An eighteen-month follow-up study on the effects of Intravitreal Dexamethasone Implant in diabetic macular edema refractory to anti-VEGF therapy

AIM： To evaluate the long-term efficacy and safety of dexamethasone implants in subjects affected by diabetic macular edema（DME） resistant to anti-vascular endothelial growth factor（VEGF） ***： Thirty-two DME patients were enrolled.A700 microgram slow release Intravitreal Dexamethasone Implant（Ozurdex~） was placed in the vitreous *** patients were followed for 18 ***-corrected visual acuity（BCVA） measured with Early Treatment Diabetic Retinopathy Study（ETDRS） and central macular thickness（cmT） exams were carried out at baseline（T0）and after 1（T1）,3（T3）,4（T4）,6（T6）,9（T9）,12（T12）,15（T15）,and 18mo（T18） post injection. RESULTS： Repeated measures ANOVA showed an effect of treatment on ETDRS（P〈0.0001）.Post hoc analyses revealed that ETDRS values were significantly increased at T1,T3,T4,T9,and T15（P 〈0.001） as compared to baseline value（T0）.At T6,T12,and T18,ETDRS values were still statistically higher than baseline（P〈0.001 vs T0）.However,at these time points,we observed a trend to return to baseline *** also showed an effect of treatment（P 〈0.0001）.cmT decreased significantly at T1,T3,T4,T9,and T15（P〈0.001）.At T6（P〈0.01）,T12 and T18（P〈0.001） cmT was also significantly lower than T0 although a trend to return to the baseline conditions was also ***： Our findings demonstrate that Intravitreal Dexamethasone Implant is a good option to improveBCVA and cmT in DME patients resistant to anti-VEGF *** data also show that the use of drugs administered directly into the vitreous allows achieving appropriate and long-lasting concentration at the site of disease without systemic side effects.

Effectiveness of 0.1% topical salicylic acid on blepharoconjunctivitis affecting glaucoma patients treated with topical prostaglandin analogues:a prospective randomized trial

AIM: To evaluate the efficacy of 0.1% topical salicylic acid（TSA） to treat iatrogenic chronic blepharoconjunctivitis in patients with primary open angle glaucoma（POAG）, treated with topical prostaglandin analogues（TPAs）.METHODS: Totally 60 patients were randomly distributed into 3 equal size groups, two of which treated with 0.1% TSA（OMKASA;） and 0.1% topical clobetasone butyrate（TCB; VISUCLOBEN;） respectively, and one consisting of untreated controls. The parameters taken into account at baseline（T0） and after 30 d（T1） of therapy were: conjunctival hyperemia, lacrimal function tests [Schirmer I test and break up time（BUT）] and intraocular pressure（IOP）.RESULTS: Conjunctival hyperemia showed a substantial improvement in both treated groups（P<0.001） but not among controls. Similarly, lacrimal function tests displayed an improvement of Schirmer I test in both treated groups（P<0.05） and an extension of BUT only in the group treated with 0.1% TSA（P<0.05）. The IOP increase was statistically significant only in those patients treated with 0.1% TCB（P<0.001）.CONCLUSION: The 0.1% TSA has proved to be an effective anti-inflammatory treatment of blepharoconjunctivitis affecting glaucoma patients on therapy with TPAs, leading to a sizeable decrease of inflammation as well as both quantitative and qualitative improvement of tear film. Furthermore, differently from 0.1% TCB, it does not induce any significant IOP increase.

Neurofibromatosis type 2 is a well known disease of the human skin. Its microscopic and ultramicroscopic features are also well defined. Aim： The aim of this work was to study the involvement of catecholaminergic nerve fibers in the human skin neurofibromatosis. Bioptic fragments of the human skin have been harvested from healthy and diseased subjects. On these specimens the following analysis were performed： 1） light microscopic observation after colouring with hematoxyline-Eosine. 2） lmmunochemical staining for Protein Gene Product 9.5.3） Fluorescent staining for catecholaminergic nerve fibers. 4） Quantitative analysis of images by means of the Quantimet analyzer Leica. 5） Statistical analysis of the quantitative morphological data comparing the healthy with diseased subjects. Comparing the light microscopy images in normal and pathological subjects the authors can affirm that the skin neurofibromatosis induces a strong decrease of the nerve fibers cutted in small pieces and destroyed. Also the catecholaminergic nerve fibers are strongly reduced and destroyed. Quantitative analysis of images and statistical analysis of the morphological data confirm that neurofibromatosis induces strong changes of the skin nerve fibers. The authors＇ results confirm that the neurofibromatosis type 2 induces an almost total destruction of the skin nerve fibers.

Self Consistent Calibration of Detectors and Sources for Hard and Soft X-Ray Diagnostics

X-Ray sources, detectors and optical components are now used in a wide range of applications. What is crucial is the absolute calibration of such devices to permit a quantitative assessment of the system under study. A new X-ray laboratory has been built in Frascati (ENEA) to develop diagnostics for nuclear fusion experiments and study applications of these X-ray techniques in other domains, like new material science, non destructive tests and so on. An in-house developed selfconsistent calibration procedure is described that permits the absolute calibration of sources (X-ray emitted fluxes) and detectors (detection efficiencies) as function of the X-ray photon energy, in the range 2 - 120 keV. The calibration procedure involves the use of an in-house developed code that also predicts the spectral response of any detector in any experimental condition that can be setup in the laboratory. The procedure has been then applied for the calibration and characterisation of gas and solid state imaging detectors, such as Medipix-2, GEM gas detector, CCD camera, Cd-Te C-MOS imager, demonstrating the versatility of the method developed here.

During the last two decades, various local thermal ablative techniques for the treatment of unresectable hepatocellular carcinoma(HCC) have been developed. According to internationally endorsed guidelines, percutaneous thermal ablation is the mainstay of treatment in patients with small HCC who are not candidates for surgical resection or transplantation. Laser ablation(LA) represents one of currently available loco-ablative techniques. In this article, the general principles, technique, image guidance, and patient selection are reported. Primary effectiveness, long-term outcome, and complications are also discussed. A review of published data suggests that LA is equivalent to the more popular and widespread radiofrequency ablation in both local tumor control and long-term outcome in the percutaneous treatment of early HCC. In addition, the LA technique using multiple thin laser fibres allows improved ablative effectiveness in HCCs greater than 3 cm. Reference centres should be equipped with all the available techniques so as to be able to use the best and the most suitable procedure for each type of lesion for each patient.
