Caregiver Burden in the Management of Frail Elderly Patients with Diabetes in Internal Medicine

Aim: Advanced age and fragility often lead to dependence, making caregiver assistance necessary for performing the activities of daily living. Patients with diabetes are characterized by an elevated presence of comorbidities and subsequent frailty—a relevant consideration when making decisions regarding treatment goals. Health professionals may overlook the caregiver experience when assessing the circumstances surrounding frail elderly patients with diabetes, but this factor can have a large impact on relevant caregiver and patient outcomes. Methods: An observational, retrospective study was carried out to assess the impact of improved management in diabetes on caregiver burden. We assessed patients’ performance of activities of daily living using the Barthel scale. Our primary outcome was caregiver burden, as assessed using the Zarit scale. Results: At baseline, 81.4% of the caregivers reported severe overburden (>55 points on the Zarit scale). At three months, the proportion of caregivers reporting this level of burden had dropped to 76.7% and remained stable at six months (77.1%). In contrast, 8.5% of the caregivers reported moderate overburden (47 to 55 points) at baseline, compared to 13.3% at three months and 12.5% at six. The proportion of caregivers reporting no overburden (Conclusions: Caregiver burden remained persistently high, decreasing only slightly after six months.

Background: Both competitive and recreational sports are considered a positive practice that is good for human health and quality of life, so it is difficult to understand how young, energetic and apparently healthy people can die while playing their usual sports. When these events occur in elite athletes during monitored events, they normally receive considerable media attention. Objective: To describe the case of sudden death in a young professional athlete who was successfully aided and resuscitated outside our hospital, and to review the literature about sudden death in sports. Materials and Method: We searched for Sudden Death (according to the International Classification of Diseases (ICD-9 and ICD-10)) in our hospital but found no other relevant cases. We performed a literature search on the state of the art in sudden death (Google Scholar, PubMed, LILACS, Scielo and the Revista Española de Cardiología). Results: Although we observed that the prevalence of sudden death is not very high, it still represents a potentially avoidable event. An adequate medical examination prior to regular sporting practice can decrease the incidence of sudden death in athletes under the age of 35. Data available in the literature support medical check-ups that include history-taking, physical examination and electrocardiogram. The findings of this evaluation may also prompt further assessment (electrocardiograph, ergometry). Conclusions: We could not identify standard, obligatory guidelines for screening possible heart diseases causing cardiorespiratory failure and sudden death. As a measure for primary prevention, we propose a protocol for reducing the risk of sudden death in athletes.

We have already shown that IL-10 plays an important role in immunosuppression and metastatic dissemination in the rat B-cell lymphoma L-TACB *** was suggested that the up-regulation of IL10 production and IL-10 receptor(IL-10R)expression would be part of the transition from primary tumor to metastatic phenotype and that IL-10,besides its immunosuppressive activity,may act as a growth factor for metastatic L-TACB *** treatment of L-TACB-bearing rats with a single low-dose cyclophosphamide decreased IL-10 production,reverted immunosuppression and induced the immunologic rejection of tumor metastasis without any effect on primary tumor *** current aim was to investigate the effects of cyclophosphamide on the expression of IL-10 and IL-10R on primary and metastatic L-TACB *** that cyclophosphamide is a prodrug,we used mafosfamide,a compound that yields in vitro the same active metabolites as cyclophosphamide does in *** induced down-regulation of IL-10 production and IL-10R expression on metastatic cells and,concomitantly,inhibited metastatic cell *** suggest that mafosfamide would inhibit the regulatory loop mediated by the IL-10/IL-10R system and,as a consequence,metastatic cell *** results may have a considerable impact on the design of new therapies for metastatic lymphomas.

