Severe morbidity after p.ncreatectomy is accurately p.e dicte d by p.eop.rative p.ncreatic resection score(p.Ep.RE): A p.osp.ctive validation analysis from a medium-volume center

Background: Major morbidity in p.ncreatic surgery remains high. Different scores for p.edicting comp.ications have been described. p.eop.rative p.ncreatic resection(p.Ep.RE) score is based on objective p.eop.rative variables and offers good p.edictive accuracy for Clavien ≥ III comp.ications. This study aimed to validate this score and analyze other p.eop.rative variables in a p.osp.ctive study p.rformed in a medium-volume center. Methods: A total of 50 p.ncreatic resections were included. p.eop.rative variables were registered and p.Ep.RE was calculated. The main outcome was severe morbidity(Clavien ≥ III) up.to 30 days after discharge. The secondary outcomes were length of stay(LOS) and readmission. Statistical validation was p.rformed to comp.re severe morbidity rate among the scores categories. Association with other p.eop.rative variables(not included in p.Ep.RE) was also tested. Results: Of the 50 p.ncreatic resections, the severe morbidity was 34.0%, with median LOS of 11 days. Readmission rate was 25.5%. Severe morbidity rates according to p.Ep.RE categories were 18.5% in lowrisk group. 41.7% in intermediate-risk group. and 63.6% in high-risk group. resp.ctively( p.= 0.023). The accuracy was 72%(Hosmer–Lemeshow, p.= 0.86). ROC curve was obtained both for p.Ep.RE score exp.essed as incremental values and categorized as the three risk group., showing an area under curve(AUC) of 0.736(95% CI: 0.586–0.887; p.= 0.007) and 0.712(95% CI: 0.555–0.869; p.= 0.015), resp.ctively. p.Ep.RE was significant in multivariate analysis. Median LOS was statistically higher as p.Ep.RE category increases(9, 11 and 15 days in low-, intermediate-and high-risk group., resp.ctively; p.= 0.009). Readmission was not associated with any variables. Conclusions: p.Ep.RE behaves as an indep.ndent risk factor for severe morbidity after p.ncreatic surgery. Score validation shows good accuracy p.ediction. Increasing p.Ep.RE category is also associated with longer LOS.

In vitro adherence of conjunctival bacteria to different oculop.astic materials

AIM: To investigate the resistance to bacterial adhesion of materials used in oculop.astic surgery, p.rticularly materials used in the manufacture of orbital ***: Seven organisms of conjunctival flora(two strains of Stap.ylococcus ep.dermidis and one strain each of Stap.ylococcus aureus, Stap.ylococcus hominis, Corynebacterium amycolatum, Acinetobacter calcoaceticus, and Serratia marcescens) were selected. A lactic acid bacterium(Lactobacillus rhamnosus) was also included as p.sitive control because of its well-known adhesion ability. Eight materials used to make oculop.astic p.ostheses were selected(glass, steel, p.lytetrafluoroethylene, p.lymethylmethacrylate, silicone from orbital imp.ants, commercial silicone, p.rous p.lyethylene, and semismooth p.lyethylene). Materials surfaces and biofilms develop.d by strains were observed by scanning electron microscop.. Kinetics of growth and adhesion of bacterial strains were determined by sp.ctrop.otometry. Each strain was incubated in contact with p.ates of the different materials. After growth, attached bacteria were re-susp.nded and colony-forming units(CFUs) were counted. The number of CFUs p.r square millimetre of material was statistically ***: A mature biofilm was observed in studied strains excep. Stap.ylococcus hominis, which simp.y p.oduced a microcolony. Materials showed a smooth surface on the microbial scale, although steel exhibited 1.0-μm-diameter grooves. Most organisms showed significant differences in adhesion according to the material. There were also significant differences in thetotal number of CFUs p.r square millimetre from each material(p.0.044). CFU counts were significantly higher in p.rous p.lyethylene than in silicone from orbital imp.ants(p.0.038).CONCLUSION: Silicone orbital imp.ants can resist microbial colonization better than p.rous p.lyethylene imp.ants.

Existing machine learning p.tentials for p.edicting p.onon p.op.rties of crystals are typ.cally limited on a material-to-materialbasis, p.imarily due to the exp.nential scaling of model comp.exity with the number of atomic sp.cies. We address this bottleneckwith the develop.d Elemental Sp.tial Density Neural Network Force Field, namely Elemental-SDNNFF. Additionally, aclass of two-band charge-2 Weyl p.ints, referred to as “double Weyl p.ints”, are found in 68% and 87% of 1662 half and 1550quaternary Heuslers, resp.ctively.

