OBJECTIVE: To assess the indications, efficacy and clinical significance of percutaneous cryoablation for liver carcinoma after transcatheter liver artery chemoembolization (TACE). METHODS: Thirty-four patients with histologically or clinically confirmed primary or metastatic carcinomas were treated with TACE. One week to 1 month later, they were treated percutaneously under ultrasound guidance using cryosurgical system in the period of July 2001 -June 2002. All patients were followed up to determine serum tumor marker, CT scans, MRI images or ultrasound images. RESULTS: This therapy was performed in 34 patients including 32 patients with Child A liver reserve, 2 patients with Child B and no patient with Child C. There were 28 patients with primary liver cancer and 6 patients with metastatic liver cancer. During the follow-up period (3 to 15 months), 41.1% patients were recognized clinically cured because the serum tumor markers became normal, or CT scans and MRI images revealed that the lesion became completely necrotic. 44.1% patients were recognized effectively treated. CONCLUSIONS: Percutaneous cryoablation combined with TACE is a choice of treatment for liver carcinoma. It is minimally invasive, safe and effective for those patients with liver cancer unsuitable for surgery.

Objective: To assess the influence of preoperative transcatheter arterial chemoembolization (TACE) on liver resection in patients with resectable hepato- cellular carcinoma (HCC). Methods: Of 126 patients with resectable HCC, 62 received preoperative TACE (TACE group) and the remaining 64 patients were selected as controls (non- TACE group). Perioperative risk factors including liver function alteration, mean blood loss during op- eration, mean time of clamping the porta hepatis, length of operation, postoperative abdominal drain- age at day 1, 2 and 3, morbidity and mortality were compared between the two groups. Results: Neither significant difference in liver func- tion alteration nor mortality was observed between the two groups. More severe hepatic cirrhosis, lon- ger operation time, more blood loss and postopera- tive abdominal drainage were noted in the TACE group than in the non-TACE group. There was no significant difference in postoperative morbidity be- tween the two groups. Conclusions: Preoperative TACE for resectable HCC increases surgical difficulty and risk, and therefore should be considered prudently according to the indi- viduality of patients.

Objective To evaluate the clinical efficacy of the combined treatment with transcatheter arterial chemoembolization (TACE) and percutaneous ethanol injection (PEI) on hepatocellular carcinoma (HCC) .Methods 312 patients with moderate or advanced HCCs were divided into two groups; 170 cases underwent TACE treatment alone, 142 cases were treated with TACE and PEI under B-ultrasound guidance.Results The rates of reduction in tumor diameter and the decline in serum AFP level were 41.2% and 40.4% in the TACE group and 75.4% and 74.1 % in the TACE + PEI group respectively. The 6, 12 and 24 months survival rates in the TACE group were 77.1 % , 34.1% and 18.8% ,respectively and in the TACE + PEI group 87.3% , 62.0% and 38.0% , respectively. Overall, there was a significant difference between the two treatment groups ( P han TACE alone.

Objective: To study the indications for resection ofvery big primary liver cancer and the operative ***: From January 1985 to June 1996, 86 pa-tients with very big primary liver cancer (≥15cm indiameter) underwent hepatectomy in our *** volume of bleeding and blood transfusion wasrecorded during the operation. After the operation,the draining quantity from their abdominal cavities,and the days of transfusion and hospitalization wererecorded. The occurrence of complications and sur-vival time of the patient were followed ***: The postoperative mortality was 3.48% andthe occurrence rate of complications was 31.40%, whichwas significantly correlated with preoperative lowerlevel of serum albumin or the elevated γ-globulin lev-el and the amount of resected liver tissue. But their liverfunction before operation was fairly good, the 1-, 3-and 5-year survival rates after hepatectomy were58.2%, 35.7% and 17.64%.Conclusions: Patients with very big primary liver can-cer, should be subjected to hepatectomy if their liverfunction before operation are normal and the marginsare distinct between the tumor and liver tissues. Afterthe operation, other treatments are suitable for goodeffects.

OBJECTIVES: To detect the expression of human alpha-fetoprotein mRNA (AFPmRNA) in peripheral blood (PBL) of recurrent hepatocellular carcinoma (HCC) by using nested reverse transcriptase polymerase chain reaction (nested RT-PCR) and to discuss relations between AFPmRNA and recurrent HCC. METHODS: Five ml peripheral blood samples were obtained from 19 patients with recurrent HCC. To identify HCC cells in PBL, liver-specific AFPmRNA was amplified from the total RNA extracted from the whole blood by nested RT-PCR. RESULTS: Seven patients (36.8%) with recurrent HCC were detected AFPmRNA, and 12 patients with recurrent HCC were AFPmRNA negative (63.2%). There was no significant relation between patients with AFPmRNA positive and AFPmRNA negative (P>0.05) in judging the origin of recurrent HCC. CONCLUSION: AFPmRNA may not be used as a marker of origin of recurrent HCC.
