[目的]虽然人工全膝关节置换术中假体旋转定位的重要性已得到公认,但术中以哪条轴线为参照能够更加精确的保证股骨假体的旋转定位,目前尚存争议。研究表明股骨外上髁线(TEA)与膝关节屈曲轴线平行,但这一轴线术中难以精确确定。本文采用间隙平衡技术(BG),对比TEA技术在股骨假体实际旋转角度测量的差异。[方法]30例人工全膝关节置换分为2组(每组15膝),分别采用TEA和BG技术,术后行CT扫描测量股骨假体旋转角度并行膝关节学会评分(KSS)。[结果]BG组中股骨假体平均外旋角度为2.7°±1.1°,TEA组为5.6°±1.6°(p=0.001)。术后KSS功能评分改善BG组高于TEA组(p=0.002),但两组的KSS膝评分无显著性差异(p=0.39)。[结论]研究表明,与BG技术相比,术中应用TEA参照确定股骨假体的旋转定位可导致股骨假体的过度外旋,其术后KSS功能评分亦较差。

BACKGROUND Our understanding of the severe acute respiratory syndrome coronavirus 2 has evolved since the first reported cases in December 2019,and a greater emphasis has been placed on the hyper-inflammatory response in severely ill *** purpose of this study was to determine risk factors for mortality and the impact of anti-inflammatory therapies on *** To determine the impact of various therapies on outcomes in severe coronavirus disease 2019 patients with a focus on anti-inflammatory and immune-modulating *** A retrospective analysis was conducted on 261 patients admitted or transferred to the intensive care unit in two community hospitals between March 12,2020 and June 17,*** 167 patients received glucocorticoid(GC)*** patients received GC alone,94 received GC and tocilizumab,28 received tocilizumab monotherapy,and 66 received no anti-inflammatory *** patient survival was associated with GC use,either alone or with tocilizumab,and decreased vasopressor *** administration of GC was found to decrease the survival benefit of GC *** difference in survival was found with varying anticoagulant doses,convalescent plasma,tocilizumab monotherapy;prone ventilation,hydroxychloroquine,azithromycin,or intravenous ascorbic acid *** This analysis demonstrated the survival benefit associated with anti-inflammatory therapy of GC,with or without tocilizumab,with the combination providing the most *** studies are needed to assess the optimal timing of anti-inflammatory therapy initiation.

The gut microbiome plays a key role in the pathogenesis and disease activity of inflammatory bowel disease(IBD).While research has focused on the bacterial microbiome,recent studies have shifted towards host genetics and host-fungal *** mycobiota is a vital component of the gastrointestinal microbial community and plays a significant role in immune *** fungi,Candida species,particularly Candida albicans(***),have been extensively studied due to their dual role as gut commensals and invasive *** findings indicate that various strains of *** exhibit consid-erable differences in virulence factors,impacting IBD's *** fungal dysbiosis and antifungal mucosal immunity may be associated to IBD,especially Crohn's disease(CD).This article discusses intestinal fungal dysbiosis and antifungal immunity in healthy individuals and CD *** discusses factors influencing the mycobiome's role in IBD pathogenesis and highlights significant contributions from the scientific community aimed at enhancing understanding of the mycobiome and encouraging further research and targeted intervention studies on specific fungal *** article also provided insights into a recent study by Wu et al in the World Journal of Gastroenterology regarding the role of the gut microbiota in the pathogenesis of CD.

Objective: The current study evaluated various new colchicine analogs for their anticancer activity and to study the primary mechanism of apoptosis and in vivo antitumor activity of the analogs with selective anticancer properties and minimal toxicity to normal ***: Sulforhodamine B(SRB) assay was used to screen various colchicine analogs for their in vitro cytotoxicity. The effect of N-[(7S)-1,2,3-trimethoxy-9-oxo-10-(pyrrolidine-1-yl)5,6,7,9-tetrahydrobenzo[a] heptalene-7-yl] acetamide(ⅢM-067) on clonogenicity, apoptotic induction, and invasiveness of A549 cells was determined using a clonogenic assay, scratch assay, and staining with 4’,6-diamidino-2-phenylindole(DApI) and annexin v/propidium iodide. Mitochondrial membrane potential(MMp) and reactive oxygen species(ROS) levels were observed using fluorescence microscopy. Western blot analysis was used to quantify expression of proteins involved in apoptosis, cell cycle, and phosphatidylinositol 3-kinase(pI3K)/protein kinase B(AKT)/mammalian target of rapamycin(mTOR) signaling. pharmacokinetic and in vivo efficacy studies against Ehrlich ascites carcinoma(EAC) and Ehrlich solid tumor models were conducted using Swiss albino ***: ⅢM-067 showed potent cytotoxicity and better selectivity than all other colchicine analogs screened in this study. The selective activity of ⅢM-067 toward A549 cells was higher among other cancer cell lines,with a selectivity index(SI) value of 2.28. ⅢM-067 demonstrated concentration-and time-dependent cytotoxicity against A549 cells with half-maximal inhibitory concentration values of 0.207, 0.150 and0.106 μmol/L at 24, 48 and 72 h, respectively. It also had reduced toxicity to normal cells(SI > 1) than the parent compound colchicine(SI = 1). ⅢM-067 reduced the clonogenic ability of A549 cells in a dose-dependent manner. ⅢM-067 enhanced ROS production from 24.6% at 0.05 μmol/L to 82.1% at 0.4 μmol/L and substantially decreased the MMp(100% in control to 5.6% at 0.4 μmol

It is common to see patients with atherosclerotic coronary disease and peripheral arterial disease in routine clinical practice. One needs to have a comprehensive and integrated multi-speciality approach and panvascular revascularization in such patients. We report a 54-year-old diabetic hypertensive male with extensive atherosclerotic coronary and peripheral arterial disease, who presented with congestive heart failure, claudication of both lower limbs and mesenteric ischemia. He underwent successful percutaneous panvas- cular revascularization of coronary, renal, mesenteric, aorto-iliac and superficial femoral arteries. Long-term patency of all the stents was also documented.

Warfarin is the most common oral anticoagulant prescribed around the world. Adverse drug interactions with warfarin are a huge problem especially in the elderly and in patients who take multiple medications. Most adverse drug interactions involve concomitantly prescribed oral or intravenous medications. Occasionally, topical or mucosally absorbed drugs can interact, leading to fluctuations in warfarin levels with adverse consequences. In this case report, we describe a case of intestinal intramural hematoma, a rare but known consequence of a supra therapeutic international normalized ratio (INR). The supra therapeutic INR was a consequence of mucosally absorbed miconazole, prescribed for vaginal candidiasis. We wish to highlight this rare and potentially fatal drug interaction, along with the need for frequent INR monitoring when new drugs are added or removed in patients taking warfarin.

Anomalous origin of the coronary artery from opposite coronary sinus is infrequently observed during coronary angiography. percutaneous coronary intervention (pCI) of anomalous coronary artery is technically difficult and challenging. It requires appropriate selection of guide catheters for adequate stability, coaxial alignment and backup support during the intervention. We hereby report a rare case of anomalous origin of left main coronary artery (LM) from the right coronary sinus, having a retro-aortic course to the left side before its bifurcation into left anterior descending (LAD) and circumflex artery. The 59-year-old man had successful pCI of atherosclerotic LAD lesions. A 64-slice Multi-Detector Computed Tomography (MDCT) performed at 4 years of follow-up demonstrated patency of coronary stents and also delineated the origin and course of the anomalous LM. The case illustrates the rarity of anomalous LM, and describes technical issues during pCI and the role of MDCT in coronary anomaly imaging.
