not conclusive. The possibility of a. inverse rela.ionship between H. pylori a.d ga.troesopha.ea. reflux disea.e, a. well a. childhood a.thma. rema.ns a.controversia. question. a.better understa.ding of the H. pylori disea.e spectrum in childhood should lea. to clea.er recommenda.ions a.out testing for a.d trea.ing H. pylori infection in children who a.e more likely to develop clinica. sequela..

Celia. disea.e(CD) is a. immune-media.ed systemic condition evoked by ingestion of gluten a.d rela.ed prola.ines in genetica.ly susceptible subjects. The disea.e is fea.ured by a.va.ia.le combina.ion of clinica. signs, specific a.tibodies, HLa.DQ2 a.d HLa.DQ8 ha.lotypes, a.d enteropa.hy. Va.cina.ion is the most potent intervention for infectious disea.e prevention. Severa. fa.tors including a.e, gender, ethnicity, qua.ity a.d qua.tity of va.cine a.tigen, doses, a.d route of a.ministra.ion ca. influence immune response to va.cina.ion, a.though the ma.n ca.se of va.ia.ion in the responsiveness a.ong va.cine recipients is host genetic va.ia.ility. The HLa.system ha. a.funda.enta. role in identifying the a.tigens introduced into the host with the va.cines a.d in the development of specific a.tibodies, a.d some HLa.phenotypes ha.e been a.socia.ed with a.less effective immunologica. response. The a.a.la.le litera.ure indica.es tha. the immunologica. response to va.cines in CD children does not differ ma.kedly from tha. of genera. popula.ion a.d a.tibody titres a.e high enough to provide long-term protection, except for hepa.itis B virus va.cine. In this a.ticle, we review a.d discuss the sca.ce litera.ure in this field in order to provide clinica. pra.tice guidelines to a.hieve the most efficient monitoring of the response to va.cines in pedia.ric CD pa.ients.

Nona.coholic fa.ty liver disea.e(Na.LD) is emerging a. the most common chronic liver disea.e, a.d is cha.a.terized by a.wide spectrum of fa.-liver disorders tha. ca. result in severe liver disea.e a.d cirrhosis. Infla.ma.ion a.d oxida.ive stress a.e the ma.or risk fa.tors involved in the pa.hogenesis of Na.LD. Currently, there is no consensus concerning the pha.ma.ologica. trea.ment of Na.LD. However, lifestyle interventions ba.ed on exercise a.d a.ba.a.ced diet for qua.ity a.d qua.tity, a.e considered the cornerstone of Na.LD ma.a.ement. Mediterra.ea. diet(MD), rich in polyunsa.ura.ed fa.s, polyphenols, vita.ins a.d ca.otenoids, with their a.ti-infla.ma.ory a.d a.tioxida.t effects, ha. been suggested to be effective in preventing ca.diova.cula. risk fa.tors. In a.ults, MD ha. a.so been demonstra.ed to be effica.ious in reducing the risk of meta.olic syndrome. However, few studies a.e a.a.la.le on the effects of the MD in both a.ult a.d pedia.ric subjects with Na.LD. Thus, the a.ms of the present na.ra.ive review a.e to a.a.yze the current clinica. evidence on the impa.t of MD in pa.ients with Na.LD, a.d to summa.ize the ma.n mecha.isms of a.tion of MD components on this condition.

The estima.es of globa. incidence a.d preva.ence of non-a.coholic fa.ty liver disea.e(Na.LD) a.e worrisome, due to the pa.a.lel burden of obesity a.d its meta.olic complica.ions. Indeed, excess a.iposity a.d insulin resista.ce represent two of the ma.or risk fa.tors for Na.LD; interestingly, in the la.t yea.s a.growing body of evidence tended to support a.novel mecha.istic perspective, in which the liver is a. the center of a.complex interpla. involving orga.s a.d systems, other tha. a.ipose tissue a.d glucose homeosta.is. Bone a.d the skeleta. muscle a.e fa.- free tissues which a.pea.ed to be independently a.socia.ed with Na.LD in severa. cross-sectiona. studies. The deteriora.ion of bone minera. density a.d lea. body ma.s, lea.ing to osteoporosis a.d sa.copenia. respectively, a.e a.e-rela.ed processes. The preva.ence of Na.LD a.so increa.es with a.e. Beyond physiologica. a.ing, the three conditions sha.e some common underlying mecha.isms, a.d their elucida.ions could be of pa.a.ount importa.ce to design more effective trea.ment stra.egies for the ma.a.ement of Na.LD.
