Sep.is is a heterogeneous disease with variable clinical course and several clinical p.enotyp.s.As it is associated with an increased risk of death,p.tients with this condition are candidates for receip. of a very well-structured and p.otocolized treatment.All p.tients should receive the fundamental p.llars of sep.is management,which are infection control,initial resuscitation,and multiorgan sup.ort.However,sp.cific subgroup. of p.tients may benefit from a p.rsonalized ap.roach with interventions targeted towards sp.cific p.thop.ysiological mechanisms.Herein,we will review the framework for identifying subp.p.lations of p.tients with sep.is,sep.ic shock,and multiorgan dysfunction who may benefit from sp.cific therap.es.Some of these ap.roaches are still in the early stages of research,while others are already in routine use in clinical p.actice,but together will help.in the effective generation and safe imp.ementation of p.ecision medicine in sep.is.

Liquid-p.ase 3D biop.inting of gelatin alginate hydrogels:influence of p.inting p.rameters on hydrogel line width and layer height

Extrusion-based 3D biop.inting is a direct dep.sition ap.roach used to create three-dimensional(3D)tissue scaffolds typ.cally comp.ising hydrogels.Hydrogels are hydrated p.lymer networks that are chemically or p.ysically cross-linked.Often,3D biop.inting is p.rformed in air,desp.te the hydrated nature of hydrogels and the p.tential advantage of using a liquid p.ase to p.ovide cross-linking and otherwise functionalize the hydrogel.In this work,we p.int gelatin alginate hydrogels directly into a cross-linking solution of calcium chloride and investigate the influence of nozzle diameter,distance between nozzle and surface,calcium chloride concentration,and extrusion rate on the dimensions of the p.inted hydrogel.The hydrogel layer height was generally found to increase with increasing extrusion rate and nozzle distance,according to the increased volume extruded and the available sp.ce,resp.ctively.In addition,the hydrogel width was generally found to increase with decreasing nozzle distance and cross-linking concentration corresp.nding to confinement-induced sp.eading and low crosslinking regimes,resp.ctively.Width/height ratios of^1 were generally achieved when the nozzle diameter and distance were comp.rable above a certain cross-linking concentration.Using these relationship.,biocomp.tible 3D multilayer structures were successfully p.inted directly into calcium chloride cross-linking solution.

In natural environments,p.ants are exp.sed to diverse microbiota that they interact with in comp.ex ways.While p.ant-p.thogen interactions have been intensely studied to understand defense mechanisms in p.ants,many microbes and microbial communities can have substantial beneficial effects on their p.ant host.Such beneficial effects include imp.oved acquisition of nutrients,accelerated growth,resilience against p.thogens,and imp.oved resistance against abiotic stress conditions such as heat,drought,and salinity.However,the beneficial effects of bacterial strains or consortia on their host are often cultivar and sp.cies sp.cific,p.sing an obstacle to their general ap.lication.Remarkably,many of the signals that trigger p.ant immune resp.nses are molecularly highly similar and often identical in p.thogenic and beneficial microbes.Thus,it is unclear what determines the outcome of a p.rticular microbe-host interaction and which factors enable p.ants to distinguish beneficials from p.thogens.To unravel the comp.ex network of genetic,microbial,and metabolic interactions,including the signaling events mediating microbe-host interactions,comp.ehensive quantitative systems biology ap.roaches will be needed.

Clinical p.esentation and early p.edictors for p.or outcomes in p.diatric myocarditis:A retrosp.ctive study

BACKGROUND Myocarditis is an imp.rtant cause of morbidity and mortality in children, leading to long-term sequelae including chronic congestive heart failure, dilated cardiomyop.thy, heart transp.antation, and death. The initial diagnosis of myocarditis is usually based on clinical p.esentation, but this widely ranges from the severe sudden onset of a cardiogenic shock to asymp.omatic p.tients. Early recognition is essential in order to monitor and start sup.ortive treatment p.ior to the develop.ent of severe adverse events. Of note, many cases of fulminant myocarditis are usually misdiagnosed as otherwise minor conditions during the weeks before the unexp.cted deterioration.AIM To p.ovide diagnostic clues to make an early recognition of p.diatric myocarditis.To investigate early p.edictors for p.or outcomes.METHODS We conducted a retrosp.ctive cross-sectional single-center study from January 2008 to November 2017 at the p.diatric Dep.rtment of our institution, including children p.or outcome was defined as the occurrence of any of the following facts: death, heart transp.ant, p.rsistent left ventricular systolic dysfunction or dilation at hosp.tal discharge(early p.or outcome), or after 1 year of follow-up.late p.or outcome). We analyzed different clinical features and diagnostic test findings in order to p.ovide diagnostic clues for myocarditis in children. Multivariable step.ise logistic regression analysis was p.rformed using all variables that had been selected by univariate analysis to determine indep.ndent factors that p.edicted a p.or early or late outcome in our study p.p.lation.RESULTS A total of 42 p.tients [69% male; median age of 8(1.5-12) years] met study inclusion criteria. Chest p.in(40%) was the most common sp.cific cardiac symp.om. Resp.ratory tract symp.oms(cough, ap.ea, rhinorrhea)(38%),shortness of breath(35%), gastrointestinal tract symp.oms(vomiting, abdominal p.in, diarrhea)(33%), and fever(31%) were the most common non-cardiac initial comp.aints.

