Efficacy of a Diabetes Specific Nutritional Supplement (DSNS) on Glycemic Response in prediabetic Adults: A Two-Armed, Open-Labelled Randomized Controlled Study

It is well known that Diabetes Specific Nutritional Supplements (DSNSs) are linked to improved glycemic control in individuals with diabetes. However, data on efficacy of DSNSs in prediabetics is limited. This was a two-armed, open-labelled, randomized controlled six-week study on 199 prediabetics [30 - 65 years;Glycosylated Hemoglobin (HbA1c) 5.7% - 6.4% and/or Fasting Blood Glucose (FBG) 100-125 mg/dl]. Two parallel phases were conducted: Acute Blood Glucose Response (ABGR) and Intervention phase. prediabetic participants were randomized into test (n = 100) and control (n = 99). The primary objective was to assess the ABGR of DSNS versus an isocaloric snack, measured by incremental Area under the Curve (iAUC). Test and control received 60 g of DSNS and 56 g of isocaloric snack (cornflakes) respectively, both in 250 ml double-toned milk on visit days 1, 15, 29 and 43. postprandial Blood Glucose (ppG) was estimated at 30, 60, 90, 120, 150 and 180 minutes. During the 4 weeks intervention phase, the test group received DSNS with lifestyle counselling (DSNS + LC) and was compared with the control receiving lifestyle counselling alone (LC alone). Impact was studied on FBG, HbA1C, anthropometry, body composition, blood pressure, nutrient intake, and physical activity. The impact of DSNS was also studied using CGM between two 14-day phases: CGM1 baseline (days 1 - 14) and CGM2 endline (days 28 - 42). DSNS showed significantly lower ppG versus isocaloric snack at 30 (p 12, and chromium were reported by DSNS + LC versus LC alone. No other significant changes were reported between groups. It may be concluded that DSNS may be considered as a snack for prediabetic or hyperglycemic individuals requiring nutritional support for improved glycemic control.

Objective:Laparoscopic cholecystectomy is currently the gold standard for treating symptomatic gallstone *** its success,approximately 10%of patients may experience persistent biliary symptoms,leading to the post-cholecystectomy syndrome.A remnant gallbladder with cystic duct or bile duct stones is one of the causes of this *** objective of this study was to shed light on the clinical manifestations,evaluation,therapeutic strategies,and outcomes associated with laparoscopic management of symptomatic remnant ***:This was a retrospective study,conducted over a five-year period(January 2017 to December 2022)at Apollo Hospitals in South *** patients who underwent laparoscopic completion cholecystectomy for a remnant gall bladder were *** following data were collected:patient demographics,symptoms,preoperative investigations,intraoperative details and post operative ***:In total,36 patients were included and *** majority of patients were male(25,69.4%),with a mean age of 50.7±12.1 *** most common presentation was pain in the upper abdomen or right upper quadrant region(24,66.7%).The laparoscopic approach was attempted in all patients,with a success rate of 94.4%.Two patients required conversion to open *** fistula to the colon was observed in one *** was observed in 7 patients(19.4%),and stone clearance was successfully achieved using endoscopic retrograde cholangiopancreatography in all patients ***:Incomplete gall bladder removal either intentionally or unintentionally leaves a remnant gall bladder that is at risk for stone formation and *** who have this clinical entity with symptoms require a redo or complete cholecystectomy,a complex procedure associated with certain *** study highlights the feasibility and safety of laparoscopic completion cholecystectomy for the management of remnant gallbladder

Hormonal contraception for human males:prospects

Development of an ideal hormonal contraceptive for man has been the goal of several research workers during thepast few decades. Suppression of pituitary gonadotropic hormones, which in turn would inhibit spermatogenesis whilemaintaining normal libido and potentia has been the approach for a contraceptive agent. Intramuscularly administeredand orally active testosterone or testosterone in combination with progesterone have been shown to cause inhibition ofspermatogenesis resulting in azoospermia in normal men. Similarly testosterone has been used in combination with go-nadotropin releasing hormone antagonists and agonists to inhibit pituitary gonadotropic hormone release. Immunologicalapproach to neutralize the circulating levels of follicle stimulating hormone has also been shown to cause inhibition ofspermatogenesis. The available literature shows that testosterone causes reversible azoospermia without any significantside effects in Asian population effectively and appears to be a promising chemical for control of fertility in man.

