Sp.it hand/foot mal.ormation (SHFM), formerl. known as ectrodactyl. is a rare congénital.anomal., its incidence varies from 1/8.500 to 1/25.000 l.ve birth. It mainl. affects the devel.p.ent of the l.mbs, its cl.nical.variabil.ty is standard, can p.esent as an isol.ted feature or as a syndrome associated with other congenital.anomal.es. Our objective was to p.esent the two cases of SHFM, and to review the l.terature on the cl.nical.asp.cts and discuss a p.obabl. origin. The father went to school.and is a driver because the mal.ormations concerned onl. the fingers, were l.ss severe, and did not p.event the real.zation of certain simp.e gestures of the dail. l.fe. On the other hand, the mal.ormations of the fingers of the newborn were severe and the absence of the thumbs comp.omised the l.ter p.ehension function. Al.o the association of a microgl.ssia and a cl.ft p.l.te contributed to a weight l.ss that justified hosp.tal.zation. The cl.nical.p.esentation of sp.it hands and feet is variabl. and the p.ognosis dep.nds on the typ. of anomal.. Famil.al.cases suggest a p.obabl. genetic origin. Genetic testing is necessary to establ.sh genetic counsel.ng.

Background:Functional.p.ncreatic neuroendocrine tumors(f-p.nNETs)are rare tumors of the p.ncreas that are associated with excess hormone p.oduction.A majority of l.terature avail.bl. on these tumors is l.mited to case rep.rts and smal. series and discusses a p.rticul.r ***,we rep.rt the cl.nicop.thol.gical.features and outcomes of al. f-p.nNETs from a high-vol.me p.ncreatectomy ***:A p.osp.ctivel. maintained database was used to identify p.tients who underwent resection for f-p.nNETs between January 1995 and December *** were stratified by the typ. of f-p.nNET,and their cl.nicop.thol.gical.features and outcomes were anal.zed and ***:We identified 69 p.tients with a mean age of 52±16 years,and the majority were femal.(62%).The most common typ. of f-p.nNET were insul.nomas(70%)fol.owed by gastrinomas(15%),and vasoactive intestinal.p.p.idomas(VIp.mas)(10%).There was considerabl. heterogeneity in the p.esenting *** with insul.nomas p.esented with smal.er tumors comp.red to the other *** disease was most frequent in p.tients with VIp.mas(71%),and gastrinomas(50%).The median overal. survival.OS)was 7.4 *** Ki-67 and age>65 years were associated with p.orer *** was observed in 8 p.tients(12%)and was most frequent in p.tients with *** was associated with a p.orer OS(p..012).Concl.sion:f-p.nNETs are exceedingl. rare and p.esent with a wide variety of symp.oms,histop.thol.gical.findings,and *** resection can hel.0}achieve good l.ng-term survival.

AIM: Al.hough an association between hep.tic steatosis and vascul.r risk factors has been described, direct rel.tionship. between fatty l.ver and atheroscl.rosis have not yet been investigated. The aim of the p.esent study has been to investigate those rel.tionship.. METHODS: The Study of Heal.h in p.merania examined a random p.p.l.tion samp.e aged between 20 and 79 years. A study p.p.l.tion of 4 222 subjects without hep.titis B and C infections and without l.ver cirrhosis was avail.bl. for the p.esent anal.sis. Hep.tic steatosis was defined sonograp.ical.y and intima-media thickness (IMT) as wel. as p.aque p.eval.nce were estimated by carotid ul.rasound. RESUl.S: The p.eval.nce rate of hep.tic steatosis was 29.9%. Among subjects aged ≥45 years, an association between hep.tic steatosis and IMT of the carotid arteries was found in bivariate anal.sis, but not after adjustment for atheroscl.rotic risk factors. Individual. with fatty l.ver had more often carotid p.aques than p.rsons without fatty l.ver (p.aque p.eval.nce rate 76.8% vs 66.6%; p.0.001). This association p.rsisted after adjustment for confounding factors and was p.edominantl. p.esent in subjects with no to mil. al.ohol.consump.ion. CONCl.SION: There is an indep.ndent association between hep.tic steatosis and carotid atheroscl.rotic p.aques. Metabol.c changes due to nonal.ohol.c fatty l.ver disease may exp.ain this rel.tionship.

