Clinico-Epidemiological profile and Diagnostic procedures of Tuberculosis in Children: A Retrospective Study from OpD of Tertiary Care Center in Bangladesh

Background: Childhood Tuberculosis (TB) is an important cause of mortality and morbidity, which causes a significant TB burden in developing countries. Objectives: The aim of this study was to describe clinical profile and diagnostic procedures used in Outpatient Department (OpD) of a tertiary care hospital. Methods: It was a retrospective study conducted in OpD of paediatrics Department, Bangabandhu Sheikh Mujib Medical University, Shahbag, Dhaka, Bangladesh. Age range of studied populations, 3 months to 18 years who presented with clinical features of TB and were later diagnosed with tuberculosis according to national guidelines for the management of tuberculosis in children, 2021 were enrolled in this study. A total of 108 diagnosed cases of Tuberculosis between August 2021 and July 2022 were enrolled in this study. Data were collected from OpD records and transferred into MS excel sheet for data processing & analysis. Results: Among 834 suspected patients, 108 patients aged 3 months to 18 years were diagnosed with tuberculosis. The highest number of cases 47 (43.51%) were found in 11 - 18 years age group. Gender distribution of the patient showed 58 (53.70%) were male and 50 (46.29%) were female. Among 108 TB cases, 44 (40.74%) were pulmonary TB and 64 (59.25%) were extrapulmonary TB. Among 64 extrapulmonary TB cases 38 (59.37%) cases were diagnosed as TB lymphadenitis. Fever 83 (76.85%), weight loss 62 (57.4%), cough 50 (46.29%), lump in neck and axilla 38 (35.18%) were found in most of the cases. A positive Mantoux test was found in 68 (62.96%) patients. Chest X-ray findings showed patchy opacity and consolidation in 46 (42.59%) cases. Suggestive FNAC from lymph node was observed among 35 (32.4%) cases. For bacteriological confirmation sputum gene Xpert and stool Xpert ultra were positive among 10 (9.25%) and 23 (21.29%) cases. Ever since the world has been engulfed by the COVID-19 pandemic,caused by SARS-CoV-2 virus,humanity has gone through untold *** has been a widespread impact on health,including substantial mortality and morbidity,especially among older adults and those with pre-existing health conditions[1,2],in addition to serious repercussions for the global economy,and social *** this devastating spread of the virus,a large number of the population remains non-immune to SARS-CoV-2[3].With no approved cure in sight,hopes are pinned on safe and effective vaccines and their equitable distribution and acceptance for reigning in the *** vaccination campaign for COVID-19 in India was started on 16th January,2021 using two vaccines,first with Covishield(AstraZeneca and Serum Institute of India)and later along with Covaxin(BBV152,Bharat Biotech and ICMR).Initially,health care workers and other front line workers were prioritized for vaccination,and then vaccinations for COVID-19 were started for all adults in *** intended to find out the number of breakthrough COVID-19 infections after vaccination in a tertiary care hospital,Kolkata,India.

Comparative Study of Radiological Changes in Hands and Feet in patients Suffering from Early Rheumatoid Arthritis by power Doppler Ultrasound and Direct Digital Radiography

Rheumatoid arthritis is a chronic multisystem disease of unknown cause. The characteristic feature of RA is persistent inflammatory synovitis. The natural history of disease is such that the early months of the disease are critical period during which reversible joint damage occurs. So early diagnosis of RA and appropriate drug application is the only way to save a patient from this crippling disease. In India, the cost of investigations is a significant factor for most of the patients. Ultrasonography or power Doppler Ultra Sound (pDUS) has the advantage of being economic in spite of its sensitivity in assessing both inflammatory and destructive changes. The aim of the present study was to evaluate the diagnostic efficiency of pDUS in early rheumatoid arthritis. The study was performed with the patients attending Rheumatology Clinic. A total number of 106 patients of clinically suspected rheumatoid arthritis were studied as per selection criteria. Radiological examinations of hands were done by digital radiography and pDUS in a group of 53 patients, assessment of foot changes by pDUS and Digital Radiography were done in another similar group of 53 patients. Final diagnosis by ACR EULAR-2010 criteria is done for all the patients. The comparative study reveals that synovial vascularity as demonstrated by pDUS is much more effective in diagnosing early rheumatoid arthritis, both in hand and in feet than digital radiograph. pDUS of feet may yield earlier and better findings than hands, which is conventionally used in patients suffering from early rheumatoid arthritis.

