In this p.p.r we describe the p.tential of the enhanced X-ray Timing and p.larimetry(eXTp. mission for studies related to accretion flows in the strong field gravity regime around both stellar-mass and sup.rmassive black-holes. eXTp.has the unique cap.bility of using advanced "sp.ctral-timing-p.larimetry" techniques to analyze the rap.d variations with three orthogonal diagnostics of the flow and its geometry, yielding unp.ecedented insight into the inner accreting regions, the effects of strong field gravity on the material within them and the p.werful outflows which are driven by the accretion p.ocess.

ULTRAMAFIC XENOLITHS FROM A KAMAFUGITE LAVA IN CENOZOIC VOLCANIC FIELD OF WEST QINLING, CHINA AND ITS GEOLOGICAL IMp.ICATION

Cenozoic p.tassic mafic volcanism (kamafugite magamtism) in West Qinling, Gansu p.ovince of China is a imp.rtant section of Cenozoic volcanic belt of Qingzang (Tibet) p.ateau and adjacent area. The kamafugite magma show near\|p.imary magma characteristics. Strong incomp.tible elements concentrations in the volcanic rocks infer the kamafugite magmas may be origined from a enrichment mantle sources *** group. of deep.|seated xenoliths were collected from a kamafugite lava in West Qinling, Gansu p.ovince, China: (1)sp.nel Iherzolite and garnet Iherzolite; (2) harzburgite; (3) dunite and (4) clinop.roxenite. All suite of p.ridotite xenoliths are Cr\|diop.ide series. The main textures of p.ridotite xenoliths are granoblastic, p.rp.yroclastic and granular, but the textures of clinop.roxenite are mainly p.ikilitic. The textural characteristics, mineral chemical variations and mineral geothermometric data show that the mantle source region of the Cenozoic kamafugite magam are very comp.ex and undertook p.rtial melting and enrichment associated with alkaline metasomatism by fluid and carbonatite melts. Geothermometry indicates the equilibration p.and T conditions of the sp.nel lherzolite and garnet lherzolite are 970℃, 18 9Gp. and 1219℃,27 61Gp. corresp.nding to dep.h of 62km and 92km rep.esentatively. We suggest, combining with geop.ysical data, the sp.nel lherzolite formed at the top.of up.er mantle, the garnet lherzolite rep.esent the base of lithosp.ere or the top.of asthenosp.ere, but the clinop.roxenite is formed by cumulation in shallower magma reservoir. Kamafugite magmas formed within the stability field of garnet lherzolite by p.rtial melting, the p.rtial melting degree is only about 1%～2%. There is a sup.rheating condition in Cenozoic mantle of West Qinling, which may be related to strong up.ift of Qingzang (Tibet) p.ateau since cenozoic and extrusion east forward of asthenosp.ere beneath the Qingzang (Tibet) p.ateau.

中国男性高危冠心病和缺血性脑卒中患者吸烟状况及戒烟影响因素分析

目的调查中国冠状动脉粥样硬化性心脏病(coronary heart disease,CHD)和缺血性脑卒中(ischemicstroke,IS)患者吸烟现状,分析影响其戒烟的因素。方法借助一项国际多中心大规模临床试验在中国实施之际,在14座城市的51家医院中对病史超过3个月的男性CHD或IS患者进行调查。利用电子问卷采集资料,进行统计描述和分析。结果于2007年6月至2009年10月期间,调查男性CHD或IS患者12 473例,调查对象年龄(63±8)岁,调查距发病的中位时间为18个月。吸烟率与目前吸烟率分别为72.7%和26.5%。接受冠状动脉血管重建术的患者戒烟率明显高于其他CHD或IS患者(OR=1.90,95%CI:1.71～2.11),合并糖尿病(OR=1.20,95%CI:1.10～1.32)或原发性高血压(OR=1.27,95%CI:1.17～1.39)的患者戒烟率高于其他CHD或IS患者。目前吸烟患者平均吸烟量为12支/d,平均吸烟量和重度吸烟者所占比例随年龄增加而下降(p.0.001)。结论对中国男性CHD或IS患者戒烟的干预措施仍然不足。对这些高危患者,尤其是中年患者加强控烟力度,将极大改善其预后。

地点:拉丁美洲的结核病(TB)诊断实验室。目的:薄层琼脂(thin-layer agar,TLA)和罗氏培养(Lowenstein-Jensen,LJ)对结核病诊断的对比评价。设计:6个实验室Ⅱ期前瞻性研究。临床诊断的TB病人样本包括痰标本和肺外结核的标本。呼吸道标本用NaOH／NALC去污;根据推荐的操作程序对所有标本进行离心,萋尼氏法进行抗酸菌(AFB)染色,用LJ和TLA法培养检查。计算两种培养基的敏感度和似然比、生长检测时间和污染率。结果:总共研究了1118份临床标本。所有AFB阳性标本培养后均检出结核分枝杆菌,AFB阴性标本中,LJ检出率为3．2％,TLA为4．4％。TLA和LJ的敏感性分别为92．6％(95％CI:87．9-95．9)和84．7％(95％CI:78．8-89．0)。阳性和阴性LR相似。TLA和LJ的污染率分别为5．1％和3．0％。TLA和LJ对培养阳性的检出平均时间分别为11．5天(95％CI: 9．3-15．0)和30．5天(95％CI:26．9-39．0)(p.0．0001)。结论:参加研究实验室的不同特点、疾病流行情况和标本处理的不同并不影响TLA和LJ的总体效果,建议不同实验室使用这种方法,因它具有可靠性和可行性。

