proximal and distal esophageal sensitivity is decreased in patients with Barrett’s esophagus

AIM: To investigate sensations to multimodal pain stimulation in the metaplastic and normal parts of the esophagus in patients with Barrett’s esophagus (BE). METHODS: Fifteen patients with BE and 15 age-matched healthy volunteers were subjected to mechanical, thermal and electrical pain stimuli of the esophagus. Both the metaplastic part and the normal part (4 and 14 cm, respectively, above the esophago-gastric junction) were examined. At sensory thresholds the stimulation intensity, referred pain areas, and evoked brain potentials were recorded. RESULTS: patients were hyposensitive to heat stimulation both in the metaplastic part [median stimulation time to reach the pain detection threshold: 15 (12-34) s vs 14 (6-23) s in controls; F = 4.5, p = 0.04] and the normal part of the esophagus [median 17 (6-32) s vs 13 (8-20) s in controls; F = 6.2, p = 0.02]. Furthermore, patients were hyposensitive in the metaplastic part of the esophagus to mechanical distension [median volume at moderate pain: 50 (20-50) mL vs 33 (13-50) mL in controls; F = 5.7, p = 0.02]. No indication of central nervous system abnormalities was present, as responses were comparable between groups to electrical pain stimuli in the metaplastic part [median current evoking moderate pain: 13 (6-26) mA vs 12 (9-24) mA in controls; F = 0.1, p = 0.7], and in the normal part of the esophagus [median current evoking moderate pain: 9 (6-16) mA, vs 11 (5-11) mA in controls; F = 3.4, p = 0.07]. Furthermore, no differences were seen for the referred pain areas (p-values all > 0.3) or latencies and amplitudes for the evoked brain potentials (p-values all > 0.1). CONCLUSION: patients with BE are hyposensitive both in the metaplastic and normal part of esophagus likely as a result of abnormalities affecting peripheral nerve pathways.

AIM To assess risk factors of hospital admission for acute colonic *** The study was conducted as part of the second wave of the population-based North Trondelag Health Study(HUNT2), performed in North Trondelag County, Norway, 1995 to 1997. The study consisted of 42570 participants(65.1% from HUNT2) who were followed up from 1998 to 2012. Of these, 22436(52.7%) were females. The cases were defined as those 358 participants admitted with acute colonic diverticulitis during follow-up. The remaining participants were used as controls. Univariable and multivariable Cox regression analyses was used for each sex separately after multiple imputation to calculate *** Multivariable Cox regression analyses showed that increasing age increased the risk of admission for acute colonic diverticulitis: Comparing with ages p 70 years, HR = 6.19, p ponding values were HR = 1.85, p = 0.004 and 2.56, p patients with obesity(body mass index ≥ 30) the HR = 2.06, p p present(HR = 2.11, p previous(HR = 1.65, p = 0.007) cigarette smoking increased the risk of admission. In males, breathlessness(HR = 2.57, p p = 0.007) increased the risk. Level of education, physical activity, constipation and type of bread eaten showed no association with admission for acute colonic *** The risk of hospital admission for acute colonic diverticulitis increased with increasing age, in obese individuals, in ever cigarette smoking females and in males living in rural areas.

Lignans from <i>Morinda citrifolia</i>(Noni) Fruit Inhibit Fatty Acid Amide Hydrolase and Monoacylglycerol Lipase <i>in Vitro</i>

Morinda citrifolia (noni) fruit juice has exhibited a variety of biological activities in human clinical trials, indicating that it influences multiple systems of the body. Since the 1990s, the endocannabinoid system (ECS) has been found to modulate the activity of other organ systems. To investigate noni’s potential impact on the ECS, extracts from freeze-dried noni fruit were evaluated in fatty acid amide hydrolase (FAAH) and monoacylglycerol lipase (MAGL) inhibition assays. The ethyl acetate extract demonstrated the greatest activity against both enzymes. Lignans in this extract also inhibited enzyme activities, with americanin A being the most active in both assays. Americanoic acid and 3,3’-bisdemethylpinoresinol were the next most active compounds. These results suggest that lignans in noni fruit may influence endocannabinoid levels within the body via FAAH and MAGL inhibition. This reveals another set of probable mechanisms of action by which noni juice affects human health.

This paper presents the power hardware in the loop(pHIL)validation of a feed forward DC voltage control scheme for the fault ride through(FRT)of voltage source converter(VSC)high voltage DC(HVDC)connected offshore wind power plants(Wpps).In the proposed FRT scheme,the Wpp collector network AC voltage is actively controlled by considering both the DC voltage error and the AC current from the Wpp AC collector system which ensures fast and robust FRT of the VSC HVDC connected offshore *** pHIL tests were carried out in order to verify the efficacy of the proposed feed forward DC voltage control scheme for enhancing the FRT capability of the VSC HVDC connected *** pHIL test results have demonstrated the proper control coordination between the offshore Wpp and the Wpp side VSC and the efficient FRT of the VSC HVDC connected Wpps.