Background Forest ecosystems undergo significant transformations due to harvesting and climate fluctuations, emphasizing the critical role of seeding in natural regeneration and long-term structural preservation. Climate change further amplifies these dynamics, affecting phenology across species and regions. In Tierra del Fuego(Argentina), Nothofagus pumilio(lenga) forests represent the most important timber resource, and it is managed through different silvicultural strategies. This species demonstrates notable post-disturbance regeneration, yet seed fall exhibits signifi-cant variability, leading to variations in seed quality(e.g., viability). This study aims to assess fluctuations in N. pumilio seed quality, determine how it varies concerning forest management strategies, annual productivity, and the co-occurrence of climatic phenomena including El Ni?o-Southern Oscillation(ENSO) and the Southern Annular Mode(SAM).Results Viable seeds represented 18.4% of the total, notably higher in unharvested than in managed areas. Con-versely, empty seeds were more prevalent in harvested areas(> 75%). Seed quality exhibited significant differences across silvicultural treatments, except for insect-predated seeds, which had similar proportions across all areas, though dispersed retention showed higher predation. When considering years with varying production levels, high-production years favoured full and viable seeds, particularly in unharvested forests and aggregated retention, while low-production years saw reduced viability across all treatments. Quadratic models revealed that viability increased with seed production, where unharvested forests achieved the highest values. Traumatic brain injury, chronic traumatic encephalopathy, and Alzheimer's disease are three distinct neurological disorders that share common pathophysiological mechanisms involving neuroinflammation. One sequela of neuroinflammation includes the pathologic hyperphosphorylation of tau protein, an endogenous microtubule-associated protein that protects the integrity of neuronal cytoskeletons. Tau hyperphosphorylation results in protein misfolding and subsequent accumulation of tau tangles forming neurotoxic aggregates. These misfolded proteins are characteristic of traumatic brain injury, chronic traumatic encephalopathy, and Alzheimer's disease and can lead to downstream neuroinflammatory processes, including assembly and activation of the inflammasome complex. Inflammasomes refer to a family of multimeric protein units that, upon activation, release a cascade of signaling molecules resulting in caspase-induced cell death and inflammation mediated by the release of interleukin-1β cytokine. One specific inflammasome, the NOD-like receptor protein 3, has been proposed to be a key regulator of tau phosphorylation where it has been shown that prolonged NOD-like receptor protein 3 activation acts as a causal factor in pathological tau accumulation and spreading. This review begins by describing the epidemiology and pathophysiology of traumatic brain injury, chronic traumatic encephalopathy, and Alzheimer's disease. Next, we highlight neuroinflammation as an overriding theme and discuss the role of the NOD-like receptor protein 3 inflammasome in the formation of tau deposits and how such tauopathic entities spread throughout the brain. We then propose a novel framework linking traumatic brain injury, chronic traumatic encephalopathy, and Alzheimer's disease as inflammasomedependent pathologies that exist along a temporal continuum. Finally, we discuss potential therapeutic targets that may intercept this pathway and ultimately minimize long-term neurological decline.

Main environmental variables influencing the abundance of plant species under risk category

Determining climatic and physiographic variables in Mexico’s major ecoregions that are limiting to biodiversity and species of high conservation concern is essential for their ***,at the national level to date,few studies have been performed with large data sets and crossconfirmation using multiple statistical ***,we used 25 endemic,rare and endangered species from 3610 sampling points throughout Mexico and 25 environmental attributes,including average precipitation for different seasons of the year,annual dryness index,slope of the terrain;and maximum,minimum and average temperatures to test our hypothesis that these species could be assessed with the same weight among all variables,showing similar indices of *** results using principal component analysis,covariation analysis by permutations,and random forest regression showed that summer precipitation,length of the frost-free period,spring precipitation,winter precipitation and growing season precipitation all strongly influence the abundance of tropical *** contrast,annual precipitation and the balance at different seasons(summer and growing season)were the most relevant variables on the temperate region *** dry areas,the minimum temperature of the coldest month and the maximum temperature of the warmest month were the most significant *** these different associations in different climatic regions could support a more precise management and conservation plan for the preservation of plant species diversity in forests under different global warming scenarios.