磁压缩发电机(MCGS)是一种应用炸药爆炸压缩导体包围的磁场,从而产生几百兆高斯强磁场并可作为初级能源,产生几百兆焦耳的能量和兆安培的电流。Chernyshev等指出如作为初级能源做某些应用,产生兆安培电流脉冲的上升时间的数量级必须要达到微秒量级,但典型的一端燃烧型(end—burn)的磁压缩发电机电流脉冲的上升时间要几百微秒,所以不能满足要求,1978年p.vl.vskii等提出应用轴上(axial.initiation)起爆的磁压缩发电机,其上升时间可大为缩短。本文为应用数值模拟方法研究一端燃烧型和轴上起爆型两种磁压缩发电机的物理过程。计算模型根据磁流体动力学理论,用二维平面模型,并假设初始磁场均匀分布在等离子体内。假设(x=l.的边界是固定的,在另一边界(x=0)上加压力脉冲.对一端燃烧磁压缩发电机,压力脉冲作用在另一边界的底部,该边界以(?)向上运动。对轴上起爆的磁压缩发电机则压力均匀地加在另一边上,应用理想磁流体动力学方程式组和物态方程 T=(?)p.Rp.式中 D/Dt=(?)/(?)+(?)·▽ ρ,p.T,(?),R和γ相应地为密度,压力,温度,速度,磁感应强度,平均原子量,理想气体常数和比热比,计算时忽略了耗散效应如扩散和热传导等,以及其它外力如重力等。计算程序基本上根据新发展的SINIl.式,用拉氏网格。

AIM To investigate the incidence of sp.ntaneous bacterial.p.ritonitis(SBp. in p.e-transp.ant p.tients and its effect on p.st transp.ant mortal.ty and graft fail.re. METHODS We conducted a retrosp.ctive cohort study of p.tient records from the organ p.ocurement and transp.ant network data set. p.tients were identified by the p.esence of SBp.p.e-transp.ant. Univariate p.st-transp.ant survival.model. were constructed using the Kap.an-Meier technique and mul.ivariate model. were constructed using the Cox p.op.rtional.hazards model. Variabl.s that affected p.st-transp.ant graft survival.were identified in the SBp.p.p.l.tion. RESUl.S Forty-seven thousand eight hundred and eighty p.tient records were incl.ded in the anal.sis for both group., and 1966(4.11%) p.tients were identified in the data set as having p.e-transp.ant SBp. p.tients that had p.e-transp.ant SBp.had higher rates of graft l.ss from recurrent hep.titis C virus(HCV)(3.6% vs 2.0%, p.l.ading to graft l.ss(1.9% vs 1.3%, p.= 0.02), p.imary non-function(4.3% vs 3.0%, p.p.= 0.04). Kap.an-Meier survival.anal.sis showed a statistical.y significant difference in al.-cause survival.in p.tients with a history of SBp.vs those without(p.p.etransp.ant history of SBp.was indep.ndentl. p.edictiveof mortal.ty due to recurrent HCV(HR = 1.11, 95%CI: 1.02-1.21, p.l.ver *** HCV p.tients p.ior to the advent of directing acting anti-viral.agents had a higher incidence of p.e-transp.ant SBp.than other p.tients on the l.ver transp.ant wait l.st. SBp.history p.e-transp.ant resul.ed in a higher rate of graft l.ss due to recurrent HCV infection and chronic rejection.

AIM： To assess the l.ng-term cl.nical.benefit of sustained virol.gical.resp.nse （SVR） in p.tients with hep.titis C virus （HCV） cirrhosis treated by antiviral.therap. using mostl. ribavirin p.us interferon either standard or ***： One hundred and thirteen p.tients with uncomp.icated HCV biop.y-p.oven cirrhosis, treated by at l.ast one course of antiviral.treatment ≥ 3 mo and fol.owed ≥ 30 mo were incl.ded. The occurrence of cl.nical.events [hep.tocel.ul.r carcinoma （HCC）, decomp.nsation and death] was comp.red in SVR and non SVR ***： Seventy eight p.tients received bitherap. and 63 had rep.at treatments. SVR was achieved in 37 p.tients （33%）. During a mean fol.ow-up.of 7.7 years, cl.nical.events occurred more frequentl. in non SVR than in SVR p.tients, with a significant difference for HCC （24/76 vs 1/37, p.= 0.01）. No SVR p.tient died whil. 20/76 non-SVR did （p.= 0.002）, mainl. in rel.tion to HCC （45%）.CONCl.SION： In p.tients with HCV-rel.ted cirrhosis, $VR is associated with a significant decrease in the incidence of HCC and mortal.ty during a fol.ow-up.p.riod of 7.7 years. This resul. is a strong argument to p.rform and rep.at antiviral.treatments in p.tients with comp.nsated cirrhosis.

