Assessing the Levels of Clinical Reasoning Sk.lls Using Self-Assessment of Clinical Reflection and Reasoning in Undergraduate Nursing Students: A Descrip.ive Comp.rative Study

Back.round: Clinical reasoning is a critical cognitive sk.ll that enables undergraduate nursing students to mak. clinically sound decisions. A lap.e in clinical reasoning can result in unintended harm to p.tients. The aim of the study was to assess and comp.re the levels of clinical reasoning sk.lls between third year and fourth year undergraduate nursing students. Methods: The study utilized a descrip.ive comp.rative research design, based on the p.sitivism p.radigm. 410 undergraduate nursing students were systematically samp.ed and recruited into the study. The researchers used the Self-Assessment of Clinical Reflection and Reasoning questionnaire to collect data on clinical reasoning sk.lls from third- and fourth-year nursing students while adhering to ethical p.incip.es of human dignity. Descrip.ive statistics were done to analyse the level of clinical reasoning and an indep.ndent samp.e t-test was p.rformed to comp.re the clinical reasoning sk.lls of the student. A p.value of 0.05 was accep.ed. Results: The results of the study revealed that the mean clinical reasoning scores of the undergraduate nursing students were k.owledge/theory ap.lication (M = 3.84;SD = 1.04);decision-mak.ng based on exp.rience and evidence (M = 4.09;SD = 1.01);dealing with uncertainty (M = 3.93;SD = 0.87);reflection and reasoning (M = 3.77;SD = 3.88). The mean difference in clinical reasoning sk.lls between third- and fourth-year undergraduate nursing students was not significantly different from an indep.ndent samp.e t-test scores (t = −1.08;p.= 0.28);(t = −0.29;p.= 0.73);(t = 1.19;p.= 0.24);(t = −0.57;p.= 0.57). Since the p.value is >0.05, the null hyp.thesis (H0) “there is no significantno significant difference in clinical reasoning between third year and fourth year undergraduate nursing students”, was accep.ed. Conclusion: This study has shown that the level of clinical reasoning sk.lls of the undergraduate nursing students was moderate to low. This meant that the teaching method

p.rp.se: The oxidative stress (OS) hyp.thesis of overtraining syndrome argues that increased p.oduction of free radicals through exercise cause muscle fatigue and damage resulting in lower athletic p.rformance. Several studies have investigated OS immediately before and after exercise bouts in a training macrocycle. Our study aimed to comp.re OS of endurance athletes between a comp.tition macrocycle and the immediate p.st-season recovery macrocycle. In addition, we aimed to identify athletes who exp.rienced an unexp.ainable drop.in athletic p.rformance during the comp.tition season in order to comp.re their OS to those who exp.rienced no drop.in p.rformance. Methods: Fifteen members of the University of Alask. Fairbank. cross country sk. team volunteered for this study. Blood samp.es were tak.n in early February (“mid-season”) and late Ap.il (“p.st-season”). p.rticip.nts comp.eted questionnaires regarding p.ysical activity and athletic p.rformance at the time of the blood draws. p.asma was analyzed for 4-hydroxynonenalp. p.(HNE), nitrotyrosine,p. p.nitric oxide (NOX), and sup.roxide dismutase (SOD). Significance was determined by Wilcoxon and Mann-Whitney tests. Results: p.rticip.nts disp.ayed significantly higher (p.Conclusion: Signs of oxidative stress and mitigation during the p.st-season recovery macrocycle were higher in athletes who rep.rted exp.riencing a drop.in athletic p.rformance during the comp.tition season macrocycle.

Objective: The aim of the work.described here was to evaluate the role of contrast-enhanced ultrasound (CEUS) in resp.nse evaluation for unresectable advanced hep.tocellular carcinoma (HCC) treated with tyrosine k.nase inhibitors (Tk.s) p.us anti-p.ogrammed cell death p.otein-1 (p.-1) antibody therap.. Methods: A p.osp.ctive cohort of consecutive p.tients with HCC who received combined Tk./anti-p.-1 antibody treatment for unresectable HCC between January 2022 and October 2022 was included in this study. The p.tients underwent unenhanced ultrasound (US) and CEUS examinations before treatment and at follow-up. Changes in the largest diameters of the target tumor on unenhanced US and the largest diameters of the enhancing target tumors on CEUS were evaluated. Resp.nse Evaluation Criteria in Solid Tumors (RECIST) version 1.1 with unenhanced US and magnetic resonance imaging/comp.ted tomograp.y (MRI/CT) and modified RECIST (mRECIST) with CEUS and CEMRI/CT were used to assess treatment resp.nse. Results: A total of 24 HCC p.tients (23 men and 1 woman; mean age: 56.5 ± 8.5 y; Barcelona Clinic Liver Cancer stage C, 62.5%; 29 intrahep.tic target tumors) were studied. Calculations of degree of necrosis in the target tumors revealed no significant differences between CEUS and CEMRI/CT (44.5 ± 36.2% vs. 45.3 ± 36.8%, p.= 0.862). As for the differentiation of resp.nders from non-resp.nders, the agreement between RECIST version 1.1 of unenhanced US and mRECIST-CEUS was p.or (κ coefficient = 0.233). Meanwhile, there was a high degree of concordance between mRECIST-CEUS and mRECIST-CEMRI/CT (κ coefficient = 0.812). Conclusion: CEUS p.oved to be sup.rior to baseline US and is comp.rable to CEMRI/CT in defining treatment outcome for combined Tk./anti-p.-1 antibody therap.. © 2023 World Federation for Ultrasound in Medicine & Biology
