Evaluation of CA125 and NT-proBNP values in patients undergoing transcatheter aortic valve implantation

Background Transcatheter aortic valve implantation （TAVI） is a minimally invasive, emerging therapy in surgically high risk, or in- operable patients. Parameters used for risk classification have some deficiencies in the selection of patients. The objective of this study is to evaluate the impact of TAVI on carbohydrate antigen 125 （CA125） and N-terminal pro brain-type natriuretic peptide （NT-proBNP） as biomarkers that have been used frequently in recent years, and also the relationship of these biomarkers to prognosis. Methods ＆ Results Transcatheter aortic valve implantation was practiced on 31 patients in this study. Then, CA125 and NT-proBNP levels studied in patients prior to and after the TAVI were evaluated. The patients were also grouped in accordance with their left ventrieular ejection fraction （LVEF） and CA125 levels （LVEF 〉 40% and 〈 40%; CA125 ≤ 35 U/L and 〉 35 U/L）. The TAVI operation was successfully performed in all patients. There was no in-hospital mortality and substantial improvement in functional capacity was detected at follow ups. In addition, a statistically significant decrease was detected in post-TAVI CA 125 and NT-proBNP levels of all patients （CA 125 83.8 ± 18.1 U/L vs. 64.3 ±14.2 U/L, P = 0.008; NT-proBNP： 4633.6± 627.6 pg/mL vs. 2866.3±536.8 pg/mL, P 〈 0.001）. In groups divided according to the CA125 levels, there was also statistically significant post-TAVI decline in CA125 levels. Within CA125 〉 35 U/L and LVEF 〈 40% groups, the permanent need for a pacemaker was required in one （3.2%） patient and mortality was observed in two （6.4%） patients after TAVI at follow up. Conclusions The results show that TAVI can be performed effectively and reliably in patients with high baseline levels of CA125 and NT-proBNP. These biomarkers are reduced substantially with TAVI, while high biomarker levels are associated with undesired events, and certainly, these biomarkers can be used for risk classifications in patient selection for TAVI.

Tuning the metal filling fraction in metal-insulator-metal ultra-broadband perfect absorbers to maximize the absorption bandwidth

In this paper, we propose a methodology to maximize the absorption bandwidth of a metal-insulator-metal(MIM) based absorber. The proposed structure is made of a Cr-Al_2O_3-Cr multilayer design. At the initial step,the optimum MIM planar design is fabricated and optically characterized. The results show absorption above 0.9 from 400 nm to 850 nm. Afterward, the transfer matrix method is used to find the optimal condition for the perfect light absorption in an ultra-broadband frequency range. This modeling approach predicts that changing the filling fraction of the top Cr layer can extend light absorption toward longer wavelengths. We experimentally proved that the use of proper top Cr thickness and annealing temperature leads to a nearly perfect light absorption from 400 nm to 1150 nm, which is much broader than that of a planar design. Therefore, while keeping the overall process lithography-free, the absorption functionality of the design can be significantly improved. The results presented here can serve as a beacon for future performance-enhanced multilayer designs where a simple fabrication step can boost the overall device response without changing its overall thickness and fabrication simplicity.

Neuroprotective effects of daidzein on focal cerebral ischemia injury in rats

Daidzein, a plant extract, has antioxidant activity. It is hypothesized, in this study, that daidzein exhibits neuroprotective effects on cerebral ischemia. Rat models of middle cerebral artery oc- clusion were intraperitoneally administered daidzein. Biochemical and immunohistochemical tests showed that superoxide dismutase and nuclear respiratory factor 1 expression levels in the brain tissue decreased after ischemia and they increased obviously after daidzein administra- tion; malondialdehyde level and apoptosis-related cysteine peptidase caspase-3 and caspase-9 immunoreactivity in the brain tissue increased after ischemia and they decreased obviously after daidzein administration. Hematoxylin-eosin staining and luxol fast blue staining results showed that intraperitoneal administration of daidzein markedly alleviated neuronal damage in the ischemic brain tissue. These findings suggest that daidzein exhibits neuroprotective effects on ischemic brain tissue by decreasing oxygen free radical production, which validates the afore- mentioned hypothesis.

