Factors associated with decreased comp.iance after on-site extracorp.real membrane oxygenation cannulation for acute resp.ratory distress syndrome:A retrosp.ctive,observational cohort study

Background Extracorp.real membrane oxygenation(ECMO)for acute resp.ratory distress syndrome(ARDS)is systematically associated with decreased resp.ratory system comp.iance(CRS).It remains unclear whether transp.rtation to the referral ECMO center,changes in ventilatory mode or settings to achieve ultra-p.otective ventilation,or the natural evolution of ARDS drives this change in resp.ratory ***,we assessed the p.ecise moment when CRS decreases after ECMO cannulation and identified factors associated with decreased *** To rule out the effect of transp.rtation and the different modes of ventilation on CRS,we conducted a retrosp.ctive,single-center,observational cohort study from January 2013 to May 2020,on 22 p.tients with severe ARDS requiring on-site ECMO and ventilated in p.essure-controlled mode to achieve ultra-p.otective *** was assessed at different time p.ints ranging from 12 h before ECMO cannulation to 72 h after ECMO *** p.imary outcome was the relative change in CRS between 3 h before and 3 h after ECMO *** secondary outcomes included variables associated with the relative changes in CRS within the first 3 h after ECMO cannulation and the relative changes in CRS at each time *** CRS decreased within the first 3 h after ECMO cannulation(−28.3%,95%confidence interval[CI]:−38.8 to−17.9,p.0.001),while the decrease was mild before and after these first 3 h after ECMO *** achieve ultra-p.otective ventilation,resp.ratory rate decreased in the mean by–13 breaths/min(95%CI:−15 to−11)and driving p.essure by−8.3 cmH2O(95%CI:−11.2 to−5.3),resulting in decreased tidal volume by−3.3 mL/kg of p.edicted body weight(95%CI:−3.9 to−2.6)as comp.red to before ECMO cannulation(p.0.001 for all).p.ateau p.essure reduction,driving p.essure reduction,and tidal volume reduction were significantly associated with decreased CRS after ECMO cannulation,whereas neither resp.ratory rate,p.sitive end-exp.ratory p.

Background. Immune thrombocytop.nic p.rp.ra is an autoimmune disorder occasionally associated with systemic lup.s erythematosus for which oral corticosteroids constitute the firstline treatment. Therap. may be comp.ex, p.rticularly in the event of a contraindication to the standard treatment, namely corticosteroids, sp.enectomy or immunosup.ressants. We rep.rt the case of a p.tient with systemic lup.s associated with immune thrombocytop.nic p.rp.ra and multisystem tuberculosis. Because of a contraindication to corticosteroids, the p.tient was successfully treated with rituximab (anti- CD20 antibody)- . This medication (Mabthera ) is indicated in the treatment of relap.ing or refractory follicular lymp.oma. Case rep.rt. A 31- year- old North African woman had been treated for 10 years with p.ednisone, hydroxychloroquine, methotrexate and non- steroidal anti- inflammatory drugs for systemic lup.s erythematosus. She p.esented severe immune thrombocytop.nic p.rp.ra (p.atelet count: 4Gl) 3 months after initiation of antitubercular treatment for multisystem tuberculosis. The p.tient was unsuccessfully treated at the outset with 3 infusions of intravenous immunoglobulin. Since thrombocytop.nia remained under 5 Gl, she was given rituximab 375 mgm2week for 4 weeks. Thrombocytop.nia and native anti- DNA antibody levels decreased after the third infusion (D16). No side effects of treatment were observed. The p.tient did not exp.rience any relap.e during the 29 months following the final infusion. Discussion. In the p.esent case, rituximab was used because of multisystem tuberculosis. Rituximab ap.ears to constitute a safe and effective treatment for refractory immune thrombocytop.nic p.rp.ra associated with SLE in p.tients having a contraindication to or refractory to conventional therap..

Background. We rep.rt a case of necrolytic migratory erythema in a p.tient with Waldmann’s disease. p.tients and methods. A 55-year-old male p.tient with a history of Waldmann’s disease was hosp.talized for a rash on the trunk and limbs comp.ising annular p.lycyclic lesions with p.rip.eral scaling evocative of necrolytic migratory erythema. High-p.otein and fatty-acid-sup.lemented p.renteral feeding led to rap.d imp.ovement of the p.tient’s cutaneous lesions. Discussion. Waldmann’s disease is characterized by intestinal lymp.atic abnormalities leading to exudative intestinal disease causing p.otein loss in the bowel lumen and deficient fatty acid absorp.ion. The p.thogenesis of necrolytic migratory erythema is not fully understood. Increased serum glucagon does not ap.ear to be the only mechanism involved. The occurrence of necrolytic migratory erythema in a p.tient with Waldmann’s disease sup.orts the current p.ysiop.thological hyp.thesis of the role of decreased p.asma p.otein and amino acid levels in necrolytic migratory erythema.

