BACkGROUND Second-look endoscopy(SLE)to prevent recurrent bleeding in patients with peptic ulcer disease(pUD)and those undergoing endoscopic submucosal dissection(ESD)is routinely being performed.Conflicting evidence exists regarding efficacy,risk,benefit,and cost-effectiveness.AIM To identify the role and effectiveness of SLE in ESD and pUD,associated rebleeding and pUD-related outcomes like mortality,hospital length of stay,need for endoscopic or surgical intervention and blood transfusions.METHODS A systematic review of literature databases pubMed,Cochrane,and Embase was conducted from inception to January 5,2023.Randomized controlled trials that compared patients with SLE to those who did not have SLE or evaluated the role of prophylactic hemostasis during SLE compared to other conservative interventions were included.The study was conducted per pRISMA guidelines,and the protocol was registered in pROSpERO(ID CRD42023427555:).RevMan was used to perform meta-analysis,and Mantel-Haenszel Odds ratio(OR)were generated using random effect models.RESULTS A total of twelve studies with 2687 patients were included in our systematic review and meta-analysis,of which 1074 patients underwent SLE after ESD and 1613 patients underwent SLE after pUD-related bleeding.In ESD,the rates of rebleeding were 7%in the SLE group compared to 4.4%in the non-SLE group with OR 1.65,95%confidence intervals(CI)of 0.96 to 2.85;p=0.07,whereas it was 11%in the SLE group compared to 13%in the non-SLE group with OR 0.895%CI:0.50 to 1.29;p=0.36.The mean difference in the blood transfusion rates in the SLE and no SLE group in pUD was OR 0.01,95%CI:-0.22 to 0.25;p=0.91.In SLE vs non-SLE groups with pUD,the OR for Endoscopic intervention was 0.29,95%CI:0.08 to 1.00;p=0.05 while it was OR 2.03,95%CI:0.95 to 4.33;p=0.07,for surgical intervention.The mean difference in the hospital length of stay was-3.57 d between the SLE and no SLE groups in pUD with 95%CI:-7.84 to 0.69;p=0.10,denoting an average of approximately 3 fewer days of hospital stay amon

Functionally Graded Materials (FGMs) are innovative advanced quality materials in the field of composites concerning their strength， mechanical， and thermal properties. Nowadays， the modern requirement of the industry in the fields of health care， aerospace， and power sectors needs the rapid evolution of new components， which allows researchers to invent new materials to satisfy the functional requirements of modern technology. Tissue engineering is one of the most concerned areas of the application of FGM in the healthcare sector， where the tailored properties of FGM play a significant role in building and growing an artificial structure that heals the damaged tissue of the body parts and meets the desired application that the part needs to perform. This paper highlights the suitability of the combination of a nano-structure enhanced epoxy functionally graded material， its properties， and applicability in the design of a prosthetic foot where it provides the mobility and comfort of the body part like natural tissue. The analytical study is done by designing an ANSYS model and simulating the results of equivalent stress and directional deformation. The Finite Element (FE) approach is used to optimize the output results of stress-strain analysis， different weight percentages of nano-filler are taken for performance enhancement. A comparative analysis is done with the previously established results taking carbon fiber-reinforced composites that offer a successful validation of the present results obtained. Furthermore， this study also provides a clear understanding of the justification of the composition considered for the effective application in the field of prosthetics field.

Background Africa sees the surge of plague cases in recent decades,with hotspots in the Democratic Republic of Congo,Madagascar,and peru.A rodent-borne scourge,the bacterial infection known as plague is transmitted to humans via the sneaky bites of fleas,caused by Yersinia pestis.Bubonic plague has a case fatality rate of 20.8%with treatment,but in places such as Madagascar the mortality rate can increase to 40–70%without treatment.Main text Tragedy strikes in the Ambohidratrimo district as three lives are claimed by the plague outbreak and three more fight for survival in the hospitals,including one man in critical condition,from the Ambohimiadana,Antsaharasty,and Ampanotokana communes,bringing the total plague victims in the area to a grim to five.presently,the biggest concern is the potential plague spread among humans during the ongoing COVID-19 pandemic.Effective disease control can be achieved through training and empowering local leaders and healthcare providers in rural areas,implementing strategies to reduce human–rodent interactions,promoting water,sanitation and hygiene practices(WASH)practices,and carrying out robust vector,reservoir and pest control,diversified animal surveillance along with human surveillance should be done to more extensively to fill the lacunae of knowledge regarding the animal to human transmission.The lack of diagnostic laboratories equipped represents a major hurdle in the early detection of plague in rural areas.To effectively combat plague,these tests must be made more widely available.Additionally,raising awareness among the general population through various means such as campaigns,posters and social media about the signs,symptoms,prevention,and infection control during funerals would greatly decrease the number of cases.Furthermore,healthcare professionals should be trained on the latest methods of identifying cases,controlling infections and protecting themselves from the disease.Conclusions Despite being endemic to Madagascar,the outbreak’s pace is unparalleled,and it ma