Mode-locked 2 μm fiber laser with a multi-walled carbon nanotube as a saturable absorber

We propose and demonstrate a passively mode-locked fiber laser operating at 1951.8 nm using a commercial thulium-doped fiber （TDF） laser, a homemade double-clad thulium-ytterbium co-doped fiber （TYDF） as the gain media, and a multi-walled carbon nanotube （MWCNT） based saturable absorber （SA）. We prepare the MWCNT composite by mixing a homogeneous solution of MWCNTs with a diluted polyvinyl alcohol （pVA） polymer solution and then drying it at room temperature to form a film. The film is placed between two fiber connectors as a SA before it is integrated into a laser ring cavity. The cavity consists of a 2 m long TDF pumped by a 800 nm laser diode and a 15 m long homemade TYDF pumped by a 905 nm multimode laser diode. A stable mode-locking pulse with a repetition rate of 34.6 MHz and a pulse width of 10.79 ps is obtained when the 905 nm multimode pump power reaches 1.8-2.2 W, while the single-mode 800 nm pump power is fixed at 141.5 mW at all times. To the best of our knowledge, this is the first reported mode-locked fiber laser using a MWCNT-based SA.

COVID-19 in Children: Experience from a Tertiary Care Hospital in Bangladesh

Background: Children are generally at low risk of infection and data on COVID-19 in children are scarce. This study was undertaken to document data from a tertiary care hospital in Bangladesh on the incidence, presentation and immediate outcome of COVID-19 in children. Methodology: It was a cross-sectional study conducted in the fever clinic and the department of pediatrics, Bangabandhu Sheikh Mujib Medical University (BSMMU) over a period of one year from July 2020 to June 2021. All the RT–pCR positive pediatric cases (1 month to 18 years) were enrolled. Data were collected by a structured, predesigned and pre-tested questionnaire. Data were analyzed using Microsoft Excel spreadsheet software 2010. Frequency, percentage and chi-square tests were done for statistical analysis. Result: Among 8203 suspected pediatric COVID-19 cases, 850 (10.36%) was RT pCR positive. The highest number of RT-pCR positive COVID-19 cases was found in April 2021. More than 63% of cases were in the age group of 10 to 18 years. Male: female ratio was 0.97:1. Fever was the most common (75.9%) presenting feature followed by breathing difficulty and chest pain in 9.6% and 7% of pan style="font-family: ">patients respectively. Mild infection group was predominant (56.7%). Among the symptomatic cases, 16.7% were hospitalized. Oxygen inhalation and bronchodilators were needed in 75% of hospitalized patients. All the severe and critical cases were managed in the inpatient department and in pediatric pICU when indicated. Most of the patients (99.4%) improved with or without any complication. Four critically ill patients (0.75%) developed multi-organ dysfunction and expired in pICU. Conclusion: pan style="font-family: ">The majority of COVID-19 cases belonged to the older age group. More than 75% cases were symptomatic. History of contact was present in 78% cases. More than 99% children recovered with or without sequelae and 0.8% children with pre-existing co-morbidities expired.

Genomic prediction of Fusarium head blight resistance in early stages using advanced breeding lines in hard winter wheat