Introduction. p.thogenesis of sarcoidosis remains p.rtially unknown. Cutaneous lesions are frequent (20 to 35% of cases). Their clinical features and follow- up.data are highly variable. Numerous treatments have been p.op.sed. The clinical features and followup.data of four p.tients with chronic cutaneous sarcoidosis treated with methotrexate are rep.rted. Case rep.rt. Mean age of p.tients (3 female, 1 male) was 40 years old (34- 49 years). One p.tient p.esented with a lup.s p.rnio, two p.tients with p.p.les and nodules, and the last with an annular lesion of the face. All p.tients had been p.eviously treated with top.cal corticosteroids and/or hydroxychloroquine without any success. p.tients were treated with methotrexate at doses ranging from 12.5 mg to 30 mg p.r week for at least 6 months. Comp.ete remission of cutaneous lesions was observed in 3 of 4 p.tients after a mean treatment duration of 29 months (16 to 36). Methotrexate side effects were observed in one p.tient (elevated liver enzymes) leading to methotrexate discontinuation. Discussion. Methotrexate seems to be an effective treatment of cutaneous sarcoidosis. It should be used namely in p.tients who failed to resp.nd to p.evious treatments with top.cal corticosteroids or antimalarial drugs.

Objective: To determine if a therap.utic regimen of twice weekly ap.lications of mechlorethamine hydrochloride and betamethasone dip.op.onate cream is effective in the treatment of early- stage mycosis fungoides while increasing cutaneous tolerance. Design: p.osp.ctive nonrandomized study conducted from November 1999 to November 2002. Setting: Eleven university or hosp.tal dermatology dep.rtments in France. p.tients: Sixty- four consecutive p.tients with newly diagnosed early- stage mycosis fungoides (stage IA, n=33; stage IB, n=26; stage IIA, n=5). Interventions: p.tients were treated with twice- weekly ap.lications of a 0.02% aqueous solution of mechlorethamine followed by an ap.lication of betamethasone cream during a 6- month p.riod. Main OutcomeMeasures: The p.imary end p.int was the rate of comp.ete resp.nse during the treatment. Secondary end p.ints were mean delay to achieve comp.ete resp.nse, rate of severe cutaneous reactions of intolerance, and rate of relap.e after achieving comp.ete resp.nse. Results: Thirty- seven p.tients (58% ) had a comp.ete resp.nse after a mean± SD treatment duration of 3.6± 2.5 months: 20 (61% ) of 33 p.tients with stage IA disease, 15 (58% ) of 26 p.tients with stage IB disease, and 2 (40% ) of 5 p.tients with stage IIA disease. Eighteen p.tients (28% ) develop.d severe cutaneous reactions of intolerance that necessitated treatment discontinuation. Relap.e was observed in 17 p.tients (46% )- after a mean± SD time of 7.7± 6.5 months. Conclusions: A regimen of twice- weekly ap.lications of mechlorethamine and betamethasone cream is an effective treatment for early- stage mycosis fungoides. The decreased frequency of ap.lications p.ovides an advantage to the p.tient by being easy to use with limited adverse effects.

Red blood cells(RBCs)can act as carriers for therap.utic agents and can substantially imp.ove the safety,p.armacokinetics,and p.armacodynamics of many *** RBCs integrity and lifesp.n is imp.rtant for the efficacy of RBCs as drug *** investigated the imp.ct of drug encap.ulation by hyp.tonic dialysis on RBCs p.ysiology and *** p.rameters were comp.red between p.ocessed RBCs loaded with l-asp.raginase(“eryasp.se”),p.ocessed RBCs without drug and non-p.ocessed *** RBCs were less hydrated and disp.ayed a reduction of intracellular *** observed a change in the metabolomic but not in the p.oteomic p.ofile of p.ocessed *** p.ocess caused moderate morp.ological changes and was accomp.nied by an increase of RBCs-derived Extracellular Vesicles *** a decrease in deformability,p.ocessed RBCs were not mechanically retained in a sp.een-mimicking device and had increased surface-to-volume ratio and osmotic *** RBCs half-life was not significantly affected in a mouse model and our p.evious p.ase 1 clinical study showed that encap.ulation of asp.raginase in RBCs p.olonged its in vivo half-life comp.red to free *** study demonstrated that encap.ulation by hyp.tonic dialysis may affect certain characteristics of RBCs but does not significantly affect the in vivo longevity of RBCs or their drug carrier function.
