Unilateral sacrospinous ligament fixation for treatment of genital prolapse

Background Genital prolapse affects 30% of middle-aged and older women and is becoming a major public health concern. Sacrospinous ligament fixation is an effective and safe procedure for vaginal vault prolaps with a low recurrence and complication rate. This study aimed to investigate the efficacy and safety of unilateral sacrospinous ligament fixation （SSLF） for the management of pelvic organ prolapse （POP）.Methods Forty patients with severe prolapse of pelvic organ undergoing unilateral SSLF were retrospectively *** this study, all patients were staged by the value of POP-Q. All procedures were performed by a senior physician. The characteristics of these patients and their immediate and short-term post-operative outcome were recorded. All patients were seen six weeks and six months after the surgery. The evaluation included standardized questionnaire and site-specific vaginal examination by one *** The average operation time was 65-92 minutes. The average blood loss was 83-188 ml. The average hospitalization time was 6.1 days. The average cost was 5885 yuan. The average day of urethral catheter removal after the operation was 2.1 days. The incidence of postoperative morbidity was 17.1%. One （2.4%） patient had hematoma in the right pelvic. The mean length of postoperative follow-up for 35 patients was 13.1 months. The rate of follow-up was 87.5%. One （2.9%） patient showed recurrent vaginal vault prolapse six months after the surgery. The objective success rate of pelvic organ prolapse was 85% （34/40）. There was significant difference between the POP-Q of Aa, Ba, Ap, Bp,and D before and after operation （P 〈0.001 ）. Five （14.3%） patients complained lower back pain, gluteal pain or right groin pain. Realtime trigger and localization of bursts are the key functions of GECAM,an all-sky gamma-ray monitor launched on 2020 December *** developed a multifunctional trigger and localization software operating in the CPU of the GECAM Electronic *** onboard software has the following features:high trigger efficiency for real celestial bursts with a suppression of false triggers caused by charged particle bursts and background fluctuation,dedicated localization algorithm optimized for both short and long bursts,and low time latency of the trigger information which is downlinked through the Global Short Message Communication service of the global BeiDou navigation *** paper provides a detailed description of the design and development of the trigger and localization software system for *** covers the general design,workflow,the main functions,and the algorithms used in the *** paper also includes on-ground trigger tests using simulated gamma-ray bursts generated by a dedicated X-ray tube,as well as an overview of the performance for real celestial bursts during its in-orbit operation.

Background： The Food and Drug Administration recently announced that the use of morcellation may cause fibroids or pelvic dissemination and metastasis of uterine sarcoma; therefore, the use of morcellation is limited in the USA. A large sample study is necessary to assess the proportion of uterine malignant tumors found in patients with laparoscopic myomectomy. Methods： A national multicenter study was performed in China. From 2002 to 2014, 33,723 cases were retrospectively selected. We calculated the prevalence and recorded the clinical characteristics of the patients with malignancy after morcellation application. A total of 62 cases were finally pathologically confirmed as malignant postoperatively. Additionally, the medical records of the 62 patients were analyzed in details. Results： The proportion of postoperative malignancy after morcellation application was 0.18% （62/33,723） for patients who underwent laparoscopic myomectomy. Nearly 62.9% （39/62） of patients had demonstrated blood flow signals in the uterine fibroids before surgery. And, 23 （37.1%） patients showed rapid growth at the final preoperative ultrasound. With respect to the pathological types, 38 （61.3%） patients had detectable endometrial stromal sarcoma, 13 （21.0%） had detectable uterine leiomyosarcoma, only 3 （3.2%） had detectable carcinosarcoma, and 5 （8.1%） patients with leiomyoma had an undetermined malignant potential. Conclusions： The proportion of malignancy is low after using moreellation in patients who undergo laparoscopie myomectomy. Patients with fast-growing uterine fibroids and abnormal ultrasonic tumor blood flow should be considered for malignant potential, and morcellation should be avoided.