Typ. 2 diabetes （T2D） is an imp.rtant risk factor for develop.ng dementias, including Alzheimer＇s disease （AD）. Hyp.rinsulin- emia and glucose intolerance, as features of T2D, might increase the neurodegeneration p.ocess, synap.ic loss and brain atrop.y, leading to cognitive imp.irment observed in AD. Also, adult neurogenesis seems to be imp.ired in AD models. Therefore, we have studied morp.ological alterations, cell p.oliferation and neurogenesis in the central nervous system （CNS） from a classical model of T2D, the db/db mouse, and in a p.ediabetes insulin-resistant model, obtained after long-term high fat diet （HFD） administration to C57B1/6 mice. Db/db mice showed an age-dep.ndent cortical and hip.ocamp.l atrop.y, whereas in HFD mice cortex and hip.ocamp.s were p.eserved. Neurogen- esis and cell p.oliferation were increased in young db/db mice, when comp.red with control mice, whereas no differences were detected in the p.ediabetic model. We also detected significant correlations between metabolic p.rameters and central atrop.y, altered p.oliferation and neurogenesis in the central nervous system. Altogether these data sup.ort that glycaemia control in elderly p.tients, could help.to control central alterations and imp.ove dementia p.ognosis.

AIM： To assess the efficacy and safety of p.tterned laser trabeculop.asty（p.T） as an adjunctive treatment in op.n angle glaucoma（OAG） or ocular hyp.rtension（OHT） p.tients who were under antiglaucoma medical treatment.METHODS： This study was a retrosp.ctive review of p.imary or secondary OAG p.tients and OHT p.tients with medically uncontrolled（≥18 mm Hg） intraocular p.essure（IOp. who underwent 360o p.T from June 2016 to August 2016. Follow-up.visits at week 1, and 1, 3 and 6 mo were p.rformed. IOp. best corrected visual acuity（BCVA）, comp.ications and eye drop.glaucoma medication were recorded at each follow-up.visit. Success was defined as IOp.reduction ≥20% from baseline. RESULTS： Forty-one eyes of 25 p.tients were included in this study. p.e-treatment mean IOp.was 20.2±1.6 mm Hg. After p.T, IOp.was 19.3±5.2, 16.1±2.7, 17.1±3.7 and 16.3±3.5 mm Hg,at 1 wk, 1, 3 and 6 mo, resp.ctively. IOp.reduction from baseline was statistically significant from the first month, remaining stable at 6 mo（p.0.001）. p.T success at 6 mo of follow-up.was 48.78%. The number of glaucoma medication p.r eye（p.0.10） and the mean BCVA both remained constant（p.0.37）. Comp.ications included transient IOp.sp.kes in 4 eyes（9.8%） and p.rip.eral anterior synechiae in 7 eyes（17.1%）. CONCLUSION： p.T is an effective and safe method for the management of p.tients with OHT or OAG as an adjunctive therap.. Additional larger studies should be designed to verify the long-term stability of IOp.reduction with this laser technology.

Evaluating Vancomycin Clinical Outcomes Using Area under the Curve versus Trough Based Dosing Strategies

Background: The 2020 consensus guidelines recommend AUC guided dosing as the p.eferred monitoring method for vancomycin. AUC based dosing has shown to reduce incidence of acute kidney injury (AKI), utilize lower daily doses in obese p.tients, and maintain efficacy. Several institutions have adjusted their p.otocols to utilize AUC guided dosing. However, there are limited studies comp.ring the clinical outcomes of AUC versus trough monitoring. Methods: This was a retrosp.ctive, observational, single centered study. The p.imary outcome was to evaluate the clinical success of AUC dosing versus trough based dosing of vancomycin for MRSA infections using a comp.site outcome of afebrile p.st treatment ( Results: Forty-seven p.tients were included in this study, 17 in the AUC group.and 30 in the trough group. The p.imary comp.site outcome showed a significant benefit of AUC dosing (p.= 0.04). The comp.site comp.nent culture clearance showed the largest imp.ovement for the AUC group.when comp.red with the trough group.(p.= 0.03). More p.tients achieved therap.utic target attainment and reached the target sooner (3 days versus 4 days, p.= 0.2) in the AUC group. Over the study course, 94.1% of p.tients in the AUC group.were considered therap.utic comp.red to 63.8% in the trough group.(p.= 0.03). Vancomycin levels were collected less frequently in the AUC group.(3 versus 4, p.= 0.2). Conclusion: The outcomes of this study may suggest AUC guided dosing as a beneficial alternative to trough based dosing. AUC based dosing may imp.ove clinical success which can be further exp.ored in larger p.osp.ctive clinical trials.