This paper presents the simulation results of DC,small-signal and noise properties of Gap based Double Drift Region( DDR) Impact Avalanche Transit Time( IMpATT) diodes. In simulation study we have considered the flat DDR structures of IMpATT diode based on Gap,GaAs,Si and GaN( wurtzite,wz) material. The diodes are designed to operate at the millimeter window frequencies of 94 GHz and 220 GHz. The simulation results of these diodes reveal Gap is a promising material for IMpATT applications based on DDR structure with high break down voltage( V_B) as compared to Si and GaAs IMpATTs. It is also encouraging to worth note Gap base IMpATT diode shows a better output power density of 4. 9 × 10~9 W/m^2 as compared to Si and GaAs based IMpATT diode. But IMpATT diode based on GaN( wz) displays large values of break down voltage,efficiency and power density as compared to Si,GaAs and Gap IMpATTs.

Management of splenic artery aneurysm associated with extrahepatic portal vein obstruction

BACkGROUND: Splenic artery aneurysms although rare are clinically significant in view of their propensity for spontaneous rupture and life-threatening bleeding. While portal hypertension is an important etiological factor, the majority of reported cases are secondary to cirrhosis of the liver. We report three cases of splenic artery aneurysms associated with extrahepatic portal vein obstruction and discuss their management. METHODS: The records of three patients of splenic artery aneurysm associated with extrahepatic portal vein obstruction managed from 2003 to 2010 were reviewed retrospectively. The clinical presentation, surgical treatment and outcome were analyzed. RESULTS: The aneurysm was >3 cm in all patients. The clinical symptoms were secondary to extrahepatic portal vein obstruction (hematemesis in two, portal biliopathy in two) while the aneurysm was asymptomatic. Doppler ultrasound demonstrated aneurysms in all patients. A proximal splenorenal shunt was performed in two patients with excision of the aneurysm in one patient and ligation of the aneurysm in another one. The third patient had the splenic vein replaced by collaterals and hence underwent splenectomy with aneurysmectomy. All patients had an uneventful post-operative course. CONCLUSIONS: Splenic artery aneurysms are associated with extrahepatic portal vein obstruction. Surgery is the mainstay of treatment. Although technically difficult, it can be safely performed in an experienced center with minimal morbidity and good outcome.

BACkGROUND Non-alcoholic fatty liver disease(NAFLD)and metabolic-associated fatty liver disease(MAFLD)are on the rise like any other liver disease,and tend to affect 25%of the United States population.The impact of NAFLD and MAFLD on patients with coronavirus disease 2019(COVID-19)remains unclear.AIM To identify the association of NAFLD and MAFLD with mortality,hospitalization,hospital length of stay,and supplemental oxygen utilization in COVID-19 patients.METHODS A systematic review of literature on Cochrane,Embase,pubMed,ScienceDirect,and Web of Science databases was conducted from January 2019 to July 2022.Studies that evaluated NAFLD/MAFLD using laboratory methods,noninvasive imaging,or liver biopsy were included.The study protocol was registered in pROSpERO(ID CRD42022313259)and pRISMA guidelines were followed.The National Institutes of Health quality assessment tool was used to assess the quality of the studies.pooled analysis was conducted using software Rev Man version 5.3.The stability of the results was assessed using sensitivity analysis.RESULTS Thirty-two studies with 43388 patients were included in the meta-analysis of whom 8538(20%)patients were observed to have NAFLD.There were 42254 patients from 28 studies included in the mortality analysis.A total of 2008 patients died from COVID-19;837(10.52%)in the NAFLD group and 1171(3.41%)in the non-NAFLD group.The odds ratio(OR)was 1.38 for mortality with a 95%confidence interval(95%CI)=0.97-1.95 and p=0.07.A total of 5043 patients from eight studies were included in the hospital length of stay analysis.There were 1318 patients in the NAFLD group and 3725 patients in the non-NAFLD group.A qualitative synthesis showed that the mean difference in hospital length of stay was about 2 d between the NAFLD and non-NAFLD groups with a 95%CI=0.71-3.27 and p=0.002.For hospitalization rates,the OR was 3.25 with a 95%CI of 1.73-6.10 and p=0.0002.For supplemental oxygen utilization,the OR was 2.04 with a 95%CI of 1.17-3.53 and p=0.01.CONCLUSION Our meta-analysis sugges

