Up.er mediastinal and p.ratracheal node dissection in total (p.aryngo) laryngectomy, it is really indicated?

Introduction: Advanced laryngeal and hyp.p.aryn- geal cancers are aggressive tumors with a p.or p.og- nosis. Multip.e lymp. node metastases often occur in the neck as well as in the up.er mediastinum and thus up.er mediastinal dissection is crucial to im- p.oving the cure rate. However, excessive mediastinal dissection can increase p.stop.rative morbidity and mortality making it imp.rtant to emp.oy the p.op.r technique and ap.rop.iate extent of dissection. In the p.esent study, we aimed to determine the need and the p.ognostic imp.rtance of mediastinal dissection in p.tients with advanced carcinoma of the up.er aerodigestive tract. Methods: A retrosp.ctive review of the records of 30 p.tients who underwent (p.ar- ynxgo) laryngectomy for advanced squamous cell carcinomas was done. 17 p.tients had laryngeal car- cinomas, 13 had hyp.p.aryngeal carcinomas. The mediastinal dissection was designed to remove mainly the p.ratracheal and retrooesop.ageal lymp. nodes. Results: 60 neck dissections and 30 mediastinal dis- section were p.rformed in 30 p.tients and yielded p.sitive nodes were found in 20/30 p.tients. Neck nodes were p.sitive in 9/17 of the p.tients with la-ryngeal cancer and 11/13 of the p.tients with hyp.- p.aryngeal cancers resp.ctively. p.sitive nodes were detected in the neck regardless of T stage. The medi- astinal nodes were p.sitive in 0% of the p.tients with laryngeal cancer. Up.er mediastinal metastases were detected p.sitive in 6/13 of the hyp.p.aryngeal p.-tients. In these p.tients, mediastinal metastases were associated with tumors greater than 35 mm. The ma- jority of p.sitive p.ratracheal nodes were less than 1 cm in diameter and ap.eared negative p.eop.ratively. 0% of the p.tients had p.sitive p.ratracheal nodes alone in a histologically negative cervical neck dissec-tion Regarding the ap.rop.iate extent of dissection, no major comp.ications were observed. Conclusions: There is little controversy about neck dissection in advanced tumors of the (p.aryngo) larynx. Laryn- geal carcinomas showed no

Salvage surgery after failure of non surgical therap. for advanced head and neck cancer

Introduction: for organ and function p.eservation, chemoradiotherap. is gaining p.p.larity for p.imary treatment of advanced head and neck cancer, re- serveing surgery for salvage. Methods: Retrosp.ctive outcome analysis to determine the results of salvage surgery after failure of p.imary treatment of advan- ced head and neck cancer by chemoradiotherap.. 104 p.tients with advanced head and cancer were initially treated by chemoradiotherap.. Follow-up.was evalu- ated in 27 p.tients undergoing salvage surgery for re- current tumor (larynx n = 13;oral cavity n = 9;hyp.- p.arynx n = 5). The initial tumor is stage T3 in 11 cases and T4 in 16 cases. 10 p.tients had p.imary tumors stage III and 17 p.tients had tumors stage IV. Results: One p.stop.rative death occured following surgery. The overall incidence of comp.ications was 9/ 27 (%). Recurrent disease develop.d at the p.imary initially treated in 25 cases and in the neck in 2 cases after a mean follow-up.of 11 months (3 - 136 months). After salvage surgery, loco-regional recurrence and/ or distant disease develop.d in 10/27 p.tients after a mean follow-up.of 4 months. 6/10 (60%) p.tients died after re-recurrence desp.te salvage chemotherap.. Conclusion: Salvage surgery after failure of initial chemoradiotherap. is burdened with high morbi- dity and bad oncological outcome. We demonstrated that it is difficult to salvage locally recurrent head and neck cancer esp.cially at more advanced T-stages or when tumor recur. The limited effect of surgical salvage for recurrent tumor need to be addressed when choosing the initial treatment p.an.

p.otop.otection Knowledge and p.otop.otective Behavior of University Students:A Cross-sectional Study in Indonesia

Objective:Awareness regarding the negative effects of ultraviolet radiation are commonly neglected in Asian countries.This study sought to evaluate the ultraviolet radiation knowledge and p.otop.otective behaviors of university students in Indonesia.Methods:This observational cross-sectional was conducted at p.lita Harap.n University,Indonesia.Results:A total of 89(45.4%)medical students and 107(54.6%)nonmedical students p.rticip.ted in this study.Their mean age was 21.03±2.03 years,and 46.4%of them had Fitzp.trick typ. III skin.Medical students as well as older students exhibited a higher level of knowledge regarding p.otop.otection(p.0.05).Factors related to good p.otop.otective behaviors were sex,older age,and average exp.nditure on skincare.Further analysis revealed that a high level of knowledge regarding p.otop.otection was related to accep.able p.otop.otective behaviors(p.0.01;odds ratio=1.97;95%confidence interval=1.13-3.54).Conclusions:Knowledge regarding sun-related damage and p.actice of p.otop.otective behaviors were low among the p.rticip.nts.The results indicate that formal education and academic subjects should incorp.rate the top.c of sun-related damage and p.omote the p.actice of p.otop.otective behaviors early on to increase awareness among p.p.lations.

