Colorectal cancer(CRC) is one of the most prevalent malignancies in the world. CRC-associated morbidity and mortality is continuously increasing, in part due to a lack of early detection. The existing screening tools such as colonoscopy, are invasive and yet high cost, affecting the willingness of patients to participate in screening programs. In recent years, evidence is accumulating that the interaction of aberrant genetic and epigenetic modifications is the cornerstone for the CRC development and progression by alternating the function of tumor suppressor genes, DNA repair genes and oncogenes of colonic cells. Apart from the understanding of the underlying mechanism(s) of carcinogenesis, the aforementioned interaction has also allowed identification of clinical biomarkers, especially epigenetic, for the early detection and prognosis of cancer patients. One of the ways to detect these epigenetic biomarkers is the cell-free circulating DNA(circ DNA), a blood-based cancer diagnostic test, mainly focusing in the molecular alterations found in tumor cells, such as DNA mutations and DNA *** this brief review, we epitomize the current knowledge on the research in circ DNA biomarkers-mainly focusing on DNA methylation-as potential blood-based tests for early detection of colorectal cancer and the challenges for validation and globally implementation of this emergent technology.

Neuroendocrine tumors of the gastrointestinal tract are rare neoplasms. Rectal neuroendocrine tumors consist approximately the 5%-14% of all neuroendocrine neoplasms in Europe. These tumors are diagnosed in relatively young patients,with a mean age at diagnosis of 56 years. Distant metastases from rectal neuroendocrine tumors are not very common. Herein we describe a case of a rectal neuroendocrine tumor which metastasized to the lung,mediastinum and orbit. This case underscores the importance of early identification and optimal management to improve patient's prognosis. Therefore,the clinical significance of this case is the necessity of physicians' awareness and education regarding neuroendocrine tumors' diagnosis and management.

加固近乎垂直的边坡,土钉已经被证明是一种多用途和有效的结构形式。钉固结构物的分析涉及到沿着潜在滑面破坏时钉固杆件的抗剪、抗弯、抗拉和抗拔的评价。分析中的关键因素是钉的抗拔力,它与破坏是渐近的。为了增加钉的抗拔力,研制了一种名为“锚固钉”的新型钉。这种钉在嵌入端固定有一个大直径的片状尖头。在大尺寸剪切盒试验仪中,对嵌入在粗颗粒土中的这种类型钉的剪切特性进行了研究。同时,还研究了钉相对于剪切面的倾角变化的影响。试验结果表明:与常规的钉相比,锚固钉呈现出更大的剪切阻力。钉的抗拔试验分有锚头和无锚头嵌入土中两种形式且在不同的正应力下进行,试验结果表明:钉的屈服强度随着锚固端直径的增大而成比例的增大。本文给出了试验室剪切和抗拔试验结果并且讨论了锚固钉的特点。

AIM: To evaluate the effect of chemotherapy to the acute toxicity of a hypofractionated radiotherapy(HFRT) schedule for breast cancer. METHODS: We retrospectively analyzed 116 breast cancer patients with T1, 2N0 Mx. The patients received3-D conformal radiotherapy with a total physical dose of 50.54 Gy or 53.2 Gy in 19 or 20 fractions according to stage, over 23-24 d. The last three to four fractions were delivered as a sequential tumor boost. All patients were monitored for acute skin toxicity according to the European Organization for Research and Treatment of Cancer/Radiation Therapy Oncology Group criteria. The maximum monitored value was taken as the final grading score. Multivariate analysis was performed for the contribution of age, chemotherapy and 19 vs 20 fractions to the radiation acute skin ***: The acute radiation induced skin toxicity was as following: grade Ⅰ 27.6%, grade Ⅱ 7.8% and grade Ⅲ 2.6%. No significant correlation was noted between toxicity grading and chemotherapy(P = 0.154, χ2 test). The mean values of acute toxicity score in terms of chemotherapy or not, were 0.64 and 0.46 respectively(P = 0.109, Mann Whitney test). No significant correlation was also noted between acute skin toxicity and radiotherapy fractions(P = 0.47, χ2test). According to univariate analysis, only chemotherapy contributed significantly to the development of acute skin toxicity but with a critical value of P = 0.05. However, in multivariate analysis, chemotherapy lost its statistical significance. None of the patients during the 2-years of follow-up presented any locoregional ***: There is no clear evidence that chemotherapy has an impact to acute skin toxicity after an HFRT schedule. A randomized trial is needed for definite conclusions.

Our aims were to compare the specificity and sensitivity of the reflux index ( RI), Euler and Byrne score (EBS), and area under the curve (AUC) at pH pathologic gastroesophageal reflux (GE- R) and to identify new cutoffs on formula feedings. We undertook a prospective investigation of extended pH monitoring (EPM) in 117 infants with symptoms of G ER. Infants were categorized as having pathologic reflux by three measures: RI ≥5%, EBS ≥50, or AUC pHpathologic reflux. When EBS and AUC were used, 65 (56%), and 67 (57%) respectively, were classified as having pathologic reflux. The specificities of RI,EBS, and AUC w ere 97.8, 100.0, and 100.0%, respectively, and the sensitivities were 93.0, 91. 5, and 94.4, respectively. A new cutoff value for RI of 2.1%was determined usin g ROC curve analysis to improve the specificity and sensitivity of RI to the abo ve values. The number of infants with pathologic GER is likely to be significant ly less using the traditional RI alone on formula feedings, however, the revised RI cutoff value and AUC analysis by computerized pH tracings can enhance the acc uracy on formula feedings.

Objective: To determine prospectively the incidence of an inlet patch (IP) in children requiring esophagogastroduodenoscopy (EGD) and assess the prevalence of presenting symptoms between children with and without an IP. Study design: All patients undergoing EGD in a 2-year period were assessed for the presence of an IPwith biopsy confirmation. IP, distal esophagus, and stomach biopsy specimens were blindly reviewed by a pathologist for the presence and degree of inflammation and intestinal metaplasia. Symptoms from children with and without an IP were compared. Results: From 407 EGDs done by a single endoscopist, 24 patients had confirmed IP (incidence of 5.9% ). The presence and degree of inflammation were always relatively greater in the columnar mucosa of the IP than in the antral/body gastric mucosa in the same patient (P = .0027)-Inflammation was similar in the squamous epithelium around the IP and in the distal esophagus (P = .46). Two patients had intestinalmetaplasia of the IP. The patients with IPs had a higher prevalence of respiratory symptoms than the control group (P = .03). Conclusions: Children with IPs may have a higher frequency of respiratory symptoms. Periodic surveillance should be performed in children with intestinal metaplasia of an IP.