目的:评估雷珠单抗反应欠佳患者治疗早期转为阿柏西普治疗疗效。方法:该研究包括38例湿性年龄相关性黄斑变性(W-AMD)患者。18例反应欠佳患者改用玻璃体内阿柏西普(IVA)每月3次疗法,其余20例反应欠佳患者持续进行玻璃体内雷珠单抗(IVR)治疗,每月增加3次使用。所有改变均行荧光素血管造影术(FA)和光学相干断层扫描成像术(OCT)评估。结果:患者术前平均视力(VA)和黄斑中心厚度(CMT)分别为0.84±0.47 logM AR和360±84μm。经过6次IVR或3次IVR联合3次IVA治疗1mo后,两组患者视力分别为1.1±0.14(P=0.11)logM AR和0.48±0.37(P=0.019)logM AR,黄斑中心厚度分别为300±79μm(P=0.002)和271±51μm(P=0.002)。结论:对于雷珠单抗治疗反应欠佳患者,减少重复性治疗早期转为使用阿柏西普疗法对于视力康复是更好的选择。

Evaluating the safety of intracameral bevacizumab application using oxidative stress and apoptotic parameters in corneal tissue

·AIM: To investigate the possible effects of intracameral bevacizumab on oxidative stress parameters and apoptosis in corneal tissue.·METHODS: In total, 30 rats were assigned randomly into the following three groups of 10 rats each: a sham group(Group 1; n =10), a control group [Group 2;balanced salt solution(BSS) was administered at 0.01 m L;n =10], and a treatment group(Group 3; bevacizumab was administered at 0.25 mg/0.01 m L; n =10). The total antioxidant status(TAS) and the total oxidant status(TOS) in the corneal tissue and blood samples were measured, and the oxidative stress index(OSI) was calculated. Additionally, corneal tissue histopathology was evaluated for caspase-3 and 8 staining and apoptotic activity.·RESULTS: In the blood samples, the TAS, TOS, and OSI levels were not significantly different(all P >0.05).Compared with the sham and control groups, the TOS and OSI levels in the corneal tissues were significantly different in the bevacizumab group(all P 0.05).However, compared with the sham and control groups,greater immunohistochemical staining for caspases-3and 8 and an elevated level of apoptotic activity were observed in the bevacizumab group.·CONCLUSION: This study revealed that intracameral bevacizumab injections seemed to be systemically safe but may have elicited local toxic effects in the corneal tissue, as indicated by the oxidative stress parameters and histopathological evaluations.

Calculation of the Spinning Speed of a Free Electron

In a recent work, we calculated the magnetic field inside a free electron due to its spin, and found it to be about B = 8.3 × 1013 T. In the present study we calculate the spinning speed of a free electron in the current loop model. We show that spinning speed is equal to the speed of light. Therefore it is shown that if electron was not spinning the mass of electron would be zero. But since spinning is an unseparable part of an electron, we say that mass of electron is non-zero and is equal to (m = 9.11 × 10−28 g).

Origin of Dolomitization in Demirkazik Formation(Adana-Turkey)

Demirkazık formation consisting of the limestones and dolomites is common in the Adana, Turkey. In this study, it is aimed to find the origin of dolomitization by preparing a detailed geological map. For this purpose, firstly, thin section/polished section and, chemical analysis of the twenty-one samples characterizing dolomitization of the field were prepared and examined. In the chemical analyzes, 21 samples were determined MgO-CaO ratio. Geological map was prepared by field study and MgO contents of the samples that are known coordinates in the chemical data, and the regions where dolomitization was effective were determined. Dolomitization in these carbonates occurs at early stages and late stages. Two dolomite types are petrographically determined;(1) fine crystalline dolomite (TD-1) with 10 - 25 μm crystal size and (2) coarse crystalline dolomite (TD-2) with 50 - 120 μm crystal size. TD-1 is formed by synsedimentary replacement and TD-2 derived from TD-1. The evidence of dolomitization due to pressure changes in the TD-2 dolomites is evidence of late diagenetic. There are three types of carbonate by chemical data;limestone, dolomitic limestone, calcitic dolomite and MgCO3 ratio of these is increased respectively. The rate of dolomitization is very variable and at the fracture regions, the Mg ratio increases, indicating that tectonism is the cause of dolomitization.

Background:Emergency department(ED)overcrowding is among the biggest and most important problems experienced by ED *** number of ED visits is on the increase and remains an unresolved *** department overcrowding has become an important problem for emergency care services *** is a relationship between overcrowding and patients’negative experi-ences of using ED and therefore reporting reduced patients’*** study aimed to identify the causes of ED overcrowding,de-termine the reasons for people’s use of EDs,and develop solutions for reducing ED ***:This study used quantitative methods using a descriptive *** participants were patients who visited the ED.A questionnaire was administered to 296 participants between December 2021 and February *** study included 5 different hospitals in *** data were analyzed using descriptive ***:This study identified the most common presenting medical problems in the ED and why patients used the *** for using the ED included patients perceiving their condition as really urgent(62.8%),the ED being open for 24 hours(36.1%),and receiving faster care in the ED(31.4%).This study also developed recommendations for alleviating ED ***:This study identified causes of ED overcrowding and some solutions for alleviating the *** department over-crowding should be perceived as an international problem,and initiatives for solutions should be implemented quickly.