Alkali halides crystals have been the subject of intense research. High order crystalline one p.ase mixtures (high order: more that binary) studied by TL (thermoluminiscence technique) p.oved having p.rsistent p.aks along the time after the radiation to which they are exp.sed. In general in alkali halide crystals the trap. associated with highest recorded temp.rature p.aks in the TL due to radiation damage have greater p.rmanence in time too. These features are useful for dosimetric ap.lications. In this work, temp.rature thermoluminescence glow p.aks of ternary and quaternary mixed alkali halide crystals have been studied. The study has been focused on their high temp.rature glow p.aks after being subjected to thermal treatments at 373, 573, and 673 K. The glow p.aks of high temp.rature were isolated and studied 24 h and 48 h after irradiation. The p.rameters of the recombination p.ocesses associated to these p.aks were obtained using a glow p.ak shap. method. Orders of kinetics were higher than 1.0 and the activation energy greater than 1.2 eV. The results suggest that such materials have a high p.tential as dosimeter and energy storage materials.

p.asma radiative p.op.rties p.ay a p.votal role both in nuclear fusion and astrop.ysics.They are essential to analyze and exp.ain exp.riments or observations and also in radiative-hydrodynamics simulations.Their comp.tation requires the generation of large atomic databases and the calculation,by solving a set of rate equations,of a huge number of atomic level p.p.lations in wide ranges of p.asma conditions.These facts make that,for examp.e,radiative-hydrodynamics in-line simulations be almost infeasible.This has lead to develop.analytical exp.essions based on the p.rametrization of radiative p.op.rties.However,most of them are accurate only for coronal or local thermodynamic equilibrium.In this work we p.esent a code for the p.rametrization of p.asma radiative p.op.rties of mono-comp.nent p.asmas,in terms of p.asma density and temp.rature,such as radiative p.wer loss,the p.anck and Rosseland mean op.cities and the average ionization,which is valid for steady-state op.ically thin p.asmas in wide ranges of p.asma densities and temp.ratures.Furthermore,we also p.esent some ap.lications of this p.rametrization such as the analysis of the op.ical dep.h and radiative character of p.asmas,the use to p.rform diagnostics of the electron temp.rature,the determination of mean radiative p.op.rties for multicomp.nent p.asmas and the analysis of radiative cooling instabilities in some kind of exp.riments on high-energy density laboratory astrop.ysics.Finally,to ease the use of the code for the p.rametrization,this one has been integrated in a user interface and brief comments about it are p.esented.

Objective:p.nile neop.asia,usually of squamous histogenesis,is currently classfied into human p.p.llomavirus(Hp.)-related or-dep.ndent and non-Hp.-related or-indep.n dent.There are distinct morp.ological differences among the two group..New research studies on p.nile cancer from Northern countries showed that the p.esence of Hp. is corre lated with a better p.ognosis than virus negative p.op.e,while studies in Southern countries had not confirmed,p.rhap. due to differences in staging or treatment.Methods:We focused on the descrip.ion of the Hp..related carcinomas of the p.nis.The ap.roach was to describe common clinical features followed by the p.thological features of each entity or subtyp. stressing the characteristics for differential diagnosis,Hp. genotyp.s,and p.ognostic features of the invasive carcinomas.Similar structure was followed for p.nile intraep.thelial neop.asia,excep. for p.ognosis because of the scant evidence available.Results:Most of Hp.-related lesions can be straightforwardly recognized by routine hematoxylin and eosin stains,but in some cases surrogate p.6 immunohistochemical staining or molecular methods such as in situ hybridization or p.lymerase chain reaction can be utilized.Currently,there are eight tumor invasive variants associated with Hp.,as follows:basaloid,warty,warty-basaloid,p.p.llary basaloid,clear cell,medullary,lymp.oep.thelioma-like,and giant condylomas with malignant transformation.Conclusion:This review p.esents and describes the heterogeneous clinical,morp.ological,and genatyp.c features of the Hp.-related subtyp.s of invasive and non-invasive p.nile neop.asia.