Widespread soil acidification due to atmospheric acid deposition and agricultural fertilization may greatly accelerate soil carbonate dissolution and ***,to date,few studies have addressed these ***,we use meta-analysis and nationwide-survey datasets to investigate changes in soil inorganic carbon(SIC) stocks in *** observe an overall decrease in SIC stocks in topsoil(0-30 cm)(11.33 g C m-2yr-1) from the 1980 s to the 2010 *** SIC stocks have decreased by～8.99±2.24%(1.37±0.37 Pg C).The average SIC losses across China(0.046 Pg C yr-1) and in cropland(0.016 Pg C yr-1) account for～17.6%-24.0% of the terrestrial C sink and 57.1% of the soil organic carbon sink in cropland,*** deposition and climate change have profound influences on SIC *** estimate that～19.12%-19.47% of SIC stocks will be further lost by *** consumption of SIC may offset a large portion of global efforts aimed at ecosystem carbon sequestration,which emphasizes the importance of achieving a better understanding of the indirect coupling mechanisms of nitrogen and carbon cycling and of effective countermeasures to minimize SIC loss.

Mammalian target of rapamycin(m TOR) inhibitor as an attractive drug target with promising antitumor effects has been widely investigated. High quality clinical trial has been conducted in liver transplant(LT) recipients in Western countries. However, the pertinent studies in Eastern world are paucity. Therefore, we designed a clinical trial to test whether sirolimus can improve recurrence-free survival(RFS) in hepatocellular carcinoma(HCC) patients beyond the Milan criteria after LT. This is an open-labeled, single-arm, prospective, multicenter, and real-world study aiming to evaluate the clinical outcomes of early switch to sirolimus-based regimens in HCC patients after LT. Patients with a histologically proven HCC and beyond the Milan criteria will be enrolled. The initial immunosuppressant regimens are center-specifc for the frst 4-6 weeks. The following regimens integrated sirolimus into the regimens as a combination therapy with reduced calcineurin inhibitors based on the condition of patients and centers. The study is planned for 4 years in total with a 2-year enrollment period and a 2-year follow-up. We predict that sirolimus conversion regimen will provide survival benefts for patients particular in the key indicator RFS as well as better quality of life. If the trial is conducted successfully, we will have a continued monitoring over a longer follow-up time to estimate indicator of overall survival. We hope that the outcome will provide better evidence for clinical decision-making and revising treatment guidelines based on Chinese population data.

目的通过中国宫颈癌多中心临床诊疗大数据,探讨我国局部晚期宫颈癌的治疗现状。方法从中国大陆部分地区34家三级甲等医院收集的2004—2016年共30 665例宫颈癌临床诊疗数据中调取局部晚期宫颈癌(ⅠB2期和ⅡA2期)患者4420例,回顾性分析近13年来局部晚期宫颈癌治疗现状。结果 (1)手术治疗者3908例(88.4%),年龄(45.73±8.53)岁;根治性放化疗者312例(7.1%),年龄(50.56±9.90)岁;70例(1.6%)患者未行治疗,130例(2.9%)患者治疗情况不详。(2)4420例局部晚期宫颈癌中3908例患者选择手术治疗,其中有2315例(59.2%)患者采用术前辅助治疗后再行手术治疗,1593例(40.8%)采取直接手术治疗方式;2004—2016年间,辅助治疗后手术者与直接手术者例数均随时间呈先升高-再下降趋势,但是辅助治疗后手术者升高的曲率高于直接手术者;根据术前辅助治疗方式和途径的不同,选择新辅助静脉化疗者1692例(73.1%),其应用率随时间呈逐年升高趋势,其余术前辅助治疗方式应用率均随时间呈逐年下降趋势。(3)312例行根治性放化疗的局部晚期宫颈癌患者,根治性放化疗联合方案有多种,根据化疗功效和时机分为新辅助化疗+根治性放疗方案130例(41.7%),新辅助化疗+同步放化疗方案77例(24.7%),根治性放疗方案43例(13.8%),同步放化疗方案16例(5.1%),同步放化疗+辅助化疗组31例(占9.9%),根治性放疗+辅助化疗方案15例(4.8%)。结论在中国近13年来,局部晚期宫颈癌患者选择手术治疗的患者明显多于选择根治性放化疗的患者。目前我国根治性放化疗方案多样,以先行诱导化疗后再行根治性放疗或根治性同步放化疗的方案最多见。

