Clinical outcomes of patterned laser trabeculoplasty as adjuvant therapy in open angle glaucoma and ocular hypertension

AIM： To assess the efficacy and safety of patterned laser trabeculoplasty（PLT） as an adjunctive treatment in open angle glaucoma（OAG） or ocular hypertension（OHT） patients who were under antiglaucoma medical ***： This study was a retrospective review of primary or secondary OAG patients and OHT patients with medically uncontrolled（≥18 mm Hg） intraocular pressure（IOP） who underwent 360o PLT from June 2016 to August 2016. Follow-up visits at week 1, and 1, 3 and 6 mo were performed. IOP, best corrected visual acuity（BCVA）, complications and eye drop glaucoma medication were recorded at each follow-up visit. Success was defined as IOP reduction ≥20% from baseline. RESULTS： Forty-one eyes of 25 patients were included in this study. Pre-treatment mean IOP was 20.2±1.6 mm Hg. After PLT, IOP was 19.3±5.2, 16.1±2.7, 17.1±3.7 and 16.3±3.5 mm Hg,at 1 wk, 1, 3 and 6 mo, respectively. IOP reduction from baseline was statistically significant from the first month, remaining stable at 6 mo（P〈0.001）. PLT success at 6 mo of follow-up was 48.78%. The number of glaucoma medication per eye（P=0.10） and the mean BCVA both remained constant（P=0.37）. Complications included transient IOP spikes in 4 eyes（9.8%） and peripheral anterior synechiae in 7 eyes（17.1%）. CONCLUSION： PLT is an effective and safe method for the management of patients with OHT or OAG as an adjunctive therapy. Additional larger studies should be designed to verify the long-term stability of IOP reduction with this laser technology.

The use of systems theory to attempt to determine which ＂place in La Mancha＂ was the one whose name Cervantes could not quite recall in his universal novel, appears to be ordained to change certain attitudes towards science held in Cervanfine literature. The reason： after four centuries of literary analysis, systemic methodology has proven able to identify the famous ＂place＂ with acceptable accuracy. Nonetheless, certain reasonable doubts persist around the suitability of a strictly scientific analysis of literature. In light of those doubts, the present article aims primarily to facilitate critique both of the systemic approach adopted and its outcome.

AIM:To describe the safety and efficacy of patterned laser trabeculoplasty(PLT)as an adjunctive treatment in primary open angle glaucoma(POAG)and ocular hypertension(OHT)after 18-month follow-up in Hispanic ***:A single-center,retrospective study was *** patients with OHT or POAG undergoing PLT from June 2016 to August 2016 were included in the *** parameters were intraocular pressure(IOP),the number of IOP-lowering medications,best corrected visual acuity(BCVA),laser parameters and postoperative adverse *** efficacy outcome measures were the proportion of eyes achieving an IOP reduction≥20%at 18 mo versus baseline medicated IOP or a reduction in the number of medications while maintaining IOP ***:From 40 PLT-treated eyes(mean baseline IOP 20.3±1.7 mm Hg),24 patients were analyzed(age 63.4±7.3 y).The mean IOP reductions from baseline across visits(months 1,3,6,9,12,and 18)ranged from 14.1%to 20.8%.Success rate after 18-month follow-up was 61.7%with a mean IOP of 16±3.2 mm Hg(P<0.001).The number of glaucoma IOP-lowering medications per eye(preoperative 2.1±1.1 and postoperative 2.3±1.1,P=0.86)and the mean BCVA(preoperative 0.10±0.22 and postoperative 0.11±0.22 logMAR,P=0.42)remained *** events comprised transitory IOP spikes in 4 eyes(10%)and peripheral anterior synechiae in 7 eyes(17.5%).CONCLUSION:Mid-term results of PLT show that this procedure may be an efficacious and safe technique to approach medically uncontrolled OHT or POAG patients.

This work has two main purposes：（i） introducing the basic concepts of molecular dynamics analysis to material scientists and engineers, and （ii） providing a better understanding of instrumented indentation measurements, presenting an example of nanoindentation and scratch test simulations. To reach these purposes, three-dimensional molecular dynamics （MD） simulations of nanoindentation and scratch test technique were carried out for generic thin films that present BCC crystalline structures. Structures were oriented in the plane （100） and placed on FCC diamond substrates. A pair wise potential was employed to simulate the interaction between atoms of each layer and a repulsive radial potential was used to represent a spherical tip indenting the sample. Mechanical properties of this generic material were obtained by varying the indentation depth and dissociation energy. The load-unload curves and coefficient of friction were found for each test; on the other hand, dissociation energy was varied showing a better mechanical response for films that present grater dissociation energy. Structural change evolution was observed presenting vacancies and slips as the depth was varied.

Given a significative class F of commutative rings, we study the precise conditions under which a commutative ring R has an S-envelope. A full answer is obtained when F is the class of fields, semisimple commutative rings or integral domains. When .F is the class of Noetherian rings, we give a full answer when the Krull dimension of R is zero and when the envelope is required to be epimorphic. The general problem is reduced to identifying the class of non-Noetherian rings having a monomorphic Noetherian envelope, which we conjecture is the empty class.

Patent foram ovale(PFO)is the most common anatomical cause of an interarterial *** is usually asymptomatic but may cause paradoxical embolism and is a risk factor for non-lacunar cryptogenic cerebral ischemia in young *** the first clinical trials did not show a significant superiority of PFO closure in the secondary prevention of cerebral ischemia as compared with standard antithrombotic treatment,six subsequent randomized clinical trials(CLOSURE I,PC Trial,RESPECT,CLOSE,REDUCE,and DEFENSE-PFO)performed in a sample of cryptogenic stroke in patients aged 60 years or younger provided evidence of a significant reduction of recurrent cerebral ischemia after percutaneous PFO ***,the use of percutaneous PFO closure cannot be generalized to the entire population of patients with cerebral ischemia and PFO,but it is indicated in highly selected patients with non-lacunar cryptogenic cerebral infarction with a large right-to-left shunt,an atrial septal aneurysm and no evidence of atrial fibrillation,as well as in association with antithrombotic treatment for an optimal secondary prevention of cerebral ischemia.

A multi-fingered robotic hand with curved fingertips enables contact re-positioning without reattaching at the expense of fingertip rolling. This rolling stands for a characteristic that facilitates dexterous manipulation but results in an algebraically complex dynamic model subject to such constraints. The hemispherical shape of fingertips allows a dexterous manipulation when controlling the tangent forces, which are essential to rotate object. However, the measurement of the object angle in practice requires tactile-optical sensing. In this paper, considering robotic fingers with curved soft tips, we propose a feedback control that ensures optimal dynamical grasping of a circular rigid object. It is shown that the collaboration of the contact forces, to get a minimum pose of internal forces, and the tangential forces, to induce the conditions for assuring the grasp closure, is necessary to get a skillful manipulation. In this case, the orientation control of a circular object to the desired angle while avoiding direct measurement of the object angle is presented. Stability conditions of the system are presented in the sense of stability-in-the-manifold. Finally, representative simulations are shown and discussed.