Background: Duodenoscope-related multidrug-resistant organism(MDRO) infections raise concerns. Disposable duodenoscopes have been recently introduced in the market and approved by regulatory agencies with the aim to reduce the risk of endoscopic retrograde cholangiopancreatography(ERCp) associated infections. The aim of this study was to evaluate the outcome of procedures performed with single-use duodenoscopes in patients with clinical indications to single-operator cholangiopancreatoscopy. Methods: This is a multicenter international, retrospective study combining all patients who underwent complex biliopancreatic interventions using the combination of a single-use duodenoscope and a single-use cholangioscope. The primary outcome was technical success defined as ERCp completion for the intended clinical indication. Secondary outcomes were procedural duration, rate of cross-over to reusable duodenoscope, operator-reported satisfaction score(1 to 10) on performance rating of the single-use duodenoscope, and adverse event(AE) rate. Results: A total of 66 patients(26, 39.4% female) were included in the study. ERCp was categorized according to ASGE ERCp grading system as 47(71.2%) grade 3 and 19(28.8%) grade 4. The technical success rate was 98.5%(65/66). procedural duration was 64(interquartile range 15-189) min, cross-over rate to reusable duodenoscope was 1/66(1.5%). The satisfaction score of the single-use duodenoscope classified by the operators was 8.6 ± 1.3 points. Four patients(6.1%) experienced AEs not directly related to the single-use duodenoscope, namely 2 post-ERCp pancreatitis(pEp), 1 cholangitis and 1 ***: Single-use duodenoscope is effective, reliable and safe even in technically challenging procedures with a non-inferiority to reusable duodenoscope, making these devices a viable alternative to standard reusable equipment.

Recently,the application of Wireless Sensor Networks(WSNs)has been increasing *** requires privacy preserving data aggregation protocols to secure the data from *** privacy of the sensor data is a challenging *** paper presents a non-linear regression-based data aggregation protocol for preserving privacy of the sensor *** proposed protocol uses non-linear regression functions to represent the sensor data collected from the sensor *** of sending the complete data to the cluster head,the sensor nodes only send the coefficients of the non-linear *** will reduce the communication overhead of the *** data aggregation is performed on the masked coefficients and the sink node is able to retrieve the approximated results over the aggregated *** analysis of experiment results shows that the proposed protocol is able to minimize communication overhead,enhance data aggregation accuracy,and preserve data privacy.

The early Archean oceans were marked by significant redox changes which have subsequently shaped the Earth’s *** chemical sediments of banded Iron and Manganese formations provide important geochemical proxies for these historical shifts in the redox conditions and to trace the ancient sedimentation patterns and *** this study,we investigate the proto-ore of the Archean Mn-formations of the Sandur,Chitradurga and Shimoga greenstone belts of Dharwar Craton of southern peninsular India,which is geochemically characterised as quartz arenites,Mn-arenites,Fe-arenites,Mn-argillites and *** geochemical systematics suggest their deposition in shallow to deeper shelf in the Archean *** detrital zircon U-pb systematics of Mn arenites and argillites indicate their maximum depositional age of 3230±52 Ma representing the oldest onset of sedimentation during the paleo-Mesoarchean timeframe in the Chitradurga Group of Dharwar *** detrital influx proxies suggest variations in sedimentation rates associated with the Archean transgressive-regressive cycles and fluctuating hydrodynamic conditions,together reflecting an increasing trend in the contributions of recycled sediment from Sandur to Chitradurga and Shimoga greenstone *** available detrital zircon ages of the Mn arenites and argillites from these greenstone belts indicate a~600 Ma prolonged period of Mn deposition for which high-T hydrothermal fluids from Archean mid-oceanic ridges supplied the *** trace element compositions of the concordant detrital zircons suggest 3.3-3.1 Ga Dharwar basement TTG/granitoid source which is corroborated by the zircon crystallization temperatures of 690-820℃.The source-normalisedα-dose rates of the detrital zircons signify greater degrees of sediment transport and multi-cycle nature which correspond to the earliest episode of crustal growth in the Indian sub-continent associated with the Mesoarchean Ur supercontinent.T