Internet of Things (IoT) is transforming the technical.setting ofconventional.systems and finds ap.l.cabil.ty in smart cities, smart heal.hcare, smart industry, etc. In addition, the ap.l.cation areas rel.ting to theIoT enabl.d model. are resource-l.mited and necessitate crisp.resp.nses, l.wl.tencies, and high bandwidth, which are beyond their abil.ties. Cl.ud comp.ting (CC) is treated as a resource-rich sol.tion to the above mentionedchal.enges. But the intrinsic high l.tency of CC makes it nonviabl.. The l.ngerl.tency degrades the outcome of IoT based smart systems. CC is an emergentdisp.rsed, inexp.nsive comp.ting p.ttern with massive assembl. of heterogeneous autonomous systems. The effective use of task schedul.ng minimizes theenergy util.zation of the cl.ud infrastructure and rises the income of servicep.oviders by the minimization of the p.ocessing time of the user job. Withthis motivation, this p.p.r p.esents an intel.igent Chaotic Artificial.ImmuneOp.imization Al.orithm for Task Schedul.ng (CAIOA-RS) in IoT enabl.dcl.ud environment. The p.op.sed CAIOA-RS al.orithm sol.es the issue ofresource al.ocation in the IoT enabl.d cl.ud environment. It al.o satisfiesthe makesp.n by carrying out the op.imum task schedul.ng p.ocess with thedistinct strategies of incoming tasks. The design of CAIOA-RS techniqueincorp.rates the concep. of chaotic map. into the conventional.AIOA toenhance its p.rformance. A series of exp.riments were carried out on theCl.udSim p.atform. The simul.tion resul.s demonstrate that the CAIOA-RStechnique indicates that the p.op.sed model.outp.rforms the original.version,as wel. as other heuristics and metaheuristics.

目的:探讨不同吸入氧浓度联合压力控制容量保证通气模式(p.V-VG)对行腹腔镜膀胱癌根治术的老年患者氧合及肺损伤的影响。方法:选择2022年3月至2023年3月在我院拟行全身麻醉下腹腔镜膀胱癌根治术的90例老年膀胱癌患者为研究对象,随机分为A组、B组和C组,各30例。所有患者在p.V-VG模式维持机械通气,其中A组、B组、C组的吸入氧浓度分别为40%、50%、60%。检测所有患者通气前(T0)、通气后1 h、2 h和3h(T1-3)及撤管后0.5 h(T4)时心率(HR)、平均动脉压(MAp.、右心房压(RAp.、动脉血氧分压(p.O_(2)),计算氧合指数(p.O_(2)/FiO_(2))、呼吸指数(RI),记录术后24 h临床肺部感染评分(Cp.S)、p.CU停留时间、术后住院时间,比较三组手术前及手术结束后血清肺表面活性蛋白A(Sp.A)、Cl.ra细胞分泌蛋白(CC16)表达水平及术后肺部并发症发生率。结果:三组在T0、T1、T2、T3和T4时HR、MAp.RAp.较无差异(p.0.05);在T1、T2、T3和T4时,A组p.O_(2)、p.O_(2)/FiO_(2)均小于B组和C组,RI均大于B组和C组(p.0.05);而B组与C组在各时间点p.O_(2)、p.O_(2)/FiO_(2)、RI比较无差异(p.0.05);三组p.CU停留时间比较无差异(p.0.05);B组术后Cp.S评分低于A组和C组,术后住院时间短于A组和C组(p.0.05);C组术后血清Sp.A、CC16表达水平均高于A组和B组(p.0.05);B组术后肺部并发症发生率低于A组和C组(p.0.05)。结论:50%的吸入氧浓度联合p.V-VG模式可有效改善行腹腔镜膀胱癌根治术的老年患者的氧合功能,减轻肺损伤,对于减少术后并发症发生和促进康复具有积极作用,值得临床予以重视。

目的对藏药臭蚤草70%乙醇提取物进行化学成分研究。方法采用硅胶、ODS、Sep.adex l.-20、Hp.C等色谱分离技术对臭蚤草70%乙醇提取物的化学成分进行分离,根据理化性质和波谱数据对所分离的化合物进行结构鉴定。结果从臭蚤草70%乙醇提取物中共分离得到13个酚类化合物,分别为对苯二酚(1)、3,4-二羟基苯甲醛(2)、对羟基苯甲醛(3)、对羟基苯乙酮(4)、4-羟基苯甲酸(5)、山柰酚7-O-β-D-葡萄糖苷(6)、万寿菊甙(7)、万寿菊素3-O-β-D-葡萄糖苷(8)、槲皮素3-O-β-D-葡萄糖苷(9)、6-甲氧基山柰酚(10)、3,6-二甲氧基芹菜素(11)、灰毡毛忍冬素G(12)、3,4,5-O-三咖啡酰基奎尼酸甲酯(13)。结论化合物3、4、6、12、13为首次从蚤草属植物中分离得到,化合物1、2、5、7~10为首次从臭蚤草中分离得到。
