AIM： To investigate the efficacy of a high-potency probiotic preparation on prevention of radiation-induced diarrhea in cancer patients.METHODS： This was a double-blind, placebo-controlled trial. Four hundred and ninety patients who underwent adjuvant postoperative radiation therapy after surgery for sigmoid, rectal, or cervical cancer were assigned to either the high-potency probiotic preparation VSL#3 （one sachet t.i.d.,） or placebo starting from the first day of radiation therapy. Efficacy endpoints were incidence and severity of radiation-induced diarrhea, daily number of bowel movements, and the time from the start of the study to the use of Ioperamide as rescue medication. RESULTS： More placebo patients had radiation-induced diarrhea than VSL#3 patients （124 of 239 patients, 51.8%, and 77 of 243 patients, 31.6%; p 〈 0.001） and more patients given placebo suffered grade 3 or 4 diarrhea compared with VSL#3 recipients （55.4% and 1.4%, p 〈 0.001）. Daily bowel movements were 14.7 ± 6 and 5.1 ± 3 among placebo and VSL#3 recipients （p 〈 0.05）, and the mean time to the use of Ioperamide was 86 ± 6 h for placebo patients and 122 ± 8 h for VSL#3 patients （p 〈 0.001）. CONCLUSION： probiotic lactic acid-producing bacteria are an easy, safe, and feasible approach to protect cancer patients against the risk of radiation-induced diarrhea.

AIM To evaluate the impact of the Glu167Lys(E167K) transmembrane 6 superfamily member 2(TM6SF2) variant on the biochemical and morphologic expression of liver lesions in human immunodeficiency virus(HIV)/hepatitis C virus(HCV) co-infected *** The study comprised 167 consecutive patients with HIV/HCV coinfection and biopsy-proven chronic hepatitis. A pathologist graded liver fibrosis and necroinflammation using the Ishak scoring system, and steatosis using Kleiner's scoring system. patients were genotyped for TM6SF2 E167K(rs58542926) by real-time polymerase chain reaction. The 167 patients, 35 therapy-naive and 132 receiving ART, were prevalently males(73.6%), the median age was 40.7 years and the immunological condition good(median CD4+ cells/mm3 = 505.5).RESULTS The 17 patients with the TM6SF2 E167 K variant, compared with the 150 with TM6SF2-E/E, showed higher AST(p = 0.02) and alanine aminotransferase(p = 0.02) and higher fibrosis score(3.1 ± 2.0 vs 2.3 ± 1.5, p = 0.05). In a multivariate analysis, TM6SF2 E167 K was independently associated with severe fibrosis. The same analysis showed that HCV-genotype 3, present in 42.2% of patients was an independent predictor of severe steatosis. The association of TM6SF2 E167 K with severe steatosis, absent for the whole group of 167 patients, was re-evaluated separately for HCVgenotype 3 and non-3 patients: No factor was independently associated with severe steatosis in the HCV-genotype-3 subgroup, whereas an independent association was observed between severe steatosis and TM6SF2 E167 K in non-3 HCV genotypes. 背景患者在出院后接受的卫生服务质量不佳会引起医疗费用增加、对急诊服务的过度利用、再住院率增加,还会使患者感觉就诊体验糟糕、治疗结局不良。本研究旨在评估应用远程医疗提高患者在出院后接受的卫生服务效果的可行性。方法本项为期12个月的随机对照试验对比了采用和未采用远程医疗(对患者进行远程监测和视频诊疗服务)的治疗效果。主要评价指标为再住院率和急诊服务利用率。次要评价指标为医疗服务可及性、用药管理和依从性以及患者对治疗的参与度。数据来源于患者电子病历、健康信息交流系统和电话调查。结果研究于2017年6月-2018年12月进行,共纳入102例患者。与对照组相比,远程医疗组患者更能严格遵守用药治疗(p=0.013),用药依从性高于对照组7倍(p=0.03)。远程医疗组对远程医疗更感兴趣(p=0.000 1),更相信远程医疗能改善其出院后的卫生服务水平(p=0.000 1)。但是,两组在急诊服务利用率(p=0.691)和再住院率(p=0.31)方面无差异。远程医疗组患者认为远程医疗有价值,并且98%的患者表示,如果有机会,还会继续利用,94%的患者表示远程监测技术很有用。结论患者在出院后接受的卫生服务有所变化,但是采用远程医疗可以改善其治疗参与度和用药依从性。尽管本研究未能发现远程医疗对降低医疗服务利用率有效果,未来研究可继续探索远程医疗是否能预防患者不必要的再住院和急诊利用情况。

AIM To evaluate the role of a therapeutic regimen with plasma exchange, intravenous immunoglobulins and rituximab in chronic-active antibody-mediated rejection(c AMR) *** We compared 21 kidney transplant recipients(KTRs) with a diagnosis of c AMR in a retrospective casecontrol analysis: nine KTRs treated with plasmapheresis, intravenous immunoglobulins and rituximab(pE-IVIGRTX group) vs 12 patients(control group) not treated with antibody-targeted therapies. We examined kidney survival and functional outcomes 24 mo after diagnosis. Histological features and donor-specific antibody(DSA) characteristics(MFI and C1 q-fixing ability) were also *** No difference in graft survival between the two groups was noted: three out of nine patients in the pE-IVIG-RTX group(33.3%) and 4/12 in the control group(33.3%) experienced loss of allograft function at a median time after diagnosis of 14 mo(min 12-max 18) and 15 mo(min 7-max 22), respectively. Kidney functional tests and proteinuria 24 mo after cA MR diagnosis were also similar in both groups. Only microvascular inflammation(glomerulitis + peritubular capillaritis score) was significantly reduced after pE-IVIG-RTX in seven out of eight patients(87.5%) in the pE-IVIG-RTX group(median score 3 in pre-treatment biopsy vs 1.5 in post-treatment biopsy; p = 0.047), without any impact on kidney survival and/or DSA characteristics. No functional or histological parameter at diagnosis was predictive of clinical *** Our data showed no difference in the two year posttreatment outcome of kidney grafts treated with pE-IVIGRTX for c AMR diagnosis, however there were notable improvements in microvascular inflammation in posttherapy protocol biopsies. Further studies, especially involving innovative therapeutic approaches, are required to improve the management and long-term results of this severe condition.
