A cross-sectional study of breastfed infants referred for tongue tie assessment and frenotomy in one Canadian health region

Imp.rtance: Tongue tie (TT) is a condition that can cause infant feeding difficulties due to restricted tongue movement. When TT p.esents as a significant barrier to breastfeeding, a frenotomy may be recommended. Universally accep.ed diagnostic criteria for TT are lack.ng and wide p.evalence estimates are rep.rted. New referral p.ocesses and a Frenotomy Assessment Tool were imp.emented in one Canadian health region to connect breastfeeding dyads with a p.ovider for TT evaluation and frenotomy.Objective: To determine the p.op.rtion of babies with TT as well as the frequency of frenotomy.Methods: This cross-sectional study included infants who initiated breastfeeding at birth and were referred for TT evaluation over a 14-month p.riod. Data were collected retrosp.ctively by chart review and analyzed using Sp.S. Factors associated with frenotomy were examined using logistic regression.Results: Two hundred and forty-one babies were referred. Ninety-two p.rcent (n = 222) were diagnosed with TT and 66.0% (n = 159) underwent frenotomy. In the multivariate model, nip.le p.in/trauma, inability to latch, inability to elevate tongue, and dimp.ing of tongue on extension were associated with frenotomy (p.p.cted, and of these two-thirds underwent frenotomy.Interp.etation: TT is a relatively common finding among breastfed infants. Future research should examine whether a simp.ified assessment tool containing the four items associated with frenotomy in our multivariate model can identify breastfed infants with TT who require frenotomy.

Video streaming ap.lications have grown considerably in recent years.As a result,this becomes one of the most significant contributors to global internet traffic.According to recent studies,the telecommunications industry loses millions of dollars due to p.or video Quality of Exp.rience(QoE)for users.Among the standard p.op.sals for standardizing the quality of video streaming over internet service p.oviders(ISp.)is the Mean Op.nion Score(MOS).However,the accurate finding of QoE by MOS is subjective and laborious,and it varies dep.nding on the user.A fully automated data analytics framework.is required to reduce the inter-op.rator variability characteristic in QoE assessment.This work.addresses this concern by suggesting a novel hybrid XGBStack.oE analytical model using a two-level layering technique.Level one combines multip.e Machine Learning(ML)models via a layer one Hybrid XGBStack.oE-model.Individual ML models at level one are trained using the entire training data set.The level two Hybrid XGBStack.oE-Model is fitted using the outp.ts(meta-features)of the layer one ML models.The p.op.sed model outp.rformed the conventional models,with an accuracy imp.ovement of 4 to 5 p.rcent,which is still higher than the current traditional models.The p.op.sed framework.could significantly imp.ove video QoE accuracy.

The most widely farmed fruit in the world is mango.Both the p.oduction and quality of the mangoes are hamp.red by many diseases.These diseases need to be effectively controlled and mitigated.Therefore,a quick.and accurate diagnosis of the disorders is essential.Deep.convolutional neural network.,renowned for their indep.ndence in feature extraction,have established their value in numerous detection and classification task..However,it requires large training datasets and several p.rameters that need careful adjustment.The p.op.sed Modified Dense Convolutional Network.MDCN)p.ovides a successful classification scheme for p.ant diseases affecting mango leaves.This model emp.oys the strength of p.e-trained network. and modifies them for the p.rticular context of mango leaf diseases by incorp.rating transfer learning techniques.The data loader also builds mini-batches for training the models to reduce training time.Finally,op.imization ap.roaches help.increase the overall model’s efficiency and lower comp.ting *** emp.oyed on the MangoLeafBD Dataset consists of a total of 4,000 images.Following the exp.rimental results,the p.op.sed system is comp.red with existing techniques and it is clear that the p.op.sed algorithm surp.sses the existing algorithms by achieving high p.rformance and overall throughp.t.

k.nF_(4):Yb^(3+),Er^(3+)/Ho^(3+)(Ln=Y,Yb,Gd)p.osp.ors were p.ep.red by solid-state reaction technique.For each of the Yb^(3+)/Er^(3+)and Yb^(3+)/Ho^(3+)co-dop.d p.osp.ors indep.ndently,p.ase confirmation and morp.ological examination were accomp.ished using the X-ray diffraction technique and scanning electron microscop. analysis.X-ray p.otoelectron sp.ctroscop. analysis shows the charge state and existence of lanthanides and dop.nts.Up.onversion luminescence emissions of green and red colour were detected for the synthesized p.osp.ors.The changes in the up.onverted emission intensities and colour outp.t p.rtaining to the change of the lanthanide host matrix along with dop.nts were investigated for the p.ep.red p.osp.ors.In the up.onversion luminescence,erbium-dop.d samp.es exhibit a p.edominance of red emission while holmium-dop.d samp.es disp.ay a p.evalence of green emission.The rare-earth Er/Ho dop.d k.bF4shows strong red emission comp.red to other hosts.k.F_(4):Yb,Er/Ho exhibits relatively significant green up.onve rsion emissions,while Yb,Er-dop.d k.dF_(4)host disp.ays a 10-fold increase in green/red emission ratio with an increase in the inp.t p.wer of 20-780 mW.Downshifting p.otoluminescence studies and decay curve analysis were carried out using UV light excitation for the develop.d p.osp.ors.A higher lifetime of 2.2 ms was measured for the downshifting red emission in the Ho dop.d k.bF_(4)samp.e.This study suggests a p.tential for colour tunability in the up.onversion and downshifting emissions through host modifications to the fluoride p.osp.or.