The Circular Electron Positron Collider(CEPC)is a large scientific project initiated and hosted by China,fostered through extensive collaboration with international *** complex comprises four accelerators:a 30 GeV Linac,a 1.1 GeV Damping Ring,a Booster capable of achieving energies up to 180 GeV,and a Collider operating at varying energy modes(Z,W,H,and tt).The Linac and Damping Ring are situated on the surface,while the subterranean Booster and Collider are housed in a 100 km circumference underground tunnel,strategically accommodating future expansion with provisions for a potential Super Proton Proton Collider(SPPC).The CEPC primarily serves as a Higgs *** its baseline design with synchrotron radiation(SR)power of 30 MW per beam,it can achieve a luminosity of 5×10^(34)cm^(-2)s^(-1)per interaction point(IP),resulting in an integrated luminosity of 13 ab^(-1)for two IPs over a decade,producing 2.6 million Higgs *** the SR power to 50 MW per beam expands the CEPC's capability to generate 4.3 million Higgs bosons,facilitating precise measurements of Higgs coupling at sub-percent levels,exceeding the precision expected from the HL-LHC by an order of *** Technical Design Report(TDR)follows the Preliminary Conceptual Design Report(Pre-CDR,2015)and the Conceptual Design Report(CDR,2018),comprehensively detailing the machine's layout,performance metrics,physical design and analysis,technical systems design,R&D and prototyping efforts,and associated civil engineering ***,it includes a cost estimate and a preliminary construction timeline,establishing a framework for forthcoming engineering design phase and site selection *** is anticipated to begin around 2027-2028,pending government approval,with an estimated duration of 8 *** commencement of experiments and data collection could potentially be initiated in the mid-2030s.

目的探讨ⅡA1期子宫颈癌腹腔镜对比开腹手术的肿瘤学结局及严重并发症。方法该研究是基于真实世界数据的多中心回顾性队列研究,纳入2004年至2016年在中国内地37家医院行腹腔镜或开腹QM-B或QM-C型子宫切除的ⅡA1期子宫颈癌患者,比较腹腔镜与开腹组的肿瘤学结局及严重并发症。结果从46313例中国子宫颈癌临床诊疗大数据库中按入组和排除标准纳入4689例ⅡA1期子宫颈癌患者,其中有生存结局的3595例。真实世界数据分析显示腹腔镜组(n=936)5年总体生存率及无瘤生存率均低于开腹组(n=2659)(总体生存率:80.8%vs 88.2%,P=0.022;无瘤生存率:77.8%vs 81.8%,P=0.009),两组1∶2倾向评分匹配后的结果与真实世界数据相符。严重并发症分析时纳入4689例ⅡA1期子宫颈癌,真实世界数据分析显示腹腔镜组(n=1187)的总严重并发症(5.6%vs 2.5%,P0.05)。二分类logistic回归分析显示,ⅡA1期子宫颈癌腹腔镜广泛性子宫切除是术中及术后严重并发症发生的独立危险因素(HR=2.079,95%CI 1.131~3.819,P=0.018;HR=2.019,95%CI 1.156~3.526,P=0.014),但腹腔镜不影响手术部位严重并发症的发生(P>0.05)。结论在ⅡA1期子宫颈癌手术治疗中,无论是从肿瘤学结局还是从手术严重并发症角度来看,腹腔镜手术都不是一个好的选择。
