Background:This study aimed to develop a comprehensive instrument for evaluating and ranking clinical practice guidelines,named Scientific,Transparent and Applicable Rankings tool(STAR),and test its reliability,validity,and ***:This study set up a multidisciplinary working group including guideline methodologists,statisticians,journal editors,clinicians,and other *** review,Delphi methods,and hierarchical analysis were used to develop the STAR *** evaluated the instrument’s intrinsic and interrater reliability,content and criterion validity,and ***:STAR contained 39 items grouped into 11 *** mean intrinsic reliability of the domains,indicated by Cronbach’sαcoefficient,was 0.588(95%confidence interval[CI]:0.414,0.762..Interrater reliability as assessed with Cohen’s kappa coefficient was 0.774(95%CI:0.740,0.807)for methodological evaluators and 0.618(95%CI:0.587,0.648)for clinical *** overall content validity index was ***’s r correlation for criterion validity was 0.885(95%CI:0.804,0.932..The mean usability score of the items was 4.6 and the median time spent to evaluate each guideline was 2. ***:The instrument performed well in terms of reliability,validity,and efficiency,and can be used for comprehensively evaluating and ranking guidelines.

The number ofψ(3686)events collected by the BESⅢdetector during the 2.2. run period is determined to be(2.59.3±11.1)×10~6 by counting inclusiveψ(3686)hadronic *** uncertainty is systematic and the statistical uncertainty is ***,the numbers ofψ(3686)events collected during the 2.09 and 2.12.un periods are updated to be(107.7±0.6)×10~6 and(345.4±2.6)×10~6,*** numbers are consistent with the previous measurements within one standard *** total number ofψ(3686)events in the three data samples is(2.12.4±14.3)×10~6.

The prevalence of SARS-CoV-2.variants of concern(VOCs) is still escalating throughout the world. However, the level of neutralization of the inactivated viral vaccine recipients’ sera and convalescent sera against all VOCs,including B.1.1.7(Alpha), B.1.351(Beta), P.1(Gamma), B.1.617.2.Delta), and B.1.1.52.(Omicron) remains to be lack of comparative analysis. Therefore, we constructed pseudoviruses of five VOCs using a lentiviral-based system and analyzed their viral infectivity and neutralization resistance to convalescent and BBIBP-CorV vaccinee serum at different times. Our results show that, compared with the wild-type strain(WT), five VOC pseudoviruses showed higher infection, of which B.1.617.2.and B.1.1.52. variant pseudoviruses exhibited higher infection rates than wild-type or other VOC strains, respectively. Sera from 10 vaccinated individuals at the 1, 3and 5-month post second dose or from 10 convalescent at 14 and 2.0 days after discharge retained neutralizing activity against all strains but exhibited decreased neutralization activity significantly against the five VOC variant pseudoviruses over time compared to WT. Notably, 100%(30/30) of the vaccinee serum samples showed more than a 2.5-fold reduction in neutralizing activity against B.1.1.52., and 90%(18/2.) of the convalescent serum samples showed more than 2.5-fold reduction in neutralization against B.1.1.52.. These findings demonstrate the reduced protection against the VOCs in vaccinated and convalescent individuals over time, indicating that it is necessary to have a booster shot and develop new vaccines capable of eliciting broad neutralization antibodies.

Percent free prostatic-specific antigen （%fPSA） has been introduced as a tool to avoid unnecessary biopsies in patients with a serum PSA level of 4.0-10.0 ng ml^-1, however, it remains controversial whether %fPSA is effective in PSA range of 10.1-2..0 ng ml^-1 in both Chinese and Western population. In this study, the diagnostic performance of %fPSA and serum PSA in predicting prostate cancer （PCa） and high-grade PCa （HGPCa） was analyzed in a multi-center biopsy cohort of 5915 consecutive Chinese patients who underwent prostate biopsy in 2. hospitals across China from January 1, 2.10 to December 31, 2.13. The indication for biopsy was PSA〉4.0 ng ml^-1 or/and suspicious digital rectal examination. Total and free serum PSA determinations were performed by three types of electrochemiluminescence immunoassays with recalibration to the World Health Organization standards. The diagnostics accuracy of PSA, %fPSA and %fPSA in combination with PSA （%fPSA ＋ PSA） was determined by the area under the receivers operating characteristic curve （AUC）. %fPSA was more effective than PSA in men aged ≥60 years old. The AUC was 0.584 and 0.635 in men aged ≥60 years old with a PSA of 4.0-10.0 ng ml^-1 and 10.1-2..0 ng ml^-1, respectively. The AUC of %fPSA was superior to that of PSA in predicting HGPCa in patients ≥60 years old in these two PSA range. Our results indicated that %fPSA is both statistically effective and clinical applicable to predict prostate biopsy outcome in Chinese patients aged ≥60 years old with a PSA of 4.0-10.0 ng ml^-1 and 10.1-2..0 ng ml^-1.