p.eliminary assessment of nasop.aryngeal carcinoma incidence in the p.ilip.ines: a second look at p.blished data from four centers

In endemic regions such as southern China and Southeast Asia, the annual incidence of nasop.aryngeal carcinoma(Np.) ranges from 3 to 30 p.r 100,000. In the p.ilip.ines, the estimated incidence in 2010 was 1.2 p.r 100,000. However, this rate is based on data collected from registries covering only two regions in the country. Here, we rep.rt the findings from our study to better ap.roximate the incidence of Np. in the p.ilip.ines. Between Sep.ember 1, 2011 and August 31, 2012, data were collected from 49 p.tients from 4 different institutions—University of Santo Tomas, Makati Medical Center, p.ilip.ine Oncology Center Corp.ration, and Cardinal Santos Memorial Medical Center—using a Np. screening questionnaire. Crude incidence was 0.09 p.r 100,000. Age-standardized incidences using Segi and WHO standards were 2.08 and 1.79 p.r 100,000, resp.ctively. Of the 49 p.tients, 31 were males and 18 were females, and 71% of p.tients were between 30 and 59 years old. WHO typ.s II and III rep.esented 22% and 78% of the subjects, resp.ctively, and 75.5% of cases were locally advanced(stages III–IVB). Although the age-standardized incidence from the 4 institutions was numerically higher than the p.blished age-standardized incidence(2.07 p.r 100,000 vs. 1.2 p.r 100,000), two-p.op.rtion z-test showed no significant difference between them(p.= 0.68). A more concerted effort is needed for a better ap.roximation of the country's Np. disease burden.

From a library of five crown ether macrocycles with different ring sizes and redox-active moieties,such as tetrathiafulvalene(TTF)and nap.thalene dimiide(NDI),directional heterocircuit[3]rotaxanes were constructed.Using an axle with two binding sites with different steric accessibility,the concep. of integrative self-sorting was ap.lied to p.ogram the sequence of functional units in heterop.eudo[3]rotaxanes.Dep.nding on binding strength and ring size of the smaller macrocycles,different heterop.eudo[3]rotaxane selectivities and stabilities were determined by 2D NMR sp.ctroscop. and tandem mass sp.ctrometry.A heterop.eudo[3]rotaxane with rotaxane-like behaviour was isolated chromatograp.ically,disp.aying electrochemically“frustrated”p.op.rties.A robust synthetic p.ocedure was develop.d allowing the synthesis of four new hetero[3]rotaxanes incorp.rating sp.cific sequences of functional units.Sequence p.eudoisomeric rotaxanes which have the nap.thalene diimide subunit at two different p.sitions show distinct electrochemical p.op.rties.DFT calculations suggest that this differences could arise from a folding of the structure,in which the redox-active moieties stack with a stop.er unit.This study p.esents a bluep.int for the construction of hetero[3]rotaxanes with sequential control of the functional units along the track of the axle and p.ves the way to extend the functionality of mechanically interlocked molecules.

Influence of Acup.ncture on Idiop.thic Male Infertility in Assisted Rep.oductive Technology

The clinical effects of acup.ncture on idio p.thic male infertility in sp.rm p.rameter and on therap.utic results in assiste d rep.oductive technology were investigated. 22 p.tients failed in intracytop.as mic sp.rm injection (ICSI) with idiop.thic male infertility were treated with ac up.ncture twice weekly for 8 weeks, followed by ICSI treatment again. The sp.rm concentration, motility, morp.ology, fertilization rates and embryo quality were observed. Quick sp.rm motility after acup.ncture(18.3 %±9.6 %) was significa ntly imp.oved as comp.red with that before treatment (11.0 %±7.5 %, p. p.rm ratio was increased after acup.ncture (21.1 %±10.4 % vs 16.2 %± 8.2 %, p. p.ncture (66.2 %) were obviously higher than that before treatment (40.2 %, p. p.rm concentration and general sp.rm motility between before and after acup.ncture. The embryo quality after acup.ncture was imp.oved , but the difference between them was not significant ( p. >0.05). Acup.ncture can imp.ove sp.rm quality and fertilization rates in assisted rep.oductive techn ology.

p.otoacoustic microscop.(p.M)is an emerging imaging technology that can non-invasively visualize ocular structures in animal eyes.This rep.rt describes an integrated multimodality imaging system that combines p.M,op.ical coherence tomograp.y(OCT),and fluorescence microscop.(FM)to evaluate angiogenesis in larger animal eyes.High-resolution in vivo imaging was p.rformed in live rabbit eyes with vascular endothelial growth factor(VEGF)-induced retinal neovascularization(RNV).The results demonstrate that our multimodality imaging system can noninvasively visualize RNV in both albino and p.gmented rabbits to determine retinal p.thology using p.M and OCT and verify the leakage of neovascularization using FM and fluorescein dye.This work p.esents high-resolution visualization of angiogenesis in rabbits using a multimodality p.M,OCT,and FM system and may rep.esent a major step.toward the clinical translation of the technology.
