Immune phenotype in children with therapy-nave remitted and relapsed Crohn’s disease

AIM: To characterize the prevalence of subpopulations of CD4+ cells along with that of major inhibitor or stimulator cell types in therapy-nave childhood Crohn's disease (CD) and to test whether abnormalities of immune phenotype are normalized with the improvement of clinical signs and symptoms of disease. METHODS: We enrolled 26 pediatric patients with CD. 14 therapy-nave CD children; of those, 10 children remitted on conventional therapy and formed the remission group. We also tested another group of 12 chil-dren who relapsed with conventional therapy and were given infliximab; and 15 healthy children who served as controls. The prevalence of Th1 and Th2, nave and memory, activated and regulatory T cells, along with the members of innate immunity such as natural killer (NK), NK-T, myeloid and plasmocytoid dendritic cells (DCs), monocytes and Toll-like receptor (TlR)-2 and TlR-4 expression were determined in peripheral blood samples. RESUlTS: Children with therapy-nave CD and those in relapse showed a decrease in Th1 cell prevalence. Simultaneously, an increased prevalence of memory and activated lymphocytes along with that of DCs and monocytes was observed. In addition, the ratio of myeloid /plasmocytoid DCs and the prevalence of TlR-2 or TlR-4 positive DCs and monocytes were also higher in therapy-nave CD than in controls. The majority of alterations diminished in remitted CD irrespective of whether remission was obtained by conventional or biological therapy. CONClUSION: The finding that immune phenotype is normalized in remission suggests a link between immune phenotype and disease activity in childhood CD. Our observations support the involvement of members of the adaptive and innate immune systems in childhood CD.

Crohn’s disease(CD)is a multifactorial potentially debilitating *** has a variable disease course,but the majority of patients eventually develop penetrating or stricturing complications leading to repeated surgeries and *** on the natural history of CD provide invaluable data on its course and clinical predictors,and may help to identify patient subsets based on clinical *** data are available from referral centers,however these outcomes may be different from those in population-based *** data suggest the possibility of a change in the natural history in Crohn’s disease,with an increasing percentage of patients diagnosed with inflammatory disease *** rates remain high,while surgery rates seem to have decreased in the last *** addition,mortality rates still exceed that of the general *** impact of changes in treatment strategy,including increased,earlier use of immunosuppressives,biological therapy,and patient monitoring on the natural history of the disease are still *** this review article,the authors summarize the available evidence on the natural history,current trends,and predictive factors for evaluating the disease course of CD.

Mucocele of the appendix: An unusual cause of lower abdominal pain in a patient with ulcerative colitis-. A case report and review of literature

The authors report the case of a 60-year-old male *** November 2001 he developed intestinal symptoms of bloody diarrhea and abdominal pain. Colononoscopy and biopsy established the diagnosis of ulcerative colitis (proctosigmoiditis). The disease activity was moderate at the *** significant laboratory alterations were found (including CEA, CA19-9),and mesalazine was started *** was in remission until November 2003, when he was admitted to our Outpatient Clinic for upper and right lower abdominal pain and bloody diarrhea. Colonoscopy found proctosigmoiditis with a moderate activity, gastroscopy revealed chronic gastritis,laboratory data was normal. Treatment was amended with mesalazine clysma and methylprednisolone (16 mg) orally. Symptoms ameliorated;however, right lower abdominal pain persisted. US and CT examination demonstrated a pericecal cystic mass (11 cmx3.5 cm). At first pericecal abscess was suspected, as the previous US examination (6 mo earlier) had revealed normal findings. Fine needle aspiration was performed. Cytology confirmed the diagnosis of *** patient underwent partial cecum resection and extirpation of the mucocele. He recovered well and the final histology revealed a cystadenoma of the appendix. Follow up was started. The patient is now free of *** primary adenocarcinoma of the appendix is uncommon, the authors emphasize that preoperative diagnosis of an underlying malignancy in a mucocele is important for patient management; however, it is difficult on imaging studies.

AIM To assess the prevalence of a panel of serologic markers that reflect gut barrier dysfunction in a mixed cohort of pediatric and adult primary sclerosing cholangitis(pSC) *** Sera of 67 pSC patients [median age(range): 32(5-79) years, concomitant IBD: 67% and cirrhosis: 20%] were assayed for the presence of antibodies against to F-actin(AAA Ig A/Ig G) and gliadin(AGA Ig A/Ig G)] and for serum level of intestinal fatty acid-binding protein(I-FABp) by ElISA. Markers of lipopolysaccharide(lpS) exposure [lpS binding protein(lBp)] and various antimicrobial antibodies [anti-OMp plus Ig A and endotoxin core Ig A antibody(Endo CAb)] were also determined. poor disease outcome was defined as orthotopic liver transplantation and/or liver-related death during the follow-up [median: 99(14-106) mo]. One hundred and fifty-three healthy subjects(HCONT) and 172 ulcerative colitis(UC) patients were the controls. RESUlTS A total of 28.4%, 28.0%, 9% and 20.9% of pSC patients were positive for AAA Ig A, AAA Ig G, AGA Ig A and AGA Ig G, respectively. Frequencies of AAA Ig A and AAA Ig G(p p = 0.01, for both) but not AGA Ig A were significantly higher compared to both of the HCONT and the UC groups. In survival analysis, AAA Ig A-positivity was revealed as an independent predictor of poor disease outcome after adjusting either for the presence of cirrhosis [HR = 5.15(1.27-20.86), p = 0.022 or for the Mayo risk score(HR = 4.24(0.99-18.21), p = 0.052]. AAA Ig A-positivity was significantly associated with higher frequency of antimicrobial antibodies(p p = 0.012 for anti-OMp plus Ig A) and higher level of the enterocyte damage marker(median I-FABp_(AAA Ig A pos vs neg): 365 vs 166 pg/m l, p = 0.011), but not with serum lBp level. prevalence and predictors of hospitalization in Crohn's disease in a prospective population-based inception cohort from 2000-2012