To achieve active control of the AC voltage magnitude of wind power plant(Wpp)collector network and improve the fault ride-through(FRT)capability,an FRT scheme based on feed forward DC voltage control is presented for voltage source converter-high voltage direct current(VSC-HVDC)connected offshore *** steady state operation,an open loop AC voltage control is implemented at the Wpp-side VSC of the HVDC system so that any possible control interactions between Wpp-side VSC and VSC of wind turbine are *** during any grid fault,a dynamic AC voltage reference is made according to both the DC voltage error and AC active current from the Wpp collector system,thus ensuring fast and robust FRT of the VSC-HVDC-connected offshore *** the unbalanced fault condition in the host power system,the resulting oscillatory DC voltage is directly used in the VSC AC voltage controller at the Wpp side so that the Wpp collector system voltage also reflects the unbalance in the main *** domain simulations are performed to verify the efficacy of the FRT scheme based on the proposed feed forward DC voltage *** results show satisfactory FRT responses of the VSC-HVDC-connected offshore Wpp under balanced and unbalanced faults in the host power system,as is shown under a serious fault in the Wpp collector network.

Articular cartilage is a connective tissue consisting of a specialized extracellular matrix （ECM） that dominates the bulk of its wet and dry weight. Type Ⅱ collagen and aggrecan are the main ECM proteins in cartilage. However, little attention has been paid to less abundant molecular components, especially minor collagens, including type Ⅳ, Ⅵ, Ⅸ, Ⅹ, Ⅺ, Ⅻ, ⅩⅢ, and ⅩⅣ, etc. Although accounting for only a small fraction of the mature matrix, these minor collagens not only play essential structural roles in the mechanical properties, organization, and shape of articular cartilage, but also fulfil specific biological functions. Genetic studies of these minor collagens have revealed that they are associated with multiple connective tissue diseases, especially degenerative joint disease. The progressive destruction of cartilage involves the degradation of matrix constituents including these minor collagens. The generation and release of fragmented molecules could generate novel biochemical markers with the capacity to monitor disease progression, facilitate drug development and add to the existing toolbox for in vitro studies, preclinical research and clinical trials.

Expression of the Epidermal Growth Factor Receptors and Ligands in paired Samples of Normal Breast Tissue, primary Breast Carcinomas and Lymph Node Metastases

purpose: In breast cancer, the EGF receptors host an increasing number of therapeutic targets and the interactive mechanisms of actions of the receptors and their ligands justify investigation of the EGF family as an entity. Experimental design: paired tissue samples of normal breast tissue and primary breast carcinomas were examined in a prospective study of 163 patients. A third sample was obtained from the paired ipsilateral metastatic lymph node from 58 of these patients. The mRNA expression of four EGF receptors (HER1 - HER4) and 11 activating ligands was quantified with real-time RT-pCR. Results: Expression of HER2, HER3, and HER4 mRNA was upregulated in primary carcinomas compared to normal breast tissue while HER1 was downregulated. The mRNA expression of HER3 and HER4 differed between primary breast carcinomas and lymph node metastases whereas there was no difference in the expression of HER1 and HER2. The combination of low HER3 and low HER4 expression in the primary carcinoma was significantly more frequent in lymph node-negative patients as compared to lymph node positive patients. Distinct correlation patterns of the receptors and their corresponding activating ligands appeared in both normal breast tissue and in carcinomas, notably for the HER3 and HER4 receptors and their 3 specific ligands: HB-EGF, NRG2, and NRG4. Conclusion: HER2, HER3, and HER4 showed increased mRNA expression in carcinomas and were positively correlated to each other and to specific activating ligands. Furthermore, low HER3 and HER4 expression in the carcinomas correlated to the absence of lymph node metastases.

Fabrication of 3D air-core MEMS inductors for very-highfrequency power conversions

We report a fabrication technology for 3D air-core inductors for small footprint and very-high-frequency power *** process is scalable and highly generic for fabricating inductors with a wide range of geometries and core *** demonstrate spiral,solenoid,and toroidal inductors,a toroidal transformer and inductor with advanced geometries that cannot be produced by wire winding *** inductors are embedded in a silicon substrate and consist of through-silicon vias and suspended *** inductors fabricated with 20 and 25 turns and 280-350μm heights on 4-16 mm2 footprints have an inductance from 34.2 to 44.6 nH and a quality factor from 10 to 13 at frequencies ranging from 30 to 72 *** air-core inductors show threefold lower parasitic capacitance and up to a 140% higher-quality factor and a 230% higher-operation frequency than silicon-core inductors.A 33 MHz boost converter mounted with an air-core toroidal inductor achieves an efficiency of 68.2%,which is better than converters mounted with a Si-core inductor(64.1%).Our inductors show good thermal cycling stability,and they are mechanically stable after vibration and 2-m-drop tests.