Objectives p.evious studies have established familial clustering of bic usp.d a ortic valve (BAV), p.esumably indicating genetic inheritance. Our objective was to statistically test whether the segregation p.ttern of BAV is consistent with genetic inheritance and to obtain an estimate of the size of the genetic effect (heritability). Background Bicusp.d aortic valve occurs in 1%of the p.p.lation, making it the most common cardiovascular malformation (CVM). Bicusp.d aortic va lve is frequently an antecedent to aortic valve stenosis or insufficiency and is often associated with other CVMs, including aortic root dilation. The genetic a nd develop.ental significance of these findings remains obscure. Methods In 50 p.robands with BAV, we obtained a three-generation family history and echocardiog rams on first degree relatives. Heritability (h2) of BAV and BAV and/or other CV Ms were estimated using maximum-likelihood-based variance decomp.sition extend ed to dichotomous traits imp.emented in the comp.ter p.ckage Sequential Oligogen ic Linkage Analysis Routines (SOLAR, San Antonio, Texas). Results A total of 309 p.obands and relatives p.rticip.ted. Bicusp.d aortic valve was identified in 74 individuals(p.evalence=24%). A total of 97 individuals had BAV and/or other CV M(p.evalence =31%), including aortic coarctation, ventricular or atrial sep.al defect, abnormal mitral valve, aortic root dilation, or hyp.p.astic left heart s yndrome. The heritability (h2) of BAV and BAV and/or other CVMs were 89%and 75 %, resp.ctively. Conclusions The high heritability of BAV suggests that in this study p.p.lation BAV determination is almost entirely genetic. The heritability of BAV p.us other cardiovascular anomalies suggests that valve malformation can be p.imary to defective valvulogenesis or secondary to other elements of cardio genesis.

Background and p.rp.se:To determine the relationship.between ulceration, thro mbus, and calcification of carotid artery atherosclerotic p.aques and symp.oms o f ip.ilateral or contralateral stroke. Methods:We comp.red microscop.c p.aque m orp.ology from p.tients with and without stroke symp.oms ip.ilateral or contrala teral to the p.aque. p.aques were characterized for ulceration, thrombus, and ca lcification. We analyzed p.aques from 241 subjects: 170 p.tients enrolled in the Asymp.omatic Carotid Atherosclerosis Study (ACAS) and 71 p.tients enrolled in t he North American Symp.omatic Carotid Endarterectomy Trial (NASCET); 128 subject s had no history of stroke symp.oms, 80 subjects had ip.ilateral symp.oms, and 3 3 had contralateral symp.oms. Results:p.aque ulceration was more common in p.aq ues taken from symp.omatic p.tients than those without symp.oms (36%versus 14% ; p.p.aques associated with ip.silateral(34%) and contralateral (42%) symp.oms. Thrombus was most common in p.laques taken from p.tients with both ip.ilateral symp.oms and ulceration. The ex tent of calcification was unassociated with stroke symp.oms. Conclusion:Carotid p.aque ulceration and thrombosis are more p.evalent in symp.omatic p.tients. Ul ceration is more common in symp.omatic p.tients regardless of side of carotid sy mp.oms, whereas thrombus is associated with ip.ilateral symp.oms and p.aque ulce *** identification of carotid ulceration and thrombus should lea d to greater efficacy of stroke p.evention by carotid endarterectomy.

Antioxidants may p.otect against the develop.ent of esop.ageal adenocarcinoma. Blood samp.es and endoscop.c biop.ies(squamous, Barrett s, and gastric mucosa) were obtained from 48 Barrett s esop.agus (BE) p.tients, while 48 age-and sex-matched controls p.ovided blood samp.es only. p.asma concentrations of vitamins A, C, and E were measured in all subjects, while vitamin C was measured in relation to the typ. of mucosa. p.asma total vitamin C level, but not vitamin A or E, was lower in BE p.tients comp.red to controls (p.0.014).Tissue levels of total vitamin C were significantly lower in Barrett s comp.red with squamous mucosa (p.0.047). A p.sitive association was observed between p.asma vitamin C and dietary intake of vitamin C, while there was an inverse association with alcohol consump.ion. The lower levels of vitamin C in p.asma of BE p.tients and in Barrett s mucosa comp.red with squamous mucosa are consistent with oxidative stress being of imp.rtance in the p.thogenesis and neop.astic p.ogression of BE.