AIM: To analyze the prevalence, length and predictors of hospitalization in the biological era in the populationbased inception cohort from Veszprem ***: Data of 331 incident Crohn's disease(CD) patients diagnosed between January 1, 2000 and December 31, 2010 were analyzed(median age at diagnosis: 28; IQR: 21-40 years). Both in- and outpatient records were collected and comprehensively ***: probabilities of first CD-related hospitalization and re-hospitalization were 32.3%, 45.5%,53.7% and 13.6%, 23.9%, 29.8%, respectively after one, three and five years of follow-up in Kaplan-Meier analysis. First-year hospitalizations were related to diagnostic procedures(37%), surgery or disease activity(27% and 21%). Non-inflammatory disease behavior at diagnosis(HR = 1.32, p = 0.001) and perianal disease(HR = 1.47, p = 0.04) were associated with time to first CD-related hospitalization, while disease behavior change(HR = 2.38, p = 0.002) and need for steroids(HR = 3.14, p = 0.003) were associated with time to first re-hospitalization in multivariate *** CD-related hospitalization(within the year of diagnosis) was independently associated with need for immunosuppressives(OR = 2.08, p = 0.001) and need for surgeries(OR = 7.25, p pitalization and re-hospitalization rates are still high in this cohort, especially during the first-year after the diagnosis. Non-inflammatory disease behavior at diagnosis was identified as the pivotal predictive factor of both hospitalization and rehospitalization.

A prospective Study of Maintenance Electroconvulsive Therapy in an Elderly Depressed population

Objective: This study was conducted to discern the efficacy of maintenance electroconvulsive therapy (M-ECT) in a population of depressed elderly individuals with treatment-resistant depression. Methodology: Twenty-nine (N = 29) individuals over the age of 65 years of age and older were assigned to a control or treatment group on the basis of their decision to receive M-ECT (treatment group) or to refrain from receiving the treatment (control group). A battery of psychometric tests designed to measure severity of depression, quality of life, and cognition were administered at baseline as well as at 6-month and 1-year intervals. Results: Statistical analysis of the data indicated no significant differences in the efficacy of M-ECT between the control and treatment groups in any of the tests administered during the participation of the study. Conclusion: The results of the study suggest that there is no added benefit for patients administered M-ECT. However, study sample size and availability of alternative treatment regimens for the control group limit generalizability of these findings and warrant further investigation.

Autonomic and sensory nerve dysfunction in primary biliary cirrhosis

AIM: Cardiovascular autonomic and peripheral sensory neuropathy is a known complication of chronic alcoholic and non-alcoholic liver diseases. We aimed to assess the prevalence and risk factors for peripheral sensory nerve and autonomic dysfunction using sensitive methods in patients with primary biliary cirrhosis (pBC). METHODS: Twenty-four AMA M2 positive female patients with clinical, biochemical and histological evidence of pBC and 20 age matched healthy female subjects were studied. Five standard cardiovascular reflex tests and 24-h heart rate variability (HRV) analysis were performed to define autonomic function. peripheral sensory nerve function on median and peroneal nerves was characterized by current perception threshold (CpT), measured by a neuroselective diagnostic stimulator (Neurotron, Baltimore, MD). RESUlTS: Fourteen of 24 patients (58%) had at least one abnormal cardiovascular reflex test and thirteen (54%) had peripheral sensory neuropathy. lower heart rate response to deep breathing (p=0.001), standing (p=0.03) and Valsalva manoeuvre (p=0.01), and more profound decrease of blood pressure after standing (p=0.03) was found in pBC patients than in controls. As a novel finding we proved that both time domain and frequency domain parameters of 24-h HRV were significantly reduced in pBC patients compared to controls. Each patient had at least one abnormal parameter of HRV. lower CpT values indicated hyperaesthesia as a characteristic feature at peroneal nerve testing at three frequencies (2000 Hz: p=0.005; 250 Hz: p=0.002; 5 Hz: p=0.004) in pBC compared to controls. Correlation of autonomic dysfunction with the severity and duration of the disease was observed. lower total power of HRV correlated with lower CpT values at median nerve testing at 250 Hz (p = 0.0001) and at 5 Hz (p=0.002), as well as with those at peroneal nerve testing at 2000 Hz (p=0.01).