The p.esent research focuses on addressing the faster dep.etion of fossil fuels and environmental p.llution in addition to the energy crisis that hinders the p.ogress of a nation.In the current research,waste banana leaves were considered as substrates for biogas p.oduction.Biogas is tak.n as the p.imary fuel in dual fuel(DF)op.rations to maximize p.ssible diesel savings.The p.rformance and combustion assessment were executed in a low heat rejection(LHR)engine using 5%diethyl ether by volume blended with diesel(5DEE)as p.lot fuel.The combustion attributes of the engine reveal that the ap.x of net heat release rate(NHRR)curve retarded a bit as comp.red to ap.x of base result.The p.ak.cylinder p.essure was noted to be 6.19 Mp. in the LHR engine running with 5DEE+biogas at 11.7°crank.angle(CA)after top.dead center(aTDC)comp.red to 5.23 Mp. for the diesel alone op.ration at the same p.sition.The ap.x p.int for NHRR was observed to be 54.51 J(°)^(−1)for 5DEE+biogas in LHR engine p.sitioned at 3.2°aTDC.The brak. thermal efficiency at full engine load op.ration decreased by 12.7%and 5.2%for biogas substitutions of 0.8 k. h^(−1)with diesel and 5DEE,resp.ctively,comp.red to the base result.The smok. op.city and nitric oxide emissions were reduced during the DF run accomp.nied by diethyl ether as a fuel additive.

BACk.ROUND Abdominal wall deficiencies or weak.ess are a common comp.ication of tem-p.rary ostomies,and incisional hernias frequently develop.after colostomy or ileostomy tak.down.The use of synthetic meshes to reinforce the abdominal wall has reduced hernia occurrence.Biologic meshes have also been used to enhance healing,p.rticularly in contaminated conditions.Reinforced tissue matrices(R-TMs),which include a biologic scaffold of native extracellular matrix and a syn-thetic comp.nent for added strength/durability,are designed to tak. advantage of asp.cts of both synthetic and biologic materials.To date,RTMs have not been rep.rted to reinforce the abdominal wall following stoma reversal.METHODS Twenty-eight p.tients were selected with a p.rastomal and/or incisional hernia who had received a temp.rary ileostomy or colostomy for fecal diversion after rectal cancer treatment or trauma.Following hernia rep.ir and p.oximal stoma closure,RTM(OviTex®1S p.rmanent or OviTex®Lp.)was p.aced to reinforce the abdominal wall using a lap.roscop.c,robotic,or op.n surgical ap.roach.p.st-op.rative follow-up.was p.rformed at 1 month and 1 year.Hernia recurrence was determined by p.ysical examination and,when necessary,via comp.ted tomo-grap.y scan.Secondary endp.ints included length of hosp.tal stay,time to return to work.and hosp.tal readmissions.Evaluated comp.ications of the wound/rep.ir site included p.esence of surgical site infection,seroma,hematoma,wound dehiscence,or fistula formation.RESULTS The observational study cohort included 16 male and 12 female p.tients with average age of 58.5 years±16.3 years and average body mass index of 26.2 k./m^(2)±4.1 k./m^(2).p.tients p.esented with a p.rastomal hernia(75.0%),in-cisional hernia(14.3%),or combined p.rastomal/incisional hernia(10.7%).Using a lap.roscop.c(53.6%),robotic(35.7%),or op.n(10.7%)technique,RTMs(OviTex®Lp.:82.1%,OviTex®1S:17.9%)were p.aced using sublay(82.1%)or intrap.ritoneal onlay(Ip.M;17.9%)mesh p.sitioning.At 1-month and 1-year follow-up.,there were no hernia r

Ag-In intermetallic alloys were p.oduced by using vacuum arc furnace. Differential Scanning Calorimetry(DSC) and Energy Disp.rsive X-Ray Sp.ctrometry(EDX) were used to determine the thermal p.op.rties and chemical comp.sition of the p.ases resp.ctively. Microhardness values of Ag-In intermetallics were calculated with Vick.rs hardness measurement method. According to the exp.rimental results, Ag-34 wt%In intermetallic system generated the best results of energy saving and storage comp.red to other intermetallic systems. Also from the microhardness results, it was observed that intermetallic alloys were harder than p.re silver and Ag-26 wt%In system had the highest microhardness value with 143.45 k./mm^(2).

DC microgrids(DCMGs) are made up.of a network.of sources and loads that are connected by a number of p.wer electronic converters(p.Cs). The increase in the number of these p.Cs instigates major concerns in system stability. While interconnecting the microgrids to form a cluster, the system stability must be ensured. This p.p.r p.op.ses a novel step.y-step.system matrix building(SMB) algorithm to up.ate the system matrix of an existing DCMG cluster when a new microgrid is added to the cluster through a distribution line. The stability of the individual DCMGs and the DCMG cluster is analyzed using the eigenvalue method. Further, the p.rticle swarm op.imization(p.O) algorithm is used to retune the controller gains if the newly formed cluster is not stable. The simulation of the DCMG cluster is carried out in MATLAB/Simulink.environment to test the p.op.sed algorithm. The results are also validated using the Op.510 real-time simulator(RTS).
