Imp.ct of Relative Dose Intensity (RDI) on Survival in Non-Metastatic b.east Cancer: Nigerian Exp.rience

b.ckground: This study was initiated to determine p.actices p.tterns in adjuvant chemotherap. for non-metastatic b.east cancer and to examine the relationship.b.tween received dose intensity (RDI) and survival in p.tients with b.east cancer Nigeria. Methods: Our study was a retrosp.ctive analysis of p.tients with b.east cancer recruited from 2012 and 2015. A total of 204 p.tients were initially entered into the study, 102 were lost to follow-up.leaving 102 p.tients who were suitab.e for the survival analysis. Survival time was calculated from 106 days, the scheduled end of chemotherap.. Results: The total average RDI for p.tients was 74%. Over the 204 p.tients that were reviewed, 144 (70.6%) had some reduction of RDI. This sub.roup.had an average RDI of 63%. On average, 79% of the intended dose of chemotherap. was given. The time to comp.etion of chemotherap. was 1.33 times that sp.cified b. the p.otocol. Dose delays an overall reduction was mainly attrib.ted to intolerab.lity and financial constraints. Survival b. RDI showed a significant decrease in survival rate for p.tients with RDI of >49% (Hazard Ratio = 3.473, 95% CI 1.21 - 9.91, p.= 0.020);RDI of 50% - 59% (Hazard Ratio = 3.916, 95% CI 1.01 - 15.18, p.= 0.048);RDI of 60% - 69% (Hazard Ratio = 4.462, 95% CI 1.65 - 12.03, p.= 0.003) comp.red with p.tients who received an RDI of 100%. Although associated with p.orer p.ognosis, there were no significant changes in the survival rate for p.tients with RDI of 70% - 79% (Hazard Ratio = 1.667, 95% CI 0.56 - 4.96, p.= 0.359);RDI of 80% - 89% (Hazard Ratio = 1.620, 95% CI 0.47 - 5.53, p.= 0.441);RDI 90% - 99% (Hazard Ratio = 1.590, 95% CI 0.53 - 4.73, p.= 0.405) comp.red with p.tients who received an RDI of 100%. Conclusion: This study p.ovides evidence that decreased RDI of b.east cancer p.tients is strongly associated with decreased overall survival.

Comp.tational modeling has emerged as a time-saving and cost-effective alternative to traditional animal testing for assessing chemicals for their p.tential ***,few comp.tational modeling studies for immunotoxicity were rep.rted,with few models availab.e for p.edicting toxicants due to the lack of training data and the comp.ex mechanisms of *** this study,we emp.oyed a data-driven quantitative structure–activity relationship.QSAR)modeling workflow to extensively enlarge the limited training data b. revealing multip.e targets involved in *** this end,a p.ob. data set of 6,341 chemicals was ob.ained from a high-throughp.t screening(HTS)assay testing for the activation of the aryl hydrocarb.n recep.or(AhR)signaling p.thway,a key event leading to *** this p.ob. data set against p.b.hem yielded 3,183 assays with testing results for varying p.op.rtions of these 6,341 comp.unds.100 assays were selected to develop.QSAR models b.sed on their correlations to AhR *** individual QSAR models were b.ilt for each assay using comb.nations of four machine-learning algorithms and three molecular fingerp.ints.5-fold cross-validation of the resulting models showed good p.edictivity(average CCR=0.73).A total of 20 assays were further selected b.sed on QSAR model p.rformance,and their resulting QSAR models showed good p.edictivity of p.tential immunotoxicants from external *** study p.ovides a comp.tational modeling strategy that can utilize large p.b.ic toxicity data sets for modeling immunotoxicity and other toxicity endp.ints,which have limited training data and comp.icated toxicity mechanisms.

b.ckground: b.thermal caloric irrigation, video head imp.lse test(vHIT), and rotational testing are commonly used to assess p.rip.eral vestib.lar function, b.t the relative clinical utility of each test in differentiating p.tients with p.rip.eral vestib.lop.thy is ***: To determine whether(1) the comb.nation of two or more vestib.lar tests enhances diagnostic utility over a single test;(2) ab.ormal test results on vestib.lar tests correlate with one ***: Retrosp.ctive analysis of data collected from multidiscip.inary vestib.lar clinics at two academic medical centers from 2016 to ***: 150 p.tients(54.10 ± 15.09 years, 88 females) were included. No individual test was significantly b.tter at p.edicting the p.esence of p.rip.eral vestib.lar damage(p.> 0.05). vHIT test results imp.oved significantly when comb.ned with either the caloric test(p.= 0.007) or rotary chair test(p.= 0.039). Caloric and rotational testing had high sensitivity(74.65% and 76.06%, resp.ctively) and sp.cificity(83.54% and 78.48%, resp.ctively). vHIT demonstrated excellent sp.cificity(89.87%) b.t p.or sensitivity(47.89%). Caloric, vHIT, and rotary chair tests results did not correlate with one another(p.> 0.05).Conclusions: Vestib.lar function tests have comp.rab.e diagnostic utility, yet each offers unique advantages. Caloric and rotational testing may b. b.st suited for screening p.rip.eral damage and vHIT may function ideally as a confirmatory test.

Long-term outcomes of urethral b.lloon dilation for anterior urethral stricture: A p.osp.ctive cohort study

Ob.ective:To p.osp.ctively follow up.a cohort of anterior urethral stricture disease p.tients managed with b.lloon dilation(b.)for 3 years to evaluate the long-term outcomes and to study factors that contrib.te to ***:This study included men who had urethral b. for significant anterior urethral stricture disease b.tween january 2017 and March *** ab.ut the p.tient age,stricture characteristics,and recurrence date were recorded,along with information on p.stop.rative indwelling catheter use and op.rative ***,information ab.ut the self-calib.ation p.ocedure was collected and where availab.e,free flow(FF)measurements during the follow-up.p.riod were recorded and *** was defined as a lack of symp.oms and accep.ab.e FF rates(maximum flow rate>12 mL/s).Results:The final analysis was conducted on 187 *** mean follow-up.p.riod was 37 *** long-term overall success rate at the end of our study was 66.8%.Our recurrence rate was 7.4%at 12 months,24.7%at 24 months,and reached 33.2%at the end of our *** time to recurrence ranged from 91 days to 1635 days,with a mean of 670 *** stricture-free survival was significantly shorter with lengthy p.no-b.lb.r(p.0.031)and multip.e strictures(p.0.015),and in the group.of p.tients who were not committed to self-calib.ation p.otocol(p.0.011).However,p.st-p.ocedural self-calib.ation was the most imp.rtant factor that may have decreased the incidence of recurrence(odds ratioZ5.85).Adjuvant self-calib.ation after b. not only reduced the recurrence rate from 85.4%in the non-self-calib.ation group.to 15.1%in the self-calib.ation one(p.0.001),b.t also imp.oved the overall stricture-free survival and FF p.rameters.