prediction of weld bead geometry is always an interesting and challenging research topic as it involves understanding of complex multi input and multi output system. The weld bead geometry has a profound impact on the load bearing capability of a weld joint, which in-turn decides the performance in real time service conditions. The present study introduces a novel approach of detecting a relationship between weld bead geometry and mechanical properties(e.g. tensile load) for the purpose of catering the best the process could offer. The significance of the proposed approach is demonstrated by a case of dissimilar aluminium alloy(AA2219 and AA5083) electron beam welds. A mathematical model of tensile braking load as a function of geometrical attributes of weld bead geometry is presented. The results of investigation suggests the effective thickness of weld-a geometric parameter of weld bead has the most significant influence on tensile breaking load of dissimilar weld joint. The observations on bead geometry and the mechanical properties(microhardness, ultimate tensile load and face bend angle) are correlated with detailed metallurgical analysis. The fusion zone of dissimilar electron beam weld has finer grain size with a moderate evaporation and segregation of alloying elements magnesium and copper *** mechanical properties of weld joint are controlled by optimum bead geometry and HAZ softening in weaker AA5083 Al alloy.

Development of cost effective personnel armour through structural hybridization

The objective of the present study is to develop cost effective thermoplastic hybrid laminate using Dyneema®HB50 and Tensylon®HSBD 30A through structural hybridization *** having 20 mm thickness were fabricated and subjected to 7.6239 mm mild steel core projectile with an impact velocity of 730±10 ms*** such as energy absorption,back face deformation and rate of back face deformation were measured as a function of hybridization *** was observed that hybrid laminate with 50:50 ratio(w/w)of Tensylon®and Dyneema®with Tensylon®as front face showed 200%more energy absorption when compared to 100%Tensylon®laminate and showed equal energy absorption as that of expensive 100%Dyneema®***,hybrid laminate with TD50:50 ratio showed 40%lower in terms of final back face deformation than Dyneema®*** of back face deformation was also found to be slow for hybrid laminate as compared to Dyneema®*** mechanical analysis showed that,Tensylon®laminate has got higher stiffness and lower damping factor than Dyneema®and hybrid *** interface between Tensylon®and Dyneema®layers was found to be separating during the penetration process due to the poor interfacial *** behaviour of laminates for different hybridization ratios were studied by sectioning the impacted *** was observed that,the Tensylon®laminate has undergone shear cutting of fibers as major failure mode whereas the hybrid laminate showed shear cutting followed by tensile stretching,fiber pull out and *** inputs are highly useful for body armour applications to design cost effective armour with enhanced performance.

Xenoesterogens and male infertility:myth or reality?

Aim: To evaluate the role of polychlorinated biphenyls (pCBs) as a potential environmental hazard in the deteriora-tion of male fertility. Methods: Fifty-three males were studied. After a thorough case history evaluation and rele-vant clinical and laboratory investigations, pCBs were estimated in the seminal plasma of 21 infertile men with 'Unex-plained Male Factor' and 32 fertile controls. peak retention times of the eluants were compared with those of the com-mercially available standard pCB Mix, and the results confirmed spectrophotometrically. Seminal pCB concentrafonswere compared between i) fertile and infertile men and ii) men from different areas and diets. The relationship betweenpCB concentrations and measures of sperm quality such as the total motile sperm count, was assessed. Results:pCBs were detected in seminal plasma of infertile men but absent from controls. Sperm quantity and quality were sig-nificantly lower in infertile men compared to controls. The highest average pCB concentrations were found in fish-eat-ing urban dwellers, and followed in succession by fish-eating rural dwellers, non fish-eating urban dwellers and nonfish-eating mral dwellers. The total motile sperm counts were inversely proportional to the pCB concentrations and weresignificantly lower than those of the respective controls. Conclusion: pCBs may be instrumental in the deteriorationof sperm quantity and quality, a contaminated fish diet being the main source of exposure.(Asian J Androl 2000 Dec; 2: 263-269 )