New threats posed by the emerging circulating variants of SARS-CoV-2.highlight the need to find conserved neutralizing epitopes for therapeutic antibodies and efficient vaccine ***,we identified a receptorbinding domain(RBD)-binding antibody,XG014,which potently neutralizesβ-coronavirus lineage B(β-CoV-B),including SARS-CoV-2.its circulating variants,SARSCoV and bat SARSr-CoV ***,antibody family members competing with XG014 binding show reduced levels of cross-reactivity and induce antibodydependent SARS-CoV-2.spike(S)protein-mediated cellcell fusion,suggesting a unique mode of recognition by *** analyses reveal that XG014 recognizes a conserved epitope outside the ACE2.binding site and completely locks RBD in the non-functional“down”conformation,while its family member XG005 directly competes with ACE2.binding and position the RBD“up”.Single administration of XG014 is effective in protection against and therapy of SARS-CoV-2.infection in *** findings suggest the potential to develop XG014 as pan-β-CoV-B therapeutics and the importance of the XG014 conserved antigenic epitope for designing broadly protective vaccines againstβ-CoV-B and newly emerging SARS-CoV-2.variants of concern.

Dear Editor,Signet-ring cell carcinoma(SRCC)is a rare subtype of colorectal cancer(CRC)characterized histologically by the accumulation of mucins in the cytoplasm and displacement of nuclei to the cellular periphery,accounting for about 1%CRC(Fig.S1A)(Borger et al.,2.07).Compare to common subtypes of CRC,such as adenocarcinoma(AC)and mucinous adenocarcinoma(MAC),SRCC is associated with aggressive behaviors and younger age at presentation(Kang et al.,2.05;Sung et al.,2.08;Nitsche et al.,2.13;Hugen et al.,2.14;Inamura et al.,2.15).A retrospective analysis of CRC patient's data at fudan university shanghai Cancer Center(FUSCC)also indicated a worse overall and disease-free survival of SRCC patients(Fig.S1B and S1C,Table S1).

Introduction The incidence of hyperglycemia is 40-60%in critically ill patients and is up to 60-80%in those who have undergone car-diac surgery.[1]The results of an epidemiological study in the United States showed that 2..6%of diabetic patients and 9.3%of non-diabetic patients had elevated mean daily glucose on the day of ICU admission.[2.In critically ill patients,elevated blood glucose is primarily the result of stress,and stress-induced hy-perglycemia is an independent risk factor associated with prog-nosis,regardless of a previous diagnosis of *** therapy has become an integral treatment option for patients in the ICU,[3,4]though nearly 30%of patients with enteral nu-trition and 44-50%with parenteral nutrition(PN)experience elevated glucose.[5,6]Intensive insulin therapy(IIT)is an impor-tant treatment for controlling hyperglycemia in critically ill pa-tients,but it also carries a corresponding risk of hypoglycemia.

目的系统评价铝碳酸镁联合幽门螺杆菌(H. pylori)根除治疗H. pylori相关性胃溃疡及铝碳酸镁联合质子泵抑制剂(PPI)治疗内镜黏膜下剥离术(ESD)后人造溃疡的有效性。方法计算机检索CBM、CNKI、Wanfang Data、VIP、Pub Med、Embase、Web of Science、Scopus及The Cochrane Library数据库,纳入联用铝碳酸镁治疗H. pylori相关性胃溃疡及联用铝碳酸镁治疗ESD术后人造溃疡的随机对照试验(RCTs),筛选文献、提取数据并评价偏倚风险后,采用Rev Man 5. 3统计软件进行Meta分析。结果①H. pylori相关性胃溃疡:与单独应用H. pylori根除相比,铝碳酸镁联合H. pylori根除治疗能降低溃疡的1年后复发率(ITT分析:RR=0. 48,95%CI:0. 36～0. 65,P 2. 92.95%CI:10. 15～15. 69,P <0. 001)。结论目前证据表明,在H. pylori根除治疗的基础上联用铝碳酸镁有助于降低H. pylori相关性胃溃疡的1年后复发率;在PPI基础上联用铝碳酸镁有助于加快ESD术后人造溃疡的愈合。