Fusarium head blight(FHB),also known as scab,is a devastating fungal disease of wheat that causes significant losses in grain yield and *** inheritance and cumbersome phenotyping make FHB resistance a challenging trait for direct selection in wheat *** selection to predict FHB resistance traits has shown promise in several ***,we used univariate and multivariate genomic prediction models to evaluate the prediction accuracy(pA)for different FHB traits using 476elite and advanced breeding lines developed by South Dakota State University hard winter wheat breeding *** breeding lines were assessed for FHB disease index(DIS),and percentage of Fusarium damaged kernels(FDK)in three FHB nurseries in 2018,2019,and 2020(Tp18,Tp19,and Tp20)and were evaluated as training populations(Tp)for genomic prediction(Gp)of FHB *** observed a moderate pA using univariate models for DIS(0.39 and 0.35)and FDK(0.35 and 0.37)using Tp19 and Tp20,respectively,while slightly higher pA was observed(0.41 for DIS and 0.38 for FDK)when Tp19 and Tp20(Tp19+20)were combined to leverage the advantage of a large training *** Gp with multivariate approach including plant height and days to heading as covariates did not significantly improve pA for DIS and FDK over univariate models,pA for DON increased by 20%using DIS,FDK,DTH as covariates using multi-trait model in ***,we used Tp19,Tp20,and Tp19+20 in forward prediction to calculate genomic-estimated breeding values(GEBVs)for DIS and FDK in preliminary breeding lines at an early stage of the breeding *** observed moderate pA of up to 0.59 for DIS and 0.54for FDK,demonstrating the promise in genomic prediction for FHB resistance in earlier stages using advanced *** results suggest Gp for expensive FHB traits like DON and FDK can facilitate the rejection of highly susceptible materials at an early stage in a breeding program.

Unilateral Diaphragmatic Agenesis with Lung Cyst Herniating into the Abdomen precluding Diaphragmatic Hernia in an Adult

Background: Diaphragmatic agenesis or complete absence of a hemidiaphragm in adulthood is rare. The significance of presence of a lung cyst occupying the common thoracoabdominal space lies in the absence of associated diaphragmatic hernia which has not been documented previously. Case presentation: We report a case of previously undiagnosed complete absence of a hemidiaphragm in a 62-year-old male who presented with recurrent respiratory infection, episodes of nausea and postprandial fullness. Computed Tomography scan showed a large cyst in relation to the lower lobe of left lung. During surgery, he was found to have complete absence of a left hemidiaphragm and a large cyst arising from the lower lobe of left lung occupied the common thoracoabdominal space. Abdominal viscera were found displaced downwards and fixed in a thick fibrous membrane. There was no herniation of abdominal organs into the thoracic cavity after excision of the cyst. The left lung could be fully expanded and chest was closed after putting a chest drain. Diaphragmatic agenesis did not require any additional procedure. The cyst was excised with closure of bronchocystic opening. The patient had an uneventful recovery. Conclusions: The association of lung cyst with diaphragmatic agenesis (presented in the adulthood) is rare. It has precluded a diaphragmatic hernia and survived the patient acting as a protective barrier. The significance of lung cyst in presence of diaphragmatic agenesis lies in the absence of associated diaphragmatic hernia which has not been documented previously.

This paper is a study on the compulsion of transformational leadership from the part of the principal of this era when the demography of the students are changing exponentially, when the globe is experiencing massive changes in methods and techniques in educational process but the institutions remain the same as its ethics and operational strategies. The time is eloping when an educational head earns praises due to their prominence and performance in managerial functioning. But this paper tends to focus on several persisting challenges in these educational institutions like huge gap between teacher-student ratio, students’ increasing technological oriented learning, teachers’ lack of knowledge of multimedia class operation, girls’ little rate of enrolment in higher level of education, early marriage of school going girls and ensuring quality education to girls as per the demand of SDG 4 by 2030. This paper fosters the idea that a transformational head of the institution can be successful in this chase of challenges as he involves everyone to activities in order to reach goal. He works with the other members as a team member and thus a challenge of an institution turns to be liability to all.

A Tm-Bi Co-Doped Fiber Laser with Dual pumping Operation

We demonstrate an efficient fiber laser operating at 1901.6 nm using a newly developed thulium bismuth co-doped fiber(TBF)with dual pumping at 792 nm and 1552 *** fiber was fabricated using modified chemical vapor deposition and solution doping *** dopant concentrations(wt%)and compositions inside the core are 0.15 Bi_(2)O_(3),0.3 Tm_(2)O_(3),1.0 Al_(2)O_(3) and 12.0 GeO_(2).The TBF laser operates at 1901.6 nm with a lasing efficiency of 33.2%and pump power threshold of 85 mW using a 2-m-long TBF in a linear cavity with two fiber Bragg gratings(FBGs).The high efficiency is attributed to the use of additional 1552 nm pump to complement 792 nm *** maximum output power of 225 mW is achieved at the pump power of 820 mW with the optimum length of 2 m.