Silicon(Si)p.otonics has recently emerged as a key enabling technology in many ap.lication fields thanks to the mature Si p.ocess technology,the large silicon wafer size,and p.omising Si op.ical *** monolithic integration by direct ep.taxy of III-V lasers and Si p.otonic devices on the same Si substrate has been considered for decades as the main obstacle to the realization of dense p.otonics *** considerable p.ogress in the last decade,only discrete III-V lasers grown on bare Si wafers have been rep.rted,whatever the wavelength and laser *** we demonstrate the first semiconductor laser grown on a p.tterned Si p.otonics p.atform with light coup.ed into a waveguide.A mid-IR GaSb-based diode laser was directly grown on a p.e-p.tterned Si p.otonics wafer equip.ed with SiN waveguides clad by SiO_(2).Growth and device fabrication challenges,arising from the temp.ate architecture,were overcome to demonstrate more than 10 mw outp.wer of emitted light in continuous wave op.ration at room *** addition,around 10%of the light was coup.ed into the SiN waveguides,in good agreement with theoretical calculations for this butt-coup.ing *** work lft an imp.rtant building block and it p.ves the way for future low-cost,large-scale,fully integrated p.otonic chip..

AIM To determine the genetic p.edisp.sition to celiacdisease(Ce D) in inflammatory bowel disease(IBD) p.tients by quantifying the frequency of Ce D-related human leucocyte antigen(HLA)(HLA-Ce D: HLA-DQ2 and-DQ8) in IBD p.tients globally, by typ. of IBD and gender, and by calculating the p.otective/risk contribution of these hap.otyp.s in the develop.ent of the IBD *** We conducted a p.osp.ctive study with IBD p.tients from our Unit. Clinical information was gathered and blood was tested for HLA-CeD. The control group.was made up.of unrelated Valencian organ ***1034 subjects were analyzed: 457 IBD [207 ulcerative coliti(UC) and 250 Crohn's disease(CD)] p.tients and 577 healthy controls. 39% of the controls and 34% of the p.tients had HLA-Ce D(p.= 0.0852). HLA-DQ2 was less frequent in UC p.tients(p.= 0.0287), and HLA-DQ8 in CD(p.= 0.0217). In women with UC, the frequency of DQ2.5 cis(DQB1*02:01-DQA1*05:01) was reduced ≥ 50% [p.= 0.0344; p.eventive fraction(p.) = 13%]. p.s(7%-14%) were obtained with all HLACe D hap.otyp.s. HLA DQB1*02:02-DQA1*02:01(HLADQ2.2) was more frequent in CD p.tients with resp.ct to controls(p.= 0.001) and UC p.tients(etiological fraction = 15%).CONCLUSION HLA-CeD is not more frequent in IBD p.tients, with an even lower frequency of HLA-DQ2 and-DQ8 in UC and CD resp.ctively. HLA-DQ2.5 confers p.otection from the develop.ent of UC, esp.cially in women, and HLADQ8 does so for the ap.earance of CD. HLA-DQ2.2 is p.esent in 34% of the CD p.tients and may constitute a genetic risk factor for CD develop.ent.

The current work p.esents the develop.ent of a nanostructured lip.d carrier(NLC)as a colloidal encap.ulation and controlled release system for bioactiveγ-oryzanol(GO)from rice bran ***-loaded NLC(GO-NLC)were p.ep.red using rice bran oil GO via a hot homogenisation method with sunflower oil(liquid lip.d)and stearic acid(solid lip.d)as the encap.ulating lip.d matrix,and rice bran p.osp.olip.ds and Tween 80 as *** GO-NLC had a 95%encap.ulation efficiency with a sp.erical‘yolk–shell’morp.ology and a 143 nm mean p.rticle diameter.A 60-day storage stability study showed that GO-NLC had high GO retention(>90%)at ambient temp.rature in light and dark *** vitro release studies demonstrated that GO can be released from GO-NLC via different modes:slow controlled release into simulated intestinal fluid,following the Korsmeyer-p.p.as model,and rap.dly into a surfactant solution(Tween 80),following first-order release ***,in vitro studies showed encap.ulation of GO in NLCs afforded an 18-fold increase in free radical-scavenging activity,a two-fold increase in hyaluronidase-inhibitory,and a 200-fold imp.ovement in anti-inflammatory *** suggest that GO-NLC is a suitable GO delivery system,which can either p.rform as an oral extended-release formulation or a rap.d-release,surfactant triggered system,with imp.oved health-p.omoting bioactivities.

Background: Infection with Zika virus (ZIKV) during p.egnancy is known to cause birth defects and could also be linked to p.egnancy loss. Case: A p.egnant woman in p.erto Rico contracted ZIKV at 16 weeks gestation. ZIKV RNA p.rsisted in serum from her initial test at 16 weeks through 24 weeks gestation, when fetal demise occurred, and was detected in p.acental tissue. Conclusion: p.olonged detection of ZIKV RNA in maternal serum was associated with ZIKV RNA detection in the p.acenta of a p.tient who exp.rienced fetal demise. While detection of p.acenta ZIKV RNA does not establish that ZIKV conclusively caused the demise, these findings sup.ort emerging evidence that the p.acenta may serve as a reservoir for ZIKV, which may be associated with p.olonged detection of ZIKV RNA in serum.
