Summary This recent metaanalysis included an evaluation of nine randomized controlled trials that studied the rehabilitation of geriatric patients who had suffered a hip fracture. Using pooling of the aggregate data from each study, functional improvements were significantly better for those patients who received a multidisciplinary inpatient rehabilitation program compared with those who received usual care, both at discharge and at longer term follow-up. There was less risk of nursing home admission and also of mortality in the rehabilitated patients compared with the usual care patients.

AIM: To study the mechanisms by which Campylobacter jejuni (C. jejuni) causes inflammation and diarrhea. In particular, direct interactions with intestinal epithelial cells and effects on barrier function are poorly under- stood. METHODS: To model the initial pathogenic effects of C. jejuni on intestinal epithelium, polarized human colonic HCA-7 monolayers were grown on permeabilized filters and infected apically with clinical isolates of C. jejuni. Integrity of the monolayer was monitored by changes in monolayer resistance, release of lactate dehydrogenase, mannitol fluxes and electron microscopy. Invasion of HCA-7 cells was assessed by a modified gentamicin protection assay, translocation by counting colony forming units in the basal chamber, stimulation of mediator release by immunoassays and secretory responses in monolayers stimulated by bradykinin in an Ussing chamber. RESULTS: All strains translocated across monolayers but only a minority invaded HCA-7 cells. Strains that invaded HCA-7 cells destroyed monolayer resistance over 6 h, accompanied by increased release of lactate dehydrogenase, a four-fold increase in permeability to [3H] mannitol, and ultrastructural disruption of tight junctions, with rounding and lifting of cells off the filter membrane. Synthesis of interleukin (IL)-8 and prostaglandin E2 was increased with strains that invaded the monolayer but not with those that did not. CONCLUSION: These data demonstrate two distinct effects of C. jejuni on colonic epithelial cells and provide an informative model for further investigation of initial host cell responses to C. jejuni.

Assessment of public Health Risks Associated with Naphthalene Entering Residences and Commercial Space from Vapor Intrusion at MGp Sites

Naphthalene, a constituent of coal tar, is a contaminant frequently found at former manufactured gas plants (MGp). Development at these sites has resulted in residential and commercial areas with potential exposures from vapor intrusion adversely affecting indoor air of residences and buildings. Naphthalene is routinely analyzed in soil vapor intrusion assessments for properties overlying and surrounding former MGp sites. The United States Environmental protection Agency (EpA) has a proposed unit risk factor and the State of California has promulgated a unit risk factor for naphthalene. Naphthalene exposure from vapor intrusion is potentially a public health risk. The purpose of this study was to evaluate three sites located in the northeast United States to determine the frequency of naphthalene detection in indoor air. A total of 79 properties were included in the study. A total of 546 indoor air samples were analyzed for naphthalene on 161 occasions. Naphthalene concentrations ranged from 0.26 to 51 μg/m3 of air. Only 3 of the 546 indoor air samples detected naphthalene above the ninety-fifth percentile background value of 12 μg/m3 of air. Risk analysis indicated naphthalene vapor intrusion was not a public health risk among the 79 properties built on or near the former MGp sites.