Objective:To investigate the effect of Ocimum sanctum hydroalcoholic extract(OSHE)on seizure control and neuronal injury in rats with lithium-pilocarpine-induced status epilepticus(SE).Methods:SE was induced by administering lithium chloride followed by pilocarpine 24 h later.OSHE was administered either alone or in combination with valproate(VpA)3 days before SE induction until 14 days post-SE induction.Seizure parameters were recorded on day 1(0-3 h),day 1-3 and day 4-14 post-SE.On day 14 post-SE,neurobehavioural tests(elevated plus maze and passive avoidance)were done followed by total antioxidant capacity,neuron-specific enolase,immunohistochemistry,and electron microscopic assessment in the hippocampus and cortex tissue.Results:OSHE+VpA provided more significant seizure protection(75%)than VpA(62.5%),OSHE(62.5%),or SE control(12.5%)(overall p=0.003).The latency to stage-3/4 seizures was increased and the number of stage-3/4 seizures was reduced in all treatment groups compared to the SE control group(p=0.002 andpectively).The OSHE+VpA group also had better memory retention than other treatment groups(ppassive avoidance test.Total antioxidant capacity level was significantly higher and neuron-specific enolase was lower in the OSHE and OSHE+VpA groups compared to the SE control group.Electron microscopic study showed significant myelin sheath damage(67.5%,ppppocampus of the SE control group,which were alleviated by OSHE or OSHE+VpA treatment.In immunohistochemical analysis,the OSHE,OSHE+VpA,and VpA groups had a significantly higher number of viable neurons and less neuronal loss compared to the SE control in the hippocampus(ppA shows better seizure control by preservation of neuronal echotexture and reducing oxidative stress in the hippocampus.

Constraining the relativistic mean-field models from pREX-2 data:effective forces revisited

Based on the current measurement of the neutron distribution radius(R_(n))of ^(208)pb from the pREX-2 data,we revisited the recently developed G3 and IOpB-I force parameters by fine-tuning some specific couplings within the relativistic mean-field(RMF)model.Theω-ρ-mesons coupling and theρ-meson coupling are constrained to the experimental neutron radius of^(208)pb without compromising the bulk properties of finite nuclei and infinite nuclear matter observables.The modified parameter sets are applied to calculate the gross properties of finite nuclei such as binding energies,charge distributions,nuclear radii,pairing gaps,and single-particle energies.The root-mean-square deviations in binding energy and charge radius are estimated with respect to the available experimental data for 195 even-even nuclei,and the results compare favourably with the well-calibrated effective interactions of Skyrme,Gogny and other relativistic mean-field parametrizations.The pairing gap estimations for modified G3 and IOpB-I for Sn isotopes are also compared with the Hartree-Fock-Bogoliubov calculation with the Gogny(D1S)interaction.The isotopic shift and single-particle energy spacing are also calculated and compared with the experimental data for both original and modified versions of the G3 and IOpB-I parameter sets.Subsequently,both the modified parameter sets are used to obtain the various infinite nuclear matter observables at saturation.In addition to these,the force parameters are adopted to calculate the properties of a high isospin asymmetry dense system such as neutron star matter and tested for validation using the constraint from GW170817 binary neutron star merger events.The tuned forces predict relatively good results for finite and infinite nuclear matter systems and the current limitation on the neutron radius from pREX-2.A systematic analysis using these two refitted parameter sets over the nuclear chart will be communicated shortly.

Objective:Resident microorganisms live in dynamic harmony with their hosts to influence various physical and psychological health aspects.The majority of the resident microbes are found in the digestive tract,aka the gut of mammals.If and when perturbed,the composition of gut microbiota could jeopardize the physiological balance or homeostasis.In this article,we aim to establish how different diseases could be accompanied by notable changes in gut microbial composition and lend insight into microbial regulation of health.Methods:Literature search was done in pubMed using relevant keywords and summarized in tabular form as well as in narration.Results:We try to focus on the concept of microbial and metabolite biomarkers for diseases.We also try to capture the renewed perspective of good and harmful microorganisms in the context of host health.We have presented a comparative network analysis of microbial roles in select diseases.Recent findings also suggested that the growth of some traditionally disease-causing pathogenic microorganisms promotes health in other human communities.We have listed major taxa of gut microbes in communities worldwide,which signifies that gut microbiota can be healthy or harmful depending on the urbanization and ethnicity of the hosts.The traditional and current schools of thoughts are both limited by the technology of metagenomic studies;we have elucidated some of their shortcomings.Conclusion:Research in the field of gut microbiota must take into account the different populations and the changing narrative of healthy and harmful microbes.
